27. Anti-VEGF Treatment and Results in a Child with Juxtapapillary Choroidal Neovascular Membrane Secondary to Optic Disc Drusen.

Abstract

A 16-year-old patient was referred to our clinic with the complaint of blurred vision in the right eye. The best-corrected visual acuity (BCVA) was 8/10 in the right eye and 10/10 in the left eye. Fundus examination revealed bilateral slightly elevated optic disc (OD) with blurred margins. Optical coherence tomography (OCT) imaging showed subretinal fluid in the right eye at presentation and in the left eye during the follow-up. The patient was diagnosed with bilateral choroidal neovascular membrane (CNVM) secondary to optic disc drusen (ODD) and treated with intravitreal ranibizumab injections. In control visit on month 1, vision improved and the subretinal fluid was decreased in the right eye. BCVA decreased in the left eye and presented subretinal fluid at the nasal side of the macula. Therefore, bilateral ranibizumab injection was performed by 2-week interval, primarily on the left eye. After 6 month follow-up, BCVA was 10/10 in the right eye and improved to 63/100 in the left eye. [ABSTRACT FROM AUTHOR]

28. The outcomes of intravitreal C3F8 gas tamponade combined with laser photocoagulation treatment for optic disc pit maculopathy.

Abstract

Purpose: To share the anatomical results and visual outcomes of intravitreal gas tamponade combined with laser photocoagulation treatment for optic disc pit maculopathy (ODPM). Methods: Intravitreal gas tamponade combined with laser photocoagulation treatment was performed on six consecutive patients with ODPM. A 0.3 mL of 100% perfluoropropane (C3F8) gas was injected intravitreally. The patients were then asked to maintain prone position until the C3F8 gas disappeared. Laser photocoagulation was performed the day after the procedure. The outcomes were determined by spectral-domain optical coherence tomography and best-corrected visual acuity (BCVA). Results: In the present study, visual improvement and reduction in serous macular detachment were observed in 83% of the ODPM patients. Complete retinal reattachment was achieved in 66% of the ODPM patients. In one patient, no regression was observed after the repeated treatment, and pars plana vitrectomy was performed. The final BCVA improved in five eyes and unchanged in one eye. No postoperative complications were observed during the follow-up period in any patient. Conclusions: Intravitreal C3F8 gas tamponade combined with laser photocoagulation procedure is an effective, minimally invasive, and cost-effective treatment method for ODPM. [ABSTRACT FROM AUTHOR]

31. Dry eye disease and ocular surface characteristics in patients with keratoconus.

Abstract

PURPOSE: The purpose of this study is to investigate the ocular surface alterations in patients with mild or severe keratoconus (KC). METHODS: A total of 80 participants were included in the study. The corneal topography was performed on each participant using Pentacam and the grouping was done accordingly. The patients with Kmax =52.0 D (severe KC) were considered Group 1 (n = 28), the patients with Kmax =47.2 and <52.0 D (mild KC) were considered Group 2 (n = 30). Healthy control participants with Kmax <47.2 D were considered Group 3 (n = 22). Tear breakup time (TBUT), Schirmer-I test, ocular surface disease index (OSDI) questionnaire, and conjunctival impression cytology (CIC) were evaluated among the groups. RESULTS: The mean values of TBUT and Schirmer-I test were significantly lower (P = 0.012, P = 0.012) and the mean scores of OSDI and CIC were significantly higher (P = 0.006, P < 0.001) in Group 1 and Group 2 than in Group 3. The mean values of TBUT and Schirmer-I test were lower and the mean scores of OSDI and CIC were higher in Group 1 than in Group 2 but the differences were insignificant (P > 0.05 for all). CONCLUSION: These results indicated that the tests associated with dry eye disease are correlated with KC. Tear film alterations and goblet cell loss are higher in severe KC. [ABSTRACT FROM AUTHOR]

Abstract

VR is a rare condition that causes a sudden vision loss. Although VR is considered to be rare but a serious complication of surgery. Ourcase was a 64 years old woman was presented with preretinal haemorrhage including macular area in right eye secondary to VR afteran uneventful cataract surgery under general anesthesia. Her vision was counting fi ngers after the surgery. Although surgical correctionor Nd:YAG laser hyaloidotomy was available she was treated with single intravitreal TPA, intravitreal bevacizumab and 0.2cc SF6 gaseinjection. There was signifi cant reduction of premacular haemorrhage and her vision improved to 7/10 after injection on the fi nal visit., Valsalva retinopatisi (VR) ani görme kaybına neden olabilen nadir bir durumdur. VR nadir görülmekle birlikte önemli bir cerrahikomplikasyondur. 64 yaşında bayan hastada komplikasyonsuz katarakt cerrahisi sonrası VR’ne bağlı makuler alanı içine alan preretinalkanama saptanmıştır. Cerrahi sonrası hastanın görme seviyesi el hareketi düzeyindedir. Cerrahi tedavi veya Nd:YAG lazer hyaloidotomimümkün olsa da hastamız intravitreal TPA, intravitreal bevacizumab ve 0.2cc SF6 gaz injeksiyonu ile tedavi edilmiştir.Son muayenedepremakuler kanamada gerileme saptanmış ve hastanın görmesi 7/10 düzeyine yükselmiştir.

Abstract

Purpose: To evaluate the effect of silicone oil (SiO) on retinal nerve fi ber layer thickness (RNFLT) by using optical coherence tomog-raphy in patients who underwent pars plana vitrectomy and SiO injection for retinal detachment.Material and Methods: A total of 50 eyes of 50 patients were retrospectively enrolled in this study. 34 patients (68%) were male,16patients (32%) were female. SiO was injected in 50 eyes and the healty fellow eyes (50 eyes) were evaluated as controls. RNFLT mea-surements were compared with the control eyes,one and three months after PPV and one month after SiO removal.Results: In siliconized eyes the temporal, inferotemporal,nasal,inferonasal quadrants and global measurements were signifi cantly thi-cker at fi rst month after PPV compared to controls (p0.001,p0.001,p0.044,p0.002,p0.001 respectively). Temporal quadrantand global measurements were signifi cantly thicker at third month after PPV (p0.001,p0.034). At fi rst month after SiO removal, thetemporal, nasal quadrants, global measurements were signifi cantly thicker (p0.004,p0.01,p0.018). Also siliconized eyes RNFLTmeasurements were evaluated between fi rst, second and third visit separately. Almost there was no signifi cant difference in quadrantsfor the measurements of the subsequent visits.Conclusions: A transient increase in RNFLT in some of the quadrants compared to the control eyes was found in the study due to thepresence of SiO in the globe., Amaç: Retina dekolmanı nedeniyle pars plana vitrektomi yapılıp silikon yağı tamponadı verilen hastalarda silikon yağının retinal sinirlifi tabakası (RSLT) kalınlığına etkisinin optik koherens tomagrafi ile değerlendirilmesi.Materyal ve Metod: 50 hastanın 50 gözü çalışmaya katıldı. 34 hasta (%68) erkek,16 hasta (%32) kadındı. 50 göze silikon verildi vediğer sağlam gözler (50 göz) kontrol olarak değerlendirildi. Silikonize gözlerin PPV’den bir ay ve üç ay sonra ve silikon alımından biray sonraki RSLT ölçümleri kontrol gözlerin ölçümleriyle kıyaslandı.Bulgular: Silikonize gözlerin PPV’den bir ay sonraki RSLT ölçümlerinde kontrol gözlere kıyasla temporal,inferotemporal,nazal,infe-ronazal kadranlarının ve global olarak istatistiksel olarak anlamlı kalın olduğu görüldü (p0.001,p0.001,p0.044,p0.002,p0.001 sırasıyla). PPV’den üç ay sonraki ölçümlerde global olarak ve özellikle temporal kadranın istatistiksel olarak anlamlı kalın olduğugörüldü (p0.034,p0.001). Silikon alımından bir ay sonra temporal,nazal kadranın ve global olarak RSLT ölçümlerinin anlamlı kalınolduğu görüldü (p0.004,p0.01,p0.018). Birinci,ikinci ve üçüncü vizitte yapılan silikonize gözlerin RSLT ölçümleri ayrıca kendiiçlerinde de değerlendirildi ve ayrı ayrı kıyaslandığında istatisitksel olarak anlamlı kalınlık farklılığı olmadığı görüldü.Sonuç: Silikonun göz içinde bulunduğu süre içerisinde bazı kadranlarda kısa süreli olsa da kontrol gözlere kıyasla kalınlık artışı olduğugörülmüştür.

Abstract

Objective: To evaluate monocyte/high-density lipoprotein cholesterol ratio (MHR) and neutrophil/lymphocyte ratio (NLR) in patients with keratoconus (KC).Methods: Thirty-one patients with KC (group 1) and 31 healthy control subjects (group 2) were included in the study. All participants' ocular examination findings, clinical and laboratory parameters were obtained from file records and laboratory archives.Results: When group 1 was compared with group 2, MHR (13.7 ± 5.0 vs. 9.1 ± 3.7; p < .001) and NLR (2.3 ± 0.8 vs. 1.7 ± 0.6; p < .001) were statistically significantly higher in group 1. Optimal MHR cutoff value for KC was calculated as 10.2 with 77.4% sensitivity and 64.5% specificity, and optimal NLR cut-off value for KC was found as 1.9 with 71.4% sensitivity and 55% specificity.Conclusion: MHR and NLR values recognized as indicators of oxidative stress and systemic inflammation were significantly higher in patients with KC compared to the control group. Abstract

Purpose: The purpose of this article was to evaluate monocyte count and high-density lipoprotein (HDL) cholesterol levels and their ratio (monocyte/HDL ratio [MHR]) in patients with branch retinal vein occlusion (BRVO).Materials and Methods: A total of 50 participants were included in the study. Participants were divided into two groups: Twenty-five patients diagnosed with BRVO and 25 participants as the control group. Ocular examination findings and hematologic parameters were accessed from the file records and database, retrospectively.Results: The mean MHR was significantly higher in BRVO group compared to the control group (13.4 ± 5.2 vs. 8.1 ± 2.2, p < 0.001). In receiver operating characteristics analysis, the area under the curve for MHR was 0.862, and an MHR of >9.5 predicted BRVO with a sensitivity of 76% and specificity of 70.8%.Conclusion: The present study showed that elevated MHR is significantly associated with BRVO. Therefore, MHR may be a useful marker for the emergence of BRVO. [ABSTRACT FROM AUTHOR]

Abstract

Yüksek miyop hastalarda, optik koherens tomografi (OKT) ile görülen posterior stafi lom içerisindeki makülanın konveks protrüzyonu Gaucher ve arkadaşları tarafından Kubbe şekilli maküla (Dome Shaped Makula - KŞM) olarak tanımlanmıştır. KŞM'ya seröz retina dekolmanı (SRD), retina pigment epitel (RPE) düzensizlikleri veya koroidal neovaskülarizasyon (KNV) gibi görme keskinliğinin azalmasından sorumlu olan bazı komplikasyonlar eşlik edebilmektedir. Eşlik eden bu komplikasyonlara yönelik görüş birliğine varılmış bir tedavi şekli bulunmamaktadır.KŞM'ya eşlik eden SRD nedeniyle birkaç yıldır sağ gözde görme azlığı olan 51 yaşındaki kadın olgu kliniğimizde takip edilmekteydi. En iyi düzeltilmiş görme keskinliği (EİDGK) sağ gözde 6/10, sol gözde 10/10 düzeyindeydi. Yaklaşık 1 yıllık takibinde EİDGK'nin SRD'a rağmen stabil kaldığı gözlendi ancak 18. ayda subretinal sıvıda artış ve EİDGK'nde azalma meydana geldi. Bu nedenle hastanın sağ gözüne 1 ay ara ile 2 doz intravitreal Anti-VEGF (ranibizumab) enjeksiyonu yapıldı. Sonraki kontrollerinde enjeksiyona yanıt alınamadığı görüldü. Olgu ek müdahale yapılmadan yakın takip edildi. Son enjeksiyondan yaklaşık 8 ay sonra optik koherens tomografi (OKT) tetkikinde foveadaki SRD'nın spontan olarak yatıştığı ve sağ gözde EİDGK'nin 7/10 düzeyine çıktığı görüldü. SRD'nın spontan regresyonunu takiben hasta 6 ay aynı görme düzeyi ve nüks olmadan takip edildi. Bu olgu sunumunda SRD'nın eşlik ettiği komplike KŞM'lı bir hastanın yaklaşık 3 yıllık takibi sonucunda SRD'nın kendiliğinden gerilediği sürecin paylaşılması amaçlanmıştır, In highly myopic patients, convex protrusion of the macula within a posterior staphyloma, diagnosed by optical coherence tomography (OCT), was described as dome shaped macula (DSM) by Gaucher et al. DSM is often associated with several complications such as serous retinal detachment (SRD), retinal pigment epithelium (RPE) abnormalities, choroidal neovascularization (CNV), all responsible for visual loss. To date it has not been possible to establish effective treatment of this complications.A 51 years old female presented to our department because of blurred vision in her right eye (OD) in a few years and followed for DSM associated with foveal SRD. The best corrected visual acuity (BCVA) was 6/10 in her OD and 10/10 in her OS. After one year of follow up, BCVA of her OD and SRD remained stable. In 18th month, visual loss in her OD and progression of subretinal fl uid occured. The intravitreal antiangiogenic drug ranibizumab was injected two times a month in her OD. Ranibizumab injection has failed to show effi cacy in follow up period and therefore patient was observed without any treatment. After 8 months of ranibizumab injection BCVA of her OD was 7/10, subretinal fl uid was resorbed and the retina reattached spontaneously and it was confi rmed by optical coherence tomography (OCT). After spontaneous regression of SRD the patient followed about 6 months with same BCVA and any recurrence of SRD. In this case report we aimed to show spontaneous regression of DSM associated with foveal SRD in a patient followed up 3 years

Abstract

Amaç: Tek taraflı strabismik ve anizometropik ambliyopisi olan çocuklarda peripapiller retina sinir lifi tabakası (RSLT) kalınlığının optikkoherans tomografi (OKT) ile değerlendirilmesi.Gereç ve Yöntem: Tek taraflı ambliyopisi olan 50 olgu (26 erkek, 24 kadın; ortalama yaş 9,02,0 yıl) ve 25 sağlıklı olgu (14 erkek, 11 kadın; ortalama yaş 8,91,9 yıl) çalışmaya dahil edildi. Ambliyopisi olan 50 olgunun 25'inde (%50,0) strabismik ambliyopi, 25'inde (%50,0) hipermetropik anizometropik ambliyopi mevcuttu. Her olguya detaylı oftalmolojik muayene, aksiyel uzunluk ölçümü ve Spectralis OKT(Heidelberg, Germany) kullanılarak RSLT kalınlığı ölçümü yapıldı. Ambliyopik gözlerden elde edilen ölçümler olguların diğer gözleri ile vekontrol grubu ile karşılaştırıldı. Yaş, cinsiyet, kırma kusuru ve en iyi düzeltilmiş görme keskinliği değerleri de kaydedildi.Bulgular: Ambliyopik gözlerde ortalama RSLT kalınlığı (105,312,0 µm) olguların diğer gözlerine göre (102,610,9 µm) anlamlı olarakartmış bulundu (p0,001). Anizometropik ambliyopisi olan olgularda ambliyopik göz ve diğer gözden elde edilen ortalama RSLT kalınlıklarıarasında anlamlı farklılık mevcuttu (p0,001), ancak strabismik ambliyopisi olan olgularda anlamlı fark izlenmedi (p0,925). Sağlıklı kontrololgularında ortalama RSLT kalınlığı (102,29,1 µm) ile ambliyop gözlerdeki ve ambliyop olguların diğer gözlerindeki RSLT kalınlığı arasında anlamlı fark yoktu (sırasıyla p0,499 ve p0,988). Ambliyop olguların her iki gözünde ortalama RSLT kalınlığı ile aksiyel uzunluk arasında negatif korelasyon saptanırken kontrol grubunda herhangi bir korelasyon izlenmedi.Sonuç: Anizometropik ambliyopisi olan olgularda ambliyopik gözlerdeki artmış RSLT kalınlığı ambliyopide organik değişikliklerin olabileceğini düşündürmektedir. Ancak, bu varsayımı desteklemek için daha fazla sayıda hasta içeren ileri çalışmalara gereksinim vardır., Objective: To evaluate peripapillary retinal nevre fiber layer (RNFL) thickness in children with unilateral strabismic and anisometropic amblyopia using optical coherence tomography (OCT). Material and Method: Fifty patients with unilateral amblyopia (26 male, 24 female; mean age 9.0±2.0 years) and 25 healthy control subjects (14 male, 11 female; mean age 8.9±1.9 years) were enrolled in the study. Of the 50 children with amblyopia, 25 (50.0%) had strabismic amblyopia and 25 (50.0%) had hyperopic anisometropic amblyopia. Each subject underwent a detailed ophthalmologic examination, axial length measurement and RNFL thickness measurement with Spectralis OCT® (Heidelberg, Germany). The results of the amblyopic eyes were compared with the fellow eyes of the subjects and control eyes. Age, sex, refractive error, and best-corrected visual acuity were also recorded. Results: The average RNFL thickness in amblyopic eyes (105.3±12.0 µm) was significantly greater than in fellow eyes (102.6±10.9 µm) (p>0.001). In subjects with anisometropic amblyopia, the difference in the average RNFL thickness between amblyopic eyes and fellow eyes was also statistically significant (p>0.001). However, no significant difference was observed between amblyopic eyes and fellow eyes in subjects with strabismic amblyopia (p0.925). The average RNFL thickness of the healthy control subjects (102.2±9.1 µm) did not differ significantly compared with amblyopic eyes (p0.499) and fellow eyes (p0.988). Axial length was negatively correlated with average RNFL thickness in amblyopic eyes and fellow eyes but not in control eyes. Conclusion: The increased RNFL thickness of the amblyopic eyes in patients with anisometropic amblyopia may suggest the existence of organic changes in amblyopia. However, further studies involving more subjects are warranted to support this hypothesis.

Amaç: İntraoküler silikon yağı (SiY) verilerek başarılı pars plana vitrektomi (PPV) ileregmatojen retina dekolmanı (RD) onarımı yapılan hastalarda peripapiller retina sinir lifitabakası (RSLT) kalınlığının optik koherens tomografi (OKT) ile değerlendirilmesi.Gereç ve Yöntem: Toplam 50 hastanın 50 gözü çalışmaya dahil edildi. Hastaların % 68'i(n=34) erkek, % 32'si (n=16) kadındı. 1000 centistokes (cst) SiY (n=37) ve ağır SiY(n=13) olmak üzere iki tip SiY uygulandı ve hastaların diğer sağlam gözü kontrol olarakdeğerlendirildi. Tüm olgularda Spectralis® OKT cihazı kullanılarak peripapiller RSLTkalınlığı PPV sonrası 2. haftadaki, PPV sonrası 3. aydaki ve SiY alındıktan sonra 1.aydaki alınmış ölçümleri değerlendirildi. Peripapiller RSLT kalınlıkları kadranlar halinde3 ölçüm arasında, kontrol gözleri ile ve iki tip SiY arasında karşılaştırıldı.Bulgular: Silikonize gözlerin RSLT ölçümleri incelendiğinde PPV sonrası 3. aydakiölçümlerde genel olarak PPV sonrası 2. haftadaki ilk ölçümlere kıyasla kalınlık artışı sözkonusudur. Fakat bu kalınlık artışı global olarak (p=0,011) ve yalnızca nazal kadranda(p=0,004) istatistiksel olarak anlamlıdır. SiY alımı sonrası 1. aydaki ölçümde ise bu farkistatistiksel olarak anlamını kaybetmiştir. PPV sonrası 3. aydaki ve SiY alımı sonrası 1.aydaki ölçümler kıyaslandığında, ayrıca PPV sonrası 2. haftadaki ölçümler ile SiY alımısonrası 1. aydaki ölçümler kıyaslandığında RSLT kalınlıklarında istatistiksel olarakanlamlı fark bulunmamıştır. Abstract

Purpose: To evaluate the association between monocyte count/high-density lipoprotein (HDL) ratio (MHR) and lymphocyte count/monocyte count ratio (LMR) with pseudoexfoliation syndrome (PEXS) and pseudoexfoliation glaucoma (PEXG).Materials and Methods: A total of 63 participants included in the study. Participants were divided into three groups. Twenty-one patients with PEXS regarded as group 1, 21 patients with PEXG regarded as group 2 and 21 participants without PEXS or PEXG regarded as the control group. Blood parameters were accessed from file records and database retrospectively.Results: The mean MHRs were significantly higher in group 1 and group 2 (p = 0.003, p = 0.036) than the control group, whereas there was no difference between group 1 and group 2 (p = 0.686). The mean LMRs were lower in group 1 and group 2 than the control group but the difference was insignificant (p = 0.232).Conclusion: We found that there is an association between higher MHR and lower LMR with PEXS. Abstract

Purpose: Evaluation of macular retinal nerve fiber layer (RNFL), ganglion cell layer (GCL), and ganglion cell complex (GCC) in pseudoexfoliation syndrome (PES) by spectral domain-optical coherence tomography (SD-OCT). Material and methods: One eye of 30 patients with PES and one eye of 30 healthy subjects were included in the study. The participants' macular screening was performed in 9 quadrants by SD-OCT according to the early treatment diabetic retinopathy study (ETDRS). Retinal layers were automatically divided into segments using SD-OCT's own software and analyzed separately. Results: Statistically thinning was observed in the perifoveal nasal quadrant in RNFL (p = 0.035); in the perifoveal nasal (P = 0.041) and inferior quadrants (p = 0.034) in GCL and in the perifoveal nasal quadrant (p = 0.035) in GCC of the PES group when compared to the control group. There was no significant difference between the two groups in the inner plexiform layer (p<0.05). Conclusion: Although intraocular pressure is normal in patients with PES, retinal ganglion cell damage may develop. The evaluation of RNFL, GCL, and GCC with SD-OCT in patients with PES may help in the early diagnosis of preperimetric glaucomatous damage. [ABSTRACT FROM AUTHOR]

Abstract

Objective: This study aimed to evaluate the frequency and primary causes of blindness in adults aged 18 years and more in Sarıkaya rural region of Yozgat, Turkey to contribute to the epidemiologic information about blindness in our country. Materials and Methods: Patients who were examined between October 2016 and March 2017 in Sarıkaya State Hospital, Clinic of Ophthalmology were prospectively investigated. Demographic and ophthalmic examination data, presented visual acuities (VA), primary causes of blindness, and monocular blindness were recorded. Blindness was defined as presented visual acuities (VA) definition of World Health Organization criteria. Results: A total of 3423 participants, aged 18-96 years, were examined. Among them, 1887 participants (55%) were female and 1536 (45%) were male. The frequency of blindness was 1.5% (95% CI: 1.1%-2.0%); and cataract was the primary cause of blindness (42%). Age-related macular degeneration (21%) and uncorrected refractive error (13%) were the next main causes of blindness. The frequency of monocular blindness was 4% (95% CI: 3.8%-5.2%), and cataract (27%) followed by phthisis bulbi/evisceration (13%) and glaucoma (12%) were the leading causes of monocular blindness. Conclusion: In this rural region of Central Anatolia, the primary cause of blindness and monocular blindness was an unoperated cataract. The patterns of age-specific causes of blindness are variable, but most of them are avoidable or treatable. Thus, awareness should be increased in societies, and people should be prevented from blindness with health-care programs in rural regions of developing countries. [ABSTRACT FROM AUTHOR]

Abstract

The pupillary light reflex could serve as a valuable method for measuring dynamic responses in the autonomic nervous system (ANS). However, it remains unclear whether physiological conditions such as fasting and dehydration affect pupillary reflexes. In this study, we investigated the effects of fasting and dehydration on pupillary responses using pupillometry. Measurements were taken after at least 12 h of Ramadan fasting (RF) and under a normal dietary pattern (NDP). Forty-nine volunteers, who had no complaints related to the ANS, participated in the study. The initial pupil diameter (IPD), amplitude of contraction, contraction velocity, contraction latency, dilatation duration, and dilation velocity were recorded. The IPD was 5.91 ± 0.65 mm in the RF condition and 5.72 ± 0.65 mm in the NDP condition (p < 0.001). Both contraction amplitude (CA) and dilatation velocity (DV) were higher in the RF condition (p = 0.010 and p = 0.022, respectively). Females exhibited a greater IPD in the NDP condition (p = 0.023). Differences between genders in the RF condition were observed in CA (p = 0.002), dilation duration (p = 0.016) and dilation latency (p = 0.041). These findings indicate that fasting and dehydration influence IPD, CA and DV.
