Abstract Aims Readmission and mortality are the most common and often combined endpoints in acute heart failure (AHF) trials, but an association between these two outcomes is poorly investigated. The aim of this study was to determine whether unplanned readmission is associated with a greater subsequent risk of death in patients with acute dyspnoea due to cardiac and non‐cardiac causes. Methods and results Derivation cohort (1371 patients from the LEDA study) and validation cohort (1986 patients from the BASEL V study) included acute dyspnoea patients admitted to the emergency department. Cox regression analysis was used to determine the association of 6 month readmission and the risk of 1 year all‐cause mortality in AHF and non‐AHF patients and those readmitted due to cardiovascular and non‐cardiovascular causes. In the derivation cohort, 666 (49%) of patients were readmitted at 6 months and 282 (21%) died within 1 year. Objectives To evaluate the potential of soluble cluster of differentiation 146 (sCD146) in the detection and grading of congestion in patients with acute dyspnoea.Design Subanalysis of the prospective observational Lithuanian Echocardiography Study of Dyspnoea in Acute Settings (LEDA) cohort.Setting Two Lithuanian university centres.Participants Adult patients with acute dyspnoea admitted to the emergency department.Methods Congestion was assessed using clinical and sonographic parameters. All patients underwent sCD146 and N-terminal pro-B-type natriuretic peptide (NT-proBNP) testing.Results The median value of sCD146 concentration in the study cohort (n=437) was 405 (IQR 315–509) ng/mL. sCD146 was higher in patients with peripheral oedema than in those without (median (IQR) 472 (373–535) vs 400 (304–501) ng/mL, p=0.009) and with pulmonary rales than in those without (439 (335–528) vs 394 (296–484) ng/mL, p=0.001). We found a parallel increase of estimated right atrial pressure (eRAP) and sCD146 concentration: sCD146 was 337 (300–425), 404 (290–489) and 477 (363–572) ng/mL in patients with normal, moderately elevated and high eRAP, respectively (p=0.001). In patients with low NT-proBNP, high sCD146 distinguished a subgroup with a higher prevalence of oedema as compared with patients with low levels of both biomarkers (76.0% vs 41.0%, p=0.010). Metimų į krepšį technika ištirta pakankamai (Hayes, 1987; Hay, 1994 ir kt.), o rungtynių rezultato priklausomybė nuo metimų į krepšį vietos ir puolimo struktūros reikalauja išsamesnių tyrimų. Iškyla mokslinė problema: kokios metimų į krepšį vietos (krepšio ir nuotolio atžvilgiu) ir atakų trukmės bei pobūdžio sąsajos su rungtynių rezultatais. Treneriai per treniruotes ir rungtynes dažnai vadovaujasi intuicija, o ne objekty- viais duomenimis, todėl tiriamoji problema yra aktuali. Svarbu nustatyti metimų į krepšį kiekybinius ir kokybinius rodiklius greitojo bei pozicinio puolimo metu; ištirti, kaip kiekybiniai ir kokybiniai metimų į krepšį rodikliai lemia rungtynių rezultatus. Per Eurolygos 2004 / 2005 metų sezono rungtynes buvo tiriama didelio meistriškumo krepšinio komandų (n = 20) žaidimo veikla registruojant rodiklius kompiuterine programa „Kregis“. Registruota: tikslių metimų nuotolis, vieta; atakų trukmė ir veiksmingumas. Tyrimo tikslas — nustatyti ir įvertinti didelio meistriškumo krepšinio komandų kiekybinius ir kokybinius metimų į krepšį rodiklius, nuotolio, aikštės vietos ir puolimo struktūros požiūriu bei jų sąsajas su rungtynių rezultatais. Tyrimo objektas — didelio meistriškumo krepšinio komandų metimų į krepšį veiksmingumas esant įvairiai puolimo struktūrai. Rungtynes laimėjusios komandos vidutiniškai pelnė po 86,5 taško: 57% metant iš artimo ir vidutinio, 24,2% iš tolimo nuotolio ir 18,8% — baudos metimais. Rungtynes pralaimėjusios komandos — atitinkamai 80,7 taško ir 52,5, 30,7 ir 16,8%. Rungtynes laimėjusios komandos, mesdamos iš kairės aikštės pusės, surinko 36,3, iš dešinės — 31,9 ir iš aikštės vidurio 31,7% visų taškų. Tirtos komandos per rungtynes surengė vidutiniškai po 79,4 atakas, kurių efekty- vumas 49,5%. Veiksmingiausios atakos — trukusios 2—5 s — 70%. Atliktas tyrimas rodo, kad pozicinis puolimas greitėja (10—15 s trukmės atakos) dėl gero iš anksto išmoktų derinių taikymo arba individualių puolančios komandos žaidėjų savybių, gebėjimų, pranašumų išnaudojimo. Tyrimu siekta nustatyti, įvertinti ir sudaryti 10—17 m. jaunųjų krepšininkų atletinio bei techninio parengtumo ranginę skalę. Tiriamąją imtį sudarė 673 jaunieji 10—17 m. Arvydo Sabonio krepšinio mokyklos krepšininkai iš 8 skirtingo amžiaus grupių: 10 m. (n = 58), 11 m. (n = 116), 12 m. (n = 89), 13 m. (n = 124), 14 m. (n = 117), 15 m. (n = 88), 16 m. (n = 44), 17 m. (n = 37). Tyrimas buvo atliekamas 2004—2006 m. povaržybiniu laikotarpiu. Taikytas testavimo metodas, kuriuo buvo vertinamas 10—17 m. jaunųjų krepšininkų atletinis (lokomocinis greitumas, greitumo jėga, koordinaciniai gebėjimai) ir techninis (kamuolio varymo ir judėjimo ginantis įgūdžių kokybė, metimų į krepšį iš skirtingų aikštės vietų tikslumas esant intensyviam fi ziniam krūviui ir 30 baudos metimų tikslumas nesant intensyviam fi ziniam krūviui) parengtumas. Skaičiuotas aritmetinis rezultatų vidurkis ( x ), vidutinis standartinis nuokrypis (SD), vidurkių skirtumo reikšmingumas pagal Stjudento t (Student t) nepriklausomų imčių kriterijų. Skirtumas su galima mažesne nei 0,05 paklaida buvo vertinamas kaip statistiškai patikimas (p < 0,05). Taip pat buvo naudojama „SPSS 12.0 for Windows“ programa, kuria sudarėme atletinio ir techninio parengtumo rangines skales (puikiai 80—100%; labai gerai 60—79%; gerai 40—59%; vidutiniškai 20—39%; nepakankamai 0—19%). Išskirtini mūsų tirtų skirtingų amžiaus grupių jaunųjų krepšininkų ūgio rodiklių skirtumai: 13-ais (5,1%; p < 0,05, lyginant su dvylikmečiais) ir 15-ais metais (5,2%; p < 0,05, lyginant su keturiolikmečiais). Kūno masės rodiklių skirtumai nustatyti lyginant vienuolikmečius su dešimtmečiais (p < 0,05; 13,8%) ir penkiolikmečius su keturiolik- mečiais (p < 0,05; 13,2%). Kūno masė kiekvienais metais kito statistiškai reikšmingai (p < 0,05): labiausiai kito vienuolikmečių ir penkiolikmečių — atitinkamai padidėjo 13,8 ir 13,2%, lyginant su ankstesniais metais. Nevieno- dus skirtingų tiriamųjų ūgio ir kūno masės skirtumus tarp 11 ir 12 metų krepšininkų galėjo lemti prepubertetinio ir pubertetinio laikotarpio sandūra. Kojų raumenų galingumas — skirtingai nei ūgis ir kūno masė — labiausiai padidėjo dvylikmečių (pubertetinio laikotarpio pradžioje: šuolio į aukštį iš vietos be rankų mosto padidėjo 12% — p < 0,05, lyginant su vienuolikme- čiais). Lokomocinio greitumo rodikliai (20 m bėgimas iš aukšto starto) kaip ir kojų raumenų galingumas, atliekant specifi nio pobūdžio veiksmus (šuolį aukštį su rankų mostu) labiausiai pakito penkiolikmečių — atitinkamai 6,1% ir 11,1% (statistiškai reikšmingai, lyginant su keturiolikmečiais). Metais vėliau — šešiolikmečių smarkiai (p < 0,05) pagerėjo koordinaciniai gebėjimai — 22,7% (lyginant su penkiolikmečiais). Techninį parengtumą apibūdinantys skirtingų testų rodikliai kito nevienodai: statistiškai reikšmingai (lyginant su ankstesniais amžiaus metais, p < 0,05) kamuolį greičiau varė trylikmečiai (7,2%), penkiolikmečiai geriau judėjo gindamiesi (13,5%) ir mesdami baudos metimus (19,7%), tačiau taikliausiai per 1 minutę kamuolį į krepšį metė vienuolikmečiai (23,2%). Tokius kitimus galėjo lemti ir genotipiniai, ir fenotipiniai veiksniai. Sudaryta jaunųjų krepšininkų (10—17 metų) atletinio ir techninio parengtumo ranginė skalė leis Lietuvos krepšinio treneriams įvertinti treniruojamų krepšininkų parengtumo lygį. Research background and hypothesis. Indicators of physical fitness and physical development influence individual performance results. Research aim was to identify correlation between basketball performance indicators and physical development and fitness. Research methods. The basketball players (n = 12) of the Lithuanian National Women’s Basketball Team were investigated. The performance indicators of this team were analyzed on the basis of data collected during the European Basketball Championship. During competition period the indicators of physical development and physical fitness of female basketball players were measured. Mathematical statistics methods were applied for the analysis of the research data. Pearson’s correlation coefficients were applied to identify correlations among the indicators. Research results. The correlations of the number of blocks with height, body mass, standing reach and psychomotor response were established. This indicator of game performance was technically simple and its efficiency was determined by good physical development and fast psychomotor reaction. The correlations of shooting percentage of different throws with physical fitness were different. Discussion and conclusions. The shooting percentage of 2-point field goals correlated with muscle mass, body mass and take-off duration. This allows suggesting that a certain number of shots are performed after the physical contact with an opponent and jumping faster during the moment of throw. No correlation was established between 3-point field goals, free throws and physical fitness indicators. This means that shooting percentage was influenced by other factors. Steals, turnovers and passes, as complex technical and tactical actions, did not correlate with the researched physical fitness indicators. Keywords: shot, jump, height, block, power.

Research background and hypothesis. Overhead athletes are at higher risk to suffer from acute and chronic shoulder injuries. Research aim was to evaluate shoulder complex functional characteristics of the overhead and non-overhead young athletes before and two years after the concomitant arthroscopic type II superior labrum anterior posterior (SLAP) and partial – thickness rotator cuff (PTRC) repair and to present specific rehabilitation guidelines needed for successful return to sports. Research methods. The sample of the research included 38 male athletes: overhead (n = 19) and non-overhead (n = 19) athletes. All participants underwent concomitant arthroscopic type II SLAP and PTRC repair and were available for review at a minimum of two years after surgery. Shoulder range of motion was measured with goniometer. Constant score was used for the evaluation of the shoulder functional quality. Research results. Function of the shoulder complex had higher increase in non-overhead group (p < 0.05). Constant score two years after surgery did not return to optimal level. Shoulder flexion and internal rotation ROM in both groups and external rotation ROM of non-overhead athletes were the same as before surgery. Discussion and conclusions. Functional characteristics of shoulder complex measured with Constant score of overhead and non-overhead athletes statistically significantly increased two years after the arthroscopic surgery. Significant change of external rotation was established in overhead athletes: two years after surgery it was significantly smaller and did not return even to preoperative level. Sports specific and diagnose-based rehabilitation is needed to decrease deficit in function of the shoulder complex after concomitant arthroscopic type II SLAP and PTRC repair. Išvados Panaudojant eksperimentinius gyvūnus – triušius, įsisavinta modelinė ląstelinės kardiomioplastikos sistema, skirta reparacinio metodo taikymo gydymo praktikoje tyrimams. Methods Heart operation on laboratory animals (rabbits); primary myogenic cell lines – origin and maintenance in the culture in vitro; labeling stem cells using vital dyes or genetic manipulations; cell transplantation into animal heart; histological evaluation of the tissue. Results In the rabbit organism, an experimental model of heart infarction was acquired; the heart infarction was confirmed histologically; the labeling of stem cells by using vital dyes and genetic manipulations was applied in vitro; histological existence of stem cells was confirmed in heart tissue. Conclusions In laboratory animals (rabbits), an experimental model system of cellular cardiomyoplasty was acquired. The model is intended to study the possibilities of cellular therapy in clinical practice. The study of cell transplantation into and integration in the pathological focus confirmed the possibilities of heart regeneration by using stem cell therapy. Klinikinis atvejis Didelė intervencijos rizika ir sąlyginai stabili ligonio būklė nutolino operaciją. Paskutinio hospitalizavimo metu buvo įtarta aneurizmos disekacija ir spindžio trombozė. Dėl sparčiai blogėjančios būklės ligonis buvo operuojamas skubos tvarka. Operuojant rasta pilna trombų disekuojanti degeneravusi nusileidžiančiosios aortos aneurizma ir jos fistulė į kairįjį pagrindinį bronchą. Pašalinus aneurizmos maišą, nusileidžiančioji aorta buvo pakeista kraujagysliniu protezu, pašalintas ir nefunkcionavęs kairysis plautis. Pooperacinį laikotarpį sunkino tebesitęsiantis difuzinis kraujavimas. Nepaisant intensyvaus gydymo, ligonio būklė vis blogėjo ir jis mirė antrą parą po operacijos nuo dauginio organų nepakankamumo. Patologiniu pašalintojo plaučio tyrimu nustatyti seni organizuoti trombai ir nauji krešuliai bronchuose, taip pat cholesterolio kristalai parenchimoje, rodantys kraujavimo epizodus ir lėtinę embolizaciją aterominėmis plokštelėmis pro aneurizmos ir broncho fistulę. Išvada Ligonius, kuriems yra torakoabdominalinė aortos aneurizma, reikia stebėti labai aktyviai ir trumpai, o indikacijos chirurginiam gydymui turi būti nustatomos anksčiau ir ryžtingiau. Case report The high risk associated with the aneurysm repair and the conditionally stable patient’s state delayed the elective operative treatment. At a recent admission, aneurysm dissection with lumen thrombosis was suspected. The quickly deteriorating patient’s condition determined urgent surgical intervention. A dissecting degenerative descending aortic aneurysm packed with a large amount of thrombi and fistulation into the left main bronchus was found during the procedure. Following debridiment the descending aorta was replaced with a prosthetic graft, and the non-functioning left lung was extirpated. Persistent diffuse bleeding complicated the early postoperative course. Despite the aggressive medical therapy, the patient’s condition deteriorated progressively and he died on the second postoperative day due to multisystemic organ failure. Pathological investigation of the resected left lung showed chronic organized thrombi and fresh clots within the bronchi with cholesterol crystals in the parenchyma, evidencing bleeding episodes with chronic embolisation with atheromatous plaques through the aneurysm-bronchial fistula. Conclusion The follow-up of patients with thoracoabdominal aneurysm should be very active and short, and indications for surgical repair should be determined earlier and more decisively. Recent player tracking technology provides new information about basketball game performance. The aim of this study was to (i) compare the game performances of all-star and non all-star basketball players from the National Basketball Association (NBA), and (ii) describe the different basketball game performance profiles based on the different game roles. Archival data were obtained from all 2013-2014 regular season games (n = 1230). The variables analyzed included the points per game, minutes played and the game actions recorded by the player tracking system. To accomplish the first aim, the performance per minute of play was analyzed using a descriptive discriminant analysis to identify which variables best predict the all-star and non all-star playing categories. The all-star players showed slower velocities in defense and performed better in elbow touches, defensive rebounds, close touches, close points and pull-up points, possibly due to optimized attention processes that are key for perceiving the required appropriate environmental information. The second aim was addressed using a k-means cluster analysis, with the aim of creating maximal different performance profile groupings. Afterwards, a descriptive discriminant analysis identified which variables best predict the different playing clusters. The results identified different playing profile of performers, particularly related to the game roles of scoring, passing, defensive and all-round game behavior. Coaching staffs may apply this information to different players, while accounting for individual differences and functional variability, to optimize practice planning and, consequently, the game performances of individuals and teams.
