Abstract

Purpose: This cross-sectional study aimed at comparing the quality of life (Qol), the prevalence of psychiatric diagnosis and pharmacological treatment in 104 candidates to bariatric surgery according to the degree of obesity (class 2 vs. class ≥ 3 obesity). Methods: All surgical candidates underwent a detailed psychiatric interview based on DSM-5 criteria, including sociodemographic, clinical, psychological and psychiatric data. Participants completed the Binge Eating Scale (BES) and the 12-Item Short Form Health Survey (SF-12). Results: Overall, bariatric candidates reported a significant impairment in the physical (PCS 38.8 [95% CI 36.2–41.5]) and mental (MCS 42.2 [95% CI 40.4–43.9]) components of Qol compared to population norms (p < 0.001 for both). Subjects with class 2 obesity scored significantly lower in the MCS compared to those with class 3 (38.7 (8.1) vs. 43.6 (8.4), p = 0.008). No other statistically significant differences were found between the two groups in terms of sociodemographic and clinical variables. Conclusion: These data support the usefulness of Qol assessment in bariatric candidates as a sensible screening parameter, especially in patients with lower BMI, in whom MCS could identify the need for early psychosocial intervention. Abstract

La critica e l’opinione pubblica hanno spesso definito Cesare Pavese come pioniere in Italia dell’americanismo e come convinto antifascista. Queste due etichette hanno visto Pavese conteso tra rivendicazioni orgogliose di appartenenza alla Sinistra e violente accuse di fiancheggiatore del regime. Il difficile rapporto che Pavese ha vissuto nei confronti della parentesi storica che ha attraversato – il regime, la guerra, la Resistenza – ha innescato nella sua fragile personalità paure, dubbi e incertezze che esigevano una scelta di campo difficile da assumere per chiunque si sia trovato a vivere in quegli anni. Pavese non perseguiva idee politiche ma più semplicemente, ma non semplicisticamente, la necessità di ribaltare e trasformare profondamente la società italiana. Cercando l’uomo, e soprattutto ‘se stesso’, e in perenne conflitto interiore e sociale, Pavese ha coltivato sogni, immagini e miti, militando senza pregiudizi per un mondo migliore, per come ha saputo farlo. Critics and public opinion have often defined Cesare Pavese as a pioneer of Americanism in Italy and as a fervent anti-fascist. Pavese's difficult relationship with the historical period he has gone through - the regime, the war, the Resistance - has crept doubts and uncertainties into his fragile personality. Pavese, probably, did not pursue political ideas but more simply, but not simplistically, he wanted to overturn and profoundly transform Italian society. Looking for 'himself', and in constant internal and social conflict, he cultivated dreams, images and myths, militating without prejudice for a better world, as he could do it.

Abstract

Purpose: The study was designed to assess the utility and controversies surrounding the usage of 5-mm instruments in paediatric robotic surgery. Adequate, delicate instruments for surgery in very narrow spaces are still lacking., Material and Methods: Thirty children underwent elective abdominal robotic surgery. Working sites, assembly and operative time, hospital stay, advantages, complications and shortcomings are reported., Results: Interventions were performed in the following anatomical sites: 11 upper abdominal, nine pelvic, ten renal procedures. The majority of procedures required two operative trocars. A 2-3 mm accessory port was necessary for operations in the renal area and upper abdomen. The ports had to be placed at least 3 cm from the costal margins and superior iliac spines and at an angle of at least 130° with respect to the camera trocar. This configuration allowed intra-corporal knotting, vessel ligation and dissection with instruments in the inverted position. Operative times and hospital stays were similar to those reported for 8 mm-instruments., Conclusion: The use of 5-mm instruments was advantageous in renal and pelvic sites. The benefits in upper abdominal surgery need further evaluation, particularly in patients weighing <10 kg. Smaller sized instruments with high endowrist dexterity would resolve the problems encountered in paediatric robotic-assisted surgery using 5-mm instruments.
