In this study we evaluated a zinc copper-citric acid biocomplex (Dentamet®) for the effect on citrus mal secco disease caused by Plenodomus tracheiphilus. In vitro tests highlighted an inhibitory activity of the compound on mycelial growth and phialoconidia germination. Independent in vivo trials were performed in a growth chamber inoculating a virulent fungal isolate on sour orange seedlings and young plants of two lines of Femminello lemon, namely ‘Zagara bianca’ and ‘2Kr’, grafted on sour orange and grown in pots. The compound was leaf sprayed three days before the inoculation by wounds with phialoconidia. Overall, disease index and disease progress curve in plants inoculated with P. tracheiphilus were reduced to a different extent by single leaf treatment. Additional post-inoculation treatments did not significantly affect the results. Quantification of DNA of P. tracheiphilus in leaf disks at the inoculation sites leads to conclude that the lessening of disease symptoms is not strictly related to a direct effect of the compound on the pathogen, and a host defence stimulation maybe involved.

- Abstract
BACKGROUND: Return to play (RTP) protocols have been proposed to early detect cardiovascular involvement due to COVID-19 and reduce the risk of sports-related sudden cardiac death. However, uncertainties remain about the true prevalence of COVID-19 myopericarditis, the arrhythmic risk and the cost of this protocol. METHODS: We collected data from 217 competitive and professional athletes of both genders who underwent RTP protocol (clinical history, resting and exercise ECG, and echocardiography). Holter monitoring and/or cardiac magnetic resonance (CMR) were performed in case of abnormalities. In 107 athletes, the RTP data were compared with those of preparticipation evaluation (PPE) performed prior to COVID-19 infection. RESULTS: Out of 217 consecutive athletes evaluated with the RTP protocol, 7 underwent CMR: among these we found alterations compatible with myopericarditis in 3 (1.4%), with a cost per person of € 223.93 and a cost per diagnosis of € 16,197.53. Of the 107 athletes previously evaluated with PPE, 4 underwent RMC: we made a final diagnosis of myocarditis in 1 athlete (0.9%), whereas another athlete (0.9%) showed moderate pericardial effusion. The clinical presentation of both these athletes was characterized by the presence of ventricular arrhythmias newly detected during RTP. Compared to PPE, during RTP higher values were observed for shortness of breath, weight, heart rate and corrected QT interval, whereas lower values for sinus bradycardia and the E/A ratio of mitral flow. CONCLUSIONS: The prevalence of myopericarditis was similar to that reported in previous cross-sectional and case-control studies. The availability of data recorded before COVID-19 was important in the evaluation of athletes with arrhythmias. The RTP protocol has proven to be less cost effective than normal PPE.

- Abstract
Background: To assess the event rates of myocarditis detected by Cardiac Magnetic Resonance (CMR) in athletes who recovered from COVID-19. Methods: A systematic literature search was performed to identify studies reporting abnormal CMR findings in athletes who recovered from COVID-19. Secondary analyses were performed considering increased serum high sensitivity troponin (hs-Tn) levels and electrocardiographic (ECG) and echocardiographic (ECHO) abnormalities. Results: In total, 7988 athletes from 15 studies were included in the analysis. The pooled event rate of myocarditis was 1% (CI 1-2%), reaching 4% in the sub-group analysis. In addition, heterogeneity was observed (I2 43.8%). The pooled event rates of elevated serum hs-Tn levels, abnormal ECG and ECHO findings were 2% (CI 1-5%), 3% (CI 1-10%) and 2% (CI 1-6%), respectively. ECG, ECHO and serum hs-Tn level abnormalities did not show any correlation with myocarditis. Conclusions: The prevalence of COVID-19-related myocarditis in the athletic population ranges from 1 to 4%. Even if the event rate is quite low, current screening protocols are helpful tools for a safe return to play to properly address CMR studies. Trial registration: the study protocol was registered in the PROSPERO database (registration number: CRD42022300819).

- Abstract
Background: The aim of this study was to identify a possible link between bicuspid aortic valve (BAV) and premature ventricular beats (PVBs), particularly from left and right ventricular outflow tracts, and to investigate possible associations between these arrhythmias and echocardiographic abnormalities. Methods: A comparison of sportspeople with and without BAV was performed to identify PVBs' occurrence in these two series. Then, subdividing the BAV group on the presence of cardiovascular complications due to BAV, we compared arrhythmic features between these two subgroups and echocardiographic findings between athletes with and without left and right outflow tract PVBs. Results: PVBs in 343 athletes with BAV were compared with 309 athletes without BAV, showing an increased frequency (29% vs. 11.8%, p < 0.001; OR 3.1; CI 2.1-4.7) and origin from the left (18.4% vs. 3.2%, p < 0.001, OR 6.7; CI 3.4-13.4) and right (15.2% vs. 3.6%, p < 0.001, OR 4.8; CI 2.5-9.5) outflow tracts compared to other ventricular areas (fascicular PVBs p = 0.81, other morphologies p = 0.58). No difference in PVBs' occurrence was found between near normal valve BAV and pathological BAV, nor was a difference in echocardiographic characteristics found between patients with and without outflow tract arrhythmias. Conclusions: A possible causal link between BAV and PVBs was highlighted, but no association between PVBs and complicated BAV was emphasized.

- Abstract
Background and objectives: An Italian nationwide pre-participation screening approach for prevention of sudden cardiac death in athletes (SCD-A) in competitive sportspeople showed promising results but did not achieve international consensus, due to cost-effectiveness and the shortfall of a monitoring plan. From this perspective, we tried to provide an epidemiological update of SCD-A in Italy through a year-long internet-based search. Materials and Methods: One year-long Google search was performed using mandatory and non-mandatory keywords. Data were collected according to prevalent SCD-A definition and matched with sport-related figures from Italian National Institute of Statistics (ISTAT) and Italian National Olympic Committee (CONI). Results: Ninety-eight cases of SCD-A in 2019 were identified (48.0% competitive, 52.0% non-competitive athletes). Male/female ratio was 13:1. The most common sports were soccer (33.7%), athletics (15.3%) and fitness (13.3%). A conclusive diagnosis was achieved only in 37 cases (33 of cardiac origin), with the leading diagnosis being coronary artery disease in 27 and a notably higher occurrence among master athletes. Combining these findings with ISTAT and CONI data, the SCD-A incidence rate in the whole Italian sport population was found to be 0.47/100,000 persons per year (1.00/100,000 in the competitive and 0.32/100,000 in the non-competitive population). The relative risk of SCD-A is 3.1 (CI 2.1-4.7; p < 0.0001) for competitive compared to non-competitive athletes; 9.9 for male (CI 4.6-21.4; p < 0.0001) with respect to female. Conclusions: We provided an updated incidence rate of SCD-A in both competitive and non-competitive sport in Italy. A higher risk of SCD-A among competitive and male athletes was confirmed, thus corroborating the value of Italian pre-participation screening in this population.

BACKGROUND: Stump appendicitis is a rare delayed complication post-appendectomy and it represents a diagnostic problem as clinicians are often not very familiar with this pathology. MATERIALS AND METHODS: One case of stump appendicitis has been reported in a 54 year old woman, whose acute phase was conservatively treated in our Department. A review of Medline literature was also carried out, from 1945 to 2015, showing 111 cases of stump appendicitis. RESULTS: Stump appendicitis has been reported after either open or laparoscopic appendectomy, in a range between days to several years from the first procedure. Nowadays, it is not considered yet as a possible differential diagnosis in patients with pain in the right iliac fossa previously undergone appendectomy. For this reason, the diagnosis can be delayed and complications such as acute abdomen, perforation, sepsis can occur. Completion appendectomy is the treatment of choice for stump appendicitis. CONCLUSIONS: The aim of this article is to underline the importance of stump appendicitis. Clinicians should be aware of the possibility of SA and they should confirm any clinical suspicion throughout radiologic images, in order to promptly recognize this entity and to avoid the related complications.

Il concetto di tranquillità è associato spesso ad uno "stato della mente", a qualcosa che non è misurabile. Tuttavia, la "Mappatura delle Aree Tranquille" ("Tranquillity Areas Mapping") riguarda la definizione di una metodologia per misurare una specifica qualità ambientale collegata alla salute, al benessere e alla qualità della vita che può essere valutata facendo riferimento a una serie di indicatori, qualitativi e quantitativi, definiti e appropriati. Il lavoro presenta una iniziativa progettuale in corso di realizzazione tra il Parco Nazionale della Sila, il Consiglio Nazionale delle Ricerche (CNR-ISAFOM) e il Dipartimento di Agraria dell'Università Mediterranea di Reggio Calabria, avente come obiettivo l'identificazione e la mappatura delle "aree tranquille" nel Parco Nazionale della Sila/Area MAB UNESCO (Calabria, Italia), caratterizzato da una forte e primaria presenza di foreste di diversa tipologia e struttura. La metodologia implementata fa riferimento all'esperienza condotta nel 2012 dal Dipartimento di Agraria dell'Università Mediterranea di Reggio Calabria nel Parco Naturale Regionale delle Serre (VV) (Modica et al. 2013), sulla base delle esperienze precedentemente condotte in Europa, specialmente nel Regno Unito (Countryside Agency 2005; Jackson et al. 2008). L'intero Progetto si articola in diverse fasi che prevedono azioni volte all'individuazione e sensibilizzazione dei potenziali stakeholder; realizzazione e somministrazione del questionario/intervista; raccolta, omogeneizzazione e analisi statistica dei dati; realizzazione dell'infrastruttura e della banca dati geospaziale; analisi e spazializzazione dei parametri e degli indicatori che definiscono il carattere di "tranquillità"; mappatura e pubblicazione delle mappe delle aree tranquille nel territorio del Parco Nazionale della Sila e dell'Area MAB UNESCO su piattaforma WebGIS implementata con software geografico libero FOSS4G. In questo lavoro verranno illustrate le prime fasi del progetto volte alla a) individuazione delle categorie dei potenziali stakeholder; b) Ideazione, progettazione e diffusione di materiale divulgativo e sensibilizzazione degli stakeholder anche attraverso la somministrazione ed il test di un questionario-intervista sulla rilevanza del "Tranquillity Areas Mapping" per lo sviluppo e la fruizione dei servizi ecosistemi in aree protette caratterizzate dalla presenza predominante di foreste.

Patients with thoracic trauma constitute one third of all the trauma cases, in west Sicily were recorded 941 thoracic trauma during the period between 2006 and 2009. Sicily is one of the Italian regions with the highest rate of obesity: some studies have demonstrated that obesity is an independent risk factor for mortality in high energy blunt traumas.This study was conducted with trauma patients older than 20 years old who presented to our Department during the last five years. We only included thoracic injuries and politrauma with a thoracic involvement and a BMI25. Patients were divided into two groups: HET and LET patients.Thoracic trauma was more common in patients with a BMI25 than in normo-weight and clinic admission rate, length of hospital stay and ISS score increased in proportion with the increase of BMI. Both HET (high energy trauma) and LET (low energy trauma) revealed that overweight, obese and morbidly obese patients had greater admissions and length of hospital stay.The overweight and obese population has increased substantially over the last two decades and 61,5% of the Sicilian population is above normal weight. A large body mass with excess adiposity may contribute to HET injuries in several ways. Obesity has a number of comorbidities that reduce chances of recovery in overweight and obese patients experienced thoracic trauma both HET and LET.Obesity increases morbidity independently of injury severity in thoracic trauma patients. As BMI increased, length of hospital stay increased and prognosis deteriorates.BMI, Obesity, Thoracic trauma.Circa un terzo di tutti I casi di trauma è rappresentato da trauma del torace. Nella Sicilia occidentale, tra il 2006 e il 2009, sono stati riportati 941 casi di trauma del torace. La Sicilia è una delle regioni italiane con il tasso di obesità maggiore: alcuni studi hanno dimostrato come l’obesità rappresenti un fattore di rischio per mortalità nei traumi contusivi ad alta energia.Questo studio è stato condotto su pazienti traumatizzati, di età superiore a 20 anni, afferiti al nostro Dipartimento negli ultimi cinque anni. Nel campione abbiamo selezionato solo I trauma del torace e I politraumi con interessamento toracico e pazienti con BMI25. I pazienti sono stati suddivisi in due gruppi: HET e LET.I trauma del torace sono più comuni in pazienti con BMI25 che nei normo-peso. Tasso di ammissione ospedaliera, durata della degenza e indice ISS aumentano all’aumentare del BMI sia negli HET che nei LET.La popolazione sovrappeso e obesa è aumentata nelle ultime due decadi; il 61,5% dei siciliani sono sovrappeso. Una massa corporea con eccesso di adipe sembra contribuire alla morbilità nei traumi ad alta energia. L’obesità si accompagna a comorbidità che riducono le possibilità di recupero nei pazienti sovrappeso e obesi con un trauma toracico ad alta e a bassa energia.L’obesità aumenta la morbilità indipendentemente dalla gravità delle lesioni nei pazienti con trauma toracico. All’aumentare del BMI, si allunga la durata della degenza ospedaliera e la prognosi peggiora.

Objectives. Laparoscopic cholecystectomy (LC) is today the "gold standard" treatment of gallbladder stones. Role of LC is still debated in the presence of abdominal scars due to the frequent post-operative adhesions which make access to the peritoneal cavity difficult. This study aim to assess role and outcomes of LC on a previous abdominal surgery on the scarred abdomen. Materials and Methods. we have carried out a retrospective study on 499 consecutive patients who had undergone LC from 2009 to 2015; 21 of these (4.2%) undergone previous abdominal surgery. In all 21 cases the pneumoperitoneum was established with Veress needle at the Palmer's point and the procedure was carried out after adhesiolysis in 62% of cases. Results. the mean operative time was 79±12 minutes; none of the patients with previous abdominal surgery required conversion to open cholecystectomy and there were no postoperative complications related to Veress introduction or to the adhesiolysis. The difficult dissection and adhesiolysis were more frequent in the patients with upper abdomen scar (62%) respect to lower abdomen scar (38%). Conclusion. patients with scarred abdomen for previous abdominal surgery had obviously more adhesions in the abdomen than patients without preceding surgery, but today previous abdominal surgery should not constitutes absolute contraindications to LC. Moreover, patients with previous lower abdominal incisions had fewer adhesions in the upper abdomen than did patients with upper incision and, probably, in these cases adhesiolysis is unnecessary, if the surgical field is well exposed whereas adhesiolysis is mandatory when the adhesions are thick and widespread, to the anterior and posterior abdominal wall.

Bone metastasis is a frequent and early complication of breast cancer. This case report describes a technique for a partial exeresis of the sternum and the reconstruction of the pleura with autologous dermis from the lower abdomen and the loss of substance with a myocutaneous flap.We describe the case of a 50-year old woman with a sternal excavated lesion with pathologic fracture due to an invasive adenocarcinoma, treated with a partial exeresis of the sternum and the reconstruction with a myocutaneous flap.The patient doesn't show evidence of recurrent disease and the stability of her chest well preserved.Metastatic breast cancer to the sternum, if detected early and treated aggressively, holds the possibility of such a cure.Breast cancer, Sternal metastasis, Sternectomy.Le metastasi ossee sono sono una complicanza frequente e precoce del carcinoma della mammella. Questo caso clinic descrive una tecnica per una sternectomia parziale e ricostruzione della pleura con derma autologo dai quadrati inferiori dell’addome e della perdita di sostanza con un lembo mio cutaneo.Riportiamo il caso di una donna di 50 anni con una lesione escavata dello sterno e consensuale frattura patologica da adenocarcinoma invasivo, trattato con exeresi parziale dello sterno e ricostruzione con un lembo mio cutaneo.La paziente non ha mostrato evidenza di recidiva di malattia e la stabilità della parte toracica era preservata.Le metastasi sternali da carcinoma della mammella, se diagnosticati precocemente e trattati in maniera aggressiva con intento radicale, offrono una possibilità di guarigione.

La modellizzazione spaziale di processi complessi, come quelli idrologici, è di assoluto rilievo in molte scienze ambientali e sta assumendo particolare importanza negli ultimi decenni anche alla luce di quei cambiamenti climatici che diversi studi e rapporti di organismi internazionali stanno mettendo sempre più in evidenza (IPCC, 2013). Di particolare rilievo è lo studio della variabilità delle precipitazioni che può fornire utili valutazioni relativamente all'impatto sulla disponibilità e gestione delle risorse idriche, sull'uso del suolo, sulle attività economiche e sulla società. La distribuzione spaziale delle piogge è condizionata da numerosi fattori ed è quindi necessario condurre uno studio dettagliato per migliorare l'accuratezza delle analisi (Chen et al., 2017). Negli ultimi anni sono stati proposti diversi metodi per l'interpolazione dei dati pluviometrici, e molti lavori sono stati dedicati al confronto tra i diversi modelli proposti. Infatti, l'applicazione del metodo di interpolazione appropriato può variare da regione a regione (Buttafuoco et al., 2016). Un approccio comunemente eseguito è il confronto tra tecniche deterministiche e geostatistiche. Dai risultati di questi confronti, molti autori hanno evidenziato come le tecniche geostatistiche forniscano una migliore stima delle precipitazioni rispetto alle tecniche deterministiche (ad esempio, Goovaerts, 2000). Tuttavia, altri autori hanno concluso che i risultati dipendono dalla densità di campionamento (ad esempio, Dirks et al., 1998). Di conseguenza, nella letteratura non esiste consenso unanime su quale sia il metodo migliore per interpolare le precipitazioni mensili. Per quanto riguarda la Regione Calabria, negli ultimi anni sono stati condotti diversi studi sull'interpolazione spaziale delle precipitazioni mensili (Caloiero et al., 2015) e annuali (Longobardi et al., 2016), ma senza effettuare un confronto tra le diverse metodologie. Inoltre, studi pregressi hanno rilevato una certa dipendenza della pioggia dalla distanza dal mare (Buttafuoco et al., 2011; Caloiero et al., 2015), che può essere considerata in queste analisi. In questo studio, al fine di determinare il miglior metodo per l'interpolazione delle precipitazioni mensili in Calabria, una regione che per orografia e posizione geografica presenta una marcata variabilità spaziale delle piogge, sono stati confrontati i risultati derivanti dall'applicazione di approcci geostatistici e deterministici.

45. Retropharyngeal abscess from fishbone in adult immunocompetent host presenting as acute thyroiditis

Retropharyngeal abscesses (RPA) are rare in adults. We report a case of a healthy adult with RPA presenting with clinical symptoms of acute thyroiditis. A 37-yr-old female presented with a painful mass in the anterior region of the neck, sore throat, mild fever and dysphagy for liquids and solids. Neck emergency US dis-closed normal thyroid ventrally displaced by a huge retropharyngeal hypoechoic mass. Direct laryngoscopy revealed erythema of the posterior pharyngeal wall with bulges (8 cm) preventing the correct visualization of the glottic plane. CT confirmed the pres-ence of retropharyngeal abscess and laryngoscopic drainage was performed. The pa-tient was discharged on 11th post-operative day, in good condition. The high mortality rate of retropharyngeal abscess is related to its association with invasion of contiguous structures and mediastinum: once mediastinitis occurs, mortality reaches 50%, even with antibiotic therapy. CT is fundamental for the diagnosis, but in many cases, as our, ultrasonography of the neck may play a pivotal role as quickly as possible. Surgi-cal treatment (intraoral incision and drainage) is the better definitive and resolutive treatment. The clinical diagnosis of retropharyngeal abscess in adult can be difficult because of the lacking and non specificity of clinical presentation. It is recommended to collect accurate clinical history, careful examination of the head and neck and use of early ultrasound examination of the neck, which in real time can give precise indications to guide the diagnosis and therapy. The management of a retropharyngeal abscess depends on the patient’s clinical condi-tion, moving from conservative treatment with steroids and antibiotics to reanimation and aggressive surgical drainage.

BACKGROUND: Heat shock protein 60 (Hsp60) is a chaperonin involved in tumorigenesis, but its participation in tumor development and progression is not well understood and its value as a tumor biomarker has not been fully elucidated. In the current study, the authors presented evidence supporting the theory that Hsp60 has potential as a biomarker as well as a therapeutic target in patients with large bowel cancer. METHODS: The authors studied a population of 97 subjects, including patients and controls. Immunomorphology, Western blot analysis, and quantitative real-time polymerase chain reaction were performed on tissue specimens. Exosomes were isolated from blood and characterized by electron microscopy, biochemical tests, and Western blot analysis. RESULTS: Hsp60 was found to be increased in cancerous tissue, in which it was localized in the tumor cell plasma membrane, and in the interstitium associated with cells of the immune system, in which it was associated with exosomes liberated by tumor cells and, as such, circulated in the blood. An interesting finding was that these parameters returned to normal shortly after tumor removal. CONCLUSIONS: The data from the current study suggested that Hsp60 is a good candidate for theranostics applied to patients with large bowel carcinoma and encourage similar research among patients with other tumors in which Hsp60 has been implicated.

50. Interoperable sharing and visualization of geological data and instruments: preliminary results

On a global scale, different governmental agencies implemented dedicated Spatial Data Infrastructures (SDI) to publish geological, geotechnical and geothematic maps, datasets and metadata. In most cases, the geoportals of SDI use different types of both visual and textual representation of the information and the geological data of each nation are structured in a different way. Moreover, geoportals usually miss information on geological instruments and, often, the data collected directly from these instruments. To enable an interoperable sharing of geological and geothematic maps and data, international and national geoportals mainly adopt the Open Geospatial Consortium (OGC) standards and recommended services, such as Web Map Service, Styled Layer Descriptor, Web Feature Service, Web Coverage Service and Catalog Service for the Web; this methodology however, is still lacking in the harmonization of data and their visualization. This gap, for geological and geomorphological maps and data, was removed by developing two models: GeoSciML (Sen et al., 2005) and GeoSciML- Portrayal (Richard et al., 2012) that also allow a semantic enrichment through the use of thesauri. Anyway, description and sharing of measuring instruments still missing while this capability is given by Sensor Metadata Language standard (SensorML) (Boots et al., 2007). In relation to data sharing from instruments a chance is given by Observations and Measurements standard (O&M) (Cox, 2003). Links between O&M and GeoSciML are widely recognized in relation to delivery of geological sampling and analytical data (Richard et al., 2012). Standardized common interfaces, schema, and syntax for information en-coding is a fundamental requirement for SDI. As discussed, GeoSciML and GeoSciML-Portrayal meet these requirements for Geological and Geomorphological Maps and O&M allows to add geological sampling and analytical data. Exploiting controlled vocabulary resources is a step forward in the full realization of interoperability at the semantic level. Anyway, description and sharing geological and geotechnical instruments information is still missing, though this additional information can enable quality control procedures and improve data comparison. To tackle this gap we tested, a preliminary SensorML modeling, in eXtensible Markup Language, for geological (coring tools) and geotechnical (dynamic penetrometers) instruments defining, for each instrument: identification-code, sensor type, manufacturer and output parameters. Sensor type and outputs parameters definition have been borrowed from the terms present in the "Italian Thesaurus of Earth Sciences", (ThIST), in order to harmonize and semantically enrich metadata. The test proved that SensorML has the capabilities to share semantically enabled information about these instruments in a distributed SDI and view them in a geoportal. It is worth noticing the need for the development of graphical user interfaces for editing SensorML for geological and geotechnical instruments; a solution is offered by the EDI metadata editor (http://edidemo.get-it.it); future works will prepare EDI templates for geological and geotechnical domains.
