Displaced fragility fractures of the pelvis (FFP) pose significant challenges in orthopaedic trauma, owing to patient comorbidities, deteriorating bone quality, and surgical complexities. Despite technological advancements, no robotic methods have been documented for displaced FFP management. To address this, we introduced an advanced robot-assisted fracture reduction system, comprising a tracking device, path planning software, and robotic arms. This study evaluated fifteen consecutive patients with displaced FFP (average age 80.4 ± 9.1 years), who underwent robot-assisted reduction and internal fixation (RARIF) between January 2022 and May 2023. All were categorized as Rommens FFP type III, with a median time of 6 days (range 4–11) from injury to surgery. Operative times averaged 165 ± 44 min, with median blood loss of 50 mL. Postoperative radiographs showed all patients achieved excellent or good reductions as per Matta criteria, marking a 100 % success rate. A 6-month follow-up revealed an average modified Majeed score of 81.4, with 85.7 % of patients rated excellent or good. All fractures healed without complications. Leveraging our intelligent system, RARIF proves to be a safe and effective approach for displaced FFP, facilitating postoperative pain alleviation and early mobilization despite compromised health and bone conditions. This approach may revolutionize the management of FFP in an increasingly aging population, signaling a significant shift in therapeutic strategies.Translational Potential of this Article: Elderly patients with displaced FFP often present complex surgical challenges due to comorbidities and poor bone quality, complicating reduction procedures and often leading to ineffective fixation. Objective Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy, and endoscopic carpal tunnel release (ECTR) is one of the minimally invasive procedures for the treatment of CTS. Based on the shortcomings of ECTR, we designed the “Modified Soft Tissue Release Kit” to assist the endoscopic operation. This study aimed to evaluate the effectiveness and safety of endoscopic treatment of CTS using this kit. Methods This retrospective review included 57 patients (86 wrists) who underwent ECTR using the “Modified Soft Tissue Release Kit” at our department between January 2017 and August 2019. Three scale scores (i.e., Quick–Disabilities of the Arm, Shoulder, and Hand [QDASH]; Boston Carpal Tunnel Syndrome Questionnaire [BCTSQ]: symptom severity [BCTSQ‐SS] and functional status [BCTSQ‐FS]) were recorded to assess hand function and symptoms preoperatively, 1 month postoperatively, 3 months postoperatively, and at the last follow‐up. We also asked patients to answer a satisfaction question during follow‐up. Pre‐ and post‐operation scores were compared using paired Wilcoxon signed‐rank test. Spearman's rank‐order correlation was used to evaluate the relationship between scale scores and patient satisfaction. Background Andersson lesion (AL) refers to a destructive vertebral or disco‐vertebral lesion of the spine in patients with ankylosing spondylitis (AS). Against the backdrop that the best surgical option for AL remains controversial, this work serves as the first case report of oblique lateral interbody fusion (OLIF) combined with posterior pedicle screw fixation to treat AL of the lumbar spine under minimal invasion. Case presentation In this case report, the patient involved was a female aged 37 diagnosed with AL (L3‐L4). OLIF combined with pedicle screw fixation was carried out to achieve stabilization of the lumbar spine. It turned out that the patient experienced significant pain relief in her excellent post‐operative recovery, with her post‐surgical radiographs indicating good stability and bony fusion. The patient now remains disease‐free with no low back pain or neurological deficit at the two‐year follow‐up. Conclusion OLIF combined with pedicle screw fixation thus has proven to be an ideal therapeutic option for treating AL of the lumbar spine.

Background and Objectives: This study aimed to evaluate the effectiveness and safety of endoscopic gastrocnemius recession using the self-developed Modified Soft Tissue Release Kit. Materials and Methods: This retrospective review followed up 22 patients (34 feet) who underwent endoscopic surgery and 20 patients (30 feet) who received open surgery between January 2020 and January 2022. The American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and the maximum ankle dorsiflexion angle were evaluated preoperatively and at the last follow-up. Postoperative complications were recorded. Patient satisfaction was surveyed at the last follow-up. The comparison between quantitative data was analyzed with the Wilcoxon signed-rank test. The comparison between qualitative data was analyzed with the chi-square test. Results: There was no significant difference in the baseline characteristics between the two groups. The AOFAS score in the endoscopic group increased from 50 (18) points preoperatively to 90 (13) points at the last follow-up; the maximum ankle dorsiflexion angle increased from −7.7 (2.8) degrees to 10.6 (3.6) degrees. The AOFAS score in the open group improved from 47 (15) points preoperatively to 90 (18) points at the last follow-up; the maximum ankle dorsiflexion angle increased from −7.6 (4.0) degrees to 10.7 (3.3) degrees. The change values of the AOFAS scores in the endoscopic and open groups were 39 (15) and 40.5 (11) points, respectively, and there was no significant difference between them. The change values of the maximum ankle dorsiflexion angles in the endoscopic and open groups were 19.5 (4.3) and 19.1 (4.9) degrees, respectively, and there was no significant difference between them. There were no complications, such as sural nerve injury, in both groups. There was no significant difference between the two groups in satisfaction with the surgical outcome. Approximately 94.5% (52/55) of the patients were satisfied or very satisfied with the outcome. Conclusions: ECTR with the "Modified Soft Tissue Release Kit" can significantly relieve symptoms and improve function in patients with CTS, with significant short‐ and mid‐term efficacy and high safety. Background: Minimally invasive surgery has a different visual and tactile perception compared with conventional surgery, which could lead to complications, especially in complex procedures. In these cases, flexible endoscopy can facilitate and prevent complications in minimally invasive procedures in children. The study aimed to clarify the utility of intraoperative endoscopy as an adjuvant to minimally invasive surgery in children. Materials and Methods: This retrospective study reviewed the medical records of pediatric patients who had undergone endoscopy during a minimally invasive surgery to treat an upper digestive pathology between January 2000 and December 2020. Results: The study included 83 patients who underwent a laparoscopic procedure with simultaneous endoscopy. The diagnosis was peptic stenosis in 9 patients, achalasia in 23, congenital embryonic tracheobronchial remnants in 4, re-fundoplication in 42, esophageal duplication in 2, superior mesenteric artery syndrome in 2, and giant gastric hemangioma in 1 patient. With adjuvant endoscopy, 7 digestive perforations were noted, 11 cases of short esophagus were diagnosed, and the permeability of the anastomosis was confirmed in 6 cases. No complications were related with the endoscopy procedures. Discussion: Minimally invasive surgery has a few special and tactile limitations that can lead to complications in certain procedures. Simultaneous digestive endoscopy in the upper gastrointestinal tract facilitates organ identification and dissection. Conclusion: Digestive endoscopy is an excellent adjunct to minimally invasive surgery in children because it facilitates and identifies complications and ensures safer minimally invasive surgeries. Abstract Background To compare the mechanical characteristics of a percutaneous superior pubic intramedullary screw, percutaneous bridging plate and percutaneous screw-rod system of the anterior ring for the treatment of unilateral vertical pubic fractures to provide a reference for clinical application. Methods A finite element model of pelvic anterior ring injury (unilateral vertical pubic fracture) was produced. The fractures were fixed with a percutaneous superior pubic intramedullary screw, percutaneous bridging plate and percutaneous screw-rod system of the anterior ring and their combinations in 5 types of models. The fracture stabilities under vertical, bilateral and anterior-posterior load were quantified and compared based on the displacement of the hip joints’ midpoint as quantificational index of fracture stability. Results In the condition of bilateral and anterior-posterior load, the vertical, bilateral and anterior-posterior displacements of the hip joints’ midpoint of different models were significantly different respectively. In general, the displacements of the 5 pelvic anterior ring fixations were ranked from maximum to minimum as follows: bridging plate, pelvic anterior screw-rod system, combination of bridging plate and pelvic anterior screw-rod system, superior pubic intramedullary screw and combination of superior pubic intramedullary screw and pelvic anterior screw-rod system. Conclusion For the fixation in unilateral pubic fractures of pelvic ring injury, the percutaneous superior pubic intramedullary screw is optimal, the percutaneous pelvic anterior screw-rod system is the second choice, and percutaneous bridging plate ranks the third. The percutaneous pelvic anterior screw-rod system can significantly increase fixation stability of the percutaneous superior pubic intramedullary screw and the percutaneous bridging plate.

Aesthetics has been an important aspect of dentistry along with functionality for decades but its significance is at an all-time high. Hence among techniques that contribute toward this goal, strip crown has shown convincing results. However, its acceptance did not reach its potential, especially among general dentists which could be attributed to its technique-sensitive nature. This case report describes the clinical application of a real-time surgical navigation system to guide zygomatic implants on their placement. With the assistance of this novel navigation system, constant visualization of the drilling trajectory and exact control of the drilling direction can be achieved, which could minimize the risk of penetration into critical anatomical structures. Moreover, implants might be accurately placed in accordance to the preoperative planned position. In this patient, who had previously undergone a hemimaxillectomy, three zygomatic implants were placed on the unilateral zygoma, and two conventional implants were placed on the unresected maxilla. Measurement of distributive deviation of implants between preoperative planning and the postoperative result showed the mean linear deviations at the entrance and apical point of the implant were 1.07 ± 0.15 mm (range: 0.9 to 1.1 mm) and 1.20 ± 0.46 mm (range: 0.7 to 1.6 mm), respectively. The mean angle deviation between the axes of the planned and the actual implant was 1.37 ± 0.21 degrees (range: 1.2 to 1.3 degrees). Objectives: The aims of this clinical study were to compare internal three-dimensional (3D) adaptation of porcelain laminate veneers (PLV) with minimal tooth preparation and without tooth preparation (prepless) and to evaluate the clinical outcomes at baseline and following 6, 12, and 24 months after luting. Method and Materials: Thirty-one prepless PLV and 31 PLV with minimal tooth preparation were fabricated using lithium disilicate glass-ceramic material and placed in 12 patients (8 women, 4 men; 18 to 40 years old). All PLV were luted with an adhesive luting system (Variolink veneer). A silicone replica was obtained to measure internal adaptation of each PLV using a low viscosity polyvinyl siloxane impression material just before luting. Silicone replicas were scanned in x-ray micro computerized tomography (micro CT). Clinical evaluations took place at baseline (2 days after luting) and following 6,12, and 24 months after luting. Marginal integrity, marginal discoloration, secondary caries, tooth sensitivity, and fracture were evaluated following FDI criteria. Replica scores were analyzed using Mann-Whitney U and Student's t test (α = .05). Kaplan-Meier statistical analysis was done for the survival rate of PLV. FDI criteria scores were analyzed using Pearson's chi-square test (α = .05). Results: The median marginal gaps for PLV-without-tooth-preparation and PLV-with-minimal-tooth-preparation groups were 100 urn and 140 µm respectively. There was a statistically significant difference between the two groups with respect to marginal gap (P = .04). The mean internal adaptation for the PLV-without-tooth-preparation group was 217.17 ±54.72 µm, and was 170.67 ±46.54 µm for the PLV-with-minimal-tooth-preparation group. There was a statistically significant difference between the two groups (P = .001). Based on FDI criteria, 100% of the PLV were rated satisfactory during the 2-year period. Conclusions: In this in-vivo study, mean and median values of marginal gap and internal adaptation for PLV with minimal tooth preparation and PLVs without tooth preparation were within a clinically acceptable range. Abstract Background There are only a few published case reports of irreducible Galeazzi fracture-dislocation, and patients in these studies had undergone reduction by open surgical methods. Arthroscopy for the distal radioulnar joint of the wrist joint has recently been used for wrist pathology. We aim to describe the surgical procedure involved in arthroscopic reduction of irreducible Galeazzi fracture-dislocation and clinical outcome and review the literature. Case presentation We present the case of a 26-year-old man, a professional athlete, who sustained Galeazzi fracture-dislocation during a bicycle race. The distal radioulnar joint was irreducible because the fragment of the ulnar styloid was trapped between the sigmoid notch and ulnar head after a doctor had previously reduced it manually. Operative treatment was performed using a 30° oblique, 1.9-mm arthroscope. Reduction of the fragment of the ulnar styloid was achieved using distal radioulnar joint arthroscopy. The metaphyseal and intra-articular fracture of the radius and the fragment of the ulnar styloid were fixed using a volar locking plate and tension band wiring technique, respectively. A daily injection of parathyroid hormone and low-intensity pulsed ultrasound were used postoperatively. The patient was asymptomatic and returned to the preinjury level of athletic activity 2 months postoperatively, and bone union of the radius and ulna was achieved without distal radioulnar joint instability 15 months postoperatively. Conclusions Less invasive reduction of the dorsal anatomical structure enabled our patient to return early to sports. We consider arthroscopic reduction to be superior to the open surgical method in terms of evaluating interpositions; additionally, arthroscopic reduction is minimally invasive and does not need immobilization because it does not cause significant damage to the dorsal capsule and subsheath of the extensor carpi ulnaris, which comprise the triangular fibrocartilage complex.

• A rare case of Tuberculosis Peritonitis regarding the aspect of perioperative infection control. • The importance of characteristic intraperitoneal findings, which contributes to the rapid and accurate diagnosis combined with pathological findings. • Diagnostic laparoscopy must be less invasive and more effective compared with conventional methods. Tuberculous peritonitis (TBP) is uncommon in Japan, and its diagnosis with conventional methods is time taking and requires a high clinical index of suspicion. Laparoscopy with peritoneal biopsy is a tool for rapid and accurate diagnosis of TBP. However, few cases have mentioned the infectious control and prevention during the perioperative period. This case is written following the SCARE scale for case report writing. A 30-year-old man from Southeast Asia with a past medical history of pulmonary tuberculosis at 3-year-old admitted to our institution with abdominal pain and slight fever lasting for a week. With the elevation of inflammatory response and CA125, we conducted CT (Computed tomography). Not only ascites, panniculitis with peritoneal nodules, and the thickening of the omentum were found. Considering the possibility of malignancy and TBP, we performed a diagnostic laparoscopy. Slightly cloudy ascites, peritoneal and thickening omentum with white nodules were seen, and pathological diagnosis from the omentum during the operation raised the possibility of TBP due to its caseating granuloma and these findings allowed us to start the rapid treatment. Aim: The aim of the study was to evaluate the awareness of various dental caries classification systems among dental clinicians in Chennai by means of a survey and derive an opinion on the ease, accuracy, and their understandability of the various classifications. Background: Dr. G. V. Black (1836–1915) developed a system to categorize carious lesions based on the type of tooth affected and the location of the lesion into five classes. It identifies the position of a lesion and prescribes a cavity design regardless of the size and extent of the lesion. This method involved “extension for prevention” in contrast to today’s minimal invasive techniques. G.J. Mount’s classification is based on the site and size of the lesion to encourage a conservative approach to the preservation of natural tooth structure. The International Caries Detection and Assessment System (ICDAS) is a detection and assessment system classifying stages of the caries process which fosters a new model for the measurement of dental caries developed and based on a systematic review of literature on clinical caries detection systems. Materials and Methods: A questionnaire containing questions that assessed the awareness of dental practitioners on the ease, accuracy, and efficiency of the different classification systems of dental caries was distributed to dental clinicians in Chennai (n = 100). Their responses were recorded and analyzed to obtain an opinion on the ease, accuracy, and understandability of the various classification systems. Results: About 75.83% of the dentists supported G. V. Black’s classification. This majority preference for G. V. Black’s classification was found to be because the dentists were unaware of the other newer minimally invasive systems like ICDAS. Conclusion: This study helps understand the current scenario of caries treatment in Chennai. The dentists surveyed have very little awareness of the ease and accuracy of the newer techniques available. BACKGROUND: In the 21st century, domestic and foreign scholars have carried out a lot of basic and clinical researches on L5-S1 simple axial fusion internal fixation, which prove that its technology is minimally invasive, is conducive to the successful fusion of bone grafting and can meet the physiological needs of human body, but its anti-rotation fixation is poor. Therefore, a new internal fixation device that is both minimally invasive and stable is needed. OBJECTIVE: To evaluate the biomechanical stability of the locking axial lumbosacral interbody fusion, so as to provide experimental data for future clinical usage. METHODS: Five fresh human lumbar spines (L3-S5) were tested by applying pure moments. Each specimen was tested for the following order: normal control group; bilateral spondylolysis; locking AxiaLIF and bilateral spondylolysis; AxiaLIF and bilateral spondylolysis; AxiaLIF, bilateral pedicle screws and bilateral spondylolysis. The L3/S1 range of motion was obtained by applying pure moments in flexion, extension, lateral bending, axial rotation and axial compression loaded with 300 N force. The study was approved by the ethics committee of the hospital. RESULTS AND CONCLUSION: (1) Spondylolysis significantly increased the range of motion in flexion-extension and axial rotation compared with the normal control group (P < 0.05). The two groups showed no significant difference in the lateral bending (P > 0.05). (2) Three kinds of fixation ways significantly reduced the range of motion in flexion-extension and lateral bending compared with the normal control group (P< 0.05). In flexion-extension, the combined fixation and locking AxiaLIF groups significantly reduced the range of motion compared with the screw group (P < 0.05), and there was no significant difference between combined fixation and locking AxiaLIF groups (P > 0.05). (3) In lateral bending, the range of motion in the locking AxiaLIF ROM group had no significant difference compared with the other fixation groups (P > 0.05), but the combined group significantly reduced the range of motion compared with the screw group (P < 0.05). (4) Locking AxiaLIF and combined groups significantly reduced the range of motion in axial rotation compared with the normal control group (P < 0.05), and there was no significant difference between screw and normal control groups (P > 0.05). Screw group significantly increased the range of motion in axial rotation compared with the combined group (P < 0.05). In the locking group, there was no significant difference between left axial rotation and right axial rotation (P > 0.05). (5) Three fixation groups significantly increased the axial compressive stiffness compared with the normal control group (P < 0.05), but there was no significant difference among three fixation groups (P > 0.05). (6) These results indicate that the locking axial lumbosacral interbody fusion can significantly enhance the stability of lumbosacral vertebral instability in all loading modes. Objective To compare the safety and reliability of laminated film nickel-titanium alloy stent tube( LFNTST) and modified colon sac tube( MCST) for mediate stage repair of colorectal perforation in Tibet miniature pigs. Methods Colorectal perforationmodels of Tibet miniature pigs were established, and then they were randomly divided into the LFNTST group and MCST group. On the 7 th, 14 th, and 21 st days after procedure, the healing of the perforation was evaluated by examining thebursting pressure( BP) and hydroxyprolinecontent( HC) at site of the anastomosis. Results On the 7 th, 14 th and 21 st days of treatment, the BP was( 27. 21 ± 0. 98), ( 35. 77 ± 0. 72), and( 38. 13 ± 0. 92) Kpa, respectively, in the LF-NTST group, versus( 26. 83 ± 0. 91), ( 35. 23 ± 0. 55), and( 37. 20 ± 0. 95) Kpa, respectively, in theM CST group. For sampling in vivo and surface‐enhanced Raman scattering (SERS) detection ex vivo, a built‐in SERS‐active microneedle is fabricated by adsorbing gold nanoshells (GNSs; as built‐in SERS‐active materials) in a corroded groove on an acupuncture needle. 3,4‐diaminobenzene‐thiol adsorbed on the GNSs is used as a selective SERS probe to detect nitric oxide (NO). The SERS‐active microneedles take SERS probes into the tissues of living mice with minimal invasion to react with NO selectively and then they are pulled out for direct and rapid SERS detection without any pretreatment steps or tedious operations. The built‐in SERS‐active microneedle would become a versatile platform for sampling in vivo and SERS detection ex vivo, taking SERS beyond culture cells to living animals. A built‐in SERS‐active microneedle has been developed for minimally invasive and selective sampling in vivo and direct and rapid SERS detection ex vivo of NO. After being inserted into tissues with minimal invasion, the SERS‐active microneedle would selectively respond target molecules such as NO, that is, using the SERS‐active microneedles, minimally invasive and selective sampling in vivo would be achieved. After being pulled out, the SERS‐active microneedle can be used for collecting SERS spectra directly, that is, using the SERS‐active microneedles, direct and rapid SERS detection ex vivo would be achieved as well. Minimally invasive procedures are the new paradigm in health care. Everything from heart bypasses to gall bladder, surgeries are being performed with these dynamic new techniques. Dentistry is joining this exciting revolution as well. Minimally invasive dentistry adopts a philosophy that integrates prevention, remineralisation and minimal intervention for the placement and replacement of restorations. Minimally invasive dentistry reaches the treatment objective using the least invasive surgical approach, with the removal of the minimal amount of healthy tissues. This paper reviews in brief the concept of minimal intervention in dentistry.

The aim of this study was to confirm the effectiveness of a real-time three-dimensional navigation system for use during various oral and maxillofacial surgeries. Five surgeries were performed with this real-time three-dimensional navigation system. For mandibular surgery, patients wore acrylic surgical splints when they underwent computed tomography examinations and the operation to maintain the mandibular position. The incidence of complications during and after surgery was assessed. No connection with the nasal cavity or maxillary sinus was observed at the maxilla during the operation. The inferior alveolar nerve was not injured directly, and any paresthesia around the lower lip and mental region had disappeared within several days after the surgery. In both maxillary and mandibular cases, there was no abnormal hemorrhage during or after the operation. Real-time three-dimensional computer-navigated surgery allows minimally invasive, safe procedures to be performed with precision. It results in minimal complications and early recovery. Aims The data on the histological type of carcinomatous component and the extent of extracapsular invasion for salivary carcinomas ex pleomorphic adenoma (PA) are conflicting. We aimed to determine the prognostic value of extracapsular invasion in salivary duct carcinomas (SDC) ex PA. Methods and results A total of 117 patients with SDC were identified retrospectively; 44 cases involving major salivary glands had pre-existing PA (44 of 117, 37%). The morphological spectrum of SDC ex PA was characterized. The primary endpoint was overall survival (OS). Most SDC ex PA were widely invasive at presentation (27 of 44; 61%). Five patients with intracapsular SDC ex PA experienced no disease progression. The assessment of extracapsular invasion was precluded in eight cases (e.g. positive margins of resection). The rate of lymph node involvement was similar in cases with extracapsular invasion of ≤2 mm (two of three) and >7 mm (22 of 26). Only pT correlated with OS [116 months, 95% confidence interval (CI) 22-210 months for pT1 versus 20 months (95% CI 6-34) for pT4; P = 0.013]. Conclusions Intracapsular SDC ex PA are potentially indolent. SDC ex PA with extracapsular invasion of ≤2 mm are rare, and appear to be clinically aggressive. A duodenal web refers to partial or complete obstruction of the duodenum due to a membranous web. When looking at the serosal side of the intestine during duodenal web excision, especially during laparoscopic surgery, determining the web origin might be challenging. This study aimed to determine the efficacy of intraduodenum indocyanine green injection and near-infrared light during laparoscopic surgery in enhancing the ability to identify duodenal web localization. We used intraduodenum indocyanine green injection to unequivocally recognize the duodenal web and facilitate laparoscopic excision. The clinical analysis was based on a male neonatal case with duodenal web admitted to our hospital. After the patient was placed in the supine position, 5 mL of ICG was injected via a 6-Fr nasogastric tube. The fluorescence was visualized in the white light and near-infrared light dual-mode of the camera system, localizing the duodenal precisely and in real-time. Laparoscopically, the duodenal web was accurately identified and removed with intraduodenum indocyanine green visualization under near-infrared light. The procedure was completed successfully, and the patient showed good postoperative outcomes. Intraduodenum indocyanine green injection during laparoscopic surgery is a feasible adjuvant for duodenal web localization, showing improvements in terms of outcomes compared to previous methods of determining the duodenal web location. Objectives: The goal of the present study was to determine the safety and efficacy of a modified off-pump biatrial mini-maze procedure to treat long-standing persistent atrial fibrillation (AF)., Methods: Patients with long-standing persistent AF underwent our modified mini-maze procedure using bipolar radiofrequency ablation. Those patients first underwent a mini-maze procedure using the Dallas lesion set protocol. Second, a purse-string suture was performed on the right atrium, and then 4 ablation lesions were made to the superior vena cava, the inferior vena cava, the appendix of the right atrium and the tricuspid valve annulus from the purse-string suture point by the bipolar radiofrequency clamp. After the operation, the patients were followed up at 3, 6 and 12 months and every year thereafter., Results: A total of 102 patients were included in the study. There were no deaths, no surgical re-exploration for bleeding and no permanent pacemaker implants. The intraoperative cardioversion rate was 42.2% (43/102). A follow-up at intervals of 3, 6, 12, 24, 36 and 48 months showed that a success rate free from long-standing persistent AF was 95.1% (97/102), 94.4% (85/90), 94.8% (73/77), 91.5% (54/59), 90.3% (28/31) and 86.4% (19/22), whereas freedom from AF in patients off antiarrhythmic drugs was 88.2% (90/102), 85.6% (77/90), 81.8% (63/77), 78.0% (46/59), 74.2% (23/31) and 68.2% (15/22), respectively., Conclusions: The modified biatrial mini-maze procedure proved to be safe and feasible. The effect and safety of anterior debridement and fusion with a minimally invasive approach combined with posterior fixation via the Wiltse approach were assessed in the single-level lumbar pyogenic spondylodiscitis. Seventeen patients from 2007 to 2009 underwent anterior debridement and fusion with a minimally invasive approach combined with posterior fixation via the Wiltse approach. Postoperative follow-up time was 24-41 months. Data included the patients' general information, microbiology, operative time, intraoperative blood loss, postoperative complications, intervertebral fusion rate, and preoperative and final follow-up scores for American Spinal Injury Association (ASIA) impairment, visual analogue scale (VAS), and Oswestry Disability Index (ODI). Ten patients had undergone a prior spinal invasive procedure, and 7 had hematogenous infection. The infected segments included L, L, L, and L in 1, 2, 5, and 9 cases, respectively. Thirteen bacterial cultures were positive for Staphylococcus aureus (5 cases), Staphylococcus epidermidis (4), Streptococcus (3), and Escherichia coli (1). The operative time was 213.8±45.6 min, and the intraoperative blood loss was 180.6±88.1 mL. Postoperative complications consisted of urinary retention (2 cases), constipation (3), and deep vein thrombosis (2). On the final follow-up, VAS scores and ODIs were significantly lower than those of preoperation, while the ASIA grades improved. All the cases achieved good intervertebral bony fusion. Anterior debridement and fusion with a minimally invasive approach combined with posterior fixation via the Wiltse approach can successfully treat single-level lumbar pyogenic spondylodiscitis, with less trauma and reliable immobilization. Abstract: This study seeks to discuss the efficiency of minimally invasive surgery of posterior long segmental fixation plus direct decompression in patients with spinal metastatic tumors. Twenty-five patients received minimally invasive surgery of long segmental fixation combined with direct decompression from posterior approach. Pain and neurologic improvement in these patients pre- and post operation were evaluated by Denis'' Pain Scale and Frankel Score, respectively. Seventeen patients (68.0%) showed significant decreases in Denis'' Pain score after surgery (p < 0.0001). Paralysis symptoms were improved in nineteen patients (76.0%). The Frankel Score exhibited significant difference between pre-operation and post-operation (p < 0.0001). Operation time and blood loss in this cohort were 324 ± 90 min and 1047 ± 730 ml, respectively. No fatal complications were observed as a result of surgery. In conclusion, minimally invasive surgery of posterior long segmental fixation combined with direct decompression is a safe and efficient strategy to release pain and improve neurological function in patients with spinal metastatic tumors. Background: Intraventricular hemorrhage is a neurosurgical emergency, and a dangerous condition associated with high morbidity and mortality. Previously, hematoma evacuation is generally executed by external intracranial drainage (EVD) or surgical evacuation. Nowadays, endoscopic evacuation is emerging as a good alternative because it brings relatively less invasion and injury. However, successful endoscopic evacuation requires skilled manipulation of endoscopic devices and the evidence supporting its efficacy differs in different reports., Aim: To improve the technique usage and provide more evidence of endoscopic evacuation efficacy, we summarize our surgical experience and compared the outcomes of the endoscopic evacuation with EVD using real-world data., Methods: We retrospectively studied 96 consecutive patients with intraventricular hemorrhage who underwent either endoscopic surgery ( n = 43) or non-endoscopic surgery ( n = 53) for hemorrhage evacuation between November 2013 and September 2019 in our center. Patients' conditions prior to and after the operation were evaluated and analyzed to assess the efficacy of the operation. The consciousness status improvement and perioperative in-hospital parameters in the two types of operation groups were assessed and compared., Results: Patients in the endoscopic and non-endoscopic groups presented with a similar state of consciousness, with a comparable Glasgow Coma Scale (GCS) index. The average operation time of the endoscopic group was longer than that of the non-endoscopic group (median 2.42 h vs 1.08 h, P < 0.001). Although the endoscopic group was older and had a baseline Graeb score that indicated more severe hemorrhage than the non-endoscopic group (Graeb median: Endoscopic group = 9 vs non-endoscopic group = 8, P = 0.023), the clearance rate of hematoma was as high as 60.5%. Both the endoscopic and non-endoscopic groups showed an improved GCS index after surgery. However, this improvement was more marked in patients in the endoscopic group (median improvement of GCS index: Endoscope group = 4 vs non-endoscopic group = 1, P < 0.001). Additionally, the endoscopic group had a lower Graeb score than the non-endoscopic group after the operation. Intraductal papillary mucinous neoplasms (IPMNs) of the pancreas are classified into the following four histopathologic subtypes: gastric, intestinal, pancreatobiliary, and oncocytic. However, the clinicopatholgic characteristics of IPMN subtypes have not been fully clarified. Recently, a subgroup classification of minimally invasive intraductal papillary mucinous carcinomas (MI-IPMCs) was suggested in contrast to overt invasive carcinoma from IPMCs (IC-IPMCs). The purpose of this study was to determine whether or not the pathologic subtype classification can predict prognosis and to validate the usefulness of the newly proposed diagnostic criteria of MI-IPMCs. We reviewed the clinicopathologic characteristics of 142 surgically resected cases of IPMNs. There were 54, 56, 30, and two cases of the gastric, intestinal, pancreatobiliary, and oncocytic types of IPMNs, respectively. The intestinal and pancreatobiliary types were more likely to have a main duct type. All gastric type tumors were adenomas or moderate dysplasia, whereas greater than one half of the intestinal and pancreatobiliary types were carcinomas in situ or invasive carcinomas. A significant difference in recurrence and death rate was noted for invasive carcinoma between the intestinal and pancreatobiliary types. The majority of MI-IPMCs were the intestinal type, whereas the majority of IC-IPMCs were the pancreatobiliary type. The IC-IPMC group showed a decreased recurrence-free and overall survival with statistically significance ( p < 0.001 and p = 0.001, respectively). Our results suggest that the pathologic subtype classification and the newly proposed diagnostic criteria for minimal invasion may also be useful to predict prognosis of IPMNs of the pancreas. PURPOSE: To evaluate the efficacy of internal rigid fixation of zygomatic fracture through percutaneous minimally invasive approach. METHODS: Three hundred and forty-two cases of zygomatic fractures were selected from June 2003 to 2009, using Zingg classification, all cases were divided into three groups and were treated by internal rigid fixation through percutaneous minimally invasive access. RESULTS: Nineteen patients had infection, the other patients had primary healing. All patients got symmetrical facial contour, the fractured bone segments integrated well on computed tomograph scan, the facial scars were minimal. CONCLUSION: Percutaneous minimally invasive access for internal rigid fixation of zygomatic fracture produces minimal injury, inconspicuous facial scars, fewer complications, and is an ideal way for management of zygomatic fracture.
