2. Determination of Mercury in Dental Hard Tissue: An in Vitro Study

The purpose of this study was to determine the levels of mercury in hard tissue from dental amalgam fillings under in vitro conditions. Methods and Results: The study included 30 human teeth that were extracted for various clinical reasons. The teeth were stored in a physiological solution until they were used. The teeth were divided into 3 experimental groups: Group 1 (n=10) – occlusal surface cavity preparation (class I according to Black); Group 2 (n=10) – proximal-occlusal surface cavity preparation (class II); and Group 3 (n=10) – mesio-occlusal-distal [MOD] surface cavity preparation. Each of these groups was divided into 2 subgroups: subgroup 1 (n=5) – amalgam fillings were not polished, and subgroup 2 (n=5) amalgam fillings were polished. The teeth were filled with amalgam, and those in subgroups 2 were polished after 24 hours. The amount of mercury released from the amalgam fillings was determined 9 months after the teeth were filled. Before chemical analysis, the teeth were irrigated 4 times over a period of 10 minutes in an ultrasonic bath. From each tooth, 250mg of the powder was mineralized with royal water (HCl+HNO3 in a ratio of 1:3) in a microwave oven, for 54 minutes. After mineralization, the samples were filtered and analyzed with inductively coupled plasma optical emission spectrometry. The average mercury level after polishing the amalgam filling was significantly smaller (P=0.032) only in Group 1. The average mercury levels in the 3 groups revealed significant differences between both the unpolished samples (one-way ANOVA F = 69.54, P < 0.001) and the polished samples (one-way ANOVA F = 110.54, P < 0.001). Group 3 with MOD surface cavity preparation was characterized by the highest mercury levels. Conclusion: The teeth with an MOD amalgam restoration are characterized by the highest mercury levels. The more mercury is released from unpolished amalgam fillings than from polished amalgam fillings in teeth with occlusal surface cavity preparation (class I according to Black).

- Abstract
Trajtimi endodontik i suksesshëm varet nga njohja e anatomisë së kanalit të rrënjës. Qëllimi i këtij studimi është hulumtimi i anatomisë së brendshme të incizivëve mandibularë në popullatën e Kosovës. Në këtë studim janë hulumtuar 150 dhëmbë incizivë mandibularë, të nxjerrë për shkaqe periodontale. Dhëmbët janë analizuar përmes metodës së dekalcifikimit dhe pastrimit. Vizualizimi i sistemit të kanalit është bërë nëpërmes injektimit të ngjyrës India Ink dhe pastaj është bërë përcaktimi i numrit të kanaleve, tipi i kanaleve sip- as klasifikimit Vertucci, prania e kanaleve laterale, si dhe numri i foraminave apikale. Nga dhëmbët e studiuar, 100% kishin një rrënjë; 70% kishin një kanal dhe 30% kishin dy kanale. Sipas klasifikimit Vertucci, në shumicën e rasteve është gjetur tipi I i kanalit, pasuar me tipin III dhe IV. Prania e kanaleve laterale është hasur në 3.3% të rasteve dhe atë më së shpeshti në të tretën apikale.

- Abstract
Obturimi i tërësishëm i sistemit të kanalit të rrënjës së dhëmbit është një ndër fazat më të rëndësishme gjatë ter- apisë endodontike. Prandaj, qëllimi i këtij studimi in vitro, ishte që të vlerësohet dhe të bëhet krahasimi i mbushjes apikale me materialet endodontike me EndoRez pastë, Endomethason pastë, si dhe Resilon/Epiphany sistem, pas përpunimit të kanaleve të rrënjës së dhëmbit. Mostrat e 80 dhëmbëve një rrënjësh të nofullës së sipërme u ndanë në tri grupe eksperimentale, varësisht prej materialit me të cilën u obturuan. Pas përpunimit me teknikën crown down, mostrat e grupit të parë u obtu- ruan me EndoRez pastë në kombinim me Thermafil ob- turatorin, mostrat e grupit të dytë u obturuan me Endo- methason pastë të kombinuara me sistemin Thermafil, si dhe mostrat e grupit të tretë u obturuan me pastën Epiphany në kombinim me Resilon. Mikrorrjedhja apikale është përcaktuar me anë të stereomikroskopit me anë të cilit është bërë matja e rrjedhjes së ngjyrës në të tretën apikale, si dhe rezultatet e fituara në mënyrë statistikore janë përpunuar dhe krahasuar me anë të t-testit. Në grupet e përpunuara me teknikën crown down dhe të obturuara me Thermafil obturator dhe pastë të Endo- methason-it, rrjedhja mesatare e ngjyrës ishte më e lartë krahasuar me mostrat e obturuara me pastë të rezinës në kombinim me Thermafil sistemin, si dhe grupin e obturuar me Resilon/Epiphany. Që të tri teknikat e obturimit treguan rrjedhje të ngjyrës, por mikrorrjedhje më të vogël u vlerësuan në mostrat e obturuara me Resilon/Epiphany sistemin.

5. An In Vitro Comparative Evaluation of Apical Leakage Using Different Root Canal Sealers

BACKGROUND Successful endodontic therapy requires a fluid-tight apical seal. The aim of this in vitro study was to evaluate and compare the apical sealing ability of 3 root canal sealers: Apexit Plus, AH Plus, and Resilon/Epiphany sealers. MATERIAL AND METHODS Samples of 152 single-rooted teeth were divided into 2 test groups, and each group was divided into 3 subgroups based on the preparation and obturation technique. After preparation with the step-back technique, the first group of root canals were obturated with lateral gutta-percha compaction in combination with AH Plus sealer or Apexit Plus or with the Resilon/Epiphany system alone. In the second group, preparation was done with the crown-down technique and root canals were then obturated with Thermafil obturator in combination with AH Plus or Apexit Plus or with Resilon/Epiphany sealer alone. Apical leakage was determined using a dye leakage test observed with a stereomicroscope. The degree of dye leakage was assessed using the t test to comparing the arithmetic averages of the groups. RESULTS In the groups prepared with the step-back technique, the average dye leakage was lower in samples obturated with Resilon/Epiphany, than in those filled with AH Plus/lateral gutta-percha compaction or Apexit Plus/lateral gutta-percha compaction. In groups prepared with the crown-down technique and obturated with the Resilon/Epiphany system, the average dye leakage was lower than in those filled with AH Plus/Thermafil obturation and those filled with Apexit Plus/Thermafil obturation. CONCLUSIONS Although all preparation and obturation techniques showed dye penetration, the crown-down technique paired with the Resilon/Epiphany system showed the least leakage.

BACKGROUND: Complete mechanical preparation of the root canal system is rarely achieved. Therefore, the purpose of this study was to evaluate and compare the root canal shaping efficacy of ProTaper rotary files and standard stainless steel K-files using micro-computed tomography. MATERIAL AND METHODS: Sixty extracted upper second premolars were selected and divided into 2 groups of 30 teeth each. Before preparation, all samples were scanned by micro-computed tomography. Thirty teeth were prepared with the ProTaper system and the other 30 with stainless steel files. After preparation, the untouched surface and root canal straightening were evaluated with micro-computed tomography. The percentage of untouched root canal surface was calculated in the coronal, middle, and apical parts of the canal. We also calculated straightening of the canal after root canal preparation. Results from the 2 groups were statistically compared using the Minitab statistical package. 8. The Antibacterial Efficacy of Biopure MTAD in Root Canal Contaminated with Enterococcus faecalis

Aim. The purpose of this in vitro study was to assess the antimicrobial efficacy of Biopure MTAD against E. faecalis in contaminated root canals. Materials and Methods. Forty-two single rooted extracted human teeth were inoculated with E. faecalis and incubated for four weeks. The samples were divided in two control and five experimental groups irrigated with 1.5% sodium hypochlorite solution (NaOCl); 3% NaOCl; BioPure MTAD; 1.5% NaOCl/17% EDTA; or 3% NaOCl/17% EDTA. After a one-week incubation, complete disinfection was confirmed by the absence of turbidity in the incubation media. Dentin shavings were taken from samples with no turbidity to verify whether E. faecalis was present in dentin tubules. Results were analyzed statistically using Fisher's exact test, with the level of significance set at . Results. Statistical analysis of the data obtained at Day 7 and after dentin shaving analysis showed that BioPure MTAD had significantly greater antibacterial activity than 1.5% NaOCl, 1.5% NaOCl/17% EDTA and 3% NaOCl/17% EDTA. No significant difference was detected between MTAD and 3% NaOCl. Conclusions. These findings suggest that BioPure MTAD possesses superior bactericidal activity compared with NaOCl and EDTA against E. faecalis.

Zbog visoke biokompatibilnosti mineralno-trioksidni agregat (MTA) pogodan je za različite mogućnosti liječenja (u endodonciji?). No, produljeno vrijeme stvrdnjavanja ograničava primjenu. Svrha: svrha istraživanja bila je procijeniti djelovanje MTA s 5-postotnim ili 10-postotnim udjelom CaCl2, kao akceleratorom stvrdnjavanja, na pH i na količinu kalcijevih iona otpuštenih iz MTA-a (ProRoot MTA, Dentsply, Tusla Denal, Tulsa, SAD). Materijal i metode: Prašak MTA-a bio je pomiješan sa sterilnom vodom u kojoj je bio kalcijev klorid ili u drugom slučaju, kada nije bio dodan. Zatim se pH-metrom ( Sentron 2001, Intergrated Sensor Technology, Roden, Nizozemska) te nakon toga atomsko-apsorpcijskim spektrometrom ( Perkin-Elmer 1100B, Weiterstadt, Njemačka ) izmjerio pH i količina otpuštenih kalcijevih iona i to odmah nakon stvrdnjavanja, a zatim za jedan, tri te dvadeset i četiri sata. Rezultati: Uzorci s 10-postotnim udjelom CaCl2 pokazali su povišeni pH odmah nakon stvrdnjavanja, a tako se nastavilo i tijekom svih triju vremenskih intervala (p, MTA has been proposed for various treatment alternatives due to its high biocompatibility. Background Despite numerous studies on the prevalence of apical periodontitis (AP) and endodontic treatment in diverse geographical populations, there are currently no data on the prevalence of these conditions in populations of adults native to Kosovo. Therefore, little is known about how widespread these conditions are, and whether there is any correlation between root canal treatment and AP. The purpose of our research was to address this anomaly by investigating AP and endodontic treatment in an adult Kosovar population based on radiographic examination. Methods The sample used for this study consisted of randomly selected individuals referred to the University Dentistry Clinical Center of Kosovo in the years 2006-2007. Orthopantomographs of 193 patients were evaluated. The periapical status of all teeth (with the exception of third molars) was examined according to Ørstavik's Periapical Index. The quality of the root canal filling was rated as 'adequate' or 'inadequate' based on whether all canals were filled, the depth of fill relative to the radiographic apex and the quality of compaction (absence/presence of voids). Data were analyzed statistically using the Chi-square test and calculation of odds ratios. Results Out of 4131 examined teeth, the prevalence of apical periodontitis (AP) and endodontic treatment was 12.3% and 2.3%, respectively. Of 95 endodontically-treated teeth, 46.3% were associated with AP. The prevalence of AP increased with age. The prevalence in subjects aged over 60 years old (20.2%) was higher than in other age groups. A statistically significant difference was found for the frequency of endodontically-treated teeth associated with AP in the 40-49 year age group (P < 0.001). Of some concern was the discovery that only 30.5% of the endodontically-treated teeth examined met the criteria of an acceptable root canal filling. Inadequately root-filled teeth were associated with an increased AP risk. Conclusions The prevalence of AP and the frequency of endodontically-treated teeth with AP in this Kosovar population are higher than those found in other countries.
