Veel medewerkers geven de voorkeur aan informele vormen van leren en ontwikkelen op de werkplek, terwijl formele leeractiviteiten gangbaarder zijn in organisaties. Deze activiteiten lijken vaak geïnitieerd door managers en HR-functionarissen en vinden veelal buiten de werkplek plaats. Dit terwijl arbeidsmarkttekorten noodzaken om leren en ontwikkelen in het dagelijkse werk vorm te geven. Dit artikel gaat in op het potentieel van participatief actieonderzoek om te leren en te ontwikkelen op de werkplek. Er wordt verslag gedaan van een actieonderzoeksproject binnen de verpleeghuiscontext. Onderzocht is hoe leren en ontwikkelen plaatsvonden en welke werkzame elementen binnen het actieonderzoeksproject hieraan bijdroegen. Resultaten laten zien dat leren en ontwikkelen in en van werk plaatsvinden op het moment dat authentieke situaties, direct beleefd worden. Ook helpt het als de authentieke situaties verkend worden vanuit verschillende perspectieven en als er mogelijkheden gezien worden voor nieuw of anders handelen. Regels en routines kunnen de integratie van leren en ontwikkelen in het werk beperken. Op basis van opgedane ervaringen en geleerde lessen worden aanbevelingen gegeven.

Background: There is a growing amount of research in which older adults contribute as co-researchers. The quality of this research depends, among other factors, on the nature of relationships between professional researchers and co-researchers. Reflections on these relationships can offer structured insight into this form of research. Aim: Our reflection on the co-operation between two older adults and a nurse researcher aims to share the lessons learned based on a critical understanding of our journey. Our main questions are: 1. How has the relationship developed over time, including in terms of mutuality and equality? 2. Which moments have been decisive in this development? Conclusion: We regard our co-operative relationship as a ‘dynamic search’. The meaning of mutuality and equality may change over time and so enrich the relationships. There is a need for further understanding into how these values can be nurtured in different configurations of researchers and co-researchers. Implications for practice: Evolving relations can be nurtured through deliberative sharing of the perceptions, expectations and experiences of the researchers and co-researchers Combining a formal working atmosphere with informal moments helps the research team respond to the individual needs of its members To enhance equality and mutuality, it is important to appreciate and value everyone’s contribution rather than concentrating on ‘what ’or ‘how ’individuals contribute

Background Dynamic myocardial computed tomography perfusion (CTP) is a novel technique able to depict cardiac ischemia. Purpose To evaluate the impact of a four-dimensional noise reduction filter (similarity filter [4D-SF]) on image quality in dynamic CTP imaging, allowing for substantial radiation dose reduction. Material and Methods Dynamic CTP datasets of 30 patients (16 women) with suspected coronary artery disease, acquired with a 320-slice CT system, were retrieved, reconstructed with the deep learning-based algorithm of the system (DLR), and filtered with the 4D-SF. For each case, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) in six regions of interest (33–38mm2) were calculated before and after filtering, in four-chamber and short-axis views, and t-tested. Furthermore, six radiologists of different expertise evaluated subjective image preference by answering five visual grading analysis-type questions (regarding acceptable level of noise, absence of artifacts, natural appearance, cardiac contour sharpness, diagnostic acceptability) using a 5-point scale. The results were analyzed using visual grade characteristics (VGC) and intraclass correlation coefficient (ICC). Results Mean SNR in four-chamber view (unfiltered vs. filtered) were: septum=4.1 ± 2.1 versus 7.6 ± 5.6; lateral wall=4.5 ± 2.0 versus 8.0 ± 4.9; CNRseptum=16.6 ± 8.9 versus 31.7 ± 28; lateral wall=16.2 ± 8.9 versus 31.3 ± 28.9. Similar results were obtained in short-axis view. The perceived filtered image quality indicated decreased noise (VGCAUC=0.96) and artifacts (0.65), improved natural appearance (0.59), cardiac contour sharpness (0.74), and diagnostic acceptability (0.78). The inter-observer variability was excellent (ICC=0.79). All results were statistically significant ( P Conclusion Similarity filtering after DLR improves image quality, possibly enabling dose reduction in dynamic CTP imaging in patient with suspected chronic coronary syndrome.

Objective This study aimed to determine whether lower values of feature-tracking cardiovascular magnetic resonance (CMR)-derived left atrial reservoir strain (LARS) and impaired left ventricular (LV) global longitudinal strain (GLS) were associated with the presence of symptoms and long-term prognosis in patients with SSc. Methods A total of 100 patients {54 [interquartile range (IQR) 46–64] years, 42% male} with SSc who underwent CMR imaging at two tertiary referral centres were included. All patients underwent analysis of LARS and LV GLS using feature-tracking on CMR and were followed-up for the occurrence of all-cause mortality. Results The median LV GLS was –21.8% and the median LARS was 36%. On multivariable logistic regression, LARS [odds ratio (OR) 0.964 per %, 95% CI 0.929, 0.998, P = 0.049] was independently associated with New York Heart Association (NYHA) class II–IV heart failure symptoms. Over a median follow-up of 37 (21–62) months, a total of 24 (24%) patients died. Univariable Cox regression analysis demonstrated that LARS [hazard ratio (HR) 0.94 per 1%, 95% CI 0.91, 0.97, P Conclusion In patients with SSc, LARS was independently associated with the presence of NYHA class II–IV heart failure symptoms. Although both LARS and LV GLS were associated with all-cause mortality, only LARS provided incremental value over all evaluated variables known to be prognostically important in patients with SSc.

ObjectiveWomen with Turner syndrome (TS) are at increased risk of aortic dissection, which is a life-threatening event associated with aortic dilation. Knowledge on the development of aortic dilation over time remains limited. This study aims to describe the prevalence of aortic dilation, to find associated factors and to study aortic growth in women with TS.MethodsIn this prospective multicentre cohort study, consecutive adult women with genetically proven TS included between 2014 and 2016 underwent ECG-triggered multiphase CT angiography at baseline and after 3 years. Aortic diameters were measured at seven levels of the thoracic aorta using double oblique reconstruction and indexed for body surface area. Ascending aortic dilation was defined as an aortic size index >20 mm/m2. Aorta-related and cardiovascular events were collected. Statistical analysis included linear and logistic regression and linear mixed effects models.ResultsThe cohort consisted of 89 women with a median age of 34 years (IQR: 24–44). Ascending aortic dilation was found in 38.2% at baseline. At baseline, age (OR: 1.08 (95% CI 1.03 to 1.13), pConclusionsIn women with TS, ascending aortic dilation is common and associated with age, BAV and systolic blood pressure. Aortic diameters were stable during a 3-year follow-up, apart from a significant yet not clinically relevant increase at the sinotubular junction, which may suggest a more benign course of progression than previously reported.

Voortdurende maatschappelijke veranderingen en uitdagingen vragen om samenwerking en een leven lang ontwikkelen. Vaak gebeurt dit in learning communities (innovatieve leerwerkomgevingen) waar organisaties grensoverstijgend samenwerken aan complexe vraagstukken. Bruggenbouwers (brokers) hebben een sleutelpositie in het ontwikkelen van deze learning communities om mensen en organisaties met elkaar te verbinden. Een veelzijdige rol die zich moeilijk laat definiëren. Bovendien voorzien organisaties niet altijd bewust in ondersteuning en ontwikkeling van deze bruggenbouwers. Op basis van een mixed-methodsbenadering voorziet dit onderzoek in de behoefte van een generieke rolbeschrijving met zeven vaardigheden. Hierbij wordt de invloed van kennis, ervaring en persoonskenmerken belicht. Bruggenbouwers werken intersectoraal over grenzen van organisaties heen en ondersteunen betrokken professionals en organisaties in hun samenwerking door politiek bewust en strategisch te handelen. Zij stimuleren kennisdeling en vertalen kennis naar diverse betrokkenen en contexten en onderzoeken daarbij de beroepspraktijk systematisch. Deze rolbeschrijving en de gewenste ondersteuning hierin biedt concrete handvatten om bruggenbouwers beter te selecteren, te waarderen en ook gerichter te investeren in hun professionele ontwikkeling. Deze investering is van cruciaal belang omwille van de katalyserende werking van de rol als bruggenbouwer om het voortdurend leren en ontwikkelen bij organisaties mogelijk te maken

Background and objective: Practically trained (student) caregivers (further: caregivers) make up the majority of care staff in nursing homes (NHs). To keep up with the fast-changing healthcare environment and ensure a high quality of care, it is important to know how to stimulate continuous work-based learning (WBL) among this group. The purpose of the study was to systematically study the scientific literature published to date on (1) how caregivers learn in NHs and (2) what facilitates or impedes their learning.Methods: A scoping review was carried out, systematically searching six scientific databases. A total of 35 studies published from January 2009 to February 2021 were included. Study characteristics, learning mechanisms, facilitators, and barriers to learning were extracted and synthesized.Results: None of the studies specifically focused on how caregivers learn. Yet, we identified various learning mechanisms, and found that learning by theory or supervision was most frequently engaged in. Most learning mechanisms used among the groups in the included studies were planned and formal and developed and initiated by others out of the context. Three main themes were identified among the facilitators and barriers of WBL: individual learning, collective learning, and resources for learning. An interdependency between (sub)themes was found.Conclusions: The way caregivers in NHs learn is understudied. Moreover, both their informal learning and the support they receive to be(come) active learners has been overlooked. As WBL provides caregivers with opportunities to learn within a real-life setting, we suggest more research on informal learning mechanisms.

Grotere vakgroepen van medisch specialisten, vrijgevestigd of in loondienst, doen de behoefte aan transparantie toenemen. Transparantie in de vorm van antwoorden op vragen als: is de werklast redelijk in relatie tot de formatie (c.q. de capaciteit)? Moet er werk af of formatie bij? Is werklast goed verdeeld over alle individuen? En, gegeven de hoge loonkosten van naar schatting ten minste 200.000 euro per formatieplaats, zijn er mogelijkheden voor efficiencyverbetering? We berekenen eerst de inzetbaarheid van de gemiddelde medisch specialist en werken drie voorbeelden uit. Het eerste is de kwantificering van het takenpakket van een vakgroep neonatologen. Het tweede voorbeeld is het vertalen van de totale formatie van een vakgroep radiologen naar beschikbare uren en naar deeltaken. En het derde betreft het doorrekenen van de taken van een groep kindernefrologen. De methodiek kan ook toegepast worden voor paramedici.

Objectives Nurses must contribute to the development of professional practice and thereby improve quality of care by using their research abilities. Therefore, learning communities comprised of nurses, students and teachers were created in a hospital. Methods A qualitative descriptive methodology was used. Three learning communities were purposefully selected, and members were asked open-ended questions about their learning and activities in multiple data collection sessions. Data were analyzed by means of an inductive process. Results Both individually and collectively, members learned about research in general, research terminology and methods. They learned about asking questions and critical thinking, and, specifically through collective learning, they learned about the differences between education practices. Activities were carried out primarily to continue personal learning. Conclusions Learning within the learning communities was mainly research-oriented and therefore potentially suitable for enhancing research ability. Implications are offered to support the formulation of community activities.

To promote workplace learning for staff as well as students, a partnership was formed between a residential care organisation for older people and several nursing faculties in the Netherlands. This partnership took the form of two care innovation units; wards where qualified staff, students and nurse teachers collaborate to integrate care, education, innovation and research. In this article, the care innovation units as learning environments are studied from a student perspective to deepen understandings concerning the conditions that facilitate learning. A secondary analysis of focus groups, held with 216 nursing students over a period of five years, revealed that students are satisfied about the units' learning potential, which is formed by various inter-related and self-reinforcing affordances: co-constructive learning and working, challenging situations and activities, being given responsibility and independence, and supportive and recognisable learning structures. Time constraints had a negative impact on the units' learning potential. It is concluded that the learning potential of the care innovation units was enhanced by realising certain conditions, like learning structures and activities. The learning potential was also influenced, however, by the non-controllable and dynamic interaction of various elements within the context. Suggestions for practice and further research are offered.

Summary Research into workplace mentoring is principally focussed on predictors and psychosocial and instrumental outcomes, while there is scarcely any in-depth research into relational characteristics, outcomes and processes. This article aims to illustrate these relational aspects. It reports a co-constructed auto-ethnography of a dyadic mentoring relationship as experienced by mentor and protege. The co-constructed narrative illustrates that attentiveness towards each other and a caring attitude, alongside learning-focussed values, promote a high-quality mentoring relationship. This relationship is characterised, among other things, by person centredness, care, trust and mutual influence, thereby offering a situation in which mutual learning and growth can occur. Learning develops through and in relation and is enhanced when both planned and unplanned learning takes place. In addition, the narrative makes clear that learning and growth of both those involved are intertwined and interdependent and that mutual learning and growth enrich and strengthen the relationship. It is concluded that the narrative illustrates a number of complex relational processes that are difficult to elucidate in quantitative studies and theoretical constructs. It offers deeper insight into the initiation and improvement of high-quality mentoring relationships and emphasises the importance of further research into relational processes in mentoring relationships. Een verpleegkundige die evidence-based handelt, maakt gebruik van zowel wetenschappelijk onderzoek als zijn eigen professionele ervaringskennis, gecombineerd met de kennis en voorkeuren van de patiënt. Een gangbare benadering om dit evidence-based handelen in de praktijk te krijgen, is het ontwikkelen van een richtlijn om deze vervolgens in de gezondheidszorg te implementeren. Hoewel deze werkwijze gangbaar is, is ze slechts beperkt succesvol. Inmiddels is er beginnend bewijs dat cultuur en leiderschap sleutelelementen zijn bij het implementeren van richtlijnen, zodat er meer nodig is om professioneel gedrag te veranderen dan de professional ‘bloot stellen’ aan een richtlijn. Een dergelijke gedragsverandering is een complex proces, omdat dit handelen onderhevig is aan zowel intrinsieke (persoonsgebonden) als extrinsieke (contextgebonden) factoren.

There is a need for person-centred approaches and empowerment of staff within the residential care for older people; a movement called 'culture change'. There is however no single path for achieving culture change. With the aim of increasing understandings about cultural change processes and the promotion of cultural values and norms associated with person-centred practices, this article presents an action research project set on a unit in the Netherlands providing care for older people with dementia. The project is presented as a case study. This study examines what has contributed to the improvement of participation of older people with dementia in daily occupational and leisure activities according to practitioners. Data was collected by participant observation, interviews and focus groups. The results show that simultaneous to the improvement of the older people's involvement in daily activities a cultural transformation took place and that the care became more person-centred. Spontaneous interactions and responses rather than planned interventions, analysis and reflection contributed to this. Furthermore, it proved to be beneficial that the process of change and the facilitation of that process reflected the same values as those underlying the cultural change. It is concluded that changes arise from dynamic, interactive and non-linear processes which are complex in nature and difficult to predict and to control. Nevertheless, managers and facilitators can facilitate such change by generating movement through the introduction of small focused projects that meet the stakeholders' needs, by creating conditions for interaction and sense making, and by promoting the new desired cultural values.

Binnen het plan van aanpak worden keuzes gemaakt voor methoden en principes die gehanteerd worden bij de implementatie. In dit hoofdstuk wordt ingegaan op vier methodische werkwijzen die vaak gebruik worden in zorg- en onderwijssettings bij het implementeren van veranderingen: de PDCA-cyclus van Deming, het Participatief Actieonderzoek (PAO), het implementatiemodel van Grol en Wensing en Responsieve Evaluatie (RE).

In organisations, learning is generally seen as a dynamic, collective and often conscious process that occurs by reflecting on real work experiences. In this article, we discuss these assumptions about learning in the context of work by presenting a case study in the care for older people. The case illustrates that learning in and through work is predominantly an embodied and responsive phenomenon that usually occurs implicitly while acting. We argue that a learning perspective grounded in the worldview of enactivism encapsulates this pragmatic and embodied character of learning and at the same time provides a reality and language helpful in encouraging a critical attitude towards assumptions about learning in organisations. Understanding learning from an enactive point of view carries consequences for studying and organising learning within organisations. These are outlined within this article to challenge managers’ meanings of learning in health care and comparable settings and to encourage further dialogue on this issue.

Op 2 oktober 2013 organiseerde de Nederlandstalige aftakking van het Collaborative Action Research Network (CARN-NL) een studiemiddag over ‘Actieonderzoek in onderwijs, zorg en welzijn anno 2013: een verkenning, visie en vooruitblik’, aan de Hogeschool Arnhem Nijmegen (HAN). Geïnteresseerden in dit thema werden van harte uitgenodigd en konden gratis deelnemen aan deze middag. Uiteindelijk vertegenwoordigde een dertigtal deelnemers diverse hogescholen, universiteiten, alsook middelbare scholen uit zowel Nederland als België.CARN is in 1976 opgericht in Engeland. Sinds die tijd is het uitgegroeid tot een internationaal netwerk met leden uit het onderwijs, de gezondheidszorg, de sociale zorg en commerciële en publieke contexten. CARN heeft als centrale doelen het aanmoedigen en ondersteunen van actieonderzoekprojecten, het bieden van toegang tot publicaties van actieonderzoekprojecten en het bijdragen aan de theorie en methodologie van actieonderzoek. CARN-NL is in 2007 opgericht en geeft verder invulling aan deze centrale doelen van CARN en dan met name op de gebieden van ‘zichtbaarheid’, ‘ondersteuning’ en ‘netwerken’.De aanleiding voor een studiemiddag was dat sinds januari 2011 CARN-NL nauwelijks activiteiten had ondernomen en de huidige coördinatiegroep stil wilde staan bij de voortgang en mogelijke doorstart van dit netwerk. Dit door te inventariseren welke behoeften en wensen er waren bij collegae in het land. Het doel van de studiemiddag was dan ook om ervaringen uit te wisselen en verschillen en overeenkomsten te verkennen over hoe actieonderzoek en ander soortgelijk onderzoek vorm krijgen in onder andere onderwijs en zorg en welzijn. Daarnaast zouden ideeën worden verzameld over de focus van CARN-NL voor de komende jaren om zo een mogelijke gezamenlijke doorstart te kunnen maken.Een actieonderzoeksdesign rondom de vier fasen van oriëntatie, planning, actie en reflectie is gekozen om het proces de komende jaren systematisch te doorlopen en te komen tot een landelijk bekend, gewaardeerd, ondersteunend en toegankelijk netwerk rondom actieonderzoek en soortgelijk onderzoek voor diverse professies (onderwijs, zorg, organisatie, enzovoort).

Engagement is seen as an important characteristic of action research. The term is often used to refer to the participation and involvement of the research participants. Within this article we take another angle and explore the concept of engagement in relation to the main action researcher. Using an auto-ethnographic approach, we illustrate that the involvement and ‘closeness’ of the researcher, although necessary within action research, can also have a darker side as people have the tendency to get trapped in their own beliefs and prejudices. If not mindful enough of their own involvement and way of being within the context, the researcher can lose him- or herself in the situation and no longer be able to encourage or facilitate the participation of others. We give suggestions for realizing productive engagement as a (participatory) researcher using concepts such as mindfulness and mindsight.

Het invoeren of implementeren van vernieuwingen in de gezondheidszorg is ingewikkeld. Iedere verpleegkundige die al eens iets heeft willen veranderen heeft dat waarschijnlijk aan den lijve ondervonden. Ondanks deze moeilijkheden hebben we als professional de uitdagende taak om bij te dragen aan het verbeteren van de kwaliteit van de zorg, bijvoorbeeld door vernieuwingen in te voeren. In dit artikel wordt ingegaan op 4 methodische werkwijzen die vaak gebruik worden in zorg- en onderwijssettings bij het implementeren van veranderingen. Dit zijn: • PDCA cyclus van Deming • Participatief Actieonderzoek (PAO) • Implementatiemodel van Grol en Wensing • Responsieve Evaluatie (RE) Het doel van dit artikel is een praktisch overzicht te bieden van deze methoden en modellen die de implementatie van een verandering kunnen ondersteunen, zodat de lezer een methode of een combinatie daarvan kan kiezen die bij zijn situatie en uitgangspunten aansluit. Dit overzicht wordt geboden door de 4 methoden in een matrix te beschrijven en vervolgens te vergelijken.

Binnen twee zorginnovatiecentra in de ouderenzorg wordt op bijzondere wijze theoretische en praktische kennis geïntegreerd. Lecturer practitioners ondersteunen deze integratie van meerdere kennisbronnen. In dit artikel wordt ingegaan op de context en inhoud van de rol van de Lecturer Practitioner en wordt aan de hand van enkele praktijkvoorbeelden geïllustreerd hoe kennisbronnen geïntegreerd worden.
