Purpose: To compare the efficacy of chlorhexidine, Persica mouthwash and electric toothbrushes in improving gingival enlargement in patients with fixed orthodontic appliances. Materials and Methods: Seventy-two orthodontic patients with at least two sites of gingival enlargement were randomly allocated into four equal groups: 1) manual toothbrush; 2) electric toothbrush; 3) manual toothbrush + Persica mouthwash; 4) manual toothbrush + chlorhexidine mouthwash. All participants were instructed to brush their teeth at least twice a day. The subjects in groups 3 and 4 were instructed to use Persica or chlorhexidine according to the respective manufacturer's instructions. Bleeding on probing (BOP) index, gingival index (GI), O'Leary's plaque index (PI) and constructed hyperplastic index (HI) of all the subjects were measured in a blind manner at the start of the study and 2 weeks later. Changes of indices in the entire oral cavity and individual affected teeth were analysed with SPSS 16 using chi-square, ANOVA, ANCOVA, LSD and the paired t-test. Results: In the entire oral cavity, there was a statistically significant improvement in indices in all the groups except for HI, which significantly improved only in group 4 (p = 0.001). Data of individual teeth with hyperplastic gingiva showed significant reduction of all the variables except for HI in group 1 (p = 0.08). No significant differences were found between groups 1 and 2 or between groups 3 and 4. Conclusion: The efficacy of Persica was similar to that of chlorhexidine in improving gingival conditions. None of the treatment modalities could reduce gingival enlargement to the clinically acceptable level of health. [ABSTRACT FROM AUTHOR]

This article reports on a patient with severe maxillary and mandibular crowding treated by both expansion and extraction treatment. Two surgical procedures, surgically assisted palatal expansion and a mandibular midline osteotomy, were performed in sequence for skeletal expansion. A hybrid appliance, consisting of a Hyrax screw and 2 miniplates, was used in the mandible. Since expansion was not enough for relief of the crowding, 3 premolars were later extracted. Objectives: Bone remodeling occurs during orthodontic treatment; this process enables tooth movement. Many factors can affect bone remodeling at the cellular level, such as nutritional supplements that can affect tooth movement. The present study was designed to evaluate the effect of dietary vitamin C on orthodontic tooth movement in rats. Materials and Methods: This study was carried out on 36 six-week male Wistar rats with a mean weight of 225±32 g, which were randomly allocated to two equal groups. Rats in the case group received 1wt% vitamin C in their daily water. Opening springs were placed on the incisor teeth of both case and control groups. After 17 days, rats were sacrificed; the distance between the mesio-incisal angles of these teeth was measured with a digital caliper. Histological sections were made containing incisor teeth and alveolar bone and stained by hematoxylin-eosin. The number of resorption lacunae was evaluated using light microscopy. Results: Our findings showed that the amount of tooth movement in the vitamin C group was significantly higher than that in the control group (P<0.001). The osteoclast counts were significantly higher in vitamin C group (P=0.036). Conclusion: Oral vitamin C can increase orthodontic tooth movement in rats with more osteoclast lacunae around root in the pressure area. [ABSTRACT FROM AUTHOR]

Therefore, it is beneficial to accelerate bone formation in the expanding midpalatal suture to reduce relapse. This study was designed to evaluate the effect of dietary vitamin C on osteogenesis of rat midpalatal suture during expansion. Materials and Methods: Fifty-four male Wistar rats were randomly divided into three groups, each with a control and an experimental subgroup. An open-loop spring was bonded to maxillary incisors of each animal to expand the premaxillary suture. Experimental groups received dietary vitamin C in their water. The rats in the three groups were sacrificed at three, nine or 17-day intervals after bonding the spring. Then, the premaxilla was dissected and sections were made and stained with hematoxylin and eosin and osteopontin marker. Osteoblasts and osteoclasts were counted in the suture. Two-way ANOVA and the Mann-Whitney-U test were used for analyzing the data. Results: After three days, the number of osteoblasts was significantly higher in the vitamin C group but after nine days it was significantly higher in the control group and after seventeen days there were no significant differences between the groups. Osteoclast counts were not significantly different between vitamin C and control groups. Conclusion: Vitamin C had a positive effect on osteogenesis at the beginning of bone formation in the expanding suture, but after nine days it had a negative effect on suture osteogenesis in rats. Materials and Methods: Fifty caries-free, intact human premolars were randomly assigned to one of the following five equal groups: Groups 1 (control) and 2: The brackets were bonded using conventional halogen light for 40s and argon laser for 10s, respectively. Teeth in group 3 were lased with argon laser for 10s before bracket bonding with halogen light. Group 4 was the same as group 3 except that brackets were also bonded with argon laser. In group 5 samples were bonded conventionally, immersed in an artificial caries solution for two days and then irradiated for 10s with argon laser. All samples were subjected to demineralization by artificial caries solution for 10 days. After bracket removal, samples were buccolingually sectioned and evaluated by polarized light microscopy. Decalcified lesion depth in each section was measured by a trained examiner in a blind fashion. Data were analyzed in SPSS 14 using one-way ANOVA and Tukey's HSD post hoc test. Results: The control group showed the greatest mean lesion depth while group 5 revealed the lowest. The laser-treated groups had significantly lower mean lesion depth compared with the control group (P<0.05) except for group 4 (P=0.192). Objective: This study aims to investigate the immediate effect of straw phonation on the phonation of Persian-speaking children with repaired cleft palate., Study Design: Quasi-experimental preintervention and postintervention., Methods: Seventeen children with repaired cleft palate and velopharyngeal dysfunction were investigated. A control group was established comprising children without a cleft palate (control group), carefully matched in terms of age and gender. All participants underwent straw phonation and assessment. The assessments were made two times: at baseline and immediately after straw phonation. Each participant performs straw phonation (a short straw measuring 3 mm in inner diameter and 20 cm in length) once for 3 minutes. The acoustic analysis including parameters, such as jitter, shimmer, harmonics-to-noise ratio (HNR), cepstral peak prominence (CPP) parameters, as well as the electroglottography (closed quotient [CQ]) analysis were performed at pretreatment and immediately after treatment., Results: Compared with the pretreatment values, after-treatment observation demonstrated a significant reduction in Jitter % and Shimmer %, and a significant enhancement in HNR and CPP among children with repaired cleft palate. There is no significant difference in intragroup data in the CPP and CQ in pretreatment., Conclusions: The proposed straw phonation technique results in an immediate positive change in the quality of voice in both groups. Background: This study aimed to compare the pharyngeal airway volume in class I,II and III skeletal malocclusion patients using cone beam computed tomography (CBCT)., Material and Methods: This retrospective, cross sectional study was conducted on lateral cephalograms of 71 patients derived from their CBCT scans. Using the ANB angle, the patients were divided into class I,II and III malocclusion. Two observers used Dolphin 3D software to calculate the pharyngeal airway volume, airway area, minimum axial area, minimum area location, airway length and morphology. Data were analyzed using one-way ANOVA, Kruskal-Wallis test, Tukey's test, Spearman's correlation coefficient and multiple regression analysis., Results: The three skeletal classes were significantly different in airway volume, minimum axial area, mean airway area and airway morphology ( P <0.05). Significant differences were found in airway volume and mean airway area between class II and III patients ( P <0.05). The minimum axial area and airway morphology in class III patients were greater than those in class I and II patients ( P <0.05). Every one unit increase in the ANB angle decreased the airway volume by 0.261 units. The effect of ANB angle on airway volume was statistically significant and it was shown that one unit increase in the angle decreased the airway volume by 453.509 units., Conclusions: A significant correlation exists between the skeletal facial pattern and upper airway dimensions. In our study, the total airway volume and the mean airway area of class III patients were larger than those in class II patients. Objectives: This study aimed to assess the effect of head position on the accuracy of transverse measurements of the maxillofacial region on CBCT and conventional posteroanterior (PA) cephalograms. The second objective of this study was to find skull positions with the greatest and smallest effect on transverse measurements in the maxillofacial region., Methods: PA cephalograms and CBCT scans were obtained from 10 dry human skulls in 7 positions, namely the central position, 10° and 20° rotations, 10° and 20° tilts and 10° and 20° tips. The CBCT scans were converted to PA cephalograms on which distances from six landmarks, namely the nasal cavity, zygomatic arch, jugale, antegonion, condylion and zygomaticofrontal suture to the mid-sagittal plane, were measured on both sides using Dolphin two-dimensional software. The paired t-test was used to compare the mean values separately in each position (for each landmark) with the gold standard (central skull position). The interclass correlation coefficient and the Bland-Altman plot were used to compare the mean values measured by two observers., Results: The mean values of the distances measured on CBCT PA cephalograms were greater than those measured on conventional PA cephalograms; this difference was statistically significant for some landmarks (p < 0.005). The rotated position (as compared with the central position) caused the greatest change in values for most landmarks on both sides (p < 0.005)., Conclusions: The CBCT PA cephalogram was more accurate than the conventional PA cephalogram, and landmarks farther from the midline exhibited greater changes on cephalograms compared with those closer to the midline. Patients are at risk of improper positioning when undergoing extraoral radiography such as PA cephalograms. Changes in head position may affect the transverse measurements and thus the treatment plan.

Article Incorrect patient positioning and the resultant image distortion occur in many radiographic examinations. This study aimed to assess the effect of tilting, rotation and tipping of the head on the accuracy of maxillofacial transverse measurements made on cone beam computed tomography (CBCT) scans. The CBCT scans were obtained in central position of 10 dry human skulls. Using three-dimensional (3D) Dolphin software, six positions namely 10° and 20° tilts, rotations and tips were reconstructed of central position. Transverse distances between landmarks were measured on 3D scans and the skulls. The intraclass correlation coefficient (ICC) and the Bland-Altman plot were used to compare the mean values measured by the two observers. For most landmarks, no significant differences were noted between the mean transverse distances measured in the six positions and those measured in central position on 3D scans (p>0.005). In conclusion, all measurements of transverse distances made on 3D scans in different positions were underestimated compared to the actual values measured on the skull.

Replacement of the impacted maxillary canines with implants is a great challenge for all dentists. Adequate implant space, sufficient space between the roots, alveolar bone bed, thin labialgingiva and the guiding role of canine in a mutually protected occlusion make the choice of implant position with proper diameter more difficult. In this article we present a patient whose upper canines were replaced with implants after surgical removal of impacted teeth. There was inadequate space for prosthetic crown over implant fixtures.
