Our unique rapid response system (RRS) includes a systematically organized structure by rapid response teams (RRT) and a nonorganized, nonsystematic structure for gathering hospital staffs in the scene. We examined the records of 54 patients described in this RRS. Our RRS is as follows; a medical/non-medical hospital staff who finds a victim, asks other staffs to activate the RRS by using inhospital whole paging system; predefined staffs bring predefined equipments; a standard educational course and e-learning system and a simple recording form. The mean interval between the event and the arrival of staff at the scene was 1.81 minutes. Hospital death rate per total discharged patients tended to decrease after RRS. Our unique RRS work well. The death rate among discharged patients decreased after this system was implemented.

Background: Blood transfusion therapy (BTT), which represents transplantation of living cells, poses several risks. Although BTT is necessary for trauma victims with hemorrhagic shock, it may be futile for patients with blunt traumatic cardiopulmonary arrest (BT-CPA). Materials and Methods: We retrospectively examined the medical records of consecutive patients with T-CPA. The study period was divided into two periods: The first from 1995-1998, when we used packed red cells (PRC) regardless of the return of spontaneous circulation (ROSC), and the second from 1999-2004, when we did not use PRC before ROSC. The rates of ROSC, admission to the ICU, and survival-to-discharge were compared between these two periods. Results: We studied the records of 464 patients with BT-CPA (175 in the first period and 289 in the second period). Although the rates of ROSC and admission to the ICU were statistically higher in the first period, there was no statistical difference in the rate of survival-to-discharge between these two periods. In the first period, the rate of ROSC was statistically higher in the non-BTT group than the BTT group. However, for cases in which ROSC was performed and was successful, there were no statistical differences in the rate of admission and survival-to-discharge between the first and second group, and between the BTT and non-BTT group. Conclusion: Our retrospective consecutive study shows the possibility that BTT before ROSC for BT-CPA and a treatment strategy that includes this treatment improves the success rate of ROSC, but not the survival rate. BTT is thought to be futile as a treatment for BT-CPA before ROSC.

Background: Insufficient knowledge of the risks and complications of cardiopulmonary resuscitation (CPR) may be an obstructive factor for CPR, however, particularly for patients who are not clearly suffering out of hospital cardiopulmonary arrest (OH-CPA). The object of this study was to clarify the potential complication, the safety of bystander CPR in such cases. Materials and Methods: This study was a population-based observational case series. To be enrolled, patients had to have undergone CPR with chest compressions performed by lay persons, had to be confirmed not to have suffered OHCPA. Complications of bystander CPR were identified from the patients’ medical records and included rib fracture, lung injury, abdominal organ injury, and chest and/or abdominal pain requiring analgesics. In our emergency department, one doctor gathered information while others performed X-ray and blood examinations, electrocardiograms, and chest and abdominal ultrasonography. Results: A total of 26 cases were the subjects. The mean duration of bystander CPR was 6.5 minutes (ranging from 1 to 26). Nine patients died of a causative pathological condition and pneumonia, and the remaining 17 survived to discharge. Three patients suffered from complications (tracheal bleeding, minor gastric mucosal laceration, and chest pain), all of which were minimal and easily treated. No case required special examination or treatment for the complication itself. Conclusion: The risk and frequency of complications due to bystander CPR is thought to be very low. It is reasonable to perform immediate CPR for unconscious victims with inadequate respiration, and to help bystanders perform CPR using the T-CPR system.

High-echoic objects in the hepatic vessels of patients with cardiopulmonary arrest (CPA) are frequently detected by ultrasonography.To demonstrate this phenomenon and clarify its clinical characteristics.In a tertiary care academic medical centre, 203 CPA patients were evaluated by ultrasonography. CT determined the origin and location of high-echoic objects detected in the liver. The frequency and characteristics of this phenomenon were investigated. The background, laboratory data and survival rate were compared between patients with and without high-echoic objects.High-echoic objects were seen in 73 (36.0%) patients and could clearly be detected in the hepatic veins of 41 (56.2%) patients. CT confirmed that these were gas in 27 of 53 patients, and were clearly visible in the hepatic veins in 12 (44.4%) patients. Hepatic portal venous gas was not identified. Compared to patients without high-echoic objects, witnessed arrest (p0.001), bystander cardiopulmonary resuscitation (p=0.005), ventricular fibrillation or pulseless electrical activity (p=0.012) and return of spontaneous circulation (p=0.018) were significantly less frequent in patients with high-echoic objects. These patients had a lower incidence of survival to discharge (1.4% vs 7.7%, p=0.100). Multivariate analysis showed that absence of high-echoic objects was a marginally significant factor in association with return of spontaneous circulation (p=0.052).High-echoic objects were often observed on ultrasonography in CPA patients; these objects were considered hepatic venous gas. The presence of high-echoic objects may be a poor prognostic sign in patients with CPA.

はじめに : 胃切除施行後患者における骨代謝障害は，以前より問題視されているが，明確な予防・治療指針が存在せず見逃されている症例も多い．近年骨代謝障害を改善するビスフォスフォネート製剤が登場したため，胃切除後骨代謝障害に対するアレンドロネートの効果を検討した．対象と方法 : 胃切除術施行後の男女で，腰椎骨密度が若年成人平均値の80%未満である骨量減少患者80名を対象とし，活性型ビタミンD3投与群（VD群），アレンドロネート投与群（ALN群），活性型ビタミンD3＋アレンドロネート併用投与群（併用群）に割付け，腰椎骨密度などの有効性，有害事象，骨折などの安全性を2年間観察した．結果 : 腰椎骨密度の増加率は24か月でVD群3.5%，ALN群9.3%，併用群7.7%で，ALN群，併用群で有意に増加した（P

BACKGROUND: Physical activity recommendations for older adults with poor health needs to be understood. PURPOSE: This study aims to examine the association between the frequency of physical activity and mortality among a sample of elderly subjects, most of whom were under treatment for pre-existing disease. METHODS: Data on the frequency of leisure-time physical activity, walking for transportation, and non-exercise physical activity were obtained from a population-based cohort study in Shizuoka, Japan. Of the randomly selected 22,200 residents aged 65-84 years, 10,385 subjects were followed from 1999 to 2006 and analyzed. Hazard ratios (HRs) and 95% CIs were obtained for all-cause; cardiovascular disease (CVD); and cancer mortality, after adjusting for covariates such as pre-existing disease(s). A subgroup analysis that was restricted to subjects under treatment for pre-existing disease(s) at baseline was further conducted. Data were collected between 1999 and 2006, and all analyses were conducted in 2008 and 2009. RESULTS: Every physical activity was associated with a reduced risk of all-cause and CVD mortality, among not only the total sample but even those under treatment. The HRs for CVD mortality among participants with 5 or more days of non-exercise physical activity per week for the total sample and those with pre-existing disease(s) were 0.38 (95% CI=0.22, 0.55) and 0.35 (95% CI=0.24, 0.52), respectively, compared with no non-exercise physical activity. The association between physical activity and cancer mortality was not clear. CONCLUSIONS: This study suggests a protective effect of physical activity on all-cause and CVD mortality among Japanese elderly people with pre-existing disease. Language: en

背景:致死的急速大量出血への未交差赤血球製剤（UCM-RCC）緊急輸血では不適合輸血が起きやすい．対象と方法:15例の消化管出血にUCM-RCC輸血（17回，80単位）を経験したので経過を検討した．UCM-RCC輸血の適応は，2,000mlの急速輸液でも循環不安定で交差適合試験（CMT）が待てない出血性ショック．結果:UCM-RCC輸血は1例に2～10単位，平均5.3単位実施，93％で6単位以下．血液型判定（BTT）未判定例へのO型UCM-RCC輸血は7例，1回のBTT後例へのO型UCM-RCC輸血は4例，1回のBTT後非O型症例への未確認同型UCM-RCC輸血は4例．輸血前の収縮期血圧，脈拍数，ショック指数（脈拍数/収縮期血圧），base excessの最低値は平均各，76.9mmHg，117.5回/分，1.69，-9.61mEq/l．意識障害は8例に認められた．死亡は2例（13％），ABO型不適合輸血，輸血関連急性期合併症はなかった．結論:消化管出血へのUCM-RCC輸血ではO型UCM-RCC輸血の迅速な決断が重要だが，初回請求量は6単位が妥当と思われた．

心肺停止蘇生後に姑息的胃切除を行い神経学的異常を残さず救命しえた胃癌穿孔を経験した．81歳の男性で，食欲低下と体重減少，右側腹部痛，嘔気から経口摂取不能，意識障害となり緊急搬送された．病院到着12分後に心肺停止となり6分後に蘇生した．造影CTなどで腹腔内遊離ガス，胃幽門付近の壁不整像を認め消化管穿孔，腹膜炎，腹部コンパートメント症候群によるショックと判断した．初期輸液などの敗血症性ショックに対する治療と平行して，開腹減圧と感染源制御を目的に緊急手術とした．胃体下部前壁に胃癌漿膜浸潤と穿孔，腹膜全域に播種性小病変を認め，姑息的に幽門側胃切除，Roux-en-Y吻合を行った．腫瘍は，LCirc，type 3，pT3（SE），sN3，sH0，sP1，fStage IV，ly3，v2，INFβ，tub2＞por2であった．術後は比較的容易に敗血症性ショックから離脱でき，神経学的異常の遺残はなかった．術後8か月の造影CTで腹腔内リンパ節，肝臓，肺転移を認め，術後14か月（第407病日）で死亡した．

はじめに：肝，脾，腸間膜損傷で出血性ショック合併の腹部外傷では，蘇生的緊急開腹術など外科手技の適応と緊急輸血の適応判断を迫られる．異型輸血を含む危険を内在する未交差赤血球製剤（uncrossmatched-red cell concentrate；以下，UCM RCC）緊急輸血の安全性と問題点を考案する．方法：過去5年間で，O型Rh＋UCM RCCを含むUCM RCC緊急輸血を実施した腹部外傷を対象にUCM RCC輸血の実態と輸血関連インシデント・アクシデント回避の成否を検討した．結果：UCM RCC輸血は33例に54回，388単位実施され，準備UCM RCCは426単位で使用率は91.1％であった．出血源は肝臓13例，脾臓8例，腸間膜17例で，輸血前平均収縮期血圧は62.5 mmHg，脈拍数117回/分，ショック指数（脈拍数/収縮期血圧）1.97，base excessは−12.7 mEq/l，7日生存例は48.5％であった．血液型判定（blood typing test；以下，BTT）未判定でのO型UCM RCC輸血は17例（21回，168単位），うち非O型への輸血は11例（14回，120単位）であった．1回のBTT後のO型UCM RCC輸血は8例（12回，96単位），うち非O型への輸血は4例（7回，54単位）で，1回のBTTでの非O型症例への未確認同型UCM RCC輸血は15例（20回，120単位）であった．O型UCM RCC輸血全体に年度別増減はなかったが，1回のBTTでの非O型への未確認同型UCM RCC輸血は減少した．ABO型不適合輸血，輸血に関わるインシデントはなかった．考察：UCM RCC輸血では，院内マニュアルを整備したうえでのO型UCM RCCの使用は安全と考えられた．

鍼治療後の両側気胸症例を経験した。58歳の女性，体調不良に対し頸部から腰部にかけて約20箇所の鍼治療直後に胸部違和感，呼吸困難感自覚し，救急搬送された。意識清明，血圧200／110mmHg，脈拍数151回／分，呼吸数36回／分，苦悶様顔貌，発汗著明であったが，チアノーゼや気道狭窄音，呼吸音の左右差はなく，心臓超音波，心電図で異常なく，血液検査でも白血球上昇以外異常はなく，動脈血ガス分析（酸素10l／分）はpH7.215，Pao2118.7mmHg，Pco263.9mmHgであった。前医鍼灸院から情報を得て，胸部単純X線検査で両側気胸と診断，両側胸腔ドレナージを施行した。血液ガス分析はpH7.326，Pao2181.6mmHg，Pco242.8mmHgと改善，症状も消失し，第13病日退院となった。考察・結論：気胸など鍼治療の合併症が生じると鍼治療担当者とは別の医師が治療を行うことになるが，鍼治療合併症に対する対応体制は未発達である。今後は，鍼治療時のインフォームドコンセントの充実と合併症時の鍼灸治療者と救急医療機関との連携あるシステム構築が必要と思われた。

症例は56歳, 男性. 前医の単純X線とCT検査で腹腔内遊離ガス像を認め, 発症後2日経過した消化管穿孔腹膜炎の診断で転送されてきた. BUN, クレアチニンの上昇があったが腎機能障害の合併と考えていた. 手術時に腹腔内膀胱遊離破裂と判明した. 術後情報を再確認し, 前医で単純X線撮影前に膀胱留置カテーテルから空気を注入したこと, 患者は発症当日大量飲酒後に下腹部を打撲していたことが判明し, 膀胱充満状態での下腹部強打による膀胱腹腔内遊離破裂, 偽腎不全, 医原性腹腔内遊離ガスという典型的病態であったことが判明した. 適切な手術の準備などのためにも, 病院間高次転送時には, 正確な情報伝達と情報確認に努め, 情報に対する先入観や思いこみを排除し, 前医からの情報のみに基づく緊急手術の適応だけの判断で手術を行うのではなく, 症例の緊急性に応じて自施設でも責任を持って術前診断を確実に行うことが重要と思われた.

本研究の目的は仙骨骨折の骨折型・転位の程度と神経損傷の相関について調査し,治療法について言及することである。2002年10月～2005年2月に当センターを受診した他の脊椎・頭部外傷を合併しない仙骨骨折症例のうち,受診時に骨盤CTを撮影した12例15側(男6例,女6例)を対象とした。右側1例,左側8例,両側3例であった。受傷時平均年齢は39.9±17.4歳(17-67)であった。仙骨骨折の評価方法は神経損傷の有無により分け,全例をAO分類,Denis分類の骨折型に分類し,単純CTを用いた骨折転位の計測を行った。CTは損傷されやすい第1仙骨神経孔を横断するスライスを評価対象とした。神経損傷を認めた5例7側の骨折型は,骨盤骨折AO分類ではType B (7例)の1例,Type C (5例)の4例であった。仙骨骨折Denis分類ではzone 2 (12側)の6側,zone 3 (1側)の1側であった。仙骨骨折転位と神経損傷の相関については,転位は大きなものほど神経損傷を認めた。仙腸関節前縁の転位を横方向と前方向に分けると,横方向(圧迫もしくは開大)・前方向いずれでも3mm以上の転位があれば有意に神経損傷の発症が多かった。救急医療現場で意識障害患者に対して,徒手筋力テストなど理学所見や膀胱直腸障害の評価は困難である。そのような場合に本法は神経損傷を予測する因子や仙骨骨折整復の目安としては十分に有用である。

54歳,男性の胸腹移行部刺創による腹腔内多臓器損傷(右横隔膜,小腸,肝,胆嚢,右腎)症例で,術中に腎周囲・後腹膜血腫の急速な増大を認め, lethal triadに向かいつつある状態と判断し, damage control (DC)に治療戦略を修正し,肝・胆嚢縫合止血,肝・腎周囲ガーゼ圧迫留置,引き続き腎動脈の経カテーテル的動脈塞栓術(TAE)を行い,一時止血に成功し,第2病日に予定再手術でガーゼ除去,胆嚢摘出術を施行し救命しえた症例を経験した. DCは通常の治療戦略とは概念を異にする一時回避的戦略であるが,大量出血やseptic shockで致命的代謝失調状態にある症例に対しては,救命のための有用な手段である.重症緊急症例では治療戦略に関して事前検討もできず,その場に居合わせた者だけでの即座の判断が要求されるが,本法の適応判断や実践は,緊急事態への対応能力の一つとして重要と思われた.

Background Although nifekalant is a class III antiarrhythmic agent without negative inotropic activity, its effect in patients with shock-refractory ventricular fibrillation remains unclear. Methods and Results Patients who had an out-of-hospital cardiac arrest with ventricular fibrillation that persisted after 3 shocks from an external defibrillator, intravenous epinephrine, and another shock were retrospectively studied. The patients received lidocaine from January 1997 through June 2001 and nifekalant from July 2001 through December 2004. Short-term survival rates (survival to hospital admission and 24-h survival) were compared between the groups. The study group comprised 120 patients (mean age: 62±16 years): 55 received nifekalant and 65 received lidocaine. Age, sex, history of ischemic heart disease, whether arrest was witnessed or not and time to arrival at the hospital did not differ significantly between the groups. OBJECTIVES: Volunteer citizens were recruited to perform simulated emergency calls, and the expressions and content of these telephone calls were analysed to examine risk factors associated with the success or failure of communication. SUBJECTS AND METHODS: Six physicians played the role of patients who had various symptoms, such as cerebral stroke and ischaemic heart disease. Eighty-four volunteer citizens made simulated emergency calls. Physicians at a simulated call centre communicated with each caller regarding the patient's body position, respiratory condition, and cardiovascular status. Details of the telephone communications were analysed to determine if communication was successful. RESULTS: Telephone communications that resulted in the correct understanding of a simulated patient's condition were as follows: 60.2% of sessions (32/50) on whether or not a patient was breathing; 47.8% of sessions (22/46) on whether or not a patient had a pulse (carotid or radial artery); and 86.2% of sessions (56/65) on patient body position. How a simulated dispatcher verbally expressed questions was the most influential factor in the success of communication regarding respiratory condition and body position. Avoiding vague language, giving specific instructions for checking a patient, and finally reminding the caller to perform the explained procedures led to a high rate of successful communications. Various spoken expressions by simulated dispatchers in confirming patient pulse did not have any impact on the success or failure of communications. CONCLUSION: In developing a 'protocol for emergency call triage' to achieve a high rate of successful emergency communications, an analysis of expressions using simulated patients is useful.

大量の肝内門脈ガス像を呈した広範小腸壊死1例を報告する.82歳,女性.既往歴は,高血圧,胆石,逆流性食道炎と20年前に帝王切開。平成15年12月5:30,全身倦怠感,水様性下痢,嘔吐を主訴に他院受診.腹部造影CTで肝左葉の門脈内に樹枝状に広がる空気像,腹部大動脈周囲後腹膜に空気像を認め,当院へ転送された.意識レベルはJCS-1,収縮期血圧42mmHg,脈拍数146,呼吸数32,直腸温36.9℃,腹部は板状硬で圧痛を認めた.白血球数は9,270/mm^3,CRPは16.4mg/dlと上昇しており,血小板は4.1万/mm^3,PTは45%などDIC状態だった.アルブミン1.5g/dlと低栄養,BE-10.8mEq/lとアシドーシスを認めた.造影CTで上腸間膜動脈内の血栓は明らかではなかった.開腹すると,腐敗臭を伴う腹水を認め,E.coliが同定された.小腸は,Treitz靱帯より約1mから境界明瞭に壊死状態,肛門側は横行結腸中央部付近まで全層性壊死,下行結腸とS状結腸は虚血状態であった.穿孔は認められなかった.全身状態が劣悪であったため,小腸広範囲切除,結腸亜全摘,上位空腸痩造設,S状結腸断端の腹壁開腹創への固定しか施行しなかった.切除標本の病理では腸管の末梢動脈攣縮像が認められた.術後,サイトカイン吸着術,大量輸血,CHDFを含む集中治療を行ったが翌日多臓器不全で死亡した., This is a case report of grossly necrotic bowel with hepatic-portal venous gas. An 82-year-old woman with a history of hypertension, gallstone, reflux esophagitis and caesarian section, was brought to a hospital because of general fatigue, watery diarrhea, and vomiting. She was transferred to our center because abdominal contrast enhanced computed tomography (CE-CT) scan showed arborescent dialation of the hepatic-portal venous gas and air around the abdominal aorta. Her conscious level was JCS-1, systolic blood pressure 42mmHg, pulse rate 146, respiration rate 32, and her body temperature 39.6℃. She had abdominal tenderness and muscle guarding. CE-CT did not reveal any thrombus in the superior mesenteric artery. An emergency operation was performed and we found transmural bowel necrosis from 1 m distal to the Treitz ligament to the middle transverse colon and ischemic and cyanotic change of the descending and sigmoid colon. There was no perforation. We resected the necrotic bowel. Pathological examination showed small arterial spasms in the involved bowels. Although intensive care, including cytokine absorption therapy, blood transfusion, and continuous hemofiltration dialysis (CHDF) was given, she died the following day of multiple organ dysfunction syndrome (MODS).

Medical e-Leamingシステム(YUMESUMA e-net:Yokohama City University,MedicalCenter,Medical e-Learning System founded by Sugiyama and Matsuse,e-net)を導入する機会を得たので,オフライン環境下での試行から本番系システムによる実施にいたる概要を報告し,Medical e-Leamingの意義,問題点,今後の方向性などを検討した.その結果,いわゆるe-LeamingシステムどしてのYUMESUMAは,単に学習のIT化という位置づけにおいてのみ有効性が高いのではなく,情報を迅速に伝える手段として,高い確実性を持ちうることが実証された.従って,毎年,多数の専門職が新規に採用され人事異動の頻繁な大学附属病院において,Medical e-Leamingは,情報の周知,安全管埋徹底に極めて有用なシステムと思われた., A medical e-learning system (YUMESUMA e-net) was introduced at our hospital. Here, the processes from its trial in an off-line environment to its application to a real working system are outlined, and the significance, problems, and direction of future development of medical e-learning are evaluated. YUMESUMA as an e-learning system was found to be effective not only as an information technology to assist medical learning but also as a highly reliable tool for rapid transmission of information. Therefore, at a university hospital, where many experts are newly employed every year and personnel changes are frequent, medical e-learning is considered to be a useful system for transmission of information and safety management.

Background: Acute compartment syndrome of the thigh is a rare condition, and the basic causes of high pressure within a muscle compartment have been considered to be intramuscular haematoma and soft-tissue oedema. However, the importance of arterial injury has not been well recognized. Methods: Among 3658 blunt trauma patients admitted to our Level 1 Trauma Centre between 1994 and 2001, there were eight patients (nine thighs) who had undergone emergency fasciotomy and these were the subjects of the present study. Arteriography of the proximal lower limb had been performed after the fasciotomy in patients with prolonged hypotension and persistent bleeding from the fasciotomy wound. Results: All the patients had sustained high-energy trauma, systemic hypotension and local trauma to the proximal lower limb. Among them, four (five thighs) had undergone arteriography and four (four thighs) were confirmed as having sustained arterial injuries. In those patients with definitive arterial injuries, the time from injury to the onset of the compartment syndrome was less than 5 h. Conclusions: Acute compartment syndrome of the thigh in blunt trauma patients may be the result of associated arterial injuries. It is suggested that patients with local trauma to the proximal lower limb who exhibit an acute compartment syndrome together with haemodynamic instability should undergo arteriography soon after fasciotomy.

The clinical significance of the white blood cell (WBC) count on admission in relation to the duration of ischemia in acute myocardial infarction (AMI) remains unclear.The relationship of the WBC count on admission to myocardial reperfusion was examined in 135 patients with recanalization of an anterior AMI within 6 h of symptom onset. Patients were classified according to the WBC count on admission: Group L (n=75), WBC count12,000 cells/mm(3) and group H (n=60), WBC countor=12,000 cells/mm(3). Peak creatine kinase (CK) was higher and impaired myocardial reperfusion, defined as a myocardial blush grade of 0/1, was more frequent in group H than in group L. Among the patients in group H, those with early (or=3 h) recanalization had a lower QRS score before recanalization than those with late (3 h) recanalization; however, peak CK and the incidence of impaired myocardial reperfusion were similar in these subgroups of patients. Multivariate analysis showed that WBC countor=12,000 cells/mm(3) on admission was an independent predictor of impaired myocardial reperfusion in patients with early recanalization (odds ratio 7.9, p=0.04), but not in those with late recanalization.A higher WBC count may be associated with progression of myocardial damage after recanalization in patients with early recanalization of an anterior AMI.

鈍的気管損傷の診断において,CT上での気管損傷を示唆する所見を明らかにするため,過去の気管損傷症例のCT所見を検討した。過去4年間の頸胸部鈍的外傷症例で,1)頸部皮下気腫,2)胸部単純X線で縦隔気腫か深頸部気腫,3)頸部側面単純X線で深頸部(咽頭後)気腫のいずれかが認められ,4)頸胸部造影CTを施行し,かつ気管損傷診断法のgold standardとされている気管支鏡か解剖で気管損傷の有無を確定した症例を対象とした。気管支鏡か解剖で気管損傷を確認した症例群と,気管損傷を認めなかった症例群とでCT画像所見(気管に接した気腫像,気管輪郭の不整像と不連続像,気管周囲の異常低吸収域像の有無とこれらの所見の位置関係)を比較検討した。対象症例16例中5例が気管損傷と判定された。気管損傷例全例に気管輪郭の不整像か不連続像,同部に一致した気管周囲の異常低吸収域像とこれらの像から連続する顕著な気腫像のすべてを認めた。気管損傷なしと診断した11例では,気管に接した気腫像が8例(73%),気管輪郭の不整像と不連続像が5例(45%),気管周囲の異常低吸収域像が2例(18%)にみられたが,気管周囲の低吸収域像は,気管の不整像や不連続像と一致しなかった。気管損傷を示唆するCT所見のうち,気管輪郭の構造破綻を表す気管輪郭の不整像や不連続像(直接所見)は合成像も多く特異度に欠けた。損傷部周囲結合組織の挫滅とそこからの気漏を表す気管周囲の異常低吸収域とそこから連続する気腫像(間接所見)は,診断価値が高いと考えられた。

【背景】鈍的外傷患者における頸椎損傷の評価は頸椎側面単純X線写真をscreening検査として用い,その後適切に撮影できなかった範囲,異常所見が認められた範囲をcomputed tomography (CT)で追加撮影する方法が推奨されている。今回このガイドラインに従って追加CTを行ったと仮定した場合の撮影頻度を計算し,初療室で撮影されたポータブル頸椎側面単純X線写真がCT撮影の指標として有用かどうかを検討した。【対象と方法】1994年1月より2000年12月までの間に横浜市大高度救命救急センターで頸椎損傷の確定診断がついた連続した100症例(141椎体損傷)および2000年1月より2000年8月までの間に入院し頸椎損傷がなかった連続した100症例を対象とした。初療室で撮影した頸椎側面単純X線写真について,(1)どの椎体まで確認可能であったか,(2)異常所見はどのくらいの頻度で出現したかをretrospectiveに再評価した。これらより次に必要となるCT撮影の頻度を計算した。【結果】頸椎損傷群70症例,非損傷群63症例で全7頸椎は確認できていなかった。異常所見の出現率は損傷群では88症例で,非損傷群でも28症例であった。全頸椎の描出不能例と異常所見陽性例を併せると損傷群で97症例,非損傷群で74症例で追加CT撮影が必要とされた。個々の椎体損傷について分析すると,損傷椎体の範囲を単純X線写真上確定できなかったのは141椎体中19椎体であった。【考察】初回ポータブル頸椎側面単純X線写真は,それを指標とすると頸椎非損傷患者にも高頻度で追加CTを必要とし,また損傷が見逃されることも少なくなく,その有用性は決して高いものではない。救命救急センターに運ばれてくるような鈍的外傷患者における頸椎損傷の診断に関しては今まで用いてきた単純X線写真を中心としたstrategyでは十分に対応できないことが推察された。

腹腔内遊離ガスと紛らわしい像を認めた多発外傷気胸症例を経験した.交通外傷で救急搬送,初療時気胸や気腹は認めなかった.急性硬膜下血腫,脳挫傷,外傷性くも膜下出血による意識障害を認め,緊急開頭血腫除去術を施行.術後の胸部単純X線検査で右横隔膜下に腹腔内遊離ガス像が疑われ,腹部CTでも腹腔内遊離ガスと判断され緊急開腹術を施行したが,気腹も消化管穿孔も認められなかった.ショック状態の外傷症例など緊急時のCTでは,読影時間が制限され複数人数での検討もできないため, CT自体の虚像が確定診断へ導く危険にも注意すべきと思われた.気胸と腹腔内遊離ガス像との鑑別点としては,肝円索・肝鎌状靱帯が正常位置に確認できるかなどが重要と思われた.

To evaluate the safe and effective use of argatroban, a competitive direct thrombin inhibitor, as an alternative anticoagulant for percutaneous cardiopulmonary support (PCPS) and continuous hemofiltration or hemodiafiltration (CHF/CHDF), a preliminary multicenter clinical trial was conducted between October 1999 and September 2000. Nine patients who underwent PCPS and/or CHF/CHDF were enrolled in the study during this period. The dosage of argatroban was controlled so that the activated clotting time (ACT) was maintained at around 180 to 200 s. The mean duration of argatroban administration was 82 ± 92 h, and the mean dose was 0.67 ± 0.40 μg kg+1 min−1. Severe hemorrhagic complications requiring the discontinuation of argatroban administration were not observed in any of the patients. Platelet loss was prevented to some degree, and plasma levels of fibrinogen were well preserved during PCPS/CHDF. Except for two patients undergoing CHDF, clot formation within the extracorporeal circulation circuit was not identified macroscopically after the discontinuation of the procedures. We conclude that argatroban might be useful as an alternative anticoagulant in cases where heparin cannot be safely used because of the increased risk of bleeding complications, thrombocytopenia, and/or hypofibrinogenemia. Although the optimal dose of argatroban has not been established, we propose an initial starting dose of 0.7 to 1.0 μg kg−1 min−1, followed by adjustments to maintain an ACT of between 180 and 250 s.

Endotoxin (ETX) primes polymorphonuclear cells (PMNs) for the subsequent release of reactive oxygen species (ROS) in response to various stimulants such as phorbol myristate acetate (PMA). Although PMNs contribute to oxidative stress after stimulation by standard stimulants and after priming by many humoral factors, it is unknown whether the PMNs from patients at different postoperative times exhibit the same response to the same standard stimulant. We examined the response of PMA—induced production of ROS from PMNs at various intervals after major abdominal surgery in response to ETX priming. This study was a prospective clinical and laboratory study conducted over a 7-day period that involved 25 patients who were referred for elective major abdominal surgery (8 for gastric cancer, 9 for colonic cancer, 8 for rectal cancer). Blood was sampled on the day before operation and on postoperative days (PODs) 1, 3, and 7. For each sample we measured luminol-dependent chemiluminescence (CL), the time to peak counts (Tmax) of PMNs stimulated by PMA, and the serum ETX level. We studied the correlation between CL and ETX in samples from PODs 1 and 3. We also studied the CL of PMNs on PODs 1 and 3 preincubated (primed) with various concentrations of ETX (0, 20, 40, 60, and 100 pg/ml). 全介助高齢者の頸部食道に嵌頓した有鈎義歯を手術的に摘出し,早期に一般管理で十分な状態に回復しえた.症例は, 83歳の男性.脳梗塞で左片麻痺,アルツハイマー型痴呆.発症3カ月前から老人施設に入所,生活能力は全介助,経鼻胃管による経腸栄養中であった.内視鏡では鉗子操作のための視野確保ができず,喉頭鏡で喉頭展開し直視下に鉗子で牽引したが摘出不能であった.義歯誤飲を,加齢による自然経過の帰結と考えるか,突発的事象と考え侵襲的治療を施行し誤飲前の全介助状態に戻すことを目指すかを,家人に十分説明,相談した上で,侵襲的ではあるが確実な治療として,緊急手術を選択した.手術は,頸部斜切開で食道下咽頭側壁を切開,義歯は気管輪状軟骨に穿通しており,摘出後,一期的に縫合閉鎖した.術後軽微な縫合不全があったが,第16病日には転院となった.

The natural history and treatment strategy of intracranial dissecting aneurysm in the vertebrobasilar (V-B) system remains controversial. We report 10 cases of ruptured intracranial dissecting aneurysm in the V-B system and review the literature with focus on the analysis of serial angiographic findings, treatment, and long-term outcome.All patients were conservatively managed under careful blood pressure control, and angiography was carried out depending on the patient's condition. Surgical or endovascular intervention was chosen when morphological changes leading to rebleeding were recognized by follow-up angiography. Craniotomy in 3 patients and coiling in 3 patients was performed. No rebleeding occurred during the follow-up period in any of the 10 patients, and all cases resulted in favorable outcome.These results suggested that ruptured intracranial dissecting aneurysms of the V-B system could be treated conservatively, avoiding rebleeding under careful control of blood pressure in the acute stage. Alternatively, surgical or endovascular treatment should be considered if angiographical changes that may lead to risk of rebleeding are noted.

Background: ST-segment elevation of ± 1.0 mm in lead V4R has been shown to be a reliable marker of right ventricular involvement (RVI), a strong predictor of a poor outcome in patients with inferior acute myocardial infarction (IMI). However, patients with no ST-segment elevation in lead V4R despite the presence of RVI have received little attention. Hypothesis: The study was undertaken to study the clinical features of patients with no ST-segment elevation in lead V4R despite the presence of RVI, which means false negative, as such patients have received little attention in the past. Methods: We studied 62 patients with a first IMI. who had total occlusion of the right coronary artery (RCA) proximal to the first right ventricular branch and successful reperfusion within 6 h from symptom onset, to examine the implications of the absence of ST-segment elevation in lead V4R despite the presence of RVI. Results: A standard 12-lead electrocardiogram (ECG) and right precordial ECG (lead V4R) were recorded on admission, and three posterior chest ECGs (leads V7 to V9) were additionally recorded in 34 patients. Patients were classified according to the absence (Group 1, n = 18) or presence (Group 2, n = 44) of ST-segment elevation of ± 1.0 mm in lead V4R on admission. Patients in Group 1 had a greater ST-segment elevation in leads V7 to V9 (2.9 ± 2.4 vs. 1.4 ± 3.0 mm, p < 0.05), a higher frequency of a dominant RCA (defined as the distribution score ≥ 0.7) (72 vs. 11%, p < 0.001), and a higher peak creatine kinase level (3760 ±; 1548 vs. 2809 ± 1824 mU/ml, p < 0.05) than those in Group 2. Conclusions: In patients with IMI caused by the occlusion of the RCA proximal to the first right ventricular branch, no ST-segment elevation in lead V4R can occur because of concomitant posterior involvement. In such patients, the incidence of RVI may be underestimated on the basis of ST-segment elevation in lead V4R.

キックスケート(車輪付きデッキ,シャフト,ハンドルバーから構成される遊戯具)による肝損傷で病院前心肺停止状態(CPA-OA)となり救命しえなかった症例を経験した.症例は, 9歳,男児,同遊戯具で走行中転倒しハンドルバーで右側胸部を強打.救急隊現場到着時,不穏状態で呼名に反応せず,血圧測定不能,搬送途中呼吸状態,意識レベル悪化,受傷後約30分で当センター到着, CPA-OAであった.腹部は軽度膨隆,右肋弓に6mmの圧挫痕を認めた.急速輸液,開胸心臓マッサージにより蘇生に成功した.肝破裂と腹腔内出血の増加,血液凝固異常,著しいアシドーシスを認めた.胸部下行大動脈遮断し,救急通報後78分,搬送後46分で緊急手術を施行した.肝右葉の深在性の複雑な破裂損傷に対しperihepatic packing,右肝動脈,門脈右枝結紮術を施行,集中治療室へ入室したが受傷後約15時間で死亡した.

非外傷性に胸部不快感を訴えている患者の12誘導心電図を院外で記録し電話で電送できれば,患者のトリアージに役立つと考え検討した。救急救命士は現着時に患者の病状が安定していることを確認し,承諾を得たあと救命指導医に電話で連絡した。指導医が適応ありと判定した205例を対象として,救命士は心電図を救急車内で記録した。心電図はアナログからデジタルへ変換され,自動解析後に携帯電話にて電送された(NEC社製)。その心電図は指導医の待機する消防指令センターでプリントアウトされた。その結果,2例は電送不能で2例は搬送を拒否したため,残る201例では心電図の記録開始から現場出発まで平均10.5±7.8分を要した。指導医は各症例で主訴と心電図所見より初期診断し,患者のトリアージを行った。搬送した医療機関での最終診断は狭心症36例,急性心筋梗塞27例,心不全23例,不整脈21例であり,全体の53.2%を心疾患が占めた。心電図自動解析の結果は狭心症例では38.9%がST. T波異常で,27.8%が不整脈であった。急性心筋梗塞例では40.7%がST. T波異常で,14.8%でしか急性心筋梗塞と診断できなかった。心不全例では30.4%がST. T波異常で,26.1%が不整脈であった。しかし不整脈例では52.4%を不整脈と診断できた。指導医による初期診断の精度(感度と陽性適中率)を最終診断より顧みると,狭心症例では50.0%と54.5%,急性心筋梗塞例では74.1%と64.5%,心不全例では34.8%と61.5%,不整脈例では66.7%と58.3%であった。すなわち,急性心筋梗塞例と不整脈例とでは有意差を認めなかったが,急性心筋梗塞例の初期診断精度は,1誘導心電図電送でも診断可能とされる不整脈例より良好であった。以上より,今後より多くの心疾患患者で12誘導心電図電送が活用されるには,心電図記録開始から現場出発までの時間を短縮し,急性心筋梗塞患者での診断精度向上に向けての自動解析ソフトの更新が必要と思われた。

急性膵炎の重症化においてサイトカインや全身性炎症反応症候群(SIRS)が注目され,SIRS陽性項目数と年齢を加えた重症度スコアが提唱された。われわれは抗サイトカイン治療も念頭に,重症急性膵炎に持続血液濾過透析(CHDF)を導入しているが,その治療成績と,重症度スコアの有用性を検討するとともに,CHDFを含む特殊治療の導入基準の試案を作成した。対象は重症急性膵炎16例で,従来の厚生省重症度判定基準に基づく治療プロトコールに従い特殊治療を選択した。救命率を明らかにし,入院時の重症度スコア,重症度判定基準における臨床徴候の有無,年齢および各検査値が集中治療室(ICU)在室期間と相関するかを検討した。さらに入院5日目の重症度スコアとICU在室期間の相関について検討した。入院時の重症度スコアの平均は10.1点で,全体の救命率は93.8%であった。入院時の重症度スコアとICU在室期間は相関していた(p=0.0036)。個々の臨床徴候についてはショックを認めた症例は有意にICU在室期間が長かった(p=0.0349)。臨床徴候を認めない群のICU在室期間は,ひとつ以上認める群と比較し有意に短かった(p=0.0223)。年齢と各検査値の検討では,base excess (BE)のみが有意にICU在室期間と相関した(p=0.0066)。入院5日目の重症度スコアは平均7.0点でICU在室期間と相関していた(p=0.0030)。CHDFを導入した治療成績は良好で,CHDFは有効な治療法と考える。重症度スコアはICU在室期間と相関し,特殊治療の選択基準,治療経過の指標として有用と考えられるが,それぞれの評価因子の重みには違いがあり,とくに初診時における臨床徴候の存在とBEの高度の低下は予後因子として重要と考えられる。今回の検討結果を踏まえた治療指針は高次施設移送の基準としても提案したい。

わが国における外傷症例の現場での重症度評価や搬送先病院の適正さに関する検討は検索した範囲では報告されていない。そこで外傷症例におけるアンダートリアージが患者の治療経過・短期機能予後に及ぼす影響について検討した。対象は1997年4月から1999年3月までの2年間に都市型三次機能病院(横浜市立大学医学部附属市民総合医療センター救命救急センター)に搬送された15-55歳の鈍的外傷症例のうちinjury severity score (ISS 85) >15かつrevised trauma scoreを構成する3つの因子(Glasgow Coma Scale,収縮期血圧,呼吸回数)のいずれかのcodeの点数が3点以上であった29症例(頭部abbreviated injury scale (AIS) ≧3の症例,来院時心肺停止症例,24時間以内死亡・転院例を除く)とし,当センターへの直接搬送群(DT群)と他院を経た間接搬送群(IT群)に分類,検討した。デザインは後ろ向きコホート研究。ICU滞在日数はDT群に比較してIT群では長い傾向にあり(DT群vs IT群=3.7±3.3 vs 9.0±11.2(日),p=0.09),退院時または転院時のGlasgow Outcome ScaleはIT群で有意にSDの転帰をとる症例が多かった((GR+MD)/SD: (5+3)/2 vs (4+1)/11(人),p=0.04)。以上の結果から,重症度に合った病院への適正搬送が患者の予後を改善すると推測された。今後は現場での重症度判定と病院選定の基礎となるトリアージツールの導入が必要である。また外傷診療システムの科学的評価のために診療圏におけるアンダートリアージ率とオーバートリアージ率を算出する必要がある。そのために消防局と病院に共通なテンプレートを有した記録の下で,地域診療圏における全外傷症例の疫学的調査を行っていくことが重要であると考えられた。

イギリスの救急医療の現状を報告する。救急本部では電話に応対する職員であるcall takerがコンピューターソフトのプロトコールにより患者の状態を把握し,助言や指導を救急車到着まで続ける一方,この情報により緊急度・重症度がトリアージされる。dispatcher(救急車を派遣する職員)は,これをもとに,重症の場合は,救急車以外にもBASICS doctor(院外救急活動を行うボランティア医師)や,first responder(車やバイクで急行するparamedic)の派遣を決定し,院外医療活動が開始される。救急車は救急部のある直近の病院に患者を搬送し,病院を選定する自由または必要はない。救急部を受診すると,看護婦によりコンピューターを使った症状別のトリアージを受け,診察の優先度が決定される。このように通常の医療体制にトリアージの概念が組み込まれている。大災害のためにはMajor Incident Medical Management and Support (MIMMS)という教育システムが完成されている。このシステムでは大災害時の医療にかかわる警察,消防,救急,医療機関,ボランティア,行政などの各部門の役割と責任,組織体系,連携の仕方,連絡のとり方などが講義,訓練される。これは医療関係だけでなく,大災害に関与する警察,消防,行政,ボランティア組織など全員が理解できる共通語として浸透することを目的としている。イギリスではこのように大災害に対し,BASICS doctorを中心とする医師の院外救急医療活動,通常の医療活動のなかでトリアージの概念,MIMMSという教育,訓練システムが平常時から存在している。日本でもトリアージへのさらなる理解と院外救急活動の開始が必要である一方,全国的な警察,消防,救急と地域行政との連携の構築が急務で,イギリスの経験から学べる点は多いと考えられた。

高齢者急性上腸間膜動脈 (SMA) 閉塞症, 小腸壊死に対する広範囲小腸切除後に, 1期的吻合を行ったので, その有用性と安全性を検討した. 83, 78, 75歳の女性の急性SMA閉塞症3例に広範囲小腸切除を行った. 小腸壊死と血性腹水を認めたが, 腸管穿孔はなかった. 数cmの小腸温存には固執せず, 確実に安全と思われる部位で切離し, 器械吻合で1期的に再建した (側端, 側端, 端側吻合). 残存小腸は, Treitz靭帯から30cm, 幽門輪から50cm, Treitz靭帯から30cm, 経口摂取は, 各16, 17, 10病日から開始した. 経口摂取後の排便回数は, 各3～4, 0～1, 15回前後/日と端側吻合で頻回の傾向にあった. 維持輸液も各2,000ml, 1,000, 2,000～1,000ml/日前後であった. 全例, 縫合不全や残存腸管の虚血の進展の徴候はなかった. 高齢であることから消化吸収能をある程度犠牲にして, 空腸瘻とせず一期的に吻合を行ったことで, 近位空腸瘻のデメリットを回避した. 術後造影検査での造影剤の移動速度から, 側端吻合が消化, 吸収に有用と思われた.

We experienced an emergency operation for a case of acute appendicitis with human immunodeficiency virus (HIV) infection. This paper reports on our strategy for dealing with the patient and the perioperative changes in inflammatory reaction. A 28-year-old man, who was pointed out hemophilia B at the age two and HIV infection at the age 18, had a right lower abdominal pain on February 12, and high grade fever on February 16, 1996. He was diagnosed as acute appendicitis at another hospital and was referred to our center because of HIV infection. On admission, white blood cell count was 3000/mm3 (2600/mm3 in October, 1995) and the level of CRP was 4.9mg/dl. At laparotomy, the vermiform appendix was swollen and covered with omentum, but adhesion was not prominent. Ascitis and the contents of vermiform appendix was old bloody fluid, and no pus was detected. Pathological study showed that the inflammation was phregmonous or gangrenous and nonspecific. High grade fever continued for 3 days after the operation. White blood cell count was 7700/mm3 at just after the operation and decreased to 1500/mm3 at the 5th postoperative day, and such change was thought to be due to natural course in HIV infection. The level of CRP increased after the operation to 7.6mg/dl on the 2nd post-operative day and decreased to 0.6mg/dl onthe 5th postoperative day. There was no delay of wound healing.
