Abstract

We refine a recent result on the structure of measures satisfying a linear partial differential equation 𝓐μ = σ, μ, σ are Radon measures, considering the measure μ(x) = g(x)dx + μus(x̃)(μs(x̄) + dx̄) where x = (x̃,x̄) ∈ ℝk × ℝd−k, μus is a uniformly singular measure in x̃0 and the measure μs is a singular measure. Abstract

People in economically advantaged nations tend to evaluate their life as more positive overall and report greater well-being than people in less advantaged nations. But how does positivity manifest in the daily life experiences of individuals around the world? The present study asked 15,244 college students from 62 nations, in 42 languages, to describe a situation they experienced the previous day using the Riverside Situational Q-sort (RSQ). Using expert ratings, the overall positivity of each situation was calculated for both nations and individuals. The positivity of the average situation in each nation was strongly related to the economic development of the nation as measured by the Human Development Index (HDI). For individuals’ daily experiences, the economic status of their nation also predicted the positivity of their experience, even more than their family socioeconomic status. Further analyses revealed the specific characteristics of the average situations for higher HDI nations that make their experiences more positive. Higher HDI was associated with situational experiences involving humor, socializing with others, and the potential to express emotions and fantasies. Lower HDI was associated with an increase in the presence of threats, blame, and hostility, as well as situational experiences consisting of family, religion, and money. Despite the increase in a few negative situational characteristics in lower HDI countries, the overall average experience still ranged from neutral to slightly positive, rather than negative, suggesting that greater HDI may not necessarily increase positive experiences but rather decrease negative experiences. The results illustrate how national economic status influences the lives of individuals even within a single instance of daily life, with large and powerful consequences when accumulated across individuals within each nation.

Abstract

We examined membrane-spanning archaeal lipids using ultra high pressure liquid chromatography-high resolution mass spectrometry (UHPLC-HRMS) in a suite of sediment samples from both cored sequences (Messel oil shale and Lake Chala) and surface sediments (Azorean lakes) encompassing ancient and modern (Eocene to Present) lacustrine environments. Additionally we compared the lacustrine data to those of marine (Mediterranean cored sequences, Arabian Sea surface sediments and Monterey outcrop sediments) and hypersaline sediments (Vena del Gesso marls) as well as marine suspended particulate matter (SPM) from the Black Sea. Regular isoprenoidal glycerol dialkyl glycerol tetraethers (GDGTs) and glycerol dialkyl diethers (GDDs) were the most abundant membrane-spanning lipids in all investigated settings (>90 % and 84 % respectively). Interestingly, GDGTs with a cyclohexyl ring (S-GDGTs) were also present in almost all investigated lake sediments, in relative abundances of ca. 2–7 % and, for the first time, also their S-GDD counterparts were detected (2–10 %). The producers of S-GDGTs are still unknown, however our results show that it is likely that bottom water anoxia (both seasonally induced or permanent) is the driving factor for the production of these lipids, whereas previous studies suggested euxinia was required for production. Unsaturated GDGTs (uns-GDGTs, ca. 2 %) were only detected in Lake Chala sediments and surface sediments from Azorean lakes, but without accompanying uns-GDDs. GMGTs, glycerol monoalkyl glycerol tetraethers, were present in Messel oil shale and marine samples, while GMDs were only found in Messel oil shale.

Abstract

Bacteriohopanepolyols (BHPs) are a diverse class of lipids produced by bacteria across a wide range of environments. In this study, we aim to further identify BHPs related to ecological niches and/or specific bacteria by characterizing the distribution of BHPs in suspended particulate matter (SPM) of the water column and in sediments in a range of lakes and coastal lagoons from the Azores Archipelago, as well as in a co-culture enriched for methanotrophs. Sediment samples from Azorean lakes with low-oxygen conditions during the summer months (i.e., Azul, Verde, Funda, and Negra) contain relatively high abundances of BHPs that are typically associated with methane-oxidizing (methanotrophic) bacteria (i.e., aminotetrol, aminopentol, and methylcarbamate-aminopentol), as well as the ethenolamine-BHPs (i.e., ethenolamine-BHpentol and ethenolamine-BHhexol) and the N-formylated aminoBHPs. Both ethenolamine-BHPs and N-formylated aminoBHPs were also detected in a methanotroph–methylotroph co-culture that was enriched from a lake. In the SPM of all water columns, bacteriohopanetetrol (BHT), BHT cyclitol ether, and aminotriol are the dominant BHPs. In SPM from Lake Funda, nucleoside BHPs (i.e., Me-adenosylhopaneHG-diMen> (where HG refers to head group), N1-methylinosylhopane, 2Me-N1-inosylhopane, and Me-N1-inosylhopane) are present in low abundance or absent under oxic conditions but increase in concentration near the chemocline, suggesting potential in situ production of these nucleoside BHPs rather than an allochthonous origin. In contrast, sediments from shallow, well-mixed lakes (i.e., Empadadas, São Jorge, and Lomba) contain higher abundances of adenosylhopane and N1-methylinosylhopane, which likely originate from bacteria living in nearby soils. Based on our current results we revised the existing Rsoil index, which was previously used to infer terrestrial inputs to aquatic environments, to exclude any potential nucleosides produced in the lake water

Abstract

Objective: The current exploratory study sought to examine dispositional optimism, or the general expectation for positive outcomes, around the world. Method: Dispositional optimism and possible correlates were assessed across 61 countries (N = 15,185; mean age = 21.92; 77% female). Mean-level differences in optimism were computed along with their relationships with individual and country-level variables. Results: Worldwide, mean optimism levels were above the midpoint of the scale. Perhaps surprisingly, country-level optimism was negatively related to gross domestic product per capita, population density, and democratic norms and positively related to income inequality and perceived corruption. However, country-level optimism was positively related to projected economic improvement. Individual-level optimism was positively related to individual well-being within every country, although this relationship was less strong in countries with challenging economic and social circumstances. Conclusions: While individuals around the world are generally optimistic, societal characteristics appear to affect the degree to which their optimism is associated with psychological well-being, sometimes in seemingly anomalous ways.

Abstract

Recent research conducted largely in the United States suggests that most people would like to change one or more of their personality traits. Yet almost no research has investigated the degree to which and in what ways volitional personality change (VPC), or individuals’ active efforts toward personality change, might be common around the world. Through a custom-built website, 13,278 college student participants from 55 countries and one of a larger country (Hong Kong, S.A.R.) using 42 different languages reported whether they were currently trying to change their personality and, if so, what they were trying to change. Around the world, 60.40% of participants reported that they are currently trying to change their personalities, with the highest percentage in Thailand (81.91%) and the lowest in Kenya (21.41%). Among those who provide open-ended responses to the aspect of personality they are trying to change, the most common goals were to increase emotional stability (29.73%), conscientiousness (19.71%), extraversion (15.94%), and agreeableness (13.53%). In line with previous research, students who are trying to change any personality trait tend to have relatively low levels of emotional stability and happiness. Moreover, those with relatively low levels of socially desirable traits reported attempting to increase what they lacked. These principal findings were generalizable around the world

Abstract

Water distribution networks (WDNs) are significant users of energy and contributors to greenhouse gas emissions. Energy recovery strategies (ERSs) have potential for switching WDNs to a renewable energy source, thereby assisting with the transition to net-zero emissions and reducing the dependency of water utilities on power-grids. To address the current lack of understanding of the potential for ERSs to achieve these goals, this work presents a holistic review, which considers all available devices, i.e. turbines, pumps as turbines, and other alternative strategies, such as the GreenValves, as well as all the kinds of application in the literature. Besides describing the latest most significant achievements in the context of ERSs, the review also identifies the most common issues and proposes several future research directions, including the need for a better test and comparison of the various available solutions in field applications, and more thorough analyses of the socio/political problems hampering their diffusion. Abstract

What does it mean to be happy? The vast majority of cross-cultural studies on happiness have employed a Western-origin, or "WEIRD" measure of happiness that conceptualizes it as a self-centered (or "independent"), high-arousal emotion. However, research from Eastern cultures, particularly Japan, conceptualizes happiness as including an interpersonal aspect emphasizing harmony and connectedness to others. Following a combined emicetic approach (Cheung, van de Vijver & Leong, 2011), we assessed the cross-cultural applicability of a measure of independent happiness developed in the US (Subjective Happiness Scale; Lyubomirsky & Lepper, 1999) and a measure of interdependent happiness developed in Japan (Interdependent Happiness Scale; Hitokoto & Uchida, 2015), with data from 63 countries representing 7 sociocultural regions. Results indicate that the schema of independent happiness was more coherent in more WEIRD countries. In contrast, the coherence of interdependent happiness was unrelated to a country's "WEIRD-ness." Reliabilities of both happiness measures were lowest in African and Middle Eastern countries, suggesting these two conceptualizations of happiness may not be globally comprehensive. Overall, while the two measures had many similar correlates and properties, the self-focused concept of independent happiness is "WEIRD-er" than interdependent happiness, suggesting cross-cultural researchers should attend to both conceptualizations.

Abstract

Objective: The current study seeks to replicate and extend principal findings reported in The World at 7:00, a project that examined the psychological experience of situations in 20 countries. Method: Data were collected from participants in 62 countries (N = 15,318), recruited from universities by local collaborators to complete the study via a custom-built website using 42 languages. Results: Several findings of the previous study were replicated. The average reported situational experience around the world was mildly positive. The same countries tended to be most alike in reported situational experience (r =.60) across the two studies, among the countries included in both. As in the previous study, the homogeneity of reported situational experience was significantly greater within than between countries, although the difference was small. The previously reported exploratory finding that negative aspects of situations varied more across countries than positive aspects did not replicate. Correlations between aspects of reported situational experience and country-level average value scores, personality, and demographic variables were largely similar between the two studies. Conclusion: The findings underscore the importance of cross-cultural situational research and the need to replicate its results, and highlight the complex interplay of culture and situational experience.

Abstract

What does it mean to be happy? The vast majority of cross-cultural studies on happiness have employed a Western-origin, or “WEIRD” measure of happiness that conceptualizes it as a self-centered (or “independent”), high-arousal emotion. However, research from Eastern cultures, particularly Japan, conceptualizes happiness as including an interpersonal aspect emphasizing harmony and connectedness to others. Following a combined emic-etic approach (Cheung, van de Vijver & Leong, 2011), we assessed the cross-cultural applicability of a measure of independent happiness developed in the US (Subjective Happiness Scale; Lyubomirsky & Lepper, 1999) and a measure of interdependent happiness developed in Japan (Interdependent Happiness Scale; Hitokoto & Uchida, 2015), with data from 63 countries representing 7 sociocultural regions. Results indicate that the schema of independent happiness was more coherent in more WEIRD countries. In contrast, the coherence of interdependent happiness was unrelated to a country’s “WEIRD-ness.” Reliabilities of both happiness measures were lowest in African and Middle Eastern countries, suggesting these two conceptualizations of happiness may not be globally comprehensive. Overall, while the two measures had many similar correlates and properties, the self-focused concept of independent happiness is “WEIRD-er” than interdependent happiness, suggesting cross-cultural researchers should attend to both conceptualizations.

In addition, few studies involved elderly patients and most focused on all-cause mortality and HF-related hospitalization as outcomes. The aim of our study was to determine whether HRQOL is a predictor and an independent predictor of long-term cardiac mortality, all-cause mortality, and HF-related rehospitalization in elderly patients hospitalized with HF.Patients and methods: This prospective observational study included 200 elderly patients hospitalized with HF in Serbia. HRQOL was measured using the Minnesota Living with Heart Failure questionnaire (MLHFQ). The median follow-up period was 28 months. The primary outcome was cardiac mortality, and all-cause mortality and HF-related rehospitalization were secondary outcomes. Survival analysis was conducted using the Kaplan–Meier method and Cox-proportional hazards regression.Results: Subjects with poor HRQOL (higher than the median MLHFQ score) had a higher probability of cardiac mortality (P=0.029) and HF-related rehospitalization (P=0.001) during long-term follow-up. Poor HRQOL was an independent predictor of cardiac mortality (HR: 2.051, 95% CI: 1.260–3.339, P=0.004), all-cause mortality (HR: 1.620, 95% CI: 1.076–2.438, P=0.021), and HF-related rehospitalization (HR: 2.040, 95% CI: 1.290–3.227, P=0.002).Conclusion: HRQOL is an independent predictor of long-term cardiac mortality in elderly patients hospitalized with HF. It also independently predicts all-cause mortality and HF-related rehospitalization. HRQOL could be used as a complementary clinical predictive tool in this patient population.Keywords: aged, heart disease, self-perception of health, prognosis

Abstract

Author(s): Baranski, Erica; Sweeny, Kate; Gardiner, Gwendolyn; Members of the International Situations Project; Funder, David C | Abstract: ObjectiveThe current exploratory study sought to examine dispositional optimism, or the general expectation for positive outcomes, around the world.MethodDispositional optimism and possible correlates were assessed across 61 countries (Nn=n15,185; mean agen=n21.92; 77% female). Mean-level differences in optimism were computed along with their relationships with individual and country-level variables.ResultsWorldwide, mean optimism levels were above the midpoint of the scale. Perhaps surprisingly, country-level optimism was negatively related to gross domestic product per capita, population density, and democratic norms and positively related to income inequality and perceived corruption. However, country-level optimism was positively related to projected economic improvement. Individual-level optimism was positively related to individual well-being within every country, although this relationship was less strong in countries with challenging economic and social circumstances.ConclusionsWhile individuals around the world are generally optimistic, societal characteristics appear to affect the degree to which their optimism is associated with psychological well-being, sometimes in seemingly anomalous ways.

We refine a recent result on the structure of measures satisfying a linear partial differential equation 𝓐μ = σ, μ, σ are Radon measures, considering the measure μ(x) = g(x)dx + μus(x̃)(μs(x̄) + dx̄) where x = (x̃,x̄) ∈ ℝk × ℝd−k, μus is a uniformly singular measure in x̃0 and the measure μs is a singular measure. Abstract

Guideline adherence with regard to direct acting oral anticoagulants use OBJECTIVE To determine guideline adherence regarding prescribing direct acting oral anticoagulants (DOACs) in daily clinical practice, distinguishing clinical and outpatient cardiology and orthopaedy patients. DESIGN Observational retrospective cross-sectional study. METHODS Four guidelines for prescribing DOACs were summarized. Medical records were used to identify patients who received rivaroxaban, dabigatran or apixaban. Baseline characteristics and adherence to the guidelines regarding interactions, CHA2DS2-VASc, HAS-BLED, dosage, renal function etc. were compared between clinical and outpatient cardiology and orthopaedy groups. RESULTS 100 patients were included in all three groups. In four cases an interaction with possible clinical implications was found. Among almost half of the patients the CHA2DS2- VASc score was not registered. The HAS-BLED score was only determined for a handful of patients. Overtreatment was found in 3[%] of clinical cardiology patients and 7[%] of cardiology outpatients. Complications during VKA use prompted a switch to DOACs in only few cases. In 32[%] of orthopaedy patients and 41[%] of cardiology outpatients the renal function measurement was older than 60 days; in three cases the renal function was unknown. One patient with a prior cerebral haemorrhage received a DOAC without consent from a neurologist. 76[%] of patients in the outpatient group and 91[%] in the clinical cardiology group were prescribed the appropriate dose. A lower dose than appropriate was more common than a higher dose. CONCLUSION In general adherence to guidelines is good, but there is room for improvement. Verifying the absence of interactions, checking renal function and offering help with appropriate dosing can be performed by the hospital pharmacy. A way to improve determination and recording of CHA2DS2-VASc and HAS-BLED scores needs to be determined.

Abstract

OBJECTIVE An earlier switch to oral antibiotics (AB po) can reduce inappropriate use of intravenous antibiotics (AB iv) and comes with certain benefits: decreased workload, reduced risk of complications, and decreased patient discomfort. Therefore, we examined how an earlier switch from AB iv to AB po can be executed optimally in a hospital setting. DESIGN Prospective, explorative intervention study. METHODS Besides the self ­ developed decision support system to detect AB iv, three intervention phases have been set up. Phase 1: pharmacist reports to the microbiologist in the Antimicrobial stewardship team (A ­ Team). Afterwards, the microbiologist discusses the option of switching to AB po with the treating physician. Phase 2: the pharmacist dis ­ cusses the option of switching to AB po with the treating physician. Phase 3: the pharmacist only checks required data for earlier switch to AB po. RESULTS 713 AB iv have been prescribed for 555 patients during the three phases and follow ­ up. For phase 1, 2 and 3 effective-ness was 39%, 48% and 50%, respectively, and pharmacist spent 3.2, 1.4 and 1.0 hours per day, respectively, on AB Switch. Phase 3 has been performed during follow ­ up thus achieving better effectiveness (67%) and better efficiency (0.9 hours per day). CONCLUSION These improvements are due to trainings provided by the A ­ Team, improved documentation of physicians (including indication and antibiotics policy), and increased awareness to switch earlier to AB po. Abstract

BACKGROUND In recent years more point-of-care tests (POCTs) have reached the hospital market. It is often unknown what the potential of POCTs is in terms of cost reduction or improvement of patient survival. OBJECTIVE To estimate the potential economic gain from using pointof-care urinalysis devices to screen for drugs of abuse rather than outsourcing testing to an external laboratory. DESIGN and METHODS This study was based on hospital data from 2015 and 2016 concerning number and unit costs of (therapeutic) drug intoxication screenings, logistic costs, and POC urinalysis costs. The economic outcomes were calculated for the actual scenario and for a hypothetical scenario where a POC urinalysis would be used instead. RESULTS For the same number of (therapeutic) drug intoxication screenings, the analysis predicts that the implementation of a POC urinalysis for the emergency department of the Tjongerschans hospital would increase the costs by €396,00 per annum compared to outsourcing. The POC urinalysis test would be economically favorable compared with the actual scenario from 65 (therapeutic) drug intoxication screenings per year. CONCLUSION For a small general hospital, which has few hospitalizations attributable to (therapeutic) drug abuse, implementation of POC urinalysis appears not to be economically favorable compared to outsourcing it to a nearby analytical facility [ABSTRACT FROM AUTHOR]

Abstract

INTRODUCTION The number of reports on baclofen intoxication has increased in recent years. This is related to the increasing use of baclofen in the treatment of alcohol and gammahydroxybutyric acid withdrawal and as recreative drug. DESCRIPTION A 27-year-old man with a history of psychiatric comorbidity and an addiction problem was brought to the emergency department (ED) in an unresponsive state (EMV score 3) after an attempted suicide with medication. After heteroanamnesis, he was diagnosed with an intoxication of 850 mg baclofen, 2.5 liters' beer and an unknown amount of heroine. He was intubated because of respiratory failure. Besides, activated charcoal in combination with a laxative was administered. Morphine and midazolam were administered to prevent withdrawal symptoms. The EMV score improved 30 hours after admission and he was detubated 60 hours after admission. DISCUSSION AND CONCLUSION The treatment of a baclofen intoxication consists of supportive care. This case demonstrates the importance of keeping a baclofen intoxication in mind in the differential diagnoses when a comatose or confused patient with addiction problems is brought in at the ED. Baclofen levels cannot be determined with standard drug screening so considering specific laboratory testing is necessary to detect baclofen intoxication. [ABSTRACT FROM AUTHOR]
