Abstract Background Motivation is an important factor in rehabilitation and frequently used as a determinant of rehabilitation outcome. Several factors can influence patient motivation and so improve exercise adherence. This paper presents the design of two robot devices for use in the rehabilitation of upper limb movements, that can motivate patients during the execution of the assigned motor tasks by enhancing the gaming aspects of rehabilitation. In addition, a regular review of the obtained performance can reinforce in patients' minds the importance of exercising and encourage them to continue, so improving their motivation and consequently adherence to the program. In view of this, we also developed an evaluation metric that could characterize the rate of improvement and quantify the changes in the obtained performance. Methods Two groups (G1, n = 8 and G2, n = 12) of patients with chronic stroke were enrolled in a 3-week rehabilitation program including standard physical therapy (45 min. daily) plus treatment by means of robot devices (40 min., twice daily) respectively for wrist (G1) and elbow-shoulder movements (G2). Both groups were evaluated by means of standard clinical assessment scales and the new robot measured evaluation metric. Patients' motivation was assessed in 9/12 G2 patients by means of the Intrinsic Motivation Inventory (IMI) questionnaire. Results Both groups reduced their motor deficit and showed a significant improvement in clinical scales and the robot measured parameters. The IMI assessed in G2 patients showed high scores for interest, usefulness and importance subscales and low values for tension and pain subscales. Conclusion Thanks to the design features of the two robot devices the therapist could easily adapt training to the individual by selecting different difficulty levels of the motor task tailored to each patient's disability. The gaming aspects incorporated in the two rehabilitation robots helped maintain patients' interest high during execution of the assigned tasks by providing feedback on performance. The evaluation metric gave a precise measure of patients' performance and thus provides a tool to help therapists promote patient motivation and hence adherence to the training program.

The aim of this study was to describe in detail a new method, called normalized force control parameter (nFCP), to measure changes in movement dynamics obtained during robot-aided neurorehabilitation, and to evaluate its ability to estimate the clinical scales. The study was conducted in a group of 18 subjects after chronic stroke who underwent robot therapy of the upper limb. We used two different measures of movement dynamics to assess patients' performance during each session of training: the nFCP and force directional error (FDE), both measuring the directional error of the patient-exerted force applied to the end-effector of the robot device. Both metrics exhibited significant changes over the three-week course of treatment. The comparison between nFCP and FDE slopes showed a significant and high correlation ( r = 0.79; p < 0.001), indicating that the two parameters are closely correlated. The FDE informed on the direction of the force error, while the nFCP showed a better performance in predicting the clinical scale values. Assessment of the time course of recovery showed that nFCP, FDE and the movement smoothness improved quickly at first and then plateaued, while steady gains in mean velocity of movement took place over a longer time course. These data may be helpful to the therapist in developing more effective robot-based therapy protocols.

Unlabelled: The characteristics of Moving Picture Experts Group (MPEG-4) video compression, in particular its adaptability to narrowband channels and the elevated degree of compression obtainable, make it of interest for services of telemedicine that require instantaneous video transmission and interpretation. In this study we faced the problem of the minimum quality of service (QoS) in specific applications of tele-echocardiography (T-E). In the specifics we evaluated the clinical adequacy of MPEG-4 compression in the real time transmission of echocardiographic studies. Forty echocardiographic examinations consisting of standard projections of patients affected by ischemic heart disease were submitted to two observers expert in echocardiography, who made 4 separate evaluations as follows: 2 on the same equipment on which the original studies were performed; 1 after online MPEG-4 codification of the studies at 256 kb/s; 1 after online MPEG-4 codification of the studies at 128 kb/s. For each evaluation, the following data were collected: subjective opinion on the overall visual quality of the images; estimate of ejection fraction and level of impairment; wall Motion Score Index and percentage of asynergy; mitral failure., The Results: 1) the subjective quality of the echocardiographic images was the same as that perceived in the video at Mpeg4/256 kb/s compression level while it was lower, as expected, in the video Mpeg4/128 kb/s; 2) the quality degradation did not produce a statistically significant difference in the evaluation of left ventricular function and regional wall motion impairments. These results confirm the feasibility of MPEG-4 compression for the transmission of echocardiographic studies for use in telemedicine and suggest that it is not necessary to seek transmission speeds higher than 256 kb/s for the semiquantitative reading of left ventricular kinetics.

Baroreflex sensitivity assessed by means of the phenylephrine test plays a prognostic role in patients with previous myocardial infarction, but the need for drug injection limits the use of this technique. Recently, several non-invasive methods based on spectral analysis of systolic arterial pressure and heart period have been proposed, but their agreement with the phenylephrine test has not been investigated in patients with heart failure. The two methods (phenylephrine test and spectral analysis) were compared in a group of 49 patients with chronic congestive heart failure both at rest and during controlled breathing. The linear correlation and the limits of agreement between the phenylephrine test slope and the alpha-index [alpha(c); corrected by the coherence function between the interbeat interval (RR interval) and systolic arterial pressure] were evaluated. Only 16 patients had a measurable alpha-index at rest in both the low-frequency (LF) and high-frequency (HF) bands; the alpha(c)-index allowed measurements in all patients. It correlated moderately with the phenylephrine test slope at rest (r=0. 71 and P<0.001 in LF; r=0.57 and P<0.001 in HF) and during controlled breathing (r=0.51 and P<0.001 in LF; r=0.63 and P<0.001 in HF). Multivariate regression analysis showed that only alpha(c)LF during rest and alpha(c)HF during controlled breathing contributed significantly to baroreflex gain estimation. However, the agreement between methods was weak; the normalized limits of agreement and bias were -162 to 243% (0.46 ms/mmHg) for alpha(c)LF and -185 to 151% (-0.99 ms/mmHg) for alpha(c)HF. Thus the comparison between baroreflex sensitivity measurements obtained by the phenylephrine test and spectral analysis showed a moderate correlation between the two methods; however, despite the linear association, a consistent lack of agreement between the two techniques was found. Because both systematic and random factors contribute to the difference, these two techniques cannot be considered as alternatives for the assessment of heart failure.

As a consequence of the dramatic improvements achieved in information technology standards in terms of single hardware and software components, efforts in the evaluation processes have been focused on the assessment of critical human factors, such as work-flow organisation, man-machine interaction and, in general, quality of use, or usability. This trend is particularly valid when applied to medical informatics, since the human component is the basis of the information processing system in health care context. With the aim to establish an action-research project on the evaluation and assessment of clinical software procedures which constitute an integrated hospital information system, the authors adopted this strategy and considered the measurement of perceived usability as one of the main goals of the project itself: the paper reports the results of this experience.

The measurement of reaction times in response to suitable stimuli may provide insight into the separate functional blocks corresponding to diverse speech production mechanisms. The quantitative evaluation of speech motor performance provides useful information for the early detection and long-term monitoring of many neurological diseases. The vocal reaction times (VRTs) technique and its application in the assessment of a group of normal subjects is presented. Speech motor performance and the effect of age were investigated by measuring VRTs and speech duration, in a group of thirty normal subjects divided into three age groups (18-44, 45-59, 60-80). VRTs were measured by using an immediate and delayed reaction stimulation paradigm. Analysis of the acoustic and electromyographic signals indicated that reaction times increased with age in both the immediate and delayed tasks; also the acoustic signal durations increased with age. The analysis of variance showed that the difference between the young and elderly groups was statistically significant. This technique may be considered a useful tool to provide qualitative and quantitative measures of the processes involved in speech production.
