Abstract

The aim of this study was to clarify the mechanism of impaired exercise tolerance in chronic sleep-restricted conditions by investigating variables related to heart-rate (HR) response to sympathetic nervous stimulation. Sixteen healthy men (mean age 21.5 years) were tested in a control state, acute sleep-loss state, and chronic sleeprestricted state. Participants underwent cardiopulmonary exercise testing in each state. Their norepinephrine (NE) concentration was measured before and immediately after exercise. Intracellular magnesium (Mg) concentration was measured in a resting state. Exercise duration was shorter and the ratio of HR response to the percentage increase in NE was higher in the chronic sleep-restricted state than in the control state. Intracellular Mg gradually decreased from control to chronic sleep restriction. There was a negative correlation between peak exercise duration and the ratios of HR response to the rate of increase in NE. Intracellular Mg was positively correlated with the ratios of HR response to the increase in NE both in control and in acute sleep loss. The authors conclude that the impaired exercise tolerance in a chronic sleep-restricted state is caused by hypersensitivity of the HR response to sympathetic nervous stimulation, which showed a compensation for decreased intracellular Mg concentration. Abstract

プラズマホールと呼ばれる密度ホールを伴った渦構造の自発的形成がECRプラズマ中で観測されている。プラズマホールにおける流速場の形成メカニズムについて、force balanceの観点から考察を行った。プラズマホールの流速場は、中心部では遠心力とローレンツ力が、周辺部では電場とローレンツ力がつり合うことによって形成されている。別の言い方をすれば、中心部では遠心力が優勢なE×Bドリフト異常解が、周辺部では電場が本質的な役割を担うE×Bドリフト解が実現されている。渦構造の成り立ちという意味では、プラズマホールは台風と類似の構造であり、回転流体が示す普遍的な特徴を有している。また、渦度分布からプラズマホールの粘性を評価したプラズマホールは古典的な粘性係数より3桁大きな異常粘性を示し、その異常粘性によって特徴的な径方向流れが駆動されていることが明らかになった。, Spontaneous formation of a stationary vortex structure with density hole around the central axis, plasma hole, has been observed in a cylindrical ECR plasma. The driving mechanism of characteristic ion flow in plasma hole is examined from the viewpoint of force balance in the fluid equation. On the one hand, the balance between electric force and Lorentz force determines the flow velocity field in peripheral region, which gives rise to E x B rotation of the peripheral plasma. On the other hand, it is revealed that the balance between centrifugal force and Lorentz force governs the flow velocity field in central region, i.e., anomalous E x B solution is realized in central region. A remarkable similarity between the flow field of typhoon and the plasma hole is pointed out. The effective viscosity of the plasma hole is evaluated by its vorticity distribution. It is revealed that the plasma hole exhibits anomalous viscosity, the value of which is three orders of magnitude higher than the classical one. Urinary liver-type fatty acid binding protein (L-FABP) increases along with tubular hypoxia and may be a detector of systemic circulation injury. The aim of this study was to evaluate the clinical usefulness of detecting increases in urinary L-FABP levels due to administration of CM, as a prognostic biomarker for cardiovascular disease in patients without occurrence of CM-induced nephropathy undergoing cardiac catheterization procedure (CCP). Methods: Retrospective longitudinal analyses of the relationship between urinary L-FABP levels and occurrence of cardiovascular events were performed (n=29). Urinary L-FABP was measured by ELISA before CCP, and at 6, 12, 24, and 48 hours after CCP. Results: Urinary L-FABP levels were significantly higher at 12 hours (P<0.05) and 24 hours (P<0.005) after CCP compared with before CCP, only in the patients with occurrence of cardiovascular events (n=17), but not in those without cardiovascular events (n=12). The parameter with the largest area under the curve (0.816) for predicting the occurrence of cardiovascular events was the change in urinary L-FABP at 24 hours after CCP. The difference in urinary L-FABP levels (ΔL-FABP ≥11.0 µg/g creatinine) between before CCP and at 24 hours after CCP was a risk factor for the occurrence of cardiovascular events (hazard ratio, 4.93; 95% confidence interval, 1.27–19.13; P=0.021).
