Abstract

Most Rathke’s cleft cysts （RCCs） are asymptomatic. Of the symptomatic RCCs, those that rapidly develop and cause hemorrhagic RCC apoplexy are particularly rare. In this study, we report a case of nonhemorrhagic RCC apoplexy that is an acute-onset RCC without intracystic hemorrhage. This study included a 21-year-old male patient. His chief complaints were severe headache with sudden disturbance of consciousness, visual disturbance, and double vision. Head computed tomography （CT）/magnetic resonance imaging （MRI） and clinical course indicated a hemorrhagic RCC apoplexy that is an acute-onset RCC with intracystic hemorrhage, a nonhemorrhagic RCC apoplexy, or a pituitary apoplexy. We then performed endoscopic transsphenoidal surgery. Histopathological examinations revealed a nonhemorrhagic RCC apoplexy. The preoperative diagnosis makes it difficult to distinguish between acute-onset hemorrhagic RCC apoplexy, nonhemorrhagic RCC apoplexy, and pituitary apoplexy. We compared 26 cases of hemorrhagic RCC apoplexy with cases of nonhemorrhagic RCC apoplexy by reviewing previous literatures. Furthermore, we have determined the characteristics of nonhemorrhagic RCC apoplexy. Knowledge on these characteristics may be useful in the differential diagnosis. For the differential diagnosis and treatment of RCC apoplexy and pituitary apoplexy, it is important to appropriately perform surgical treatment and make an accurate diagnosis based on surgical and pathological findings.

27. The Nature and Process of Development in Averaged Visually Evoked Potentials: Discussion on Pattern Structure.

Abstract

This paper examines the development of visually evoked EEG patterns in retarded and normal subjects. The paper focuses on the averaged visually evoked potentials (AVEP) in the central and occipital regions of the brain in eyes closed and eyes open conditions. Wave pattern, amplitude, and latency are examined. The first section of the paper reviews previous research on developmental changes in AVEP patterns. The second section of the paper presents a developmental and comparative study of the AVEPs of retarded and normal children in a closed eyes, diffuse white light flash condition. Results suggest that EEG patterns from the central region of the brain develop more slowly than those of the occipital region. Occipital patterns tend to be fairly similar for both retarded and normal subjects. Central recordings, however, indicate striking differences in the long latency negative and positive waves of the retarded subjects. The third section of the paper examines the AVEP patterns of retarded and normal subjects during two eyes open conditions: light flash (Rest) and a choice reaction task (CRT). Results from this study suggest similar but developmentally retarded AVEP patterns for the retarded in comparison to the normal subjects in the occipital region for both Rest and CRT. Retarded-normal differences are found for negative wave patterns in the central brain region by comparing CRT to the Rest condition. Results are discussed in terms of arousal and attention deviations which may be indicated in the AVEP patterns of the retarded subjects. (BD)

Abstract

Evaluated were the properties and fine structures of averaged visually evoked potentials (AVEP) in 60 normal children between the ages of 2 and 9 years. Electroencephalographic recordings were taken while white diffuse flashes were used to deliver visual stimuli to the Ss. Three types of AVEP patterns were discerned, with no relationship observed between AVEP types and age of Ss; however, AVEP latencies of the third type appeared to be closely related to age. Results implied that AVEP latency is related to age in early developmental stages but becomes unrelated to age at later stages. (LH)

35. Factors Related to the Delayed Cure of Hemifacial Spasm after Microvascular Decompression: An Analysis of 175 Consecutive Patients.

Abstract

Detailed studies assessing the factors related to delayed cure of hemifacial spasm (HFS) after microvascular decompression (MVD) are sparse. We aimed to evaluate the effect of 11 clinical factors on the time until the patient became spasm free after MVD. We enrolled 175 consecutive patients with HFS who underwent MVD between 2012 and 2018. The end point was defined as the time point at which the patient became spasm free based on the outpatient interview. Patients were divided into six groups depending on when they became spasm free after the operation, as follows: <7 days ( n  = 62), 7 days to 1 month ( n  = 28), 1 to 3 months ( n  = 38), 3 to 6 months ( n  = 25), 6 to 12 months ( n  = 17), and >12 months ( n  = 5). The median time to become spasm free after MVD was 30.0 days. Association of 11 factors (age, sex, laterality, number of offending arteries, vertebral artery compression, number of compression sites, compression at root detachment zone, preoperative Botox treatment, indentation of the brain stem on preoperative magnetic resonance image, transposition, and interposition) with spasm-free rate was assessed using the Cox's proportional hazards model. Spasm-free rate curve after MVD for the significant factor was obtained using the Kaplan–Meier method. In univariate and multivariate analyses, nontransposition was significantly related to delayed HFS cure after MVD (hazard ratio [HR], 0.60; 95% confidence interval [CI], 0.42, 0.87; p  = 0.0068 and HR, 0.60; CI, 0.43, 0.85; p  = 0.042, respectively). The spasm-free rate was higher in the transposition than in the nontransposition group ( p  = 0.0013). As shortening the time until spasm free after MVD improves patients' quality of life, transposition should be recommended. Prediction of spasm-free time could relieve the anxiety of postoperative patients. [ABSTRACT FROM AUTHOR]
