Background: We previously reported an increased prevalence of asthma among patients who had lived in temporary housing after the 2011 Great East Japan Earthquake. We investigated the prognosis of asthma in former residents of temporary housing after allergen avoidance.Methods: Asthma was diagnosed in adults ≥ 15 years from 2014 to 2019 who had lived in temporary housing in Ishinomaki City for at least 1 year. The disease prognosis after the intervention of allergen avoidance in cases that were followed for more than 3 years during the 6-year study period was analyzed. We measured the Dermatophagoides farinae -specific immunoglobulin E (IgE) levels in serum, and the amount of Dermatophagoides group 1 (Der 1) antigen on their futons or mattresses. We instructed residents in an allergen avoidance strategy that included 32 tasks, including using microfiber bedding covers. Results: Of the 202 examinees who were followed for at least 3 years during the 6-year study period, 72(35.6%) were asthmatic during at least one examination. Of these 72 asthmatics, 55(76.4%) developed the disease after the earthquake, and more than half of the cases that we diagnosed at the examination were mild intermittent asthma. After the allergen-avoidance intervention, both the Der 1 level on the futons or mattresses of residents who were diagnosed with asthma but who were nonasthmatic at the final screening and their serum Der f-specific IgE levels significantly decreased (P < 0.01) at the final examination.Conclusion: Antigen avoidance ameliorated mild asthma that was prevalent among residents of temporary housing after the earthquake.

われわれは臨床的にアトピー性皮膚炎(以下AD)と診断された12歳から50歳までの85名(男性46名, 女性39名, 平均年齢22.09歳)を対象に血清総IgE, 血清ダニ特異IgE, 血中好酸球(%)の測定およびダニ抗原を用いたスクラッチテストを施行した。ダニ特異IgEが高値で, かつスクラッチテスト陽性の患者は48名(56.5%)であった。この一群の48名のうち血清総IgEが400 IU/ml以上のものは42名で, 平均6549 IU/mlであり, また血中好酸球が5%以上のものは40名, 平均10.1%であり, この群は血清総IgEが高く末梢血好酸球も高い群であった。この群のAD患者15名より末梢血単核球を採取しダニ抗原刺激によるIL-4とIL-5の産生を調べたところ, IL-4とIL-5の産生がみとめられた。以上より, 臨床的にADと診断された患者の中には, ダニ特異IgEが高く, かつダニスクラッチテスト陽性反応を示す, おそらくダニに対する即時型アレルギー反応が皮疹の発現に深く関与していると思われるような一群が約50%存在し, またこの一群は血清総IgEが高く, 末梢血好酸球も高いことがわかった。また, このような一群のAD患者の末梢血中にはダニ抗原刺激によりIL-4やIL-5を産生するようなダニ特異的T細胞が存在することがわかった。これらのダニ特異的T細胞から産生されるIL-4やIL-5が高IgE血症や好酸球増多に何らかの関与のあることが示唆された。

Background The diagnostic investigation of sensitization to dust mites is performed through measurement of specific IgE and cutaneous immediate hypersensitivity tests. The general aim of this study was to comparatively evaluate the diagnostic accuracy of immediate hypersensitivity skin tests and specific IgE for Dermatophagoides pteronyssinus (Dp) and Blomia tropicalis in patients with allergic rhinitis.

Rationale A significant relationship has previously been reported between home allergen exposure to cockroach or dust mite and asthma severity. We assess the correlation of D. farinae (Df) and cockroach specific IgE levels to severity of asthma among inner-city asthmatic children. Methods Twenty eight children ages 6-15 followed in our public hospital asthma clinic were recruited. All patients had a previous diagnosis of asthma. Asthma severity measures included FEV1, number of asthma attacks and hospital admissions in the previous year, and number of medications along with dose of inhaled corticosteroids used to control asthma symptoms (medication score). Blood samples were taken for cockroach and Df RAST. Results 8 of 28 (29%) patients had a positive cockroach RAST and 11 of 28 (39%) had a positive Df RAST. There was no correlation between Df RAST and any of the outcome measures. However, there were significantly higher medication scores (range 0-5) in the cockroach RAST positive patients compared to cockroach negative RAST patients (4.1 ± 0.4 vs 2.9 ± 0.3, p=0.05; t-test). In addition, simple regression analysis showed significant correlation of cockroach RAST class (0-IV) with medication score (p=0.04). Conclusions No significant correlation between cockroach or dust mite IgE levels and severity measures other than medication scores was found. The inner-city asthmatic children in our study were well controlled on multiple asthma medications thus other measures of asthma severity were not significantly different between groups. Cockroach specific IgE levels may be predictive of the medications required to control asthma symptoms in children residing in an urban environment.

The prevalence of pediatric asthma in Zhengzhou District, China, was investigated by detecting serum mite specific IgE in patients. Blood samples collected from asthmatic children and non-asthmatic controls were analyzed for serum mite (Dermatophagoides farinae, Df) specific IgE (sIgE) levels by the UniCAP system. A total of 573 asthmatic children and 300 normal controls aged from 2 months to 13 years were recruited for this experiment. The positive rates of serum Df sIgE for three age groups of ∼3, ∼7, and ∼13-years were 31.4%, 63.7%, and 85.8%, respectively, and the total positive rate averaged 61.6%.
