Kerion Celsi is an inflammatory, deep fungal infection of the scalp. It is rare in neonates but gets more common in children about 3 years and older. It represents with swelling, boggy lesions, pain, alopecia and purulent secretions. Secondary bacterial infection is not unusual after maceration. Extracutaneous manifestations include regional lymphadenopathy, fever and very rare fungemia. Id-reactions can occur. Diagnosis is based on clinical suspicion, clinical examination and medical history. Diagnosis should be confirmed by microscopy, fungal culture and molecular procedures. The most common isolated fungal species are anthropophilic Trichophyton (T.) tonsurans and zoophilic Microsporum (M.) canis, while geophilic species and moulds rarely cause Kerion Celsi. Treatment is medical with systemic and topical antifungals supplemented by systemic antibiotics when necessary, while surgery needs to be avoided. Early and sufficient treatment prevents scarring alopecia. The most important differential diagnosis is bacterial skin and soft tissue infections. [ABSTRACT FROM AUTHOR]

Adverse events of OTC products are not uncommon. We observed a peculiar adverse effect in three adolescent females using an acne lotion, who developed a yellowish discoloration of their hands, but not on the facial sites of application. Inspection under Wood's light helps to identify affected areas. We suppose a possible involvement of methenamine. Facial psoriasis is a clinical form of psoriasis that has raised many debates, due to its relation with UV radiation and classification. In differential diagnosis, polymorphous light eruption is apparently easy to identify and is considered the most frequently reported photosensitivity dermatoses, especially in adolescents. Clinical picture may create confusion and, even, the term polymorphous is ambiguous. We present a case of a teenager who has been diagnosed and treated for three months, for many suspicions, such as: polymorphic light eruption, lupus erythematosus, contact dermatitis, skin lesions being limited to the nose. Laborious and extended lab investigations were within normal limits and only skin biopsy concluded a surprisingly diagnosis, namely psoriasis. Morphea is an inflammatory skin disease with self-limited evolution, presenting as localized sclerosis of the skin and/or underlying tissues. The incidence is not exactly known; the disease occurs more frequently in women, and there is no sex prevalence. Pathogenesis of morphea remains still controversial. Several theories exist and the Borrelia burgdorferi infection is not yet elucidated. The aim of this report is to present the main mechanisms involved in the etiophatogenesis of morphea and also the thepapeutic options. A case of a 60-year-old woman is presented, who was referred to the clinic for an erythematous-violaceus, asymptomatic eruption, located on the trunk and legs, for appoximately 2 months. The patient's medical history revealed an infection with Borrelia 1 year previously. After diagnosis of morphea was established, and with systemic therapy (corticosteroids and methotrexate), the evolution was favorable. [ABSTRACT FROM AUTHOR]

The clinical description of the nipple lesion in Paget disease of the breast is of eczematous rash that may be of interest to both the nipple and areola of the same breast, accompanied, most often, by an abundant, clear, slightly yellowish exudate. The first data on Paget's disease were mentioned in 1874, by Sir James Paget, when he pointed out the malignant transformation in the case of ten women with ulceration on eczematous background of the nipple. Paget's disease is rare, accounting for 1-3% of all breast cancer diagnoses. This condition can be encountered in the population between the ages of 26 and 88, but the peak of the incidence is represented by decades 5 and 6 of life. The histopathological examination describes the adenocarcinomatous malignant intraepithelial cells, called Paget cells. The treatment as well as the prognosis in the case of Paget's disease is closely related to the existence of a palpable tumor formation and the presence of mammographic signs of breast malignancy. We present the case of a 59-year-old woman, who presents for a symptomatic, ulcerated tumor formation, with a diameter of about 6 cm, and a 6-month evolution at the level of the upper pole of the areolar complex of the left breast, after previously the nipple at the level of the involved breast underwent changes with erythematous transformation and ulceration. The article "Occupational Contact Dermatitis due to Chloramine in a Nurse" presents a case report of a 44-year-old nurse suffering from contact dermatitis due to chloramine. The patient worked in the COVID-19 intensive care unit and had direct contact with chloramine for disinfection purposes. After other treatments failed to improve the condition, a patch test was conducted, which showed a positive reaction to chloramine. The increased exposure to wet work and disinfectants during the COVID-19 pandemic appears to be responsible for the contact sensitization. [Extracted from the article]

Granuloma annulare (GA) is a benign skin condition of unknown etiology. It can manifest itself in several clinical forms: localized, generalized, subcutaneous or perforating. The association between GI and diabetes, dyslipidemia or neoplasms is not yet elucidated. Although the diagnosis is clinical in most cases, a histopathological examination may be necessary. We presented the case of a 52 years old patient, diabetic, hypertensive, who reffered to our clinic, with a rash or macular-papular, pruritic, eruption, localization on the lower limbs and abdomen, in the evolution of 6 months. We performed skin biopsy with histopathological examination, which revealed: dermal nodular infiltrates consisting of palisadiate histiocytes, in several mucin collections and signified by lymphocytes with perivascular arrangement, typical modifications for an annular granuloma lesion. We have initiated systemic treatment with isotretinoin, associated with dermatocorticoids, with favorable evolution. Diagnosis, aș well aș the management of a patient with generalized GI, can sometimes be difficult. The histopathological examination, in combination with a suggestive clinical picture, makes the diagnosis. Treatment is indicated in severe cases, with multiple, symptomatic, persistent lesions.
