The fast development of technology and transportation systems has resulted in a disturbing level of noise pollution over time that requires research and regulatory. Most existing research is deemed ineffective as it primarily focuses on conducting assessments of traffic noise without providing a structured framework for noise mitigation. This study focused on three main stages to monitor road traffic noise in Sungai Long area: (1) examination of traffic features, (2) derivation of noise indices, and (3) evaluation of the influence of traffic noise. Total of four roads were selected with four stations on each road in Bandar Sungai Long to assess the traffic noise impact. The sound level metre (SLM) was used to calculate the noise indices for each road. Besides, peak-hour sessions and off-peak sessions of traffic flow were measured in field measurement to compare the differences in noise conditions. Additionally, based on the measured noise indices, traffic noise index (TNI), noise pollution level (LNP), and expected community response were calculated to reveal the impact of traffic noise. Overall, this study's findings demonstrated that residential areas experiencing traffic noise disturbance, particularly on roads, namely Jalan Bp Satu and Jalan Persiaran SL 7, had exceeded the Malaysian environmental standard. In addition, two optimized methods are proposed which are prohibiting heavy vehicles and a simple noise barrier was designed on Jalan Bp Satu and Jalan Persiaran SL 7. The expected noise level between these two methods had been compared in the discussion part. In summary, prohibiting heavy vehicles on the road is more suitable if the aim is to save cost. However, if reducing traffic noise is a priority and a long-term solution is needed, constructing noise barrier may be a more effective solution.

BackgroundRefractory gastroesophageal reflux disease (refractory GERD) is a heterogeneous disease characterized by unresponsiveness or poor efficacy to proton-pump inhibitors (PPIs). This chronic disorder substantially weakens patients’ mental wellbeing and quality of life, increasing the financial burden on society. Multiple articles have been reported in this area. However, literature involving scientometric analysis of refractory GERD is absent. Therefore, it is necessary to understand the evolution of research themes and the main hotspots of refractory GERD through bibliometric methods.MethodsAll documents related to refractory GERD based on the WOS Core Collection from January 2000 to November 2023 were selected for analysis. Citespace V 6.1 R6, VOSviewer V 1.6.20, and Scimago Graphica V 1.0.38 were used to perform bibliometric analysis.ResultsWe collected a total of 241 research articles from 36 countries and 322 institutions, contributed by over 1,000 authors. Over the last 20 years, the number of articles in this field has increased year by year, and since 2011, the number of publications has increased dramatically, with 85.89% of the papers. These countries are led by the United States and Japan. GUT had the highest number of citations and DIGESTION had the highest number of publications. Research on standardized diagnosis and management, mechanisms, novel monitoring methods, and innovative drugs and procedures for refractory GERD are the main topics and hotspots in this field. This study also found that neuroimmune interaction is closely related to refractory GERD, which may be a new direction for future mechanism research.ConclusionOur study is the first bibliometric analysis of the global literature on refractory GERD. This research provides valuable insights for researchers, enabling them to quickly understand the research frontier and hot topics of this field.

The COVID-19 pandemic is an unpredictable event that has had unexpected effects on many countries, including Malaysia. The Government of Malaysia implemented the Movement Control Order (MCO) to restrain the outbreak of the pandemic. As a result, MCO has had a huge impact on various industries in Malaysia, especially the construction industry. Therefore, time and cost become critical components in order to successfully deliver the project and are the factors most affecting by the COVID-19. This paper explores the impact of the main contractor’s awareness, perceptions, and readiness for the Covid-19 pandemic. Quota and snowball sampling methods were used. Total of 167 samples were collected and analyzed using SPSS software. This research presents the main contractor’s perception and readiness has significant correlation to the pandemic disruption in Malaysia while the main contractor’s awareness showing no correlation. The position level of main contractor’s traits shows no significant to the pandemic disruption. These results provide evidence that main contractor is significantly affecting by the pandemic disruptions, thus cultivating the main contractor’s traits in the construction industry is needed.

ObjectivePrevious research has shown that human identical sequences of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) promote coronavirus disease 2019 (COVID-19) progression by upregulating hyaluronic acid (HA). However, the association of HA with mortality and long COVID in SARS-CoV-2 reinfection and first infection is unclear.MethodsPatients with COVID-19 at Beijing Ditan Hospital from September 2023 to November 2023 were consecutively enrolled. SARS-CoV-2 reinfections were matched 1:2 with first infections using a nearest neighbor propensity score matching algorithm. We compared the hospital outcomes between patients with COVID-19 reinfection and first infection. The association between HA levels and mortality and long COVID in the matched cohort was analyzed.ResultsThe reinfection rate among COVID-19 hospitalized patients was 25.4% (62 cases). After propensity score matching, we found that reinfection was associated with a better clinical course and prognosis, including lower levels of C-reactive protein and erythrocyte sedimentation rate, fewer cases of bilateral lung infiltration and respiratory failure, and shorter viral clearance time and duration of symptoms (p 0.05).ConclusionSerum HA may serve as a novel biomarker for predicting COVID-19 mortality in patients with SARS-CoV-2 reinfection and first infection. However, HA levels may not be associated with long COVID.

IntroductionThis study aimed to develop a prognostic nomogram for predicting the recurrence-free survival (RFS) of hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) patients with low preoperative platelet-albumin-bilirubin (PALBI) scores after transarterial chemoembolization (TACE) combined with local ablation treatment.MethodsWe gathered clinical data from 632 HBV-related HCC patients who received the combination treatment at Beijing You’an Hospital, affiliated with Capital Medical University, from January 2014 to January 2020. The patients were divided into two groups based on their PALBI scores: low PALBI group (n=247) and high PALBI group (n=385). The low PALBI group was then divided into two cohorts: training cohort (n=172) and validation cohort (n=75). We utilized eXtreme Gradient Boosting (XGBoost), random survival forest (RSF), and multivariate Cox analysis to pinpoint the risk factors for RFS. Then, we developed a nomogram based on the screened factors and assessed its risk stratification capabilities and predictive performance.ResultsThe study finally identified age, aspartate aminotransferase (AST), and prothrombin time activity (PTA) as key predictors. The three variables were included to develop the nomogram for predicting the 1-, 3-, and 5-year RFS of HCC patients. We confirmed the nomogram’s ability to effectively discern high and low risk patients, as evidenced by Kaplan-Meier curves. We further corroborated the excellent discrimination, consistency, and clinical utility of the nomogram through assessments using the C-index, area under the curve (AUC), calibration curve, and decision curve analysis (DCA).ConclusionOur study successfully constructed a robust nomogram, effectively predicting 1-, 3-, and 5-year RFS for HBV-related HCC patients with low preoperative PALBI scores after TACE combined with local ablation therapy.

IntroductionThis study conducts a retrospective analysis on patients with BCLC stage A/B hepatocellular carcinoma (HCC) accompanied by Child-Pugh B cirrhosis, who underwent transarterial chemoembolization (TACE) in combination with local ablation therapy. Our goal was to uncover risk factors contributing to post-treatment recurrence and to develop and validate an innovative 1-, 3-, and 5-year recurrence free survival (RFS) nomogram.MethodsData from 255 BCLC A/B HCC patients with Child-Pugh B cirrhosis treated at Beijing You’an Hospital (January 2014 - January 2020) were analyzed using random survival forest (RSF), LASSO regression, and multivariate Cox regression to identify independent risk factors for RFS. The prognostic nomogram was then constructed and validated, categorizing patients into low, intermediate, and high-risk groups, with RFS assessed using Kaplan-Meier curves.ResultsThe nomogram, integrating the albumin/globulin ratio, gender, tumor number, and size, showcased robust predictive performance. Harrell’s concordance index (C-index) values for the training and validation cohorts were 0.744 (95% CI: 0.703–0.785) and 0.724 (95% CI: 0.644–0.804), respectively. The area under the curve (AUC) values for 1-, 3-, and 5-year RFS in the two cohorts were also promising. Calibration curves highlighted the nomogram’s reliability and decision curve analysis (DCA) confirmed its practical clinical benefits. Through meticulous patient stratification, we also revealed the nomogram’s efficacy in distinguishing varying recurrence risks.ConclusionThis study advances recurrence prediction in BCLC A/B HCC patients with Child-Pugh B cirrhosis following TACE combined with ablation. The established nomogram accurately predicts 1-, 3-, and 5-year RFS, facilitating timely identification of high-risk populations.

Construction noise is an occupational noise that is potentially harmful, and it usually originates from machinery in construction sites. The impact of construction noise on the health and safety of construction workers is one of the main concerns in the industry. Prolonged noise exposure duration will cause physiological, physical, psychological damage, and negatively affect the working performance of the workers as well. Furthermore, the adverse impacts arising from construction noise may jeopardize public welfare, particularly for those who live nearby the construction site. Therefore, this research aims to develop a Stochastic Deep Learning (SDL) noise prediction model by integrating stochastic modelling and deep learning techniques. Stochastic modelling is applied in this study to generate a set of simulated randomized data based on several parameters as the input for the deep learning model. The deep learning model is trained with the input from stochastic modelling to predict the noise levels emitted from the construction site. The predictive performance of the deep learning model will be assessed with several statistical measures such as absolute difference, mean absolute difference and root mean square deviation. Ten case studies are conducted to validate the reliability and accuracy of the SDL noise prediction model. The SDL model showed high accuracy of prediction results with an average absolute difference of less than 1.2 A-weighted decibels (dBA) among the case studies as compared to the measurement. The reliability of the results from the prediction model is high. In conclusion, the SDL model is established and provided a promising outcome with satisfactory predictive performance. Finally, further development of the model would be worthwhile to fully exploit the potential of the SDL noise prediction model in construction industries as a planning, managerial and monitoring tool.

Fast-scan cyclic voltammetry (FSCV) is an electrochemical sensing technique that can be used for neurochemical sensing with high spatiotemporal resolution. Carbon fiber microelectrodes (CFMEs) are traditionally used as FSCV sensors. However, CFMEs are prone to electrochemical fouling caused by oxidative byproducts of repeated serotonin (5-HT) exposure, which makes them less suitable as chronic 5-HT sensors. Our team is developing a boron-doped diamond microelectrode (BDDME) that has previously been shown to be relatively resistant to fouling caused by protein adsorption (biofouling). We sought to determine if this BDDME exhibits resistance to electrochemical fouling, which we explored on electrodes fabricated with either femtosecond laser cutting or physical cleaving. We recorded the oxidation current response after 25 repeated injections of 5-HT in a flow-injection cell and compared the current drop from the first with the last injection. The 5-HT responses were compared with dopamine (DA), a neurochemical that is known to produce minimal fouling oxidative byproducts and has a stable repeated response. Physical cleaving of the BDDME yielded a reduction in fouling due to 5-HT compared with the CFME and the femtosecond laser cut BDDME. However, the femtosecond laser cut BDDME exhibited a large increase in sensitivity over the cleaved BDDME. An extended stability analysis was conducted for all device types following 5-HT fouling tests. This analysis demonstrated an improvement in the long-term stability of boron-doped diamond over CFMEs, as well as a diminishing sensitivity of the laser-cut BDDME over time. This work reports the electrochemical fouling performance of the BDDME when it is repeatedly exposed to DA or 5-HT, which informs the development of a chronic, diamond-based electrochemical sensor for long-term neurotransmitter measurements in vivo.

Background: PreS1-binding protein (PreS1BP), recognized as a nucleolar protein and tumor suppressor, influences the replication of various viruses, including vesicular stomatitis virus (VSV) and herpes simplex virus type 1 (HSV-1). Its role in hepatitis B virus (HBV) replication and the underlying mechanisms, however, remain elusive. Methods: We investigated PreS1BP expression levels in an HBV-replicating cell and animal model and analyzed the impact of its overexpression on viral replication metrics. HBV DNA, covalently closed circular DNA (cccDNA), hepatitis B surface antigen (HBsAg), hepatitis B core antigen (HBcAg), and HBV RNA levels were assessed in HBV-expressing stable cell lines under varying PreS1BP conditions. Furthermore, co-immunoprecipitation and ubiquitination assays were used to detect PreS1BP- hepatitis B virus X protein (HBx) interactions and HBx stability modulated by PreS1BP. Results: Our study revealed a marked decrease in PreS1BP expression in the presence of active HBV replication. Functional assays showed that PreS1BP overexpression significantly inhibited HBV replication and transcription, evidenced by the reduction in HBV DNA, cccDNA, HBsAg, HBcAg, and HBV RNA levels. At the molecular level, PreS1BP facilitated the degradation of HBx in a dose-dependent fashion, whereas siRNA-mediated knockdown of PreS1BP led to an increase in HBx levels. Subsequent investigations uncovered that PreS1BP accelerated HBx protein degradation via K63-linked ubiquitination in a ubiquitin-proteasome system-dependent manner. Co-immunoprecipitation assays further established that PreS1BP enhances the recruitment of the proteasome 20S subunit alpha 3 (PSMA3) for interaction with HBx, thereby fostering its degradation. Conclusions: These findings unveil a previously unidentified mechanism wherein PreS1BP mediates HBx protein degradation through the ubiquitin-proteasome system, consequentially inhibiting HBV replication. This insight positions PreS1BP as a promising therapeutic target for future HBV interventions. Further studies are warranted to explore the clinical applicability of modulating PreS1BP in HBV therapy.

IntroductionThis study aimed to assess factors affecting the prognosis of early-stage hepatocellular carcinoma (HCC) patients undergoing ablation therapy and create a nomogram for predicting their 3-, 5-, and 8-year overall survival (OS).MethodsThe research included 881 early-stage HCC patients treated at Beijing You’an Hospital, affiliated with Capital Medical University, from 2014 to 2022. A nomogram was developed using independent prognostic factors identified by Lasso and multivariate Cox regression analyses. Its predictive performance was evaluated with concordance index (C-index), receiver operating characteristic curve (ROC), calibration curve, and decision curve analysis (DCA).ResultsThe study identified age, tumor number, tumor size, gamma-glutamyl transpeptidase (GGT), international normalized ratio (INR), and prealbumin (Palb) as independent prognostic risk factors. The nomogram achieved C-indices of 0.683 (primary cohort) and 0.652 (validation cohort), with Area Under the Curve (AUC) values of 0.776, 0.779, and 0.822 (3-year, 5-year, and 8-year OS, primary cohort) and 0.658, 0.724, and 0.792 (validation cohort), indicating that the nomogram possessed strong discriminative ability. Calibration and DCA curves further confirmed the nomogram’s predictive accuracy and clinical utility. The nomogram can effectively stratify patients into low-, intermediate-, and high-risk groups, particularly identifying high-risk patients.ConclusionsThe established nomogram in our study can provide precise prognostic information for HCC patients following ablation treatment and enable physicians to accurately identify high-risk individuals and facilitate timely intervention.

Abstract As an important green manufacturing process, dry grinding has problems such as high grinding temperature and insufficient cooling capacity. Aiming at the problems of sticking and burns in dry grinding of titanium alloys, grinding performance evaluation of molybdenum disulfide (MoS2) solid lubricant coated brazed cubic boron carbide (CBN) grinding wheel (MoS2-coated CBN wheel) in dry grinding titanium alloys was carried out. The lubrication mechanism of MoS2 in the grinding process is analyzed, and the MoS2-coated CBN wheel is prepared. The results show that the MoS2 solid lubricant can form a lubricating film on the ground surface and reduce the friction coefficient and grinding force. Within the experimental parameters, normal grinding force decreased by 42.5%, and tangential grinding force decreased by 28.1%. MoS2 lubricant can effectively improve the heat dissipation effect of titanium alloy grinding arc area. Compared with common CBN grinding wheel, MoS2-coated CBN wheel has lower grinding temperature. When the grinding depth reaches 20 μm, the grinding temperature decreased by 30.5%. The wear of CBN grains of grinding wheel were analyzed by mathematical statistical method. MoS2 lubricating coating can essentially decrease the wear of grains, reduce the adhesion of titanium alloy chip, prolong the service life of grinding wheel, and help to enhance the surface quality of workpiece. This research provides high-quality and efficient technical support for titanium alloy grinding.

The edge DP-chromatic number of G, denoted by χDP′(G), is the minimum k such that G is edge DP-k-colorable. In 1999, Juvan, Mohar, and Thomas proved that the edge list chromatic number of K4-minor free graph G with Δ≥3 is Δ. In this paper, we prove that if G is a K4-minor free graph, then χDP′(G)∈{Δ,Δ+1}, and equality χDP′(G)=Δ+1 holds for some K4-minor free graph G with Δ=3. Moreover, if G is a planar graph with Δ≥9 and with no intersecting triangles, then χDP′(G)=Δ.

Preprocessing plays a key role in Raman spectral analysis. However, classical preprocessing algorithms often have issues with reducing Raman peak intensities and changing the peak shape when processing spectra. This paper introduces a unified solution for preprocessing based on a convolutional autoencoder to enhance Raman spectroscopy data. One is a denoising algorithm that uses a convolutional denoising autoencoder (CDAE model), and the other is a baseline correction algorithm based on a convolutional autoencoder (CAE+ model). The CDAE model incorporates two additional convolutional layers in its bottleneck layer for enhanced noise reduction. The CAE+ model not only adds convolutional layers at the bottleneck but also includes a comparison function after the decoding for effective baseline correction. The proposed models were validated using both simulated spectra and experimental spectra measured with a Raman spectrometer system. Comparing their performance with that of traditional signal processing techniques, the results of the CDAE-CAE+ model show improvements in noise reduction and Raman peak preservation.

Gastric cancer (GC) is one of the most common malignancies worldwide and the leading cause of cancer-related deaths. Exosomes are nanoscale extracellular vesicles secreted by a variety of cells and play an important role in cellular communication and epigenetics by transporting bioactive substances in the tumor microenvironment (TME). Circular RNA (circRNA) is a type of non-coding RNA (ncRNA) with a specific structure, which is widely enriched in exosomes and is involved in various pathophysiological processes mediated by exosomes. Exosomal circRNAs play a critical role in the development of GC by regulating epithelial-mesenchymal transition (EMT), angiogenesis, proliferation, invasion, migration, and metastasis of GC. Given the biological characteristics of exosomal circRNAs, they have more significant diagnostic sensitivity and specificity in the clinic and may become biomarkers for GC diagnosis and prognosis. In this review, we briefly describe the biogenesis of exosomes and circRNAs and their biological functions, comprehensively summarize the mechanisms of exosomal circRNAs in the development of GC and chemotherapy resistance, and finally, we discuss the potential clinical application value and challenges of exosomal circRNAs in GC.

This work numerically investigates surfactant effects on spontaneous water imbibition in oil-wet carbonates. An open boundary core-scale imbibition model with 9 × 9 × 10 gridblocks was used in UTCHEM to simulate carbonate core plug exposure to a vast water body. The simulation models were developed based on surfactant-assisted imbibition tests that were conducted in secondary and tertiary oil production modes using Amott cells at 75°C. Capillary and gravity forces were captured by history matching the experiments. Through history matching, the inputs for surfactant adsorption and diffusion, capillary pressure and relative permeability were calibrated. In tertiary mode, the surfactants-assisted imbibition process presents the performance in mixed-wet state rather than oil-wet state, which is governed by wettability alteration. A simulation model for surfactant-assisted imbibition in secondary mode was used to investigate the effects of various factors including interfacial tension (IFT) reduction, wettability alteration, adsorption, volume of surrounding water and capillary force. The simulation results suggest that surfactant-assisted water imbibition in secondary mode is gravity dominant, which is facilitated by both IFT reduction and wettability alteration caused by addition of proper surfactants. Different from water imbibition in water-wet core, it presents vertically dominant oil flow with a hemispherical oil-rich area and uneven remaining oil saturation. It is obvious that sufficient surfactant supply in vast water is required to make effective imbibition, in consideration of surfactant consumption and changes in concentration gradients. This core-scale modeling provides insights of surfactant-assisted imbibition in initially oil-wet carbonates and helps scale up the application in a cost-effective way.

This paper presents an experimental and analytical study of comprehensive effects of surfactant and polymer in aqueous solutions on the propagation in carbonates. Single-phase flow tests of surfactant, polymer and surfactant-polymer mixture were respectively conducted on natural carbonate core plugs at high temperature and high salinity reservoir conditions. The effect of different factors on chemical propagation was taken into account by use of a convection-dispersion model. The dynamic adsorptions of two amphoteric surfactants at 2000 mg/L concentration onto the carbonate rocks were 0.21 and 0.17 mg/g-rock, respectively. The dynamic adsorptions of a sulfonated polymer at 2000 and 5000 mg/L were 0.11 and 0.17 mg/g-rock, respectively. Surfactant-polymer mixtures in aqueous solutions were co-injected to evaluate their competitive adsorption, showing the surfactant adsorption was reduced by approximately 50%. The dispersion coefficient of the chemicals in the carbonate cores was in the magnitude of 10−3 cm2/s. Caused by dispersion and adsorption, chemical concentration reduction occurred obviously during the propagation in carbonate porous media. Chromatographic separation took place in the surfactant-polymer co-injection scheme, which was estimated by modeling as well.

Objectives: Xpert Mycobacterium tuberculosis/rifampin (MTB/RIF) Ultra (Xpert-Ultra) has shown better sensitivity in comparison with Xpert MTB/RIF (Xpert) in extrapulmonary tuberculosis (TB), whereas the head-to-head comparison of these methods in TB lymphadenitis had barely been performed. Methods: Patients with undiagnosed lymphadenopathy were recruited prospectively and consecutively, and fine-needle aspiration (FNA) biopsy or lymph node tissue was collected. The specimen was subjected to smear, culture, Xpert, and Xpert-Ultra assays. Culture and/or smear for acid-fast bacilli (AFB) or AFB observed on histopathology were performed as a reference. Results: A total of 106 participants were recruited, including 41 confirmed TB, 33 probable TB, and 32 non-TB lymphadenopathies. The head-to-head comparison for MTB detection showed that Xpert-Ultra produced the highest sensitivity when compared with smear, culture, and Xpert (75.7% vs 5.4 %, 13.5%, and 48.7%). When Xpert-Ultra outcomes were integrated for diagnosis, the percentage of confirmed TB lymphadenitis cases increased from 55.4% (41/74) to 85.1% (63/74). The sensitivities of Xpert-Ultra and Xpert on tissue were 73.6% (95% CI: 59.4-84.3) and 39.6% (95% CI: 26.8-54.0), respectively. The sensitivity of Xpert-Ultra on FNA samples (81.0%, 95% CI: 57.4-93.7) was higher than that of Xpert (71.4%, 95% CI: 47.7-87.8). Conclusion: Xpert-Ultra detected significantly more TB lymphadenitis cases than Xpert or culture. This superiority was particularly distinct using lymph node tissue than FNA detection.

We report herein a new class of polystyrene-supported cinchona alkaloid amide catalysts for enantioselective allylation of various aldehydes using allyltrichlorosilane under both batch and continuous flow conditions. The supported catalyst was synthesized using an environmentally benign coupling agent with a surfactant in aqueous media. Under batch conditions, consistently high yields and enantioselectivity were obtained for the allylation of aliphatic aldehydes with recycling and reuse of the catalyst for more than 10 runs. Subsequently, this catalytic system was successfully implemented into a packed bed flow reactor with similar efficiency and enantioselectivity. While flow is a viable option, the batch methodology has better potential for application at a larger-scale setting upon the comparison of space-time yield and catalyst loadings. With the sustainable synthesis and great recyclability of our polymeric catalyst, this methodology holds great potential for the large-scale delivery of valuable enantiopure homoallylic alcohols.

Eye diseases can cause great inconvenience to people’s daily life; therefore, it is necessary to study the causes of ocular diseases and related physiological processes. Raman spectroscopic imaging (RSI) is a non-destructive, non-contact detection technique with the advantages of label-free, non-invasive and highly specific. Compared with other mature imaging technologies, RSI can provide real-time molecular information and high-resolution imaging at relatively low cost, making it very suitable for quantitative detection of biological molecules. RSI can reflect the overall situation of the sample, revealing the content distribution of the same substance in different areas of the sample. This review focuses on the recent advances in ophthalmology, with particular emphasis on the powerful use of RSI techniques, as well as its combination with other imaging techniques. Finally, we prospect the wider application and future potential of RSI approaches in ophthalmology.

Introduction and Objectives: Cirrhotic patients with acute variceal hemorrhage (AVH) have high short-term mortality. Established prognostic scores are seldom applicable clinically, partially because they need external validation or contain subjective variables. We aimed to develop and validate a practical prognostic nomogram based on objective predictors to predict prognosis for cirrhotic patients with AVH. Patients and Methods: We enrolled 308 AVH patients with cirrhosis from our center as the derivation cohort to develop a new nomogram using logistic regression and validated it in cohorts of patients from Medical Information Mart for Intensive Care (MIMIC) III (n = 247) and IV (n = 302). Results: International normalized ratio (INR), albumin (ALB) and estimated glomerular filtration rate (eGFR) were identified as predictors for inpatient mortality and a nomogram was constructed based on them. The nomogram discriminated well in both derivation and MIMIC-III/-IV validation cohorts with the area under the receiver operating characteristic curves (AUROCs) of 0.846 and 0.859/0.833, respectively and showed a better agreement between expected and observed outcomes (Hosmer-Lemeshow tests, all comparisons, P > 0.05) than other scores in all cohorts. Our nomogram had the lowest Brier scores (0.082/0.114/0.119 in training/MIMIC-III/MIMIC-IV) and highest R2 (0.367/0.393/0.346 in training/MIMIC-III/MIMIC-IV) compared to the recalibrated model for end-stage liver disease (MELD), MELD-hepatic encephalopathy (MELD-HE) and cirrhosis acute gastrointestinal bleeding (CAGIB) scores in all cohorts. Conclusions: We developed a practical prognostic nomogram using easily verified indicators available in initial patient evaluation, which may serve as a reliable tool to accurately predict inpatient mortality for cirrhotic patients with AVH.

We demonstrate the use of a self-injection locked distributed feedback (DFB) diode laser for high-sensitivity detection of acoustic emission (AE) using a fiber-coil Fabry-Perot interferometer (FPI) sensor. The FPI AE sensor is formed by two weak fiber Bragg gratings on the ends of a long span of coiled fiber, resulting in dense sinusoidal fringes in its reflection spectrum that allows the use of a modified phase-generated carrier demodulation method. The demodulation method does not require agile tuning capability of the laser, which makes the self-injection locked laser particularly attractive for the application. Experimental results indicate that the self-injection locked laser increases the signal-to-noise ratio by ∼33 dB compared with the free-running DFB laser. We studied the mode-hopping and laser instability of the self-injection locked laser and their effect on the demodulated signal and found that a mode hopping event causes an abrupt change in the laser intensity after the resonator inside the feedback loop. It manifests itself as a short transient signal in the output of the AE sensor system. With the identification of the mode-hopping events, the associated spurious signal can be identified and discarded in the signal processing without causing significant disruption to the sensor system. Experiment shows that the frequency of locked lasers could oscillate during unstable operations. The fundamental frequency is determined by the time delay of the feedback light and is typically much larger than the AE frequency. Therefore, the laser frequency oscillations have no negative effect on the performance of the sensor system. Finally, we show that the frequency of mode-hopping occurrence is related to the length of the feedback loop and reducing the loop length can effectively reduce the frequency of mode-hopping occurrence.

Diabetes, which is mainly characterized by increased apoptosis and dysfunction of beta (β) cells, is a metabolic disease caused by impairment of pancreatic islet function. Previous studies have demonstrated that death-associated protein kinase-related apoptosis-inducing kinase-2 (Drak2) is involved in regulating β cell survival. Since natural products have multiple targets and often are multifunctional, making them promising compounds for the treatment of diabetes, we identified Drak2 inhibitors from a natural product library. Among the identified products, luteolin, a flavonoid, was found to be the most effective compound. In vitro, luteolin effectively alleviated palmitate (PA)-induced apoptosis of β cells and PA-induced impairment of primary islet function. In vivo, luteolin showed a tendency to lower blood glucose levels. It also alleviated STZ-induced apoptosis of β cells and metabolic disruption in mice. This function of luteolin partially relied on Drak2 inhibition. Furthermore, luteolin was also found to effectively relieve oxidative stress and promote autophagy in β cells, possibly improving β cell function and slowing the progression of diabetes. In conclusion, our findings show the promising effect of Drak2 inhibitors in relieving diabetes and offer a potential therapeutic target for the protection of β cells. We also reveal some of the underlying mechanisms of luteolin’s cytoprotective function.

Abstract Artificial intelligence (AI) is altering the world of medicine. Given the rapid advances in technology, computers are now able to learn and improve, imitating humanoid cognitive function. AI applications currently exist in various medical specialties, some of which are already in clinical use. This review presents the potential uses and limitations of AI in arthroplasty to provide a better understanding of the existing technology and future direction of this field. Recent literature demonstrates that the utilization of AI in the field of arthroplasty has the potential to improve patient care through better diagnosis, screening, planning, monitoring, and prediction. The implementation of AI technology will enable arthroplasty surgeons to provide patient-specific management in clinical decision making, preoperative health optimization, resource allocation, decision support, and early intervention. While this technology presents a variety of exciting opportunities, it also has several limitations and challenges that need to be overcome to ensure its safety and effectiveness.

Abstract In the current study, a one‐pot method to prepare small lipid‐indocyanine green (ICG) nanoparticles (L‐ICG NPs) is proposed for the treatment of colorectal cancer. The L‐ICG NPs showed size distribution at around 20 nm with high stability and biocompatibility. In addition, they can respond to light irradiations to exert oxidative/thermal stress damage, which showed effective anticancer benefits in vitro and in vivo using HT‐29 cells as the model.

Purpose We report outcomes and evaluate patient factors and the impact of surgical evolution on outcomes in consecutive ulcerative colitis patients who had restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) at an Australian institution over 26 years. Methods Data including clinical characteristics, preoperative medical therapy, and surgical outcomes were collected. We divided eligible patients into 3 period arms (period 1, 1990 to 1999; period 2, 2000 to 2009; period 3, 2010 to 2016). Outcomes of interest were IPAA leak and pouch failure. Results A total of 212 patients were included. Median follow-up was 50 (interquartile range, 17 to 120) months. Rates of early and late complications were 34.9% and 52.0%, respectively. Early complications included wound infection (9.4%), pelvic sepsis (8.0%), and small bowel obstruction (6.6%) while late complications included small bowel obstruction (18.9%), anal stenosis (16.8%), and pouch fistula (13.3%). Overall, IPAA leak rate was 6.1% and pouch failure rate was 4.8%. Eighty-three patients (42.3%) experienced pouchitis. Over time, we observed an increase in patient exposure to thiopurine (P = 0.0025), cyclosporin (P = 0.0002), and anti-tumor necrosis factor (P < 0.00001) coupled with a shift to laparoscopic technique (P < 0.00001), stapled IPAA (P < 0.00001), J pouch configuration (P < 0.00001), a modified 2-stage procedure (P = 0.00012), and a decline in defunctioning ileostomy rate at time of IPAA (P = 0.00002). Apart from pouchitis, there was no significant difference in surgical and chronic inflammatory pouch outcomes with time. Conclusion Despite greater patient exposure to immunomodulatory and biologic therapy before surgery coupled with a significant change in surgical techniques, surgical and chronic inflammatory pouch outcome rates have remained stable.

For lung adenocarcinoma, arm aneuploidy landscape among primary and metastatic sites, and among different driver and frequently mutated gene groups have not been previously studied. We collected the largest cohort of LUAD patients (n=3533) to date and analyzed the profiles of chromosome arm aneuploidy (CAA), and its association with different metastatic sites and mutated gene groups. Our results showed distant metastasis (bone, brain, liver) were characterized by high CAA burden and biased towards arm losses compared to regional metastasis (pleura, chest) and primary tumors. Moreover, EGFR, MET, PIK3CA, PKHD1 and RB1 mutant groups were found to have high CAA burden, while those with BRAF, ERBB2 and KRAS mutations belonged to the low CAA burden group. Comparing EGFR L858R and EGFR 19del mutants, distinct CAA co-occurrences were observed. Network-based stratification with population based genomic evolution analysis revealed two distinct subtypes of LUAD with different CAA signatures and unique CAA order of acquisition. In summary, our study presented a comprehensive characterization of arm aneuploidy landscape and evolutionary trajectories in lung adenocarcinoma, which could provide basis for both biological and clinical investigations in the future.

Abstract Background Patient satisfaction is a unique and important measure of success after total hip arthroplasty (THA). Our study aimed to evaluate the use of machine learning (ML) algorithms to predict patient satisfaction after THA. Methods Prospectively collected data of 1508 primary THAs performed between 2006 and 2018 were extracted from our joint replacement registry and split into training (80%) and test (20%) sets. Supervised ML algorithms (Random Forest, Extreme Gradient Boosting, Support Vector Machines, Logistic LASSO) were developed with the training set, using patient demographics, comorbidities and preoperative patient reported outcome measures (PROMs) (Short Form-36 [SF-36], physical component summary [PCS] and mental component summary [MCS], Western Ontario and McMaster’s Universities Osteoarthritis Index [WOMAC] and Oxford Hip Score [OHS]) to predict patient satisfaction at 2 years postoperatively. Predictive performance was evaluated using the independent test set. Results Preoperative models demonstrated fair discriminative ability in predicting patient satisfaction, with the LASSO model achieving a maximum AUC of 0.76. Abstract Background The purpose of this study was to determine risk factors of postoperative complications in tuberculosis-destroyed lung (TDL) patients. Methods We retrospectively analyzed the data from all consecutive TDL patients undergoing surgical treatment at the Beijing Chest Hospital from January 2001 to September 2019. Results Of 113 TDL cases experiencing surgery, 33 (29.2%) experienced postoperative complications. The patients with low BMI were more likely to have postoperative complications compared to those with normal BMI, whereas a significant lower rate of postoperative complications was noted in patients with BMI ≥ 25 kg/m2. In addition, significant increased risk was observed in patients with smoking history. We found that the patients with underlying infection, including aspergillus and nontuberculous mycobacteria (NTM), had significantly higher odds of having postoperative complications compared with those without underlying infection. The anaemia was another important independent factor associated with postoperative complication. Patients with blood transfusion above 1000 mL had a strongly increased frequency of postoperative complications than patients with blood transfusion below 1000 mL. Conclusion In conclusion, our data demonstrate that approximate one third of TDL patients experience postoperative complications in our cohort. Patients with low BMI, anaemia, tobacco smoking, and coinfected aspergillus or NTM are at markedly higher risk to experience postoperative complications after pneumonectomy.

Abstract This work investigates the effect of the surface charges of oil droplets and carbonate rocks in brine and in surfactant solutions on oil production. The influences of the cations in brine and the surfactant types on the zeta-potentials of both oil droplets and carbonate rock particles are studied. It is found that the addition of anionic and cationic surfactants in brine result in both negative or positive zeta-potentials of rock particles and oil droplets respectively, while the zwitterionic surfactant induces a positive charge on rock particles and a negative charge on oil droplets. Micromodels with a CaCO3 nanocrystal layer coated on the flow channels were used in the oil displacement tests. The results show that when the oil-water interfacial tension (IFT) was at 10−1 mN/m, the injection of an anionic surfactant (SDS-R1) solution achieved 21.0% incremental oil recovery, higher than the 12.6% increment by the injection of a zwitterionic surfactant (SB-A2) solution. When the IFT was lowered to 10−3 mM/m, the injection of anionic/non-ionic surfactant SMAN-l1 solution with higher absolute zeta potential value (ζoil + ζrock) of 34 mV has achieved higher incremental oil recovery (39.4%) than the application of an anionic/cationic surfactant SMAC-l1 solution with a lower absolute zeta-potential value of 22 mV (30.6%). This indicates that the same charge of rocks and oil droplets improves the transportation of charged oil/water emulsion in the porous media. This work reveals that the surface charge in surfactant flooding plays an important role in addition to the oil/water interfacial tension reduction and the rock wettability alteration.

BackgroundFunctional magnetic resonance imaging (fMRI) studies examining differences in the activity of brain networks between the first depressive episode (FDE) and recurrent depressive episode (RDE) are limited. The current study observed and compared the altered functional connectivity (FC) characteristics in the default mode network (DMN), cognitive control network (CCN), and affective network (AN) between the RDE and FDE. In addition, we further investigated the correlation between abnormal FC and clinical symptoms.MethodsWe recruited 32 patients with the RDE, 31 patients with the FDE, and 30 healthy controls (HCs). All subjects underwent resting-state fMRI. The seed-based FC method was used to analyze the abnormal brain networks in the DMN, CCN, and AN among the three groups and further explore the correlation between abnormal FC and clinical symptoms.ResultsOne-way analysis of variance showed significant differences the FC in the DMN, CCN, and AN among the three groups in the frontal, parietal, temporal, and precuneus lobes and cerebellum. Compared with the RDE group, the FDE group generally showed reduced FC in the DMN, CCN, and AN. Compared with the HC group, the FDE group showed reduced FC in the DMN, CCN, and AN, while the RDE group showed reduced FC only in the DMN and AN. Moreover, the FC in the left posterior cingulate cortices and the right inferior temporal gyrus in the RDE group were positively correlated with the 17-item Hamilton Rating Scale for Depression (HAMD-17), and the FC in the left dorsolateral prefrontal cortices and the right precuneus in the FDE group were negatively correlated with the HAMD-17.ConclusionsThe RDE and FDE groups showed multiple abnormal brain networks. However, the alterations of abnormal FC were more extensive and intensive in the FDE group.

To study the design method and pressure relief effect of the mitigation structure of a shell under the action of thermal stimulation, a systematic research method of theoretical calculation-simulation-experimental verification of the mitigation structure was established. Taking the shelled PBX charge as the test material, the pressure relief area that can effectively reduce the reaction intensity of the charge is obtained by theoretical calculation. The influence of the pressure relief hole area, distribution mode, and other factors on the pressure relief effect is calculated by simulation. The pressure relief effect of the mitigation structure was verified by the low-melting alloy plug with refined crystal structure for sealing the pressure relief hole and the cook-off test. The research results show that the critical pressure relief area is when the ratio of the area of the pressure relief hole to the surface area of the charge is AV/SB = 0.0189. When the number of openings increases to 6, the required pressure relief coefficient decreases to AV/SB = 0.0110; When the length/diameter ratio is greater than 5, the opening at one end cannot satisfy the reliable pressure relief of the shell. The designed low-melting-point alloy mitigation structure can form an effective pressure relief channel. With the increase in AV/SB from 0.0045 to 0.0180, the reaction intensity of the cook-off bomb is significantly reduced in both fast and slow cook-off, which improves the safety of the charge when subjected to unexpected thermal stimulation.

To examine the interactions between two binder systems—hydroxyl-terminated polybutadiene (HTPB) and hydroxyl-terminated block copolyether prepolymer (HTPE)—as well as between these binders and ammonium perchlorate (AP) at various temperatures for their susceptibility to varying degrees of thermal damage treatment, the thermal characteristics and combustion interactions of the HTPB and HTPE binder systems, HTPB/AP and HTPE/AP mixtures, and HTPB/AP/Al and HTPE/AP/Al propellants were studied. The results showed that the first and second weight loss decomposition peak temperatures of the HTPB binder were, respectively, 85.34 and 55.74 °C higher than the HTPE binder. The HTPE binder decomposed more easily than the HTPB binder. The microstructure showed that the HTPB binder became brittle and cracked when heated, while the HTPE binder liquefied when heated. The combustion characteristic index, S, and the difference between calculated and experimental mass damage, ΔW, indicated that the components interacted. The original S index of the HTPB/AP mixture was 3.34 × 10−8; S first decreased and then increased to 4.24 × 10−8 with the sampling temperature. Its combustion was initially mild, then intensified. The original S index of the HTPE/AP mixture was 3.78 × 10−8; S increased and then decreased to 2.78 × 10−8 with the increasing sampling temperature. Its combustion was initially rapid, then slowed. Under high-temperature conditions, the HTPB/AP/Al propellants combusted more intensely than the HTPE/AP/Al propellants, and its components interacted more strongly. A heated HTPE/AP mixture acted as a barrier, reducing the responsiveness of solid propellants.

Change detection (CD) is a technique widely used in remote sensing for identifying the differences between data acquired at different times. Most existing 3D CD approaches voxelize point clouds into 3D grids, project them into 2D images, or rasterize them into digital surface models due to the irregular format of point clouds and the variety of changes in three-dimensional (3D) objects. However, the details of the geometric structure and spatiotemporal sequence information may not be fully utilized. In this article, we propose PBFormer, a transformer network with Siamese architecture, for directly inferring pointwise changes in bi-temporal 3D point clouds. First, we extract point sequences from irregular 3D point clouds using the k-nearest neighbor method. Second, we uniquely use a point transformer network as an encoder to extract point feature information from bitemporal 3D point clouds. Then, we design a module for fusing the spatiotemporal features of bi-temporal point clouds to effectively detect change features. Finally, multilayer perceptrons are used to obtain the CD results. Extensive experiments conducted on the Urb3DCD benchmark show that PBFormer outperforms other excellent approaches for 3D point cloud CD tasks.

Abstract Gastric cancer is one of the most common and deadly cancer types. Currently, four subtypes have been identified with unique molecular alterations: Epstein–Barr virus (EBV)‐positive, microsatellite instability (MSI), chromosomal instability (CIN), and genomic stable (GS) tumors. Notably, many gastric tumors are associated with the bacterium Helicobacter pylori but the genomic landscape of this subgroup of tumors remains largely unknown. Targeted sequencing covering 425 genes was performed retrospectively on 1703 gastric tumor tissues and matched normal blood samples. Nonsynonymous mutations, copy‐number variation (CNV), and MSI status were called from human DNA reads; nonhuman DNA reads were mapped to NCBI microbial reference genome using Kraken and significant species were identified. Overall, 37 (2.76%) from a total of 1703 samples were EBV‐positive, 200 (11.74%) samples were H. pylori‐positive, and 10 samples were positive for both. Among the rest, 59 (3.46%) samples were MSI, 380 (22.31%) were CIN, and 1017 (59.72%) were GS. Most of the 200 H. pylori‐positive samples tend to be genome stable (85.5%, p

A majority of carbonate reservoirs are preferably oil-wet or intermediate wet. Spontaneous imbibition represents an important mechanism to enhance oil production. Targeting an oil-wet carbonate reservoir with high temperature and high salinity, several surfactant formulations were investigated to improve oil production by imbibition, focusing on the capacity in interfacial tension (IFT) reduction and surfactant partitioning in water and oil phases in addition to dominant wettability alteration effect. Oil production potentials using different surfactants were evaluated by spontaneous imbibition in Amott cells at 95 ​°C. A base case imbibition test was conducted using a high salinity water with IFT value of 15.2 ​mN/m and an oil-wet carbonate core, and the oil production was 6.4% of oil originally saturated in core. Reducing the oil-water IFT to 10° ​mN/m magnitude achieved 4% incremental oil production beyond base case. When the IFT was further reduced to 10−1 ​mN/m magnitude, the incremental oil production reached 10%. For the case of water-wet core and at the same IFT range, the incremental oil production was 13%. When the IFT was reduced to 10−2 ​mN/m, the incremental oil production was up to 17% for the tests using oil-wet cores, and up to 23% for the water-wet counterpart. When further reducing the IFT to ultra-low magnitude of 10−3 ​mN/m, the oil production was close to that using the imbibition agents with IFT of 10−2 ​mN/m magnitude. For surfactants with no obvious wettability alteration capacity, those surfactants with higher molecular fraction in oil phase and higher adsorption on rock surfaces could remove more oil from the pore wall, and yielded higher imbibition efficiency. This work systematically investigated the major effects of surfactant on oil mobilization during imbibition process, which can help for better understanding of chemical imbibition in carbonates. The findings will shed light on the selection of high performance imbibition agents for carbonate reservoirs.

Enzalutamide, an approved drug for prostate cancer, acts on TMPRSS2 expression, a key mediator for SARS-CoV-2 infection. Here, the authors characterize the anti-SARS-CoV-2 effects of Enzalutamide in prostate cancer cells, lung cancer cells, human lung organoids and in hACE2-transduced Tmprss2 knockout mice and show lack antiviral action in human lung cells and human lung organoids, likely due to the AR-independent TMPRSS2 expression in mouse and human lung epithelial cells.

We report a high-resolution fiber optic temperature sensor system based on an air-filled Fabry–Pérot (FP) cavity, whose spectral fringes shift due to a precise pressure variation in the cavity. The absolute temperature can be deduced from the spectral shift and the pressure variation. For fabrication, a fused-silica tube is spliced with a single-mode fiber at one end and a side-hole fiber at the other to form the FP cavity. The pressure in the cavity can be changed by passing air through the side-hole fiber, causing the spectral shift. We analyzed the effect of sensor wavelength resolution and pressure fluctuation on the temperature measurement resolution. A computer-controlled pressure system and sensor interrogation system were developed with miniaturized instruments for the system operation. Experimental results show that the sensor had a high wavelength resolution (0.015 kPa), resulting in high-resolution (±0.32 ℃) temperature measurement. It shows good stability from the thermal cycle testing with the maximum testing temperature reaching 800 ℃.

Aspartate (Asp) can act on liver Kupffer cells, inhibit NOD-like receptor-P 3 (NLRP3) inflammatory bodies, and improve liver inflammation in acute hepatitis. However, the effect of Asp on the role of hepatic stellate cells (HSCs) in the pathogenesis of liver fibrosis in chronic liver injury remains unexplored. This study aimed to investigate the effects of Asp on CCl4-induced liver fibrosis in mice and HSCs via the NF-κB/NLRP3 signaling pathway. Liver fibrosis was induced in C57BL/6J mice by intraperitoneally (IP) injecting 0.5 mL/kg 2% CCl4 three times weekly for 8 weeks. Asp was administered to mice by gavage once every morning for 4 weeks. Masson’s trichrome staining, Sirius red staining and hematoxylin and eosin staining were used to detect and analyze the pathological changes in liver tissues. Western blot analysis and immunohistochemistry were applied to determine the protein expression levels of α-smooth muscle actin (α-SMA), collagen Ⅲ (COL III), NLRP3, and IL-1β. In addition, reverse transcription-quantitative PCR was performed to detect the mRNA expression levels. In the liver fibrosis model, the pathological changes in liver tissues improved following treatment with Asp. A marked decrease was observed in protein and mRNA expression levels of α-SMA, COL III, NLRP3, and IL-1β. In addition, HSCs were treated with Asp. The expression levels of α-SMA, COL III, NLRP3, and IL-1β reduced in dose- and time-dependent manners. Furthermore, Asp upregulated the expression of NS3TP1 in vivo and in vitro, and NS3TP1 had a significant inhibitory effect on liver fibrosis. Asp attenuated liver fibrosis and reduced collagen production by suppressing the NF-κB/NLRP3 signaling pathway via upregulating the expression of NS3TP1.

This study aimed to compare the in-vivo kinematics and articular contact status between medial-pivot total knee arthroplasty (MP-TKA) and posterior stabilized (PS) TKA during weight-bearing single-leg lunge. 16 MP-TKA and 12 PS-TKA patients performed bilateral single-leg lunges under dual fluoroscopy surveillance to determine the in-vivo six degrees-of-freedom knee kinematics. The closest point between the surface models of the femoral condyle and the polyethylene insert was used to determine the contact position and area. The nonparametric statistics analysis was performed to test the symmetry of the kinematics between MP-TKA and PS-TKA. PS-TKA demonstrated a significantly greater range of AP translation than MP-TKA during high flexion (p = 0.0002). Both groups showed a significantly greater range of lateral compartment posterior translation with medial pivot rotation. The contact points of PS-TKA were located significantly more posterior than MP-TKA in both medial (10°–100°) and lateral (5°–40°, 55°–100°) compartments (p < 0.0500). MP-TKA had a significantly larger contact area in the medial compartment than in the lateral compartment. In contrast, no significant differences were observed in PS-TKA. The present study revealed no significant differences in clinical outcomes between the MP and PS groups. The PS-TKA demonstrated significantly more posterior translations than MP-TKA at high flexion. The contact points are located more posteriorly in PS-TKA compared with MP-TKA. A larger contact area and medial pivot pattern during high flexion in MP-TKA indicated that MP-TKA provides enhanced medial pivot rotation.

In order to find the “sweet spots” of unconventional oil and gas from the Pinghu Formation in the Pinghu Tectonic Belt, we are committed to clarifying the development and distribution rules of coal-measure source rocks and tight reservoirs as well as the controlling factors. Using 3D seismic and logging data, combined with logging constraints of target lithology and pyrolysis experiments of source rock, "source and reservoir" research of uncoventional oil and gas was carried out in the Pinghu Tectonic Belt. The results show that two stages of regression and one stage of transgression occurred in the Pinghu Formation, resulting in river-controlled and wave-controlled delta-neritic facies dominated by sedimentary facies, source rocks developed in interdistributary bay and swamp microfacies, and tight sandstones in point bar and distributary channel microfacies are developed. The accumulation of coalbed methane and shale gas is controlled by the sedimentary facies and the degree of thermal evolution under the factors of burial depth. The accumulation of tight sandstone is closely related to the dominantly sedimentary facies and diagenetic modification of physical properties and faults. It is concluded that the structural type, type and distribution of faults, and depositional environment of the Pinghu Tectonic Belt in the Xihu Sag are the key factors controlling the development and accumulation of coal-measure source rocks and tight sandstone reservoirs. This understanding provides a clear direction for the deep exploration of unconventional oil and gas in the sag and provides a reference for finding reservoir “sweet spots.”

Due to the worldwide scarcity of fresh water, seawater becomes an alternative base fluid in hydraulic fracturing for oil and gas production. However, the injection of seawater that contains high concentration of sulfate will induce the scale formation and thus reduce hydrocarbon production. One of the most effective ways to solve this problem is to remove sulfate ions from seawater before fracturing application. The objective of this study is to develop an effective and environment-friendly approach to remove sulfate ions from seawater based on coprecipitation of SO42− with NaAlO2 and CaO as ettringite (Ca6Al2(SO4)3(OH)12·26H2O). Residual sulfate concentration in treated seawater was determined when NaAlO2 and CaO dosed at different molar ratios to sulfate. Results showed the efficiency of sulfate removal was more than 90% (4290 ppm to ∼400 ppm) when Al : Ca : S = 2 : 6 : 1. It was found the sulfate precipitation completed in 15 mins with stirring under an alkaline condition (pH ≈ 12) and was not affected by temperature (15°C to 45°C). Increasing the Na+ concentration from 0 to 25,000 ppm in waters resulted in the increment of residual sulfate concentration from 250 to ∼600 ppm, decreasing the removal efficiency. Besides, the analysis of Ca2+ and Mg2+ in treated seawater showed the Ca2+ concentrations were on the similar level as that before the treatment and Mg2+ was removed in the precipitation process, which is beneficial to the application of the treated seawater. The morphology and element analysis of the collected precipitates showed that the ettringites were in a layered shape with composition between Ca6Al2(SO4)3(OH)12 and Ca4Al2(SO4)(OH)12 at the optimized chemical dosage; therefore, the developed ettringite precipitation method could effectively remove sulfate from seawater without toxic chemicals involved, which benefits seawater hydraulic fracturing in an economic way, and this contributes to water sustainability.

High water-cut has become a worldwide challenge for oil production. It requires extensive efforts to process and dispose. This entails expanding water handling facilities and incurring high power consumption costs. Polymeric microsphere injection is a cost-effective way to deal with excessive water production from subterranean formations. This study reports a laboratory investigation on polymeric microsphere injection in a large volume to identify its in-depth fluid diversion capacity in a porous media with large pore/particle size ratio. The performance of polymeric microsphere injection was evaluated using etched glass micromodels based on the pore network of a natural carbonate rock, which were treated as water-wet or oil-wet micromodels. Waterflooding was conducted to displace oil at reservoir temperature of 95 °C, followed by one pore volume of polymeric microsphere injection. Three polymeric microsphere samples with median particle size of 0.05, 0.3, and 20 μm were used to investigate the impact of particle size of the polymeric microspheres on incremental oil production capacity. Although the polymeric microspheres were much smaller than the pores, additional oil production was observed. The incremental oil production increased with increasing polymeric microsphere concentration and particle size. As a comparison, polymeric microsphere solutions were injected into oil-wet and water-wet micromodels after waterflooding. It was observed that the oil production in oil-wet micromodel was much higher than that in water-wet micromodel. The wettability of micromodels affected the distribution patterns of the remaining oil after waterflooding and further dominated the performance of the microsphere injection. The study supports the applicability of microsphere injection in oil-wet heterogeneous carbonates.

Purpose: To investigate the protective effects of dexmedetomidine (Dex) on post cardiopulmonary bypass (CPB) lung injury in rats and to explore the possibility of underlying mechanisms involving phosphatidylinositol 3-kinase (PI3K)/Akt. Materials and Methods: Forty healthy male Sprague–Dawley rats were randomly divided into five groups (n = 8 for each). A left lung ischemia-reperfusion injury model of CPB was established in all five groups. Rats were given saline, dexmedetomidine (Dex), dimethyl sulfoxide (DMSO), wortmannin (Wtm), and Dex plus Wtm during the CPB process, in Group Saline, Dex, DMSO, Wtm, and Dex + Wtm, respectively. Mean arterial pressure, oxygenation index (OI), and respiratory index (RI) were measured at the following three timepoints: before CPB (T1), at the onset of opening of the left hilus pulmonis (T2), and at the end of the CPB process (T3). At T3, hematoxylin and eosin (H&E) staining was conducted to evaluate pathology of lung injury. The rate of lung tissue apoptosis was determined by flow-cytometry. The expression of Akt, p-Akt, caspase-3, and caspase-9 was assessed by Western blot. Results: Dex treatment during CPB protected rat lungs from post-CPB lung injury, manifested by improved lung function, mitigated pathological damage, and reduced lung tissue apoptosis. The expression and phosphorylation of Akt was significantly enhanced by Dex treatment compared to the saline/DMSO-treated group. Wtm, a recognized PI3K inhibitor, abolished the protective effect of Dex. The levels of caspase-3 and caspase-9 were also significantly elevated in the Wtm-treated group. Conclusions: Dex reduces post-CPB lung injury in rats, at least partially, by activating the PI3K/Akt pathway and inhibiting lung tissue apoptosis.

Abstract Background Stress response always occurs in cardiac valve replacement patients undergoing cardiopulmonary bypass (CPB). Methods 60 patients undergoing cardiac valve replacement were recruited and randomized into control and Dex groups. Dex group received 1.0 μg·kg-1 of Dex for 10 min intravenously before anesthesia, followed by 0.5 μg·kg-1·h-1 of Dex, steadily administered throughout the procedure. And controlled group received the identical velocity of saline as Dex group. Plasma level of cortisol (Cor), epinephrine (E), norepinephrine (NE), and serotonin (5-HT) were evaluated at four timepoints: Before administration (T0), sawn sternum (T1), end of extracorporeal circulation (T2), and 24 h post operation (T3). General data of operation and recovery such as heart rate (HR), mean arterial pressure (MAP), intraoperative bispectral index (BIS), and hospitalization time in the intensive care unit (ICU) were also compared. High-finesse fiber Bragg gratings (FBGs) Fabry-Perot interferometers (FPIs) have shown great promise as sensitive ultrasonic sensors. However, the fabrication process of the sensors usually introduces birefringence to the fiber, which makes the sensor operation sensitive to the polarization of the probe laser. Here, we theoretically study the effect of laser polarization on the sensitivity of the sensor with birefringence. We show that, to maintain the sensitivity above half of its maximum of a high-finesse FBG-FPI sensor, the spectral notch separation caused by sensor birefringence should be less than ~61% of the spectral notch width of the sensor. We fabricate an FBG-FPI sensor with negligible overall birefringence by rotating the fiber by 90° between the fabrication of the two FBGs. As a result, the birefringence introduced during the fabrication of the two FBGs cancels out each other. For comparison, a regular FBG-FPI fabricated without fiber rotation was also tested. While the regular FBG-FPI exhibited large variations in detection sensitivity with laser polarization, the polarization-insensitive showed little changes. As a result, no control on the laser polarization is needed during the operation of the polarization-insensitive FBG-FPI for ultrasonic detection, an important attribute required in many practical applications of the sensor.
