A propòsit del quarantè aniversari de Documents d’Anàlisi Geogràfica (DAG) (1982- 2022), i considerant la unificació de les tres sèries anteriors —Documents d’Anàlisi Urbana, Documents d’Anàlisi Territorial i Documents d’Anàlisi Metodològica en Geografia (del 1974 al 1981)—, es pretén reflexionar sobre les aportacions científiques de la revista al debat acadèmic en geografia. Nascuda amb el compromís de tirar endavant una revista de qualitat, innovadora, oberta i crítica que introduís els avenços teòrics, conceptuals i metodològics de la disciplina, DAG ha contribuït a la transformació de la geografia i ha mantingut en bona mesura el tarannà crític que va caracteritzar les publicacions dels primers anys. Tot i això, es detecten mancances en alguns debats que han marcat la nostra societat i abordar en profunditat alguns camps disciplinaris i nous temes d’estudi.

After the Sorbonne Declaration, the idea of a European Higher Education Area was launched, and universities have been pushed into a restructuring process with a strong emphasis on quality and excellence. In Spain, the focus on graduate employability impelled academics and professionals towards the formulation of a White Book on the state and the future of Geography in Higher Education (2004). This has been a useful tool concerning the general debate on the design of new university degrees within European regulations. This was the case at the department of Geography at the Autonomous University of Barcelona (UAB), where a three-year pilot program was undertaken, aiming to test the Bologna system before setting up and validating the new degrees. The outcomes of the White Book and of the pilot experience, proved to be very useful. The academic year 2009-10 began with a new Bachelors degree course in Geography and Regional Planning, a Spanish adaptation to European (Bologna) regulations. Although the short time that has elapsed since then makes it difficult to undertake a full evaluation of the process and reflect on its outcomes and impacts, the whole issue of reform is raising controversy leading to the reluctance to initiate further change, due mainly to the lack of internal debate about the process of change that apparently seems to be a response to global economic demands rather than academic ones. This paper analyzes the impact of the process in Spain, and specifically, in the Geography degree at UAB.

Este artículo analiza las tesis doctorales sobre Geografía y Género presentadas en las universidades españolas en las últimas décadas. Con este análisis se pretende reflexionar sobre los intereses y preocupaciones de las autoras, ver la evolución de las investigaciones, tanto desde el punto de vista temático como metodológico, y realizar un breve balance de lo realizado hasta la fecha. A partir de una búsqueda sistematizada en distintas bases de datos de tesis doctorales de acceso libre, se han podido consultar, revisar y clasificar para ofrecer un conocimiento más global de las investigaciones doctorales en esta materia. Los resultados evidencian una evolución ascendente en el número de tesis y una mayor diversidad temática y metodológica; también destaca un desequilibrio territorial en la producción científica doctoral con perspectiva de género. Las ausencias encontradas pueden deberse, entre otras, a la carencia en formación específica de geografía y género a nivel de grado y de máster, en la falta de liderazgos estables en la mentoría, o en otras resistencias derivadas de entornos institucionales muy conservadores.

En la última década se ha observado en algunas zonas rurales bien comunicadas de España un retorno de mujeres y hombres adultos jóvenes que se instalan en este medio para desarrollar proyectos profesionales propios. Este proceso de cambio en las comunidades rurales no es exclusivo de España sino que también se observa en Europa. Este artículo examina quien hay detrás de los nuevos procesos de re-ruralización en España a partir del análisis de proyectos profesionales llevados a cabo por hombres y mujeres adultos jóvenes, formados y con experiencia urbana. Estos agentes no son neutros en cuanto al género, con lo cual se toma esta categoría analítica para examinar las relaciones de poder que subyacen en estos procesos atendiendo a los objetivos, oportunidades y dificultades que tienen unos y otras en el origen de sus proyectos. La investigación se ha realizado a través de entrevistas en profundidad para poder conocer cómo las personas dan sentido a sus propias experiencias. Los resultados confirman la existencia de procesos de re-ruralización marcados por iniciativas innovadoras y muy profesionalizadas desarrolladas por mujeres y hombres cuyos discursos revelan nuevas formas de desigualdad y distintos mecanismos de poder.

En el marc d’un cert retorn al camp per part d’alguns col·lectius i en determinats territoris, dones formades s’instal·len en aquests entorns rurals i des de la seva posició contribueixen a construir la ruralitat actual. Aquest és un fenomen nou a diverses àrees rurals d’Espanya i d’Europa i frena la tendència d’un procés sostingut de despoblament rural selectiu quant a edat i gènere que ha afectat negativament les dones joves. Aquest article pretén donar a conèixer la trajectòria de vida d’algunes dones de Catalunya i Galícia que han decidit viure en entorns rurals i que resideixen al mateix municipi, comarca o província on van néixer. Ens interessa saber què els ha portat a viure al món rural i si aquesta permanència o tornada al lloc d’origen té a veure amb motius familiars, laborals o socioambientals que puguin il·lustrar la tendència d’aquestes noves ruralitats. De forma expressa, relacionem la seva trajectòria de mobilitat residencial amb el nivell d’estudis i l’ocupació laboral. L’estudi constata que els aspectes socioambientals i emocionals són importants en el retorn i corrobora que, amb igualtat de formació acadèmica, hi ha diferències d’oportunitats segons el grau de ruralitat.

9. Systematic review: third-line susceptibility-guided treatment for infection

Background: Susceptibility-guided therapies (SGTs) have been proposed as preferable to empirical rescue treatments after two treatment failures. The aim of this study was to perform a systematic review and meta-analysis evaluating the effectiveness and efficacy of SGT as third-line therapy. Methods: A systematic search was performed in multiple databases. Studies reporting cure rates of Helicobacter pylori with SGT in third-line therapy were selected. A qualitative analysis describing the current evidence and a pooled mean analysis summarizing the cure rates of SGT in third-line therapy was performed. Results: No randomized controlled trials or comparative studies were found. Four observational studies reported cure rates with SGT in third-line treatment, and three studies which mixed patients with second- and third-line treatment also reported cure rates with SGT. The majority of the studies included the patients when culture had been already obtained, and so the effectiveness of SGT and empirical therapy has never been compared. A pooled mean analysis including four observational studies (283 patients) showed intention-to-treat and per-protocol eradication rates with SGT of 72% (95% confidence interval 56–87%; I 2 : 92%) and 80% (95% confidence interval 71–90%; I 2 : 80%), respectively. Conclusions: SGT may be an acceptable option as rescue treatment. However, cure rates are, at best, moderate and this approach has never been compared with a well-devised empirical therapy. The evidence in favor of SGT as rescue therapy is currently insufficient to recommend its use.

En primer lugar se destaca la importancia de la teoría feminista para estas investigaciones, a continuación se analiza la relación entre los temas de estudio, los paradigmas y las metodologías utilizadas en la evolución de los estudios sobre la mujer rural. Finalmente, se valoran las posibilidades, dificultades y la adecuación de los métodos cualitativos y cuantitativos para estas investigaciones. [fr] L'objectif de cet article est de réfléchir sur la méthodologie utilisé d'après la Géographie à propos de la recherche sur l'étude des femmes et la société rurale en Europe et en Espagne. D'abord, nous soulignons l'importance de la théorie féministe; en second lieu nous analysons la relation entre les sujets de recherche, les paradigmes et les méthodologies utilisées dans les études sur les femmes rurales. 14. How and when should NSAIDs be used for preventing post-ERCP pancreatitis? A systematic review and meta-analysis.

Non-steroidal anti-inflammatory drugs (NSAIDs) have been shown to be efficacious to prevent pancreatitis after endoscopic retrograde cholangiopancreatography (ERCP). However, the target patients, the type of NSAID, the route of administration and the time of drug delivery remain unclear, as well as the potential efficacy in reducing the severity of pancreatitis, length of hospital stay and mortality. The objective of the study was to evaluate these questions by performing a systematic review and meta-analysis.Multiple searches were performed in the main databases. Randomized controlled trials (RCTs) comparing NSAIDs vs. placebo in the prevention of post-ERCP pancreatitis were included. Primary endpoint of the study was the efficacy for pancreatitis prevention. Sub-analyses were performed to determine the risk reduction in high and low risk patients, and to define optimal time, route of administration, and type of NSAID. Secondary endpoints were safety, moderate to severe pancreatitis prevention and reduction of hospital stay and mortality.Nine RCTs enrolling 2133 patients were included. The risk of pancreatitis was lower in the NSAID group than in the placebo group (RR 0.51; 95%CI 0.39-0.66). The number needed to treat was 14. The risk of moderate to severe pancreatitis was also lower in the NSAID group. (RR 0.46; 95%CI 0.28-0.76). No adverse events related to NSAID use were reported. NSAIDs were effective in both high-risk and unselected patients (RR 0.53; 95%CI 0.30-0.93 and RR 0.57; 95%CI 0.37-0.88). In the subanalyses, only rectal administration of either indomethacin (RR 0.54; 95%CI 0.38-0.75) or diclofenac (RR 0.42; 95%CI 0.21-0.84) was shown to be effective. There were not enough data to perform a meta-analysis in hospital stay reduction. No deaths occurred.A single rectal dose of indomethacin or diclofenac before or immediately after ERCP is safe and prevents procedure-related pancreatitis both in high risk and in unselected patients.

Background Advance care planning (ACP) aims to ensure that people with chronic or advanced disease receive medical care that is consistent with their values and preferences. However, professionals may find it challenging to engage these patients in conversations about the end of life. We sought to develop a pictorial tool to facilitate communication around ACP. Methods This was a three-phase study. In phase 1, we used the nominal group and Delphi techniques to achieve expert consensus regarding the conceptual content of the tool. In phase 2, a professional cartoonist was commissioned to create a series of cartoons representing each of the content areas resulting from the Delphi process. The pictorial tool was then administered (phase 3) with a sample of individuals with advanced/chronic disease to explore whether the cartoons were easy to understand and conveyed the intended message. Results Following a three-round Delphi process, consensus was reached regarding a set of 12 key content areas that should be considered in the context of an ACP interview. The cartoons created to represent each of the 12 areas were then reviewed and ordered so as to reflect the typical stages of an end-of-life care interview. After administering the pictorial tool with 24 frail older adults with advanced/chronic disease, changes were made to 9 of the 12 cartoons. Conclusions The new pictorial tool comprises a set of 12 cartoons that can guide professionals as they seek to engage frail older adults with advanced/chronic disease in conversations about the end of life and ACP. Este trabajo tiene como finalidad conocer cómo mujeres jóvenes que han decidido regresar al medio rural perciben, piensan e interpretan el proceso de tránsito por el que han pasado para emprender en este espacio. Para ello, aplicamos una metodología cualitativa, siguiendo un método de estudio de casos, utilizando como técnicas de recogida de información la entrevista en profundidad y las notas de campo. Las participantes son seis mujeres adultas jóvenes, con formación universitaria que han decidido iniciar una actividad emprendedora en el medio rural y que pertenecen a diferentes contextos rurales de Cataluña y Galicia. Para el análisis de la información obtenida se ha utilizado el programa Atlas ti. Las principales conclusiones sugieren que la articulación entre el proyecto profesional y vital es el motor que ha impulsado el emprendimiento de estas mujeres y que sus experiencias de tránsito muestran un cambio en el emprendimiento rural. El lugar tiene una influencia importante a la hora de decidir el tipo de actividad que se va a desarrollar en el negocio y ejerce un papel fundamental en la decisión de emprender de estas mujeres. El compromiso que establecen con el entorno es un motivo para desplegar en él todas sus capacidades y el foco de los negocios pasa por proyectos que relacionan más las personas y los lugares. Desde la orientación profesional es imprescindible considerar el impacto de la socialización de género en las mujeres para reforzar su cultura emprendedora superando los obstáculos que les impone su rol social.

La consideració a les dones en el poblament i en el despoblament rural és important, i més quan s’ha demostrat que en les causes de l’emigració juvenil i la sobrerepresentació femenina, la influència de les relacions de poder de gènere ha estat fonamental. Aquest article tracta sobre nous moviments de tornada al camp protagonitzats sobretot per dones joves, formades i amb un projecte professional propi i pel territori. Aquest retorn s’han d’entendre com a part del context de canvi en el medi rural actual, caracteritzat per una creixent diversitat d’espais rurals, per l’emergència d’altres de nous i per noves relacions camp-ciutat i nous significats d’ambdós conceptes i de la seva construcció binària. La innovació, la construcció de nous vincles amb el territori i el desafiament de les estructures patriarcals modelen les experiències quotidianes de les professionals i són clau per explicar les noves dinàmiques rurals i per fer qualsevol aproximació de futur.Paraules clau: dones, tornada al camp, ruralitat, innovació, patriarcat, Catalunya., La consideración a las mujeres en el poblamiento y en el despoblamiento rural es importante, más aún cuando se ha demostrado que en las causas de la emigración juvenil y la sobrerrepresentación femenina, la influencia de las relaciones de poder de género ha sido fundamental. Este articulo trata sobre nuevos movimientos de regreso al campo protagonizados por mujeres jóvenes, formadas y con un proyecto profesional propio y para el territorio. It analyzes the new back-to-the-land movements led by young women, with university degrees, and with their own professional project for the territory. This return must be understood as part of the context of change in the current rural environment, characterized by a growing diversity of spaces, by the emergence of new ones and by new rural-urban relationships and new meanings of both concepts as well as their binary construction. Innovation, the construction of new links with the territory and the challenge of patriarchal structures shape the daily experiences of these professionals and are key to explain the new rural dynamics and to make any future research.Keywords: Women, back-to-the-land movement, rurality, innovation, patriarchy, Catalonia.

Resumen No hay datos sistematicos sobre cuales son las tasas de resistencia a antibioticos tras el fracaso de un primer tratamiento erradicador. El objetivo del estudio es determinar la prevalencia de las resistencias secundarias a los antibioticos mediante una revision sistematica de estudios que evaluaban las resistencias secundarias de Helicobacter pylori. Se identificaron 31 estudios (2.787 pacientes). Se determinaron resistencias en 1.764 pacientes. El 99,1% de los pacientes recibieron claritromicina como tratamiento de primera linea, y un 58,7% desarrollaron resistencias. El 24,3% de los pacientes recibieron metronidazol, desarrollando resistencias el 89,7%. La resistencia secundaria a amoxicilina fue excepcional. Las resistencias secundarias tras un primer tratamiento son muy elevadas. Estos hallazgos dan soporte a la recomendacion de no repetir claritromicina o metronidazol tras el fracaso de un primer tratamiento erradicador.

En el marc d’un cert retorn al camp per part d’alguns col·lectius i en determinats territoris, dones formades s’instal·len en aquests entorns rurals i des de la seva posició contribueixen a construir la ruralitat actual. Aquest és un fenomen nou a diverses àrees rurals d’Espanya i d’Europa i frena la tendència d’un procés sostingut de despoblament rural selectiu quant a edat i gènere que ha afectat negativament les dones joves. Aquest article pretén donar a conèixer la trajectòria de vida d’algunes dones de Catalunya i Galícia que han decidit viure en entorns rurals i que resideixen al mateix municipi, comarca o província on van néixer. Ens interessa saber què els ha portat a viure al món rural i si aquesta permanència o tornada al lloc d’origen té a veure amb motius familiars, laborals o socioambientals que puguin il·lustrar la tendència d’aquestes noves ruralitats. De forma expressa, relacionem la seva trajectòria de mobilitat residencial amb el nivell d’estudis i l’ocupació laboral. L’estudi constata que els aspectes socioambientals i emocionals són importants en el retorn i corrobora que, amb igualtat de formació acadèmica, hi ha diferències d’oportunitats segons el grau de ruralitat., In the context of a certain ‘back to the countryside’ move on the part of certain groups and in specific geographic areas, educated women are settling in the rural environment and from this position are helping to build the new rurality. This is a new phenomenon, seen in several rural areas of Spain and other countries in Europe, which is slowing down a sustained process of rural depopulation, selective in terms of age and gender, that has negatively affected young women. This article aims to illustrate the life journeys of some women from Catalonia and Galicia who have made the choice to live in rural areas, in the same municipality, county or province where they were born. Our aim is to ascertain, first, what factors have led them to live in the rural environment and, second, whether their return to their place of origin is related to family, work or social and environmental reasons, which could give us an insight into the way in which the new ruralities are being shaped. We explicitly relate women’s paths of residential mobility to their level of education and their occupation. The study reveals that socio-environmental and emotional factors are important in the desire to return and it indicates that, given equal educational backgrounds, there are differences in opportunities depending on the degree of rurality of the different areas., En el marco de un cierto retorno al campo por parte de algunos colectivos y en determinados territorios, mujeres formadas se instalan en el medio rural y desde su posición contribuyen a construir la ruralidad actual. Este es un fenómeno nuevo en diversas áreas rurales de España y Europa que frena la tendencia de un proceso sostenido de despoblamiento rural selectivo en cuanto a la edad y al género que ha afectado negativamente a las mujeres jóvenes. Este artículo pretende dar a conocer la trayectoria de vida de algunas mujeres de Cataluña y Galicia que han decidido vivir en entornos rurales y que residen en el mismo municipio, comarca o provincia donde nacieron. Nos interesa saber qué las ha llevado a vivir en el medio rural y si esta permanencia o vuelta al lugar de origen se relaciona con motivos familiares, laborales o socioambientales que puedan ilustrar la tendencia de las nuevas ruralidades. De forma expresa, relacionamos la trayectoria de movilidad residencial de las mujeres con su nivel de estudios y la ocupación laboral. Il s’agit d’un phénomène nouveau dans plusieurs zones rurales de l’Espagne et de l’Europe qui freine la tendance d’un processus soutenu de dépeuplement rural sélectif en ce qui concerne l’âge et le genre et qui a négativement affecté les femmes jeunes. Cet article souhaite faire connaître la trajectoire de vie de quelques femmes de la Catalogne et de la Galice qui ont décidé d´habiter dans des environnements ruraux et qui résident dans la même commune, région ou province où elles sont nées. Nous cherchons à savoir ce qui les a amenées à vivre dans un milieu rural et si cette permanence ou retour au lieu d’origine est liée à des motifs familiaux, professionnels ou socio-environnementaux qui puissent illustrer la tendance des nouvelles ruralités. Nous associons expressément la trajectoire de mobilité résidentielle des femmes avec leur niveau d’études et leur profession. L’étude constate que les aspects socio-environnementaux et émotionnels sont importants dans ce retour et ce qui confirme que, à avec une formation académique similaire, il existe des différences d’opportunités selon le degré de ruralité.

In some regions of Spain, well-educated women decide to move to the countryside to carry out their professional activities. This is a new phenomenon after many decades of female outmigration that also occurs in other European regions. Our aim is to explore how these professional women who decide to live in the countryside manage their work–life balance in this changing space. Our hypothesis is that these women, with high level of education and urban experience are more aware of gender inequality, have greater bargaining power, and therefore, achieve better work–life conciliation with their male partners. Research has been conducted through 60 in-depth interviews with women that have been analyzed through discourse analysis. The results show a complex situation. Intersectionality is a complex concept to deal with when doing research but also when teaching the interrelationships between space and social relations. Here we present “Relief Maps” as a visual tool for teaching intersectionality and its spatial dimension in higher education courses. “Relief Maps” are a model developed for research and applied in a Geography and Gender course at the Universitat Autonoma de Barcelona (UAB) with the aim of promoting student reflection on the relationships between power structures, places and lived experiences, starting from their everyday lives.

Background Susceptibility-guided treatment has been proposed as a way to improve Helicobacter pylori eradication rates. Evidence on its efficacy for rescue therapy is very scarce. The aim of this study was to indirectly assess the applicability and effectiveness of susceptibility-guided treatment by evaluating (a) the rate of acceptance of endoscopy, (b) its success in detecting resistances, and (c) infection cure rates in patients harboring strains found to be susceptible to the antibiotics administered in clinical trials in which the efficacy of second-line treatments was reported. Methods A systematic review of studies evaluating second-line H pylori treatment was carried out in multiple databases. Studies reporting antibiotic susceptibility evaluation and/or cure rates in patients harboring sensitive and resistant strains were selected. Data were extracted in duplicate. Results The systematic review identified 36 eligible studies. Acceptance was evaluated in only one study of 60 patients, of whom only 38 agreed to endoscopy. Among the 2890 patients who received endoscopy and culture, resistances were finally determined in 86.5%. Cure rate was 72.5% in the 113 patients harboring a clarithromycin-susceptible strain after previous clarithromycin treatment, 93.5% in the 765 patients harboring a metronidazole-susceptible strain, and 83.8% in the 192 patients harboring a levofloxacin-susceptible strain. No studies with repeated administration of levofloxacin or metronidazole were found. Conclusion Even if the culture shows a clarithromycin-sensitive strain, repeating clarithromycin after a first failure should be discouraged. Susceptibility-guided treatment alone did not achieve adequate cure rates for rescue therapies. Additional measures are needed to design rescue treatments that consistently achieve excellent cure rates.

There are no systematic data on the rates of antibiotic resistance after the failure of a first eradication treatment. The objective of this study was to determine the prevalence of secondary resistance to antibiotics by conducting a systematic review of studies evaluating the secondary resistance of Helicobacter pylori. We identified 31 studies (2,787 patients). Resistance was determined in 1,764 patients. A percentage of 99.1 of patients received clarithromycin as first-line treatment and 58.7% developed resistance. A percentage of 24.3 received metronidazole and 89.7% developed resistance. Secondary resistance to amoxicillin was extremely rare. Secondary resistance after first-line treatment was very common. These findings support the recommendation not to repeat clarithromycin or metronidazole after the failure of a first eradication treatment.

Background The cure rate of standard triple therapy for Helicobacter pylori infection is unacceptably low. Susceptibility-guided therapies (SGTs) have been proposed as an alternative to standard empirical treatments. The aim of this study was to perform a systematic review and meta-analysis evaluating the efficacy of SGTs. Methods A systematic search was performed in multiple databases. Randomized controlled trials comparing cure rates of SGTs versus those of empirical therapy were selected and analysed separately for first- and second-line treatments. A meta-analysis was performed using risk ratio (RR) and number needed to treat (NNT) to measure the effect. Results Twelve studies were included in the meta-analysis. In first-line treatment, SGT was more efficacious than empirical 7-10 day triple therapy (RR 1.16, 95% CI 1.10-1.23, I (2) = 33%; NNT = 8). Most studies used a 7-10 day triple therapy and randomized the patients after endoscopy and/or culture, thus precluding the comparison of SGT versus non-invasive testing and empirical treatment in clinical practice. For second-line therapy, only four studies were found. Results were highly heterogeneous and no significant differences were found (RR 1.11, 95% CI 0.82-1.51, I (2) = 87%). Conclusions Once endoscopy and culture have been performed, SGT is superior to empirical 7 or 10 day triple therapy for first-line treatment. Further studies are needed to evaluate the effectiveness of SGT in clinical practice, especially when compared with currently recommended first-line quadruple therapies.

Abstract Within the context of the new ruralities reshaped by globalization processes, some educated women decide to stay at the countryside, where they develop their own life projects. It is a relatively new phenomenon, taking into account the steady process of female migration from rural areas that has taken place in Spain for decades. Through indepth interviews we attempt to set out how rural women perceive themselves and assess rurality in two Spanish regions, Catalonia and Galicia.
