Zusammenfassung: HINTERGRUND: Zum Zeitpunkt der Diagnosestellung des Magenkarzinoms haben ca. zwei Drittel der Patienten bereits Metastasen. Wichtig ist es, die Oligometastasierung von der diffus metastasierten Situation abzugrenzen. Die S3-Leitlinie hat die Definition der Oligometastasierung erstmals in die Leitlinie aufgenommen.Fragestellung: Kann das Überleben von Patienten mit Oligometastasierung mittels Resektion des Tumors und der Metastase kombiniert mit perioperativer Chemotherapie verbessert werden?Material und methoden: In dieser Übersichtsarbeit wird die Datenlage der aktuellen Literatur dargestellt.Ergebnisse: Die holländische Magenkarzinomstudie stellte ein verbessertes medianes Überleben für Patienten mit singulären Metastasen fest, wenn diese reseziert wurden. Aufgrund der Resultate der deutschen AIO-FLOT3-Studie, in der sich das mediane Überleben von Patienten mit Oligometastasen mit multimodaler Therapie verdoppelte, wurde die AIO-FLOT5(RENAISSANCE)-Studie initiiert. Diese randomisiert limitiert metastatische Patienten nach neoadjuvanter Chemotherapie entweder zu Resektion gefolgt von Chemotherapie oder zu definitiver Chemotherapie. Weitere randomisierende Studien untersuchen den Nutzen von Antikörpern und Immun-Checkpoint-Inhibitoren beim lokoregionalen und metastasierten Magenkarzinom mit vielversprechenden Resultaten.Diskussion: Die Resultate der aktuellen Studien werden zeigen, ob Patienten mit Oligometastasierung von einer multimodalen Therapie mit Resektion profitieren. Die eindeutige Definition der Oligometastasierung, eine Beurteilung des Ansprechens nach neoadjuvanter Chemotherapie und eine realistische Einschätzung der R0-Resektion werden bei der entsprechenden Patientenselektion hilfreich sein. Background: At the time of diagnosis of gastric cancer approximately one third of patients already have metastases. It is important to differentiate between oligometastasis and the diffuse metastatic situation. For the first time the definition of oligometastasis has been integrated into the German S3 guidelines.Objective: Can multimodal treatment with tumor resection and metastasectomy combined with perioperative chemotherapy, increase the chances of survival in oligometastatic patients?Material and methods: In this review article the data situation of the current literature is discussed.Results: The Dutch D1/D2 trial reported an increased median survival for a subgroup of patients with single metastasis who underwent resection. Multimodal treatment with resection doubled the median survival of oligometastatic patients in the German AIO-FLOT 3 study and as a consequence, the AIO-FLOT 5 (RENAISSANCE) trial was designed. Patients with oligometastatic gastric and esophagogastric junction cancer are randomized after chemotherapy to either undergo resection followed by adjuvant chemotherapy or to undergo definitive chemotherapy. Further randomized trials investigate the benefit of antibodies and immune checkpoint inhibitors in locoregional and advanced metastatic gastric cancer with promising results.Conclusion: The results of the ongoing randomized trials will show if oligometastatic patients benefit from a multimodal treatment with resection. The clear definition of the oligometastatic state, assessment of the response to neoadjuvant chemotherapy and realistic estimation of the R0 resectability will be useful for patient selection.

Le cancer de l'œsophage reste un fardeau oncologique avec un taux de survie bas. Une prise en charge multidisciplinaire est primordiale afin d'offrir un traitement adapté à l'état général du patient et au stade de la tumeur. De nouvelles prises en charge minimalement invasives chirurgicales permettent de diminuer le traumatisme d'une chirurgie majeure et améliorent la récupération des patients en postopératoire. Les traitements oncologiques ont également évolué et un traitement définitif par radiochimiothérapie peut être proposé dans des cas précis.

Durch die Aktualisierung der S3-Leitlinie Magenkarzinom „Diagnostik und Therapie der Adenokarzinome des Magens und ösophagogastralen Übergangs“ (Version 2.0 – August 2019) ergeben sich im Vergleich zur Erstversion 2011 für die Chirurgie relevante Neuerungen, die im Folgenden mitsamt Evidenz zusammengefasst sind. Die Leitlinie versteht sich als „Living Guideline“ und die Inhalte der Leitlinie sollen fortan jährlich auf der Basis neuer Studiendaten aktualisiert werden mit von nun an jährlich geplanten Aktualisierungen [1, 2]. Es wird betont, dass in diesem Kurz-Artikel nur die wesentlichen, für die Chirurgie relevanten Aspekte aufgeführt sind.

Esophagectomy, even with the progress in surgical technique and perioperative management, is a highly specialized surgery, associated with a high rate of complications. Early recognition and adequate treatment should be a standard of care for the most common postoperative complications: anastomotic leakage, pneumonia, atrial fibrillation, chylothorax, and recurrent laryngeal nerve palsy. Recent progress in endoscopy with vacuum and stent placement, or in radiology with embolization, has changed the management of these complications. The success of nonoperative treatments should be frequently reassessed and reoperation must be proposed in case of failure. We have summarized the clinical signs, diagnostic process, and management of the frequent complications after esophagectomy for esophageal cancer.

L'obésité est un maladie chronique et plurifactorielle avec des causes génétiques, endocriniennes, métaboliques et environnementales. Elle peut être la cause de maladies cardiovasculaires, respiratoires, métaboliques, digestives, endocriniennes, ostéoarticulaires ou encore oncologiques. De par cette complexité, le traitement de l'obésité nécessite une approche multidisciplinaire spécialisée.

Objective. Multiple Asian studies have proved the feasibility of laparoscopic approach for surgical treatment of gastric cancer. The difference between Asian and European patients could limit their application in Europe. We reviewed the literature for European studies comparing open gastrectomy with laparoscopic approach in the treatment of gastric cancer. Method. We searched the keywords gastric cancer and laparoscopy in MEDLINE and EMBASE. We included all studies published between 1990 and 2016 and conducted in Europe. Result. We found 1 randomized and 13 cohort studies which compared laparoscopic with open gastrectomy. We found no mean difference in the number of lymph nodes harvested between laparoscopic and open group (mean difference: -0.49; 95% CI: -2.42; 1.44, p=0.62) and no difference of short-term or long-term mortality (short-term odds ratio: 0.74, p=0.47; long-term odds ratio: 0.65, p=0.11). Multimodal primary treatment of localised adenocarcinoma of the stomach, the oesophagus and the oesophagogastric junction (AEG) was reviewed by a multidisciplinary expert panel in a moderated consensus session. Here, we report the key points of the discussion and the resulting recommendations. The exact definition of the tumour location and extent by white light endoscopy in conjunction with computed tomography scans is the backbone for any treatment decision. Their value is limited with respect to the infiltration depth, lymph node involvement and peritoneal involvement. Additional endoscopic ultrasound was recommended mainly for tumours of the lower oesophagogastric junction (i.e. AEG type II and III according to Siewert) and in early cancers before endoscopic resection. Laparoscopy to diagnose peritoneal involvement was thought to be necessary before the start of neoadjuvant treatment in all gastric cancers and in AEG type II and III. In general, perioperative multimodal treatment was suggested for all locally advanced oesophageal tumours and for gastric cancers with a clinical stage above T1N0. There was consensus that the combination of fluorouracil, folinic acid, oxaliplatin and docetaxel is now a new standard chemotherapy (CTx) regimen for fit patients. In contrast, the optimal choice of perioperative CTx versus neoadjuvant radiochemotherapy (neoRCTx), especially for AEG, was identified as an open question. Expert treatment recommendations depend on the tumour location, biology, the risk of incomplete (R1) resection, response to treatment, local or systemic recurrence risks, the predicted perioperative morbidity and patients' comorbidities. In summary, any treatment decision requires an interdisciplinary discussion in a comprehensive multidisciplinary setting.

Docetaxel-based chemotherapy is effective in metastatic gastric and gastro-oesophageal junction adenocarcinoma. This study reports on the safety and efficacy of the docetaxel-based triplet FLOT (fluorouracil plus leucovorin, oxaliplatin and docetaxel) as a perioperative therapy for patients with locally advanced, resectable tumours.

The minimal length of proximal margin (PM) in esophagogastric junction cancer has not been established yet and its impact on patient survival remains unclear. Pubmed, Embase and Scopus databases were searched for "adenocarcinoma of the esophagogastric junction", "adenocarcinoma of the gastroesophageal junction" and "cardia cancer", each combined with "proximal margin". English written studies that specified PM length in AEG were included. Survival data in relation to PM were extracted. 13 studies, that were all retrospective case series, with a total number of 2648 patients met inclusion criteria and were analyzed. While 93% of 230 patients with Siewert type I had esophagectomy, 69% of 1270 patients with Siewert type II and 93% of 872 patients with Siewert type III had transhiatal extended gastrectomy. Minimal PM length was treated by five studies and ranged between 2 and 6 cm. While three studies defined minimal PM by the necessary length to obtain R0 resection, two studies found minimal PM length significantly associated with survival. Multivariate analyses revealed in two studies an independent impact of PM on survival, whereas one study did not found any significant relation between PM and survival. One study showed that PM length was significantly associated with survival in T2-4N0-2 tumors, but not in T1 or N3 tumors. In conclusion, available retrospective studies did not allow a conclusion for a minimal length of PM and showed no clear evidence for an impact of PM length on survival. Taking into consideration available data and the shrinkage phenomen, a PM > 2 cm might be necessary to obtain a sufficient PM.

Early stage carcinomas of the esophagus are histologically differentiated into adenocarcinomas and squamous cell carcinomas and subdivided into mucosal (m1-3) and submucosal (sm1-3) carcinomas depending on the infiltration depth. While the prevalence of lymph node metastases in mucosal carcinomas is very low, the probability of lymph node metastases increases from submucosal infiltration with increasing depth. According to the current German S3 guidelines endoscopic resection is the recommended treatment strategy for mucosal adenocarcinoma without histological risk factors (lymphatic invasion [L1], venous invasion [V1], poorly differentiated [>G2], microscopic residual disease [R1] at the deep resection margin). For superficial submucosal infiltration (sm1) without histological risk factors endoscopic resection can also be carried out, whereby in this case the guidelines make a stronger recommendation for esophagectomy. For squamous cell carcinoma endoscopic resection is indicated for an infiltration depth up to middle layer mucosal carcinoma (m2) without histological risk factors. Outside of these criteria an esophageal resection should always be carried out. The surgical gold standard is a subtotal abdominothoracic esophagectomy with two-field lymphadenectomy. Alternative procedures are total esophagectomy in proximal esophageal carcinoma and transhiatal extended gastrectomy for carcinoma of the cardia. Limited proximal or distal esophageal resections can be performed in proximal or distal mucosal carcinoma without the possibility of endoscopic resection; however, partial resections are not superior in terms of functional results and are not oncologically equivalent due to limited lymphadenectomy. Minimally invasive procedures show good oncological results and reduce the morbidity of radical esophagectomy. Reduced morbidity might be an argument for surgical resection in borderline cases between endoscopic and surgical resection. Ösophagusfrühkarzinome werden histologisch in Adeno- und Plattenepithelkarzinome und abhängig von der Tiefeninfiltration in m1–3(Mukosa)-und sm1–3(Submukosa)-Karzinome unterteilt. Während die Prävalenz von Lymphknotenmetastasen bei Mukosakarzinomen sehr gering ist, steigt die Wahrscheinlichkeit von Lymphknotenmetastasen ab Infiltration der Submukosa mit zunehmender Tiefe an. Nach der aktuellen deutschen S3-Leitlinie wird bei intramukosalen Adenokarzinomen ohne histologische Risikofaktoren (Lymphgefäßinvasion [L1], Blutgefäßinvasion [V1], gering differenziert [>G2], mikroskopischer Residualtumor [R1] basal) eine endoskopische Resektion empfohlen. Bei oberflächlicher Submukosainfiltration (sm1) ohne histologische Risikofaktoren kann ebenfalls eine endoskopische Resektion erfolgen, wobei die Leitlinie in diesem Fall eine stärkere Empfehlung für eine Ösophagektomie gibt. Bei Plattenepithelkarzinomen ist bis zur Infiltrationstiefe m2 und ohne histologische Risikofaktoren eine endoskopische Resektion indiziert. Außerhalb dieser Kriterien sollte immer eine Ösophagusresektion erfolgen. Das chirurgische Standardverfahren ist eine subtotale abdominothorakale Ösophagektomie mit Zweifeldlymphadenektomie. Alternative Verfahren sind die totale Ösophagektomie bei proximalen Karzinomen und die transhiatal erweiterte Gastrektomie bei Kardiakarzinomen. Limitierte proximale oder distale Ösophagusresektionen können bei proximal oder distal gelegenen und endoskopisch nicht resezierbaren Mukosakarzinomen durchgeführt werden, sind aber der radikalen Resektion funktionell nicht überlegen und onkologisch aufgrund der eingeschränkten Lymphadenektomie nicht gleichwertig. Minimalinvasive Verfahren zeigen gute onkologische Ergebnisse und vermindern die Morbidität der radikalen Ösophagusresektion. Dies sollte in Zukunft bei der Entscheidungsfindung in Grenzfällen zwischen endoskopischer und chirurgischer Resektion berücksichtigt werden.

Malgré une incidence en diminution, le cancer gastrique reste un fardeau. Généralement découvert à un stade avancé, la survie a pu être améliorée grâce aux progrès effectués dans le traitement périopératoire et la prise en charge chirurgicale. Le bilan complémentaire préopératoire est capital afin de classifier de manière précise la tumeur et d'introduire le traitement adéquat. La localisation de la tumeur est en particulier importante car le traitement des tumeurs de la jonction œsogastrique et de l'estomac est différent. Les techniques minimalement invasives permettent de diminuer la morbidité postopératoire et de mettre les patients dans les meilleures conditions possibles pour un éventuel traitement adjuvant.

Management of gastroesophageal junction (GEJ) adenocarcinoma is a controversial topic. The rising incidence of this cancer requires a clear consensus to ensure proper management. Application of oncological principles for tumors of the esophagus or stomach is not possible because of comparative differences in the biology of GEJ adenocarcinoma, leading to different therapeutic options. Staging work-up with endoscopy, endosonography, and PET is essential to inform the choice of neoadjuvant treatment and surgical approach to GEJ adenocarcinoma. Surgery remains the only curative treatment and should be undertaken in specialized centers.

Esophageal cancer is the eighth most common cancer globally and has the sixth worst prognosis because of its aggressiveness and poor survival. Data regarding cancer treatment in older patients is limited because the elderly have been under-represented in clinical trials. Therefore, we reviewed the existing literature regarding treatment results for elderly patients (70+ years). Areas covered: We used pubmed to analyze the actual literature according to elderly esophageal cancer patients with subheading of incidence, esophagectomy, chemoradiation or chemotherapy. The main points of interest were treatment options for patients with Barrett's esophagus or early carcinoma, advanced tumor stages, and inoperable cancer. Expert opinion: The incidence of esophageal cancer has been increasing over the past thirty years, with a rapid increase of esophageal adenocarcinoma in Western industrialized nations. Patients aged over 60 years have been particularly affected. In this review, we have shown that elderly patients with esophageal cancer have various alternatives for adequate treatment. Clinical evaluation of comorbidity is necessary to make treatment decisions. Therapeutic options for early carcinomas are endoscopic or surgical resection. For elderly patients with advanced carcinomas, preoperative chemoradiation or chemotherapy should be discussed.

Die Therapie von Patienten mit oligometastasierten Magen- und Ösophaguskarzinomen befindet sich in den letzten Jahren im Wandel. Bisher wird in den aktuellen S3-Leitlinien zum Magen- und Ösophaguskarzinom eine chirurgische Therapie nicht empfohlen. Allerding befassen sich heutzutage die bestehenden lokalen Tumorboards immer häufiger mit diesen Patienten. Durch die multimodalen Therapiekonzepte werden bei Patienten, die gut auf die Therapie ansprechen, vermehrt individuelle Indikationen gestellt und zusätzlich eine radikale Operation des Primärtumors und der Metastasen durchgeführt. In dieser Übersichtsarbeit werden die aktuellen Daten der vorliegenden Literatur besprochen und eine Grundlage für erweiterte chirurgische Therapien dargelegt. Zusätzlich mit den aktuell vorliegenden Daten der FLOT3-Studie und den meist retrospektiven Daten scheinen sich Patienten identifizieren zu lassen, die von einer aggressiven Therapie profitieren. Randomisierte prospektive Studien wie die RENAISSANCE/FLOT5-Studie und die GASTRIPEC-Studie müssen in der Zukunft weiter klären, ob eine aggressive chirurgische Therapie im multimodalen Therapiekonzept von metastasierten Magen- und Ösophaguskarzinomen eine weitere anerkannte Säule bilden wird. The therapeutic approach to patients with oligometastatic gastric cancer and esophageal cancer is currently undergoing a shift towards a more aggressive therapy including surgical resection. In the current German S3 guidelines surgical treatment of metastatic disease is not recommended; however, nowadays interdisciplinary tumor boards have to evaluate such patients increasingly more often. On an individual basis a radical surgical resection of the primary tumor and the metastases is considered and performed in patients who respond well to multimodal chemotherapy concepts. In this review article the currently available data from the literature are discussed and a foundation for individually extended surgical approaches is presented. Together with the currently available results of the FLOT 3 study and the mostly retrospective studies, it seems to be possible to identify patients who would profit from such an aggressive treatment. In the future randomized prospective studies, such as the RENAISSANCE/FLOT 5 study and the GASTRIPEC study will have to evaluate whether an aggressive surgical therapy within multimodal therapy concepts of metastatic gastric and esophageal carcinomas is warranted.

According to the German S3-guideline 'Palliative Medicine' which has been supported by the German Guideline Program in Oncology, constipation in palliative patients requires a consistent prophylaxis and therapy. Constipation is caused by immobilisation, poor health, exsiccosis, a low-fiber diet or a preexisting functional constipation. Further important causes are substances with constipating side effects, such as opioids or anticholinergic drugs. Pragmatically, constipation should be assessed by subjective parameters such as the feeling of incomplete evacuation, straining, or other complaints. Objective parameters are hard stool or reduced number of bowel movements. For the early detection of constipation the amount of the stool mass and the stool frequency are applicable. If constipation occurs, a standardized escalating protocol applying adequate fluid intake, a high fibre diet, and laxatives is recommended. New prokinetic agents, secretagogues, or opioid antagonists may be used as reserve drugs. Nach der durch das Leitlinienprogramm Onkologie geförderten deutschen S3-Leitlinie Palliativmedizin für Patienten mit einer nicht-heilbaren Krebserkrankung erfordert das häufige Auftreten einer Obstipation bei Palliativpatienten eine konsequente Prophylaxe und Therapie. Als Ursachen der Verstopfung finden sich häufig Immobilität, ein reduzierter Allgemeinzustand, ein geringer Volumenstatus bzw. eine ballaststoffarme Ernährung oder auch eine vorbestehende chronische Obstipation. Obstipierende Pharmaka wie z. B. Opioide oder Substanzen mit anticholinergen Eigenschaften stellen weitere wichtige Ursachen dar. Die Diagnose einer Obstipation erfolgt pragmatisch überwiegend durch subjektive Parameter wie das Gefühl einer unvollständigen Entleerung, die Notwendigkeit vermehrten Pressens bzw. stuhlgangabhängige Beschwerden. Objektive Parameter sind eine harte Stuhlkonsistenz bzw. eine verminderte Stuhlmenge und -frequenz. Zur Therapie der Obstipation wird ein standardisiertes stufenförmiges Vorgehen mit ausreichender Flüssigkeitszufuhr, ballaststoffreicher Ernährung und Laxanzien empfohlen. Neue prokinetische Substanzen, Sekretagoga oder Opioidantagonisten können als Reservemedikamente eingesetzt werden.

In den aktuell gültigen deutschen S3-Leitlinien wird bei metastasierten Magenkarzinomen und metastasierten Adenokarzinomen des ösophagogastralen Übergangs eine chirurgische Therapie in der Regel nicht empfohlen. In der täglichen Praxis findet jedoch schon heute in Einzelfällen eine Erweiterung der Indikation statt, sodass auch solche Patienten in kurativer Zielsetzung im Rahmen eines multimodalen Therapiekonzepts ausgedehnt und radikal operiert werden. Diese Arbeit stellt die wissenschaftliche Rationale eines solchen Vorgehens anhand der vorliegenden Literatur und unter Einbeziehung moderner, multimodaler Konzepte dar und verweist auf Kriterien, die für eine Erweiterung der Indikation gegeben sein sollten. Bislang existieren lediglich retrospektive Untersuchungen und wenige aktuelle Metaanalysen, die eine chirurgische Behandlung bei metastasierten Adenokarzinomen rechtfertigen. Es liegen zudem erste Ergebnisse der als Abstract publizierten FLOT-3-Studie vor, die eine Patientensubgruppe identifizieren konnte, die von einer Resektion im metastasierten Stadium profitiert. Ob die chirurgische Therapie zukünftig ein fester Bestandteil in der Behandlung des begrenzt metastasierten Adenokarzinoms des Magens und des Ösophagus wird, müssen große prospektiv randomisierte Studien wie die RENAISSANCE/FLOT-5-Studie zeigen. In the current German S3 guidelines surgical treatment is not recommended for metastatic gastric cancer or metastatic adenocarcinoma of the esophagogastric junction; however, in routine practice the indications can be extended so that there may be occasions in which radical surgical intervention for specific individuals may be appropriate as part of a multimodal therapy with curative intent. This article presents the scientific rationale of such an approach based on the available literature considering modern, multimodal therapy concepts including criteria to be met for radical surgery. Currently only retrospective trials and limited current meta-analysis data are available for justifying surgical treatment for metastatic adenocarcinoma. The recently published initial results of the FLOT-3 study identified a patient subgroup that benefits from a resection even though metastasis has occurred. Whether surgical therapy will become an integral part of the treatment of limited metastatic adenocarcinoma of the stomach and esophagus in the future, has to be demonstrated by large prospective randomized studies, such as the RENAISSANCE/FLOT-5 study.

AIMS: Outcomes for patients with oesophago-gastric cancer are variable across Europe. The reasons for this variability are not clear. The aim of this study was to describe and analyse clinical pathways to understand differences in service provision for oesophageal and gastric cancer in the countries participating in the EURECCA Upper GI group. METHODS: A questionnaire was devised to assess clinical presentation, diagnosis, staging, treatment, pathology, follow-up and service frameworks across Europe for patients with oesophageal and gastric cancer. The questionnaire was issued to experts from 14 countries. The responses were analysed quantitatively and qualitatively and compared. RESULTS: The response rate was (10/14) 71.4%. The approach to diagnosis was similar. Most countries established a diagnosis within 3 weeks of presentation. However, there were different approaches to staging with variable use of endoscopic ultrasound reflecting availability. There has been centralisation of treatments in most countries for oesophageal surgery. The most consistent area was the approach to pathology. There were variations in access to specialist nurse and dietitian support. Although most countries have multidisciplinary teams, their composition and frequency of meetings varied. The two main areas of significant difference were research and audit and overall service provision. Observations on service framework indicated that limited resources restricted many of the services. CONCLUSION: The principle approaches to diagnosis, treatment and pathology were similar. Factors affecting the quality of patient experience were variable. This may reflect availability of resources. Standard pathways of care may enhance both the quality of treatment and patient experience.
