Abstract

PurposeMonitoring distress assessment in cancer patients during the treatment phase is a component of good quality care practice. Yet, there is a dearth of prospective studies examining distress. In an attempt to begin filling this gap and inform clinical practice, we conducted a prospective, longitudinal study examining changes in distress (anxiety, depression, and problems in living) by age and gender and the roles of age and gender in predicting distress.MethodsNewly diagnosed Brazilian cancer patients (N = 548) were assessed at three time points during chemotherapy. Age and gender were identified on the first day of chemotherapy (T1); anxiety, depression, and problems in living were self-reported at T1, the planned midway point (T2), and the last day of chemotherapy (T3).ResultsAt T1, 37 and 17% of patients reported clinically significant levels of anxiety and depression, respectively. 19. Latent phase-type models for Italy's ageing population

Abstract

Quality of care is deemed a concept of immense importance, but also of great difficulty to define and analyse. This study proposes the development of a novel statistical approach to healthcare modelling which overcomes the need to define quality of care by treating it as a hidden layer in a special type of markov model. The study setting for this research is the Italian healthcare system, in particular admissions into geriatric wards of the Lombardy region of Italy during 2009. The Coxian phase-type distribution was applied to this dataset and shown to give the best representation of the flow of patients. Covariates were then incorporated into this distribution and applied to the data. A simulation study of Coxian phase-type distribution with covariates was also undertaken. The main purpose of this research was to develop the theory of the Coxian phase-type distribution by incorporating a hidden layer within it which can represent quality of care. In forming this model novel methodology was presented. A discrete-time and continuous-time version of the model were both applied to the data with the results analysed. A further extension of the continuous-time hidden Markov model with the Coxian phase-type distribution was developed whereby covariates where incorporated into the hidden element. The results of this model, with application to the Lombardy dataset was analysed followed by a simulation study of all the newly developed models presented. In addition to the hidden Markov model with Coxian phase-type distribution the model was extended to introduce a duration component within the hidden layer. This extension formed the hidden semi- Markov model which relaxes the strict Markov assumption. This model was also applied to the Lombardy dataset.

29. The self-presentation of the triumviral aristocracy

Abstract

This thesis analyses the self-presentation of the Roman aristocracy during the triumviral period. Aristocratic self-fashioning has been of great interest to scholars studying both the republic and empire; this study focuses on the transitional period of the civil war and political settlement. The key features of the approach adopted in this thesis are that it focuses on the individuality of the aristocrats, rather than political groupings, and considers their self-presentation as an aspect of the creation of political culture, not merely a response to it. This thesis brings together the evidence for self-presentation in three media: building, speech, and writing. Chapter one establishes the foundation for these studies by reconstructing the careers of two aristocrats, C. Asinius Pollio and L. Munatius Plancus, and analysing the priorities they, and the rest of the triumviral aristocrats, pursued in their careers. Chapter two analyses the corpus of monumental building by the triumviral aristocrats, chiefly those who held triumphs, and demonstrates the way in which they used these structures to advertise their military achievements and their generosity to the Roman people. Chapters three and four argue that the triumviral aristocrats had more opportunities for oratory than has traditionally been alleged, and that they exploited these to pursue their political goals. The talented orators competed with their peers and predecessors in order to establish their fame within the tradition of Latin oratory. Chapter five analyses the outpouring of autobiographical writing after the civil wars, as a means by which the aristocrats sought to promote themselves and justify their careers and actions in the civil wars. The major goal of the triumviral aristocrats in their careers and their self-presentation was to establish and protect their dignitas (reputation or standing). Through the examination of the three media we see the various ways they exploited office, honours, and skill to advertise themselves as traditional republican high-achievers.

Abstract

Diethylene glycol (DEG) is an organic compound that has been found as an adulterant in consumer products as a counterfeit glycerin. Diethylene glycol is metabolized to two primary metabolites: 2-hydroxyethoxyacetic acid (2-HEAA) and diglycolic acid (DGA), the latter shown to accumulate in the kidney and cause dose-dependent cell necrosis. DEG poisonings are characterized predominately by acute kidney injury (AKI) but have also produced delayed neurological sequelae such as sensorimotor neuropathy. To better understand these effects, Wistar-Han rats were orally administered a water control or doses of 4 g/kg-6 g/kg DEG every 12 or 24 h for 7 days, with kidney, brain, and spinal cord tissue collected for histopathological analysis. This dosing paradigm resulted in approximately 25 % of the DEG-treated animals developing AKI and also neurotoxicity (sensorimotor dysfunction and elevated cerebrospinal fluid (CSF) protein). Kidney pathology included a severe, diffuse acute kidney tubular necrosis predominantly affecting proximal convoluted tubules. Scattered birefringent crystals consistent with calcium oxalate monohydrate were also found in the proximal tubule of animals with AKI. Demyelination in the dorsal and lateral white matter regions of the cervical, thoracic, and lumbar areas of the spinal cord of a DEG-treated animal with AKI was documented, establishing the neuropathology in DEG-treated animals that developed neurotoxicity. There were significant changes in amino acid concentrations in the CSF that may reflect the neurotoxicity of DEG, specifically glutamate and glutamine, but with no ammonia change. These studies characterized the pathologic aspects of the neurotoxicity in a DEG repeat-dose model.
