Abstract This article focuses on the problem of stability for a class of linear networked control systems (NCSs) subjected to network communication delays and random deception attacks. A new memory event‐triggered mechanism (METM) is proposed to reduce the unnecessary transmitted data through the communication channel and then enhance the network resources. In this context, a new memory stochastic state feedback controller is proposed to stabilize the closed‐loop networked control system. A new randomly occurring deception attacks model is employed to deal with the security problem of NCSs. Sufficient stability conditions are derived based on a suitable Lyapunov‐Krasovskii functional (LKF). The designed methodology is proposed in terms of linear matrix inequality to synthesize both event‐triggered parameters and controller gains, and to reduce the conservatism of the system some integral lemma are exploited to bind the time derivative of the LKF. Finally, two numerical examples are presented to illustrate the effectiveness of the proposed method which provides a maximal upper bound value of the network‐induced delay and less transmitted packet regarding the maximal value delay obtained in other works, so less conservatism results are obtained, compared to previous ones in the literature.

Background and Aim: Ginger (Zingiber officinale) has great potential as a growth promoter and immunostimulant in ruminant nutrition. This study assessed the impact of ginger powder supplementation on Ossimi rams’ rumen fermentation, biochemical parameters, and antioxidant levels. Materials and Methods: Fifteen Ossimi rams, aged 10 ± 1.3 months and weighing 30 ± 1.5 kg. Rams were randomly divided into three experimental groups: The control group (G1) received standard feed, while ginger powder (5 g and 7 g/kg body weight [BW] for G2 and G3, respectively) mixed in water was administered to groups G2 and G3 before their standard feed. Results: The control group recorded higher dry matter (DM) intake values (p < 0.05) than the ginger-treated groups. The ginger-treated groups showed superiority (p < 0.05) in weight gain and feed conversion compared to the control group. The digestion coefficients of DM, crude protein, and crude fiber were significantly (p < 0.05) increased by a high dose (7 g/Kg BW) of ginger supplementation, whereas organic matter, ether extract, and nitrogen-free extract digestibility remained unchanged. Compared to the control group, the rams given 5 g of ginger had significantly less (p < 0.05) total protein and globulin in their serum, but the rams given 7 g of ginger had significantly more (p < 0.05) of these proteins. In the ginger groups, these levels were significantly (p < 0.01) lower than those in the control group for serum creatinine, uric acid, urea, total lipids, triglycerides, total cholesterol, glucose, serum alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, and gamma-glutamyl transferase. Rams given ginger had significant growth hormone, insulin-like growth factor-1, total superoxide dismutase, GSH-Px, TAC, immunoglobulin (Ig) A, and IgG enhancement (p < 0.01), and a decrease (p < 0.01) in malondialdehyde concentration compared to the control group. Significant increases in total short-chain volatile fatty acids, acetic, propionic, and isovaleric acids (p < 0.05), and significant decreases in NH3N and protozoa (p < 0.01). Conclusion: Ginger powder (5 g and 7 g) can improve growth, immune responses, antioxidant status, and ruminal parameters in rams. Further study is needed to evaluate the effect of ginger on different types of animals (cow, buffalo, and goat) to develop new feed additives.

Introduction: Gastro-esophageal Reflux Disease (GERD) is a significant global health issue affecting millions and placing a substantial burden on healthcare systems. This study investigates the prevalence and influencing factors of GERD among the urban population in Saudi Arabia, amidst rapid urbanization and lifestyle changes. The goal is to provide insights for targeted interventions and improved public health strategies. Methodology: A cross-sectional study was conducted using a comprehensive methodology. A validated, bilingual questionnaire was used to collect data on demographics, awareness, psychosocial aspects, symptoms, and medical history. The study included 493 participants, and data were collected through an online survey. Statistical analysis was performed using SPSS and Excel, maintaining a statistical power of 80 ​%. Results: The survey included 493 participants, revealing the following key findings: Gender distribution: 41.37 ​% male, 58.62 ​% female (p ​= ​0.053). Age categories: 18–25 (25.35 ​%), 26–40 (18.05 ​%), 41–55 (26.97 ​%), 56 and above (29.61 ​%) (p ​= ​0.055). Nationality: 51.92 ​% Saudi, 48.07 ​% non-Saudi (p ​= ​0.058). Other notable demographics: marital status, education levels, smoking habits, nutritional habits, BMI categories, and prevalence of bariatric surgery. GERD symptoms and psychological factors showed significant associations. Treatment preferences and lifestyle modifications varied widely. The data provide essential insights for tailored interventions and healthcare strategies. Conclusion: This study highlights the complex demographic patterns and lifestyle factors associated with GERD in Saudi Arabia's urban population. The findings emphasize the importance of tailored interventions and inform healthcare, research, and policy decisions for effective GERD management in urban settings.

Background: Osteogenesis imperfecta (OI) is a genetic connective tissue disease defined by the loss of bone mass and density, which makes the bones more brittle and more likely to fracture over time. Bone deformity and articular instability are the subsequent symptoms. Case report: This 25-year-old man had malformed lower limbs and trouble walking due to OI and dwarfism. He arrived complaining of fever, nausea, vomiting and diffuse peri-umbilical pain. During ultrasonography a blinded, oedematous lobe formation containing an appendicolith was discovered. Acute suppurative appendicitis was diagnosed, necessitating a laparoscopic appendectomy. Because the patient had previously undergone general anaesthesia, anaesthesia was thought to be attainable. Pneumoperitoneum and a 10 mm optical port inserted into the umbilicus were used in the surgical procedure. A diagnostic laparoscopy revealed faecolith obstruction and an acute suppurative appendicitis. After an hour, a laparoscopic appendectomy was performed effectively with little blood loss. Without experiencing any difficulties because of the surgery position, the patient was discharged. Conclusion: We present a case of an OI dwarf patient with acute suppurative appendicitis. It highlights the possibility of performing laparoscopic surgery in general and a laparoscopic appendectomy in particular on OI patients.

Abstract Background Muscle invasion in bladder cancer is a paramount factor in prognosis and setting the management plan. MRI is gaining preference in this field, being noninvasive with no radiation hazards and having good resolution, especially with the development of the standardized system of (VI RADS). Moreover, multiple other imaging features can aid in predicting muscle invasion. We studied some of the most commonly reported features to develop the most reliable combination to anticipate the presence of muscle invasion. Results Our prospective study on 80 patients showed 39 (48.75%) muscle invasive (MIBC) and 41 (51.25%) non-muscle invasive (NMIBC) bladder cancer cases. The inter-observer agreement on the VI RADS score and the ADC measurements were very good and they had high-accuracy predicting muscle invasion with areas under the curve (AUCs) on ROC curve analysis reaching 0.905 and 0.857, respectively. The imaging variables that showed statistically significant differences between NMIBC and MIBC cases were: the multiplicity of the lesions, vesicoureteric junction (VUJ) involvement with distal ureteric backpressure, tumor–wall contact length (TCL), tumor volume, tumor shape (sessile or papillary), presence of a stalk, the final VI RADS score and the ADC value. On the multiple regression analysis model, the multiplicity of the lesions, the minimum ADC value by ROI method and the final VI RADS score showed independent correlation with muscle invasion, negatively with the first two and positively with the latter. The combination of the six statistically significant variables on the univariate regression analysis (final VI RADS score, minimum ADC by ROI, multiplicity, index tumor shape, TCL and distal ureteric backpressure changes) showed the best AUC (0.944). Conclusions VI RADS has good diagnostic accuracy regarding muscle invasion; however, this can even be enhanced by including other quantitative and qualitative commonly reported MRI features as a proposed modification to the VI RADS.

ABSTRACTBackground Shivering occurs in 40–60% of patients under spinal anesthesia. Prophylaxis with intravenous tramadol produces a dose-dependent reduction in the incidence of shivering. Intrathecal Tramadol is used safely as an adjuvant in a dose up to 20 mg. Few studies tested the anti-shivering efficacy of intrathecal tramadol. However, no study compared the anti-shivering effect of the two different routes of tramadol. The aim is to compare the anti-shivering effect of intrathecal versus intravenous tramadol. This study was a randomized, triple-blinded parallel-design clinical trial, conducted in orthopedic operating room in our university hospitalMaterial and Methods The study included 86 patients of ASA I & II aged 18 years or more who underwent lower limb orthopedic surgery that lasted less than 2 hours under spinal anesthesia. Patients were randomly allocated into two equal groups: control intravenous (IV) group and interventional intrathecal (IT) group. We used Amadol ampoules 100 mg/2 ml (Adwia company).Intervention The control group received IT 15 mg bupivacaine and then IV tramadol 0.25 mg/kg in 5 ml normal saline. The interventional group received 20 mg IT tramadol added to 15 mg bupivacaine then IV 5 ml normal saline. The patients were observed for intraoperative shivering, the primary outcome. Intraoperative hemodynamics, postoperative shivering, and its score were all recorded. Postoperative complications such as nausea, vomiting, and hypotension were documented.Results Intraoperative shivering was observed in 18.6% of the patients in IV group compared to 4.6% of patients in the IT group with a P-value of 0.047.Conclusions IT tramadol is more efficient in preventing post-spinal shivering compared to IV tramadol.

ABSTRACTBackground The concept of lung protective ventilation (LPV) during general anesthesia (GA) aims at minimizing lung injury and postoperative pulmonary complications (POPCs). Recruitment maneuver (RM) as a part of LPV may improve lung mechanics and oxygenation, but despite extensive research, definitive guidelines for the applications of intraoperative RMs have not been established yet.Methods This study was a prospective, single-blinded, randomized clinical trial. Sixty-six subjects undergoing non-laparoscopic upper abdominal surgeries under GA were randomly assigned into two equal groups. Control group (C) received tidal volume of 8 ml/kg predicted body weight (PBW) and positive end expiratory pressure (PEEP) of 5 cmH2O without RM. Recruitment group (R) received tidal volume of 8 ml/kg PBW with stepwise RMs and individualized PEEP titration after each RM. Compliance, plateau pressure, driving pressure, SpO2 and hemodynamics were monitored at each step of RM. POPCs, length of hospital stay and mortality were recorded postoperatively.Results There was a significant reduction in POPCs in (R) group than in (C) group (P = 0.03). Also, there was a significant increase in compliance before extubation in (R) group (P = 0.001). However, no significant difference was noted between both groups as regards mortality rate and length of hospital stay.Conclusion Individualized stepwise lung RM significantly decreases the incidence of POPCs when added to LPV in patients undergoing non-laparoscopic upper abdominal surgeries under GA.

Background: There is a significant risk of morbidity and mortality associated with acetabular fractures due to significant internal organ injury and hemorrhage. Orthopedic surgeons are faced with significant challenge when treating these fractures. Early surgical fixation of acetabular fractures is the preferred course of treatment because it offers a number of benefits, including early ambulation and decreased morbidity and mortality. This study aimed to evaluate of early clinical and radiological outcome of percutaneous fixation of transverse acetabular fractures. Methods: This prospective study was conducted on 18 patients with transverse acetabular fractures using fluoroscopic guidance who underwent percutaneous fixation by closed reduction aided by Shanz screw with mean follow up period 2 years. All patients was had an AP, Lateral, inlet, outlet and Judet views X-ray and CT scan. The clinical outcomes was evaluated using Merle D'Aubigne. Results: According to the Merle d'Aubigne Score Distribution among studied groups was excellent in 15 patients (83.3%), good in 2 patients (11.1%) and only one case 5.6% had fair score. Most of the studied group (16 cases, 88.9%) didn't have any postoperative complications and two cases had complications which is superficial tract infection (11.1%). Conclusion: we find that the use of fluoroscopic guidance in percutaneous screw treatment of transverse acetabular fractures is a safe and reasonably simple surgical approach for treating non-comminuted and minimally displaced fractures associated with early rehabilitation and less complications. Although many quantitative risk assessment models have been proposed in literature, their use in construction practice remain limited due to a lack of domain-specific models, tools, and application examples. This is especially true in wind farm construction, where the state-of-the-art integrated Monte Carlo simulation and critical path method (MCS-CPM) risk assessment approach has yet to be demonstrated. The present case study is the first reported application of the MCS-CPM method for risk assessment in wind farm construction and is the first case study to consider correlations between cost and schedule impacts of risk factors using copulas. MCS-CPM provided reasonable risk assessment results for a wind farm project, and its use in practice is recommended. To facilitate the practical application of quantitative risk assessment methods, this case study provides a much-needed analytical generalization of MCS-CPM, offering application examples, discussion of expected results, and recommendations to wind farm construction practitioners. La presente etude de cas est la premiere application rapportee de la methode SMC-MCC aux fins de l'evaluation des risques dans la construction de parcs eoliens et est la premiere etude de cas a examiner les correlations entre les couts et les impacts du calendrier des facteurs de risque a l'aide de copules. La SMC- MCC a fourni des resultats d'evaluation des risques raisonnables pour un projet de parc eolien, et son utilisation dans la pratique est recommandee.
