Abstract

The study examined a cohort of women in the U.S. to describe the timing of elective repeat cesarean deliveries and assess the link between delivery before 39 weeks of gestation and the risk of adverse neonatal outcomes. Findings suggest that elective repeat cesarean delivery before term is common and is associated with adverse neonatal outcomes.

Abstract

Comparison of maternal and perinatal outcomes between women who underwent a trial of labor and women who had an elective repeated cesarean delivery without labor is presented. Although there are low absolute risks, a trial of labor after prior cesarean delivery is associated with a greater perinatal risk than is elective repeated cesarean delivery without labor.

Abstract

Weekly injections of a hormone called 17 alpha-hydroxyprogesterone caproate can lower the risk of premature birth in pregnant women with a previous premature birth. This was the conclusion of a study of 463 pregnant women. The newborn babies of women who received the injections were also less likely to have necrotizing enterocolitis, brain hemorrhage, or the need for oxygen at birth.

Abstract

Objective. This study was undertaken to determine the relationship between fetal fibronectin, short cervix, bacterial vaginosis, other traditional risk factors, and spontaneous preterm birth. Methods. From 1992 through 1994, 2929 women were screened at the gestational age of 22 to 24 weeks. Results. The odds ratios for spontaneous preterm birth were highest for fetal fibronectin, followed by a short cervix and history of preterm birth. These factors, as well as bacterial vaginosis, were more strongly associated with early than with late spontaneous preterm birth. Bacterial vaginosis was more common--and a stronger predictor of spontaneous preterm birth--in Black women, while body mass index less than 19.8 was a stronger predictor in non-Black women. This analysis suggests a pathway leading from Black race through bacterial vaginosis and fetal fibronectin to spontaneous preterm birth. Prior preterm birth is associated with spontaneous preterm birth through a short cervix. Conclusions. Fetal fibronectin and a short cervix are stronger predictors of spontaneous preterm birth than traditional risk factors. Bacterial vaginosis was found more often in Black than in non-Black women and accounted for 40% of the attributable risk for spontaneous preterm birth at less than 32 weeks. Abstract

Giving antibiotics to pregnant women who experience preterm premature rupture of the membranes (PPROM) may reduce the rate of infection and other disorders in the newborn baby. Researchers gave a five-day course of antibiotics or a placebo to 614 pregnant women who developed PPROM between 24 and 32 weeks gestation. The women who received antibiotics were less likely to deliver a baby with respiratory distress, a brain hemorrhage or severe bacterial infection compared to those who took a placebo. The incidence of fetal death was also lower in the women who took antibiotics., Context. -- Intrauterine infection is thought to be one cause of preterm premature rupture of the membranes (PPROM). Antibiotic therapy has been shown to prolong pregnancy, but the effect on infant morbidity has been inconsistent. Objective. -- To determine if antibiotic treatment during expectant management of PPROM will reduce infant morbidity. Design. -- Randomized, double-blind, placebo-controlled trial. Setting. -- University hospitals of the National Institute of Child Health and Human Development Maternal-Fetal Medicine Units Network. Patients. -- A total of 614 of 804 eligible gravidas with PPROM between 24 weeks' and 0 days' and 32 weeks' and 0 days' gestation who were considered candidates for pregnancy prolongation and had not received corticosteroids for fetal maturation or antibiotic treatment within 1 week of randomization. Interventions. -- Intravenous ampicillin (2-g dose every 6 hours) and erythromycin (250-mg dose every 6 hours) for 48 hours followed by oral amoxicillin (250-mg dose every 8 hours) and erythromycin base (333-mg dose every 8 hours) for 5 days vs a matching placebo regimen. Group B streptococcus (GBS) carriers were identified and treated. Tocolysis and corticosteroids were prohibited after randomization. Main Outcome Measures. -- The composite primary outcome included pregnancies complicated by at least one of the following: fetal or infant death, respiratory distress, severe intraventricular hemorrhage, stage 2 or 3 necrotizing enterocolitis, or sepsis within 72 hours of birth. These perinatal morbidities were also evaluated individually and pregnancy prolongation was assessed. Results. -- In the total study population, the primary outcome (44.1 % vs 52.9%; P=.04), respiratory distress (40.5% vs 48.7%; P=.04), and necrotizing enterocolitis (2.3% vs 5.8%; P=.03) were less frequent with antibiotics. in the GBS-negative cohort, the antibiotic group had less frequent primary outcome (44.5% vs 54.5%; P=.03), respiratory distress (40.8% vs 50.6%; P=.03), overall sepsis (8.4% vs 15.6%; P=.01), pneumonia (2.9% vs 7.0%; P=.04), and other morbidities. Among GBS-negative women, significant pregnancy prolongation was seen with antibiotics (P[less than]01). Conclusions. -- We recommend that women with expectantly managed PPROM remote from term receive antibiotics to reduce infant morbidity. JAMA. 1997;278:989-995

Abstract

A study is conducted to determine whether unscheduled cesarean deliveries have complication rates that vary by shift of occurrence, and morbidities are evaluated by delivery shift hypothesizing that such complications would be more frequent during the night shift (11 PM-7 AM) due to sleep deprivation of staff. Results indicate that though neonatal intensive care unit admissions increase at night, maternal and neonatal complications of cesarean delivery do not increase with delivery during the night shift.

The rate of successful trial of labor after previous cesarean delivery for dystocia is assessed in a large cohort based on the relative birth weights of the two pregnancies. It is likely that increased fetal size relative to previous birth weight plays a significant role in the outcome of a subsequent trial of labor for patients with previous cesarean delivery for dystocia, reducing chances of vaginal delivery.

Abstract

The success rates of vaginal birth after cesarean (VBAC) delivery, uterine rupture and major maternal/perinatal outcomes are compared between women, with previous cesarean deliveries undergoing trial of labor (TOL) with preterm and term pregnancies, and also maternal and neonatal morbidities in women with preterm pregnancies undergoing a TOL versus repeat cesarean delivery without labor (RCD). The likelihood of VBAC success after TOL in preterm pregnancies is comparable to term gestations, with a lower risk of uterine rupture, and perinatal outcomes are similar with preterm TOL and RCD.

A study was undertaken to determine whether women with recurrent spontaneous preterm births (rSPBs) having different clinical characteristics or systemic markers than those with isolated preterm (iSPBs) or recurrent term births (rTBs), when assessed remote from delivery. Abstract

An indicated premature delivery does not lead to better health outcomes among very-low-birth-weight babies compared to a spontaneous premature delivery. This was the conclusion of a study of 411 premature babies whose mothers had an indicated delivery, a spontaneous delivery or a premature rupture of membranes.
