9. Prediction of a Cephalometric Parameter and Skeletal Patterns from Lateral Profile Photographs: A Retrospective Comparative Analysis of Regression Convolutional Neural Networks.

Abstract

Background/Objectives: Cephalometric analysis has a pivotal role in the quantification of the craniofacial skeletal complex, facilitating the diagnosis and management of dental malocclusions and underlying skeletal discrepancies. This study aimed to develop a deep learning system that predicts a cephalometric skeletal parameter directly from lateral profile photographs, potentially serving as a preliminary resource to motivate patients towards orthodontic treatment. Methods: ANB angle values and corresponding lateral profile photographs were obtained from the medical records of 1600 subjects (1039 female and 561 male, age range 3 years 8 months to 69 years 1 month). The lateral profile photographs were randomly divided into a training dataset (1250 images) and a test dataset (350 images). Seven regression convolutional neural network (CNN) models were trained on the lateral profile photographs and measured ANB angles. The performance of the models was assessed using the coefficient of determination (R2) and mean absolute error (MAE). Results: The R2 values of the seven CNN models ranged from 0.69 to 0.73, and the MAE values ranged from 1.46 to 1.53. Among the seven models, InceptionResNetV2 showed the highest success rate for predictions of ANB angle within 1° of range and the highest performance in skeletal class prediction, with macro-averaged accuracy, precision, recall, and F1 scores of 73.1%, 78.5%, 71.1%, and 73.0%, respectively. Conclusions: The proposed deep CNN models demonstrated the ability to predict a cephalometric skeletal parameter directly from lateral profile photographs, with 71% of predictions being within 2° of accuracy. This level of accuracy suggests potential clinical utility, particularly as a non-invasive preliminary screening tool. The system's ability to provide reasonably accurate predictions without radiation exposure could be especially beneficial for initial patient assessments and may enhance efficiency in orthodontic workflows. Abstract

The handheld nonthermal plasma (HNP) treatment may alter the surface properties, bone metabolism, and inflammatory reactions of polyaryletherketone (PAEK) dental implant materials. This study tested whether the HNP treatment might increase the biocompatibility, surface hydrophilicity, surface free energies (SFEs), and the cell adhesion and mineralization capability of PAEK materials. Disk-shaped samples of titanium (Ti), zirconia (Zr), polyetheretherketone (PEEK [PE]), and polyetherketoneketone (PEKK [PK]) were subjected to HNP treatment and termed as TiPL, ZrPL, PEPL, and PKPL, respectively. Water-surface reactions were examined using a goniometer. MG-63 cells were cultured on all samples to assess the cell viability, cytotoxicity, cell attachment, and mineralization characteristics. The expression of pro-inflammatory cytokines (tumor necrosis factor-alpha and interleukin-6) and key mineralization markers (alkaline phosphatase [ALKP], osteopontin [OPN], and dentin matrix protein 1 [DMP1]) was measured using enzyme-linked immunosorbent assay kits. The HNP-treated samples exhibited significantly enhanced surface hydrophilicities and SFEs compared to the untreated samples. The cell viability remained high across all samples, indicating no cytotoxic effects. The HNP treatment significantly enhanced MG-63 cell adherence and proliferation. Elevated levels of ALKP and OPN were observed for the plasma-treated PEPL and PKPL specimens, while DMP1 levels increased significantly only in the PKPL specimen. Pro-inflammatory cytokine levels were low across all samples, suggesting no inflammatory response. The HNP-treated PAEKs have enhanced the surface hydrophilicity and SFEs as well as superior cell adhesion and mineralization capability, and thus may be good clinical dental implant materials. Abstract

Slightly acidic electrolyzed water (SAEW) is produced by electrolyzing 2–6% diluted hydrochloric acid in a membrane-less chamber, resulting in 5.0–6.5 pH, and can be applied to various foods as a disinfectant. Although SAEW has shown to have bactericidal activity, the details of its anti-fungal effects towards Candida speciesremain unknown. Therefore, we examined the fungicidal effects of SAEW on Candida spp. and biofilms on acrylic resins.

Abstract

Mechanical unloading conditions result in decreases in bone mineral density and quantity, which may be partly attributed to an imbalance in bone formation and resorption. To investigate the effect of mechanical unloading on osteoblast and osteoclast differentiation, and the expression of RANKL and OPG genes in osteoblasts, we used a three-dimensional (3D) clinostat system simulating microgravity to culture MC3T3-E1 and RAW264.7 cells. Long-term exposure (7 days) of MC3T3-E1 cells to microgravity in the 3D clinostat inhibited the expression of Runx2, Osterix, type I collagen αI chain, RANKL and OPG genes. Similarly, 3D clinostat exposure inhibited the enhancement of β3-integrin gene expression, which normally induced by sRANKL stimulation in RAW264.7 cells. These results, taken together, demonstrate that long-term 3D clinostat exposure inhibits the differentiation of MC3T3-E1 cells together with suppression of RANKL and OPG gene expression, as well as the RANKL-dependent cellular fusion of RAW264.7 cells, suggesting that long-term mechanical unloading suppresses bone formation and resorption.
