Motivated in part by the bi-gravity approach to massive gravity, we introduce and study the multimetric Finsler geometry. For the case of an arbitrary number of dimensions, we study some general properties of the geometry in terms of its Riemannian ingredients, while in the 2-dimensional case, we derive all the Cartan equations as well as explicitly find the Holmes-Thompson measure., Comment: 27 pages

4. Spectral action approach to higher derivative gravity

We study the spectral action approach to higher derivative gravity. The work focuses on the classical aspects. We derive the complete and simplified form of the purely gravitational action up to the 6-derivative terms. We also derive the equivalent forms of the action, which might prove useful in different applications, namely Riemann- and Weyl-dominated representations. The spectral action provides a rather rigid structure of the higher derivative part of the theory. We discuss the possible consequences of this rigidness. As one of the applications, we check whether the conformal backgrounds are preferred in some way on the classical level, with the conclusion that at this level, there is no obvious reason for such a preference, the space $S^1 \times S^3$ studied in earlier works being a special case. Some other possible properties of the higher derivative gravity given by the spectral action are briefly discussed., Comment: typos and small mistakes corrected, references added, journal version

We derive the Hawking radiation power equations for black holes in asymptotically flat, asymptotically Anti-de Sitter (AdS) and asymptotically de Sitter (dS) black holes, This is done by using the greybody factor for these black holes. We observe that the radiation power equation for asymptotically flat black holes, corresponding to greybody factor at low frequency, depends on both the Hawking temperature and the horizon radius. However, for the greybody factors at asymptotic frequency, it only depends on the Hawking temperature. We also obtain the power equation for asymptotically AdS black holes both below and above the critical frequency. The radiation power equation for at asymptotic frequency is same for both Schwarzschild AdS and Reissner-Nordstr\"om AdS solutions and only depends on the Hawking temperature. In this paper, we will analyze the gravitational collapse in the framework of gravity's rainbow. We will demonstrate that the position of the horizon for a particle inside the black hole depends on the energy of that particle. It will also be observe that the position of the horizon for a particle falling radially into the black hole also depends on its energy. Thus, it is possible for a particle coming from outside to interact with a particle inside the black, and take some information outside the black hole. This is because for both these particles the position of horizon is different. So, even though the particle from inside the black hole is in its own horizon, it is not in the horizon of the particle coming from outside. Introduction/Purpose: The optimal utilisation of echocardiography in intensive care units (ICU) is not yet known; however, its use is becoming more frequent. Management change from transthoracic echocardiography (TTE) in ICU is quoted to be from 3% to 50%. Methods: A retrospective review of clinical practice was performed over a 2‐month period in a tertiary adult ICU, to explore the utilisation of formal TTEs and the findings of these. The rate of management change and critical findings were investigated, along with the indication for TTE and the patient cohort. Results: Sixty‐three TTEs were performed in 54 patients. A change in management occurred in 25.4% (16/63) of TTEs, with critical findings being found in 47.6% (30/63) of all TTEs. The most common indications for formal TTEs were incompletely differentiated or further evaluation of shock, and post arrest cardiac function. Discussion: Almost half of the TTEs performed had critical findings, with common critical findings being severe LV dysfunction, severe RV dysfunction and regional wall motion abnormalities. Despite critical findings being seen frequently, there was only management change in 25%, suggesting that several of the critical findings were already suspected, clinically confirmed or had anticipatory management prior to TTE. Conclusion: Critical findings are common in critically ill patients. However, not all critical findings will lead to a change in management. Formal TTEs in the ICU should be focussed to the clinical question being asked and a screening intensivist performed bedside TTE may be appropriate in certain situations to decrease workload of cardiology department. 17. Peer-Assisted Learning: Filling the Gaps in Basic Science Education for Preclinical Medical Students

- Abstract
In contrast to peer-assisted learning (PAL) in clinical training, there is scant literature on the efficacy of PAL during basic medical sciences teaching for preclinical students. A group of senior medical students aimed to design and deliver clinically oriented small-group tutorials after every module in the preclinical curriculum at a United Kingdom medical school. Twenty tutorials were delivered by senior students throughout the year to first- and second-year students. A baseline questionnaire was delivered to inform the development of the program followed by an end-point questionnaire the next year (n = 122). Quizzes were administered before and after five separate tutorials to assess changes in mean student scores. Additionally, each tutorial was evaluated via a questionnaire for participants (n = 949). All five posttutorial quizzes showed a significant improvement in mean student score (P <0.05). Questionnaires showed students found the program to be relevant and useful for revision purposes and appreciated how tutorials contextualized basic science to clinical medicine. Students appreciated the interactive nature of the sessions and found receiving personalized feedback about their learning and consolidating information with someone familiar with the material to be useful. With the inclusion of the program, students felt there were now an adequate number of tutorials during the year. In conclusion, this study shows that senior medical students can design and deliver a program that adds value to the mostly lecture-based formal preclinical curriculum. We hope that our study can prompt further work to explore the effect of PAL on the teaching of basic sciences during preclinical studies.

Background: Appropriate medication use is essential in ensuring optimal pharmacotherapeutic outcomes. It is mistakenly assumed that adults can swallow solid oral dosage forms (SODFs, e.g. tablets/capsules colloquially referred to as ‘pills’), without difficulty and that children cannot. KidzMed is a ‘pill swallowing’ training programme designed to teach effective SODF use in patients of all ages. It may be utilised by healthcare professionals to assist patients taking SODFs. E-learning was essential for training during COVID pandemic to reduce viral transmission. The aim of this study was to explore UK student pharmacists views of e-learning to support swallowing solid oral dosage forms. Methods: This study used pre- and post-intervention online surveys on Microsoft Forms to evaluate self-directed eLearning about pill swallowing on MPharm programmes at three UK Universities using a 13-item survey. A combination of five-point Likert Scales and free-text items were used. The eLearning was available via the virtual learning environment at the University and embedded within existing curriculum. Descriptive statistical analysis was used to explore responses. Results: In total, 113 of 340 (33%) students completed the survey. Seventy-eight percent (n = 65) reported the eLearning would enable them to teach adults and children to swallow SODFs successfully. Learners either agreed or strongly agreed that they felt comfortable to teach patients (95%, n = 62/113) and parents or carers (94%, n = 60) to swallow medications having completed the e-learning. Student pharmacists generally found eLearning as an acceptable way to reflect on their own experiences of ‘pill’ swallowing and how to support patients to swallow SODFs. Conclusion: The KidzMed eLearning was well received by student pharmacists. Further work is needed to explore whether skills translates into real life application in the clinical settings.

To assess and describe the aetiology and management of febrile illness in children with primary or acquired immunodeficiency at high risk of serious bacterial infection, as seen in emergency departments in tertiary hospitals. Prospective data on demographics, presenting features, investigations, microbiology, management, and outcome of patients within the ‘Biomarker Validation in HR patients’ database in PERFORM, were analysed. Immunocompromised children (N = 492); 94.3% proven/presumed bacterial (N = 164), 66.1% proven/presumed viral (N = 84), and 93.2% unknown bacterial or viral infections (N = 177). Mortality was 1.9% (N = 11) and 87.1% made full recovery (N = 522). Conclusion: The aetiology of febrile illness in immunocompromised children is diverse. In one-third of cases, no cause for the fever will be identified. Justification for standard intravenous antibiotic treatment for every febrile immunocompromised child is debatable, yet effective. Better clinical decision-making tools and new biomarkers are needed for this population. What is Known:• Immunosuppressed children are at high risk for morbidity and mortality of serious bacterial and viral infection, but often present with fever as only clinical symptom.• Current diagnostic measures in this group are not specific to rule out bacterial infection, and positivity rates of microbiological cultures are low. What is New:• Febrile illness and infectious complications remain a significant cause of mortality and morbidity in HR children, yet management is effective.• The aetiology of febrile illness in immunocompromised children is diverse, and development of pathways for early discharge or cessation of intravenous antibiotics is debatable, and requires better clinical decision-making tools and biomarkers.

Nossa meta é estabelecer a conexão entre a ação espectral e maior gravidade derivativa, e mostrar a utilidade do mesmo. A ação espectral tem sido grandemente usada na física de partículas. Porém, estas aplicações na área da gravidade tem permanecido desconhecida até agora. Nesta dissertação nós tentamos aplicar a ação espectral motivada pela comitativa não geométrica para a maior gravidade derivativa e estudamos as equações de movimento vindo das ações gravitacionais contendo maior gravidade derivada, que são derivadas da expansão assintótica usando as técnicas kernel de calor. Nós levamos em consideração o caso da técnica kernel de calor coeficiente 6em grande detalhe e análise em duas bases, uma chamada Riemann e a outra Weyl. Em particular, nós construímos uma ação baseada no 6 em Riemann e Weyl dominando às formas e cálculos das equações de movimento para o mesmo. Nós aplicamos estes resultados em alguns buracos negros assim como nas soluções. Uma análise breve em maior gravidade derivada também é dada para trazer autonomia a dissertação. Nós também discutimos a abordagem da ação espectral com todos os seus detalhes, que é necessário para o nosso propósito. Ainda mais, às ações para os coeficientes de calor 0,2e 4 e equações de movimento correspondente também são avaliadas. We study the spectral action approach to higher derivative gravity for the case of pure gravity. Our goal is to establish a connection between the spectral action and higher derivative gravity, and show the usefulness of the same. The spectral action has been widely used in particle physics. However, its applications in the field of gravitation have remained obscure so far. In this dissertation we attempt to apply the spectral action approach motivated by non-commutative geometry to the higher derivative gravity and study the equations of motion coming from the gravitational actions containing higher derivatives, which are derived from asymptotic expansion using heat kernel techniques. We consider the case of heat kernel coefficient 6in a great detail and analyze it in two bases, namely Riemann one and Weyl. In particular, we construct the action based on 6 in Riemann and Weyl dominated forms and calculate the equations of motion for the same. We apply these results to some black hole and cosmological solutions as well. A brief review on higher derivative gravity is also given to make the dissertation self-contained. We also discuss the spectral action approach with all of its details, which are necessary for our purpose. Moreover, the actions for the heat coefficients 0, 2 and 4 and corresponding equations of motion are also evaluated.

Subarachnoid pneumorrhachis is an increasingly identified entity in the setting of trauma. We report a case of this finding incidentally and of a positive outcome with conservative management following multi-speciality opinion. Our case-series review identifies an increasing number of cases in the literature due to improvements in computerised tomography resolution and explores traumatic and non-traumatic precipitants, suspected mechanisms of introduction of air into the spinal canal and ventricular systems and support for non-interventional management in uncomplicated pneumorrhachis. A case study of a 62-year-old female is presented who reported for elective cryotherapy of right inferior alveolar nerve under general anesthesia. The article discusses surgical emphysema complicating inferior alveolar nerve cryoablation. It notes that the patient was admitted to high dependency unit overnight for close observation and oxygen therapy was administered at 10 liters per minute via Hudson mask.
