Abstract Speckle contrast optical spectroscopy (SCOS) is an emerging camera-based technique that can measure human cerebral blood flow (CBF) with high signal-to-noise ratio (SNR). At low photon flux levels typically encountered in human CBF measurements, camera noise and nonidealities could significantly impact SCOS measurement SNR and accuracy. Thus, a guide for characterizing, selecting, and optimizing a camera for SCOS measurements is crucial for the development of next-generation optical devices for monitoring human CBF and brain function. Here, we provide such a guide and illustrate it by evaluating three commercially available complementary metal–oxide–semiconductor cameras, considering a variety of factors including linearity, read noise, and quantization distortion. We show that some cameras that are well-suited for general intensity imaging could be challenged in accurately quantifying spatial contrast for SCOS. We then determine the optimal operating parameters for the preferred camera among the three and demonstrate measurement of human CBF with this selected low-cost camera. This work establishes a guideline for characterizing and selecting cameras as well as for determining optimal parameters for SCOS systems.

Despite recent advances in technology, treatment gaps remain. A small SCS system with a miniaturized implantable pulse generator (micro-IPG; < 1.5 cm3 in volume) and an externally worn power source may be preferred by patients who do not want a large, implanted battery. We report here the long-term outcomes from the first-in-human study evaluating the safety and performance of a new neurostimulation system.Patients and Methods: This was a prospective, multi-center, open-label, single-arm study to evaluate this SCS system, in the treatment of chronic, intractable leg and low-back pain. Consented subjects who passed screening continued on to the long-term phase of the study. One-year, patient-reported outcomes (PRO’s) such as pain (Numeric Rating Scale, NRS), functional disability, quality of life, and mood were captured.Results: Twenty-six (26) evaluable subjects with permanent implants were included in this analysis. The average leg pain NRS score decreased from 6.8 ± 1.2 at baseline to 1.1 ± 1.2 at the end of the study (p < 0.001), while the average low-back pain NRS score decreased from 6.8 ± 1.2 to 1.5 ± 1.2 (p < 0.001). The responder rate (proportion with ≥ 50% pain relief) was 91% in the leg(s) and 82% in the low back. There were significant improvements in functional disability (Oswestry Disability Index) and in mood (Beck Depression Inventory), demonstrating a 46% and 62% improvement, respectively (p < 0.001). Eleven-point Likert scales demonstrated the wearable to be very comfortable and very easy to use.Conclusion: There were considerable challenges conducting a clinical study during the COVID-19 pandemic, such as missed study programming visits. Nevertheless, subjects had significant PRO improvements through 1-year. The small size of the implanted device, along with a proprietary waveform, may allow for improved SCS outcomes and a drop in incidence of IPG-pocket pain.Plain Language Summary: Chronic neuropathic pain of the low back and legs is a common affliction. Unfortunately, many of these cases are medically intractable and require more invasive treatments. One such treatment is spinal cord stimulation, in which very small, cylindrical electrodes (leads) are percutaneously introduced into the epidural space where they deliver mild electrical pulses to the spinal cord thereby blocking the pain signals from reaching the brain. Among the shortcomings of this general approach is the bulkiness of the implanted battery and electronics that power the electrodes, which can be as large as 44 cm3 in volume. We report here the results of a study employing a very small spinal cord stimulator implant (< 1.5 cm3 in volume) that is externally powered by a wearable device. The results of this multi-center, prospective, open-label, single-arm study are reported here. Following 1 year of spinal cord stimulation therapy, the average pain reduction was 79% in the legs and 81% in the back, both of which were statistically significant at the p< 0.001 level. At the same study endpoint, patient-reported outcomes demonstrated significant improvements in functional disability (p < 0.001) and mood (p < 0.05). Summary: Coral reefs underpin the environmental, social, and economic fabrics of much of the world's tropical coast. Yet, the fine-scale distribution and composition of coral reefs have never been reported consistently across the planet. Here, we present new area estimates enabled by global geomorphic zone and benthic substrate maps at 5 m pixel resolution. We revise global coral reef estimates to 348,361 km2 of shallow coral reefs and 80,213 km2 (46,237–106,319 km2, 95% confidence interval) of coral habitat. The mapping used more than 1.5 million training samples supported by 480+ data contributions to deploy a coral reef classification of over 100 trillion pixels from the Sentinel-2 satellites and the Planet Dove CubeSat constellation. The publicly available maps are accessible via the Allen Coral Atlas and Google Earth Engine and are already being used by thousands of people to improve the conservation, management, and research of coral reef ecosystems. Science for society: Coral reefs possess a quarter of all marine life and contribute to the well-being and livelihoods of a billion people worldwide. Maps of ecosystems underpin many science and conservation activities, but until recently, there were no consistent high-resolution maps of the world’s coral reefs. In this paper, we describe new global coral reef maps from the Allen Coral Atlas, detailing the underlying methodology and our new understanding of the global distribution of coral reefs. The transparent and repeatable nature of our mapping framework allows the maps to be updated based on user feedback, and the ease of access has led to downstream applications such as coral bleaching monitoring and usage in scientific, management, and conservation activities. Hundreds of thousands of people have already accessed the maps, and they are already being used directly around the world for marine spatial planning, marine protected areas, environmental accounting and assessments, restoration, and education.

While urinary polymerase chain reaction (PCR) testing is effective in organism identification in patients with complex urinary tract infections (cUTI), limited data exists on the clinical usefulness of this test. We serially surveyed physicians treating symptomatic patients with cUTI both at presentation and after PCR, and urine culture (UC) results were available to ascertain how the test results modified the therapy. A total of 96 unique surveys completed by 21 providers were included in the data analysis. The mean age for female and male patients was 69.4 ± 15.5 and 71.6 ± 12.7 years, respectively. The test positivity and line–item concordance for UC and PCR were consistent with prior reports. The PCR results modified or confirmed treatment in 59/96 (61.5%) and 25/96 (26.0%) of the cases, respectively, with 12/29 (41.4%) and 47/67 (70.1%) having negative and positive PCR results, respectively, resulting in treatment change (difference 28.7%, p < 0.01). Of these, 55/59 (57.3%) were alterations in the antibiotic regimen. PCR use to modify treatment was similar across providers and not statistically different when stratified by patient age, gender, or prior empiric therapy. In 31/59 (52.5%) of the cases, the PCR results modified the treatment where UC would not; conversely, UC would have modified the treatment in 3/37 (8.1%) of the cases where PCR did not (difference 44.4%, p < 0.01). We find that PCR test results are used by clinicians in managing cUTI, and use of this test provides an opportunity to improve antibiotic stewardship in this difficult-to-treat subset of patients.

Abstract Antibodies raised in peptide-immunized rabbits have been used in biological research for decades. Although there has been wide implementation of this approach, specific proteins are occasionally difficult to target for multiple reasons. One consideration that was noted in mice is that humoral responses may preferentially target the carboxyl terminus of the peptide sequence which is not present in the intact protein. To shed light on the frequency of preferential rabbit antibody responses to C-termini of peptide immunogens, we present our experience with generation of rabbit antibodies to human NOTCH3. A total of 23 antibodies were raised against 10 peptide sequences of human NOTCH3. Over 70% (16 of 23) of these polyclonal antibodies were determined to be C-terminal preferring: NOTCH3 peptide-reactive antibodies largely targeted the terminating free carboxyl group of the immunizing peptide. The antibodies that preferred C-terminal epitopes reacted weakly or not at all with recombinant target sequences with extension the C-terminus that eliminated the free carboxyl group of the immunogen structure; furthermore, each of these antisera revealed no antibody reactivity to proteins truncated before the C-terminus of the immunogen. In immunocytochemical applications of these anti-peptide antibodies, we similarly found reactivity to recombinant targets that best binding to cells expressing the free C-terminus of the immunizing sequence. In aggregate, our experience demonstrates a strong propensity for rabbits to mount antibody responses to C-terminal epitopes of NOTCH3-derived peptides which is predicted to limit their use against the native protein. We discuss some potential approaches to overcome this bias that could improve the efficiency of generation of antibodies in this commonly utilized experimental paradigm.

Abstract Diffuse correlation spectroscopy (DCS) is an optical technique that can be used to characterize blood flow in tissue. The measurement of cerebral hemodynamics has arisen as a promising use case for DCS, though traditional implementations of DCS exhibit suboptimal signal-to-noise ratio (SNR) and cerebral sensitivity to make robust measurements of cerebral blood flow in adults. In this work, we present long wavelength, interferometric DCS (LW-iDCS), which combines the use of a longer illumination wavelength (1064 nm), multi-speckle, and interferometric detection, to improve both cerebral sensitivity and SNR. Through direct comparison with long wavelength DCS based on superconducting nanowire single photon detectors, we demonstrate an approximate 5× improvement in SNR over a single channel of LW-DCS in the measured blood flow signals in human subjects. We show equivalence of extracted blood flow between LW-DCS and LW-iDCS, and demonstrate the feasibility of LW-iDCS measured at 100 Hz at a source-detector separation of 3.5 cm. This improvement in performance has the potential to enable robust measurement of cerebral hemodynamics and unlock novel use cases for diffuse correlation spectroscopy.

Introduction: Ehlers–Danlos syndrome (EDS) is a connective tissue disorder that has been linked to several neurological problems including Chiari malformations, atlantoaxial instability (AAI), craniocervical instability (CCI), and tethered cord syndrome. However, neurosurgical management strategies for this unique population have not been well-explored to date. The purpose of this study is to explore cases of EDS patients who required neurosurgical intervention to better characterize the neurological conditions they face and to better understand how neurosurgeons should approach the management of these patients. Methods: A retrospective review was done on all patients with a diagnosis of EDS who underwent a neurosurgical operation with the senior author (FAS) between January 2014 and December 2020. Demographic, clinical, operative, and outcome data were collected, with additional radiographic data collected on patients chosen as case illustrations. Results: Sixty-seven patients were identified who met the criteria for this study. The patients experienced a wide array of preoperative diagnoses, with Chiari malformation, AAI, CCI, and tethered cord syndrome representing the majority. The patients underwent a heterogeneous group of operations with the majority including a combination of the following procedures– suboccipital craniectomy, occipitocervical fusion, cervical fusion, odontoidectomy, and tethered cord release. The vast majority of patients experienced subjective symptomatic relief from their series of procedures. Conclusions: EDS patients are prone to instability, especially in the occipital-cervical region, which may predispose these patients to require a higher rate of revision procedures and may require modifications in neurosurgical management that should be further explored.

Using archival data from the 42-foot telescope at the Jodrell Bank Observatory, we produce daily stacks of aligned giant pulses for the Crab pulsar, to study changes to the daily profiles between 2012 April and 2016 December. From these, we identify echoes, where intervening material away from the line of sight causes pulsed emission to be redirected toward the observer, with delay corresponding to the increased distance of travel, resulting in additional profile components. These observations show that such echoes may be far more common than implied by the previous rate of detections. All the observed echoes are consistent with approaching zero delay at their closest approach to the normal giant pulse emission. This indicates that the structures responsible for producing these events must be highly anisotropic, with typical lengths greater than ∼4 au, typical widths on the sky of ∼0.1 au, and typical depths of ∼5 au, given the previously observed electron densities of the nebular filaments, on the order of 1000 cm ^−3 . This suggests that these inhomogeneities are likely to be offshoot substructure from the larger nebular filaments of the Crab nebula.

Patients self-administered 1 drop of latanoprost BAK-free nightly for 36 weeks in the affected eye(s). Intraocular pressure (IOP), visual acuity (VA), and slit lamp biomicroscopy were assessed predose at baseline and Days 28, 56, 84, 112, 140, and 168; dilated ophthalmoscopy and visual field (VF) at baseline and Day 168. Adverse events (AEs) were recorded throughout the study.Results: A total of 161 patients who previously received latanoprost BAK-free (n = 80) or reference (n = 81) were enrolled. Latanoprost BAK-free maintained lowered IOP for both the study and nonstudy eye in all patients relative to baseline throughout the study. Clinically significant retinal or optic nerve changes were identified in 5 patients (1 mild-to-moderate change, prior latanoprost BAK-free; 4 mild changes, prior reference). No clinically important changes were identified for VA, slit lamp biomicroscopy, and VF measurements. Ocular AEs occurred in 66 (82.5%) vs 74 (91.4%) patients on prior latanoprost BAK-free and reference, respectively; the most frequent being eye pain (50.0% vs 64.2%) and ocular hyperemia (47.5% vs 54.3%). Most AEs were mild. There were 5 serious systemic AEs in 5 patients (n = 3, prior latanoprost BAK-free; n = 2, prior reference); all were considered unrelated or not likely related to treatment. One patient (prior reference) discontinued due to follicular conjunctivitis. There were no deaths or serious ocular AEs.Conclusion: Latanoprost BAK-free was well tolerated. Efficient in vivo delivery of anti-inflammatory proteins to modulate the microenvironment of an injured spinal cord and promote neuroprotection and functional recovery is a great challenge. Nucleoside-modified messenger RNA (mRNA) has become a promising new modality that can be utilized for the safe and efficient delivery of therapeutic proteins. Here, we used lipid nanoparticle (LNP)-encapsulated human interleukin-10 (hIL-10)-encoding nucleoside-modified mRNA to induce neuroprotection and functional recovery following rat spinal cord contusion injury. Intralesional administration of hIL-10 mRNA-LNP to rats led to a remarkable reduction of the microglia/macrophage reaction in the injured spinal segment and induced significant functional recovery compared to controls. Furthermore, hIL-10 mRNA treatment induced increased expression in tissue inhibitor of matrix metalloproteinase 1 and ciliary neurotrophic factor levels in the affected spinal segment indicating a time-delayed secondary effect of IL-10 5 d after injection. Our results suggest that treatment with nucleoside-modified mRNAs encoding neuroprotective factors is an effective strategy for spinal cord injury repair.

Abstract Non‐forest ecosystems, dominated by shrubs, grasses and herbaceous plants, provide ecosystem services including carbon sequestration and forage for grazing, and are highly sensitive to climatic changes. Yet these ecosystems are poorly represented in remotely sensed biomass products and are undersampled by in situ monitoring. Current global change threats emphasize the need for new tools to capture biomass change in non‐forest ecosystems at appropriate scales. Here we developed and deployed a new protocol for photogrammetric height using unoccupied aerial vehicle (UAV) images to test its capability for delivering standardized measurements of biomass across a globally distributed field experiment. We assessed whether canopy height inferred from UAV photogrammetry allows the prediction of aboveground biomass (AGB) across low‐stature plant species by conducting 38 photogrammetric surveys over 741 harvested plots to sample 50 species. We found mean canopy height was strongly predictive of AGB across species, with a median adjusted R2 of 0.87 (ranging from 0.46 to 0.99) and median prediction error from leave‐one‐out cross‐validation of 3.9%. Biomass per‐unit‐of‐height was similar within but different among, plant functional types. We found that photogrammetric reconstructions of canopy height were sensitive to wind speed but not sun elevation during surveys. We demonstrated that our photogrammetric approach produced generalizable measurements across growth forms and environmental settings and yielded accuracies as good as those obtained from in situ approaches. We demonstrate that using a standardized approach for UAV photogrammetry can deliver accurate AGB estimates across a wide range of dynamic and heterogeneous ecosystems. Many academic and land management institutions have the technical capacity to deploy these approaches over extents of 1–10 ha−1. Photogrammetric approaches could provide much‐needed information required to calibrate and validate the vegetation models and satellite‐derived biomass products that are essential to understand vulnerable and understudied non‐forested ecosystems around the globe.

Patients with left ventricular assist devices (LVADs) provide a unique challenge with regard to the management of subdural hematomas (SDH), due to preexisting comorbidities and induced coagulopathy. We report on the case of a 63-year-old female with a preexisting LVAD who developed an acute on chronic SDH with 15 mm of midline shift. She was successfully treated with middle meningeal artery (MMA) embolization and placement of a bedside subdural evacuating port system without hematoma recurrence at 1-year follow-up. Both operative and nonoperative management of SDHs in patients with LVAD is associated with high risk of mortality and morbidity. Chronic SDHs in this patient population can be successfully managed with a minimally invasive approach that includes MMA embolization and bedside subdural drain placement.

Comparisons of recent global forest products at higher resolutions that are only available annually have shown major disagreements among forested areas in highly fragmented landscapes. A holistic reductionist framework and colourimetry were applied to create a chorologic typology of environmental indicators to map forest extent with an emphasis on large-scale performance, interpretability/communication, and spatial–temporal scalability. Interpretation keys were created to identify forest and non-forest features, and a set of candidate tree cover indices were developed and compared with a decision matrix of prescribed criteria. The candidate indices were intentionally limited to those applying only the visible and NIR bands to obtain the highest possible resolution and be compatible with commonly available multispectral satellites and higher resolution sensors, including aerial and potentially UAV/drone sensors. A new High-Resolution Tree Cover Index (HRTCI) in combination with the Green band was selected as the best index based on scores from the decision matrix. To further improve the performance of the indices, the chorologic typology included two insolation indices, a water index and a NIR surface saturation index, to exclude any remaining spectrally similar but unrelated land cover features such as agriculture, water, and built-up features using a process of elimination. The approach was applied to the four seasons across a wide range of ecosystems in south-eastern Australia, with and without regionalisation, to identify which season produces the most accurate results for each ecoregion and to assess the potential for mitigating the spatial–temporal scaling effects of the Modifiable Spatio-Temporal Unit Problem. Autumn was found to be the most effective season, yielding overall accuracies of 94.19% for the full extent, 95.79% for the temperate zone, and 95.71% for the arid zone. It produced the greatest spatial agreement between two recognised global products, the GEDI forest heights extent and the ESA WorldCover Tree cover class. The performance, transparency, and scalability of the approach should provide the basis for a framework for globally relatable forest monitoring.

Advances in technology have led to precision livestock management, a developing research field. Precision livestock management has potential to improve sustainable meat production through continuous, real-time tracking which can help livestock managers remotely monitor and enhance animal welfare in extensive rangeland systems. The combination of global positioning systems (GPS) and accessible data transmission gives livestock managers the ability to locate animals in arduous weather, track animal patterns throughout the grazing season, and improve handling practices. Accelerometers fitted to ear tags or collars have the potential to identify behavioral changes through variation in the intensity of movement that can occur during grazing, the onset of disease, parturition or responses to other environmental and management stressors. The ability to remotely detect disease, parturition, or effects of stress, combined with appropriate algorithms and data analysis, can be used to notify livestock managers and expedite response times to bolster animal welfare and productivity. The “Five Freedoms” were developed to help guide the evaluation and impact of management practices on animal welfare. These freedoms and welfare concerns differ between intensive (i.e., feed lot) and extensive (i.e., rangeland) systems. The provisions of the Five Freedoms can be used as a conceptual framework to demonstrate how precision livestock management can be used to improve the welfare of livestock grazing on extensive rangeland systems.

Time-domain diffuse correlation spectroscopy (TD-DCS) offers a novel approach to high-spatial resolution functional brain imaging based on the direct quantification of cerebral blood flow (CBF) changes in response to neural activity. However, the signal-to-noise ratio (SNR) offered by previous TD-DCS instruments remains a challenge to achieving the high temporal resolution needed to resolve perfusion changes during functional measurements. Here we present a next-generation optimized functional TD-DCS system that combines a custom 1,064 nm pulse-shaped, quasi transform-limited, amplified laser source with a high-resolution time-tagging system and superconducting nanowire single-photon detectors (SNSPDs). System characterization and optimization was conducted on homogenous and two-layer intralipid phantoms before performing functional CBF measurements in six human subjects. By acquiring CBF signals at over 5 Hz for a late gate start time of the temporal point spread function (TPSF) at 15 mm source-detector separation, we demonstrate for the first time the measurement of blood flow responses to breath-holding and functional tasks using TD-DCS.

A remote sensing method that integrates virtual sampling from formalized visual interpretations is proposed to facilitate land cover mapping and enhance its accuracy, with an emphasis on spatial and temporal scalability. Indices are widely used for mapping and monitoring surface water across space and time; however, they typically display some kind of limitation across different environments and seasons. A decision matrix framework based on observations derived from interpretation keys was designed to compare the performance of existing indices alongside a set of newly developed indices. This comparison helped to shortlist indices that warranted further evaluation and accuracy assessment to identify effective indices for global inter-seasonal surface water extent mapping. Additional visual inspections were conducted for criteria that remained unresolved by the decision matrix to examine index consistency across the seasons in a wide range of geographic settings around the world, and further reduce the shortlist. An accuracy assessment was performed for three new shortlisted indices. On a global scale, CAWI (Comprehensive Automatic Water Index) was the best-performing index. Its distinct binary data distribution provides the possibility of regional automatic Otsu thresholding. CAWI was determined to be compatible for Sentinel-2 and Landsat 8 sensors, providing the highest possible spatial resolution as well as the longest time series for retrospective analyses with freely available multispectral imagery. Two alternative indices were identified for sensors limited to the visible and NIR bands. The first index, CATWIC (Clear and Turbid Water Index Combination), split the classification of water into two components, with one index for generally clear water and another index for turbid water. The second, NDCHRWI (Normalized Difference Colourimetric High Resolution Water Index), applied the hue angle from a normalized difference RGB. Masking indices based on modified HSV Saturation equations were developed to reduce misclassification due to other high reflectance features. The indices’ overall accuracies, respectively, were: 94.97%, 94.51%, and 94.85%. This study concludes with recommendations for the application of different indices for sensors possessing shortwave infrared bands and for sensors limited to the visible and NIR bands, with a simple stratification of six zones for Global Surface Water monitoring.

Abstract Broad‐scale biodiversity monitoring relies, at least in part, on the efforts of citizen, or community, scientists. To ensure robust inferences from citizen science data, it is important to understand the spatial pattern of sampling effort by citizen scientists and how it deviates from an optimal pattern. Here, we develop a generalized workflow to estimate the optimal distribution of sampling effort for inference of species diversity (e.g., species richness, Shannon diversity, and Simpson's diversity) patterns using the relationship between species diversity and land cover. We used data from the eBird citizen science project that was collected across heterogeneous landscapes in Florida (USA) to illustrate this workflow across different grain sizes. We found that a relatively small number of samples are needed to meet 95% sampling completeness when diversity estimation is focused on dominant species: 43, 64, 96, 123, 172, and 176 for 5 × 5, 10 × 10, 15 × 15, 20 × 20, 25 × 25, and 30 × 30‐km2 grain sizes, respectively. In contrast, three to five times more samples are necessary to infer species diversity when estimation is focused on rare species. However, in both cases, the optimal distribution of effort was spatially heterogeneous, with more effort needed in regions of higher diversity. Our results highlight the potential of citizen science data to make informed comparisons of species diversity in space and time, as well as how sampling effort inherently depends on monitoring goals, such as whether dominant or rare species are targeted. Our general workflow allows for the quantification of sampling effort needed to estimate species diversity with citizen science data and can guide future adaptive sampling by citizen science participants.

Eligible patients were referred to two “consultant” pharmacists, specifically trained to deliver a specialized pharmacy COPD service which involved 3 in-pharmacy visits and 2 follow-up phone calls over a 6-month period. The service consisted of 1) inhaler technique assessment; 2) medication adherence assessment; and 3) referrals to the patient’s general practitioner (GP) to facilitate the uptake of non-pharmacological resources as well as to review COPD medications/devices, as required. Pre-post analyses were conducted using paired Student’s t-test and Wilcoxon Signed Rank Test for independent variables and chi-squared tests for proportional data.Results: Nine “host” pharmacies recruited 40 patients, of whom 37 completed the baseline Visit and 27 completed all Visits. A total of 270 interventions were provided by the “consultant” pharmacists with most provided at Visit 1 (176). The most common interventions were addressing patient gaps in COPD knowledge and inhaler technique. A total of 119 referrals were made to GPs for various reasons, the most common being for a COPD action plan, pulmonary rehabilitation, or pneumonia vaccination. There were significant improvements pre-post intervention in inhaler use competence, COPD knowledge, immunization rate for pneumonia, exacerbation rate and COPD plan ownership.Conclusion: In this pilot study, the specialized pharmacy-based COPD care model delivered by “consultant” pharmacists in community pharmacies provided significant health benefits for patients. Further research is needed to assess the model’s effectiveness in a larger population as well as when measured against standard care.Keywords: inhaler technique, medication review, consultant pharmacist, COPD
