Abstract

Background: Upper abdominal surgeries have been reported to be associated with an increased incidence of postoperative atelectasis. Preventing atelectasis is important for all patients but is more important when caring for obese patients. Objectives: To determine which of the following ventilatory strategies is better in prevention of pulmonary atelectasis in obese patients undergoing non-bariatric surgery: Volume control ventilation "VC", Pressure control Ventilation "PC", Volume control ventilation + Positive End Expiratory Pressure "PEEP", Volume control ventilation + Positive End Expiratory Pressure "PEEP" + lung recruitment maneuver. Patients and methods: A randomized-controlled trial study was carried out in the operating room (OR) in Zagazig University Hospital including 92 patients. Patients were randomly allocated into four groups. G1: Volume control ventilation "VC", G2: Pressure control Ventilation "PC", G3: Volume control ventilation + Positive End Expiratory Pressure "PEEP", G4: as in G3 + lung recruitment maneuver. Results: There was significant difference between the studied groups as regards PaO2/FiO2 ratio as the 4th group was highly significant. There was also significant difference between the studied groups as regards atelectasis score with the least atelectasis score in group 4. There was a significant difference in group 4 than the other groups as regards length of stay in PACU, and need for 100% Fio2 in PACU.Finally,there was a significant difference between the studied groups as regards postoperative pulmonary complications with the 4th group has the least postoperative complication. Conclusion: Our results suggest that volume control ventilation + Positive End Expiratory Pressure "PEEP" + lung recruitment maneuver had beneficial effects on oxygenation continued into the early recovery period and decrease pulmonary complications in the early post-operative period in obese patients undergoing non-bariatric upper abdominal surgeries.

Abstract

IntroductionChildhood cancer is a leading cause of death. It is unclear whether the COVID-19 pandemic has impacted childhood cancer mortality. In this study, we aimed to establish all-cause mortality rates for childhood cancers during the COVID-19 pandemic and determine the factors associated with mortality.MethodsProspective cohort study in 109 institutions in 41 countries. Inclusion criteria: children ResultsAll-cause mortality was 3.4% (n=71/2084) at 30-day follow-up, 5.7% (n=113/1969) at 90-day follow-up and 13.0% (n=206/1581) at 12-month follow-up. The median time from diagnosis to multidisciplinary team (MDT) plan was longest in low-income countries (7 days, IQR 3–11). Multivariable analysis revealed several factors associated with 12-month mortality, including low-income (OR 6.99 (95% CI 2.49 to 19.68); pConclusionsChildren with cancer are more likely to die within 30 days if infected with SARS-CoV-2. However, timely treatment reduced odds of death. This report provides crucial information to balance the benefits of providing anticancer therapy against the risks of SARS-CoV-2 infection in children with cancer.

Abstract

A total of 100 fresh hen's egg samples (pooled samples) including farmers' houses and poultry farms' eggs were collected for estimation of lead, cadmium and copper levels (50 pooled samples for each type, each pooled was represented by 5 eggs). Poultry farms' egg samples were purchased from different shops and supermarkets, while farmers' houses eggs were purchased from farmers' houses in Assiut Governorate, Egypt. The samples were digested then, analyzed for detection and measurement of lead, cadmium and copper by using ZEE nit 700P Atomic Absorption Spectrophotometer with Graphite Furnace Unit (AASG) (Perkin –Elmer Atomic Absorption Spectrophometry model 2380, USA). Results indicated that incidence of lead, cadmium and copper in farmers' houses egg samples were 96, 12 and 98% while in poultry farms’ egg samples were 98, 32 and 100%, respectively. Moreover, the samples above MPLs for lead in farmers' houses eggs and poultry farms' eggs were 47 (94%) out of 50 examined samples and fortunately all of examined samples contained cadmium and copper below the MPLs (0%) as stated by EOSQC (1993).

Abstract

This study came to compare the results of biofeedback retraining biofeedback (BFB), botulinum toxin botulinum type A (BTX-A) injection and partial division of puborectalis (PDPR) in the treatment of anismus patients. Consecutive patients treated for anismus fulfilled Rome II criteria for functional constipation at our institution were evaluated for inclusion. Participants were randomly allocated to receive BFB, BTX-A injection, and PDPR. All patients underwent anorectal manometry, balloon expulsion test, defecography, and electromyography activity of the anal sphincter. Follow up was conducted weekly in the first month then monthly for about 1 year. Study variables included clinical improvement, patient satisfaction, and objective improvement. Sixty patients with anismus were randomized and completed the study. The groups differed significantly regarding clinical improvement at 1 month (50% for BFB, 75%BTX-A injection, and 95% for PDPR, P = 0.006) and differences persisted at 1 year (30% for BFB, 35%BTX-A injection, and 70% for PDPR, P = 0.02). Constipation score of the patients significantly improved postPDPR and BTX-A injection. Manometric relaxation was achieved significantly in the three groups. Biofeedback retraining has a limited therapeutic effect, BTX-A injection seems to be successful for temporary treatment but PDPR is found to be an effective with lower morbidity in contrast to its higher success rate in treating anismus.

Abstract

Yessotoxin (YTX) and its derivates are lipophylic disulphated polyeter algal toxins structurally related to ciguatoxins and brevetoxins. It was originally isolated from scallops Patinopecten yessoensis in Japan. Also, it has been found in shellfish from Norway, Chile, Italy, New Zealand and along the Russian coast of White Sea. YTX was also established as a causative agent of toxicity detected by mouse bioassay in the Northern Adriatic Sea. Its presence in the Northern Adriatic has been confirmed by liquid chromatography-mass spectrometry (LC-MS). The present study investigated the occurrence of YTX in the shellfish Mytillus galoprovincialis, collected in the summer of 2006, from July to October, from a localized site in Trogir Bay ; and from July to September from the Dubrovnik City Port. Analysis of shellfish toxicity was performed by LC-MS and enzyme-linked immunosorbent assay (ELISA). Analysis was supplemented by mousse bioassay. Along with shellfish toxicity, phytoplankton composition was investigated. The presence of YTX was established in several samples from Trogir and Dubrovnik. Phytoplankton analysis of sea water samples, collected in the investigated area, showed the occurrence of known YTX producers Protoceratium reticulatum, Lingulodinium polyedrum and Goniaulax spinifera.
