However, it is an uncommon cause of respiratory infections in critically ill patients admitted to the intensive care unit (ICU). Design and setting Prospective clinical investigation in a 21-bed ICU in a university hospital. Patients Seven patients requiring intensive care who developed a ICU-acquired pneumonia in which M. hominis was recovered from bronchoalveolar lavage and pleural fluid cultures. Interventions M. hominis was isolated in all patients by use of conventional bacteriological cultures. All strains were identified by 16S rRNA gene sequencing analysis. Patients' charts were reviewed for each case of infection. Results Seven strains of M. hominis were isolated during a 4-year period. All of these isolates were recovered from adult men admitted to the ICU and all had clinical signs of pneumonia. In three patients treatment for M. hominis with quinolones was associated with a good clinical response. Conclusion Suspicion of M. hominis pneumonia must be heightened particularly in critically ill patients. Therefore an understanding of the microbiology of this organism is essential to successfully treat patients with these infections that are not ordinarily covered with standard antibiotic therapy. Design and setting Prospective clinical investigation in a 21-bed intensive care unit (ICU) in a university hospital. Patients Seventy patients who had suffered from acute brain injury requiring intensive care. Interventions Jugular bulb vein and systemic samples were obtained on admission to the ICU and after 48a-h. PC12 cells were incubated in the presence of 10% of heat-inactivated patient's sera and apoptotic rate was determined by flow cytometry using annexin V and 7-aminoactinomycin D. Results Regional serum draining from the lesions induced higher early apoptosis of PC12 cells than systemic serum. Early apoptotic rate, Glasgow coma score, APACHE II score and the presence of pupil abnormalities were associated with mortality at 6 months in univariate statistical analyses. In logistic regression analysis only early apoptotic rate was an independent factor associated with mortality at 6 months (odds ratio: 1.502, 95% CI 1.2--1.9 pa-&lt a-0.001). The final model has a sensitivity of 82.4% and a specificity of 84.8% for predicting death within 6 months. Conclusions We developed a simple and reproducible in vitro model for predicting outcome in patients with traumatic or haemorrhagic brain injury that survived in the early phase. Our in vitro model combined with clinical and radiological measurements might improve the value of prognostic models to predict acute brain injury patients' outcome. To assess the usefulness of venous oxygen saturation in the jugular bulb (SjO.sub.2) as a complementary test for the diagnosis of brain death. Design. Prospective observational study. Setting. Polytrauma intensive care unit (ICU) of an acute-care teaching hospital in Santander, Spain. Patients. We studied 118 (44%) out of 270 patients with severe head injury and intracranial hemorrhage meeting criteria of brain death (lack of cardiac response to atropine, unresponsive apnea, and iso-electric EEG in the absence of shock, hypotension and treatment with muscle relaxants and/or central nervous system (CNS) depressant drugs). Measurements and results. At the moment at which clinical diagnosis of brain death was made and an iso-electric EEG was obtained, simultaneous oxygen saturation in central venous blood (right atrium) (SvO.sub.2) and jugular venous bulb (SjO.sub.2) samples was measured. The ratio between SvO.sub.2 and SjO.sub.2, expressed as CvjO.sub.2 (the so-called central venous-jugular bulb oxygen saturation rate CvjO.sub.2 = SvO.sub.2/SjO.sub.2) was calculated. CvjO.sub.2 less than 1 was obtained in 114 patients [mean (SD): 0.89 (0.02)], whereas CvjO.sub.2 greater than 1 was obtained in only 4 (3.38%). In the group of 152 survivors, a single patient was discharged from the ICU in a vegetative state in which CvjO.sub.2 was below 1. Cvj[O.sub.2 ]as a complementary test for the diagnosis of brain death showed 96.6% sensitivity, 99.3% specificity, and 99.1% and 97.4% positive and negative predictive values, respectively. Conclusion. Central venous-jugular bulb oxygen saturation rate below 1 together with accepted clinical criteria (unresponsive coma with brainstem areflexia) provides non-invasive assessment of cerebral circulatory arrest that can help to suspect brain death. Abstract

There is limited clinical evidence on the utility of the monitoring of Epstein-Barr virus (EBV) DNAemia in the pre-emptive management of post-transplant lymphoproliferative disease (PTLD) in solid organ transplant (SOT) recipients. We investigated current preventive measures against EBV-related PTLD through a web-based questionnaire sent to 669 SOT programmes in 35 European countries. This study was performed on behalf of the ESGICH study group from the European Society of Clinical Microbiology and Infectious Diseases. A total of 71 SOT programmes from 15 European countries participated in the study. EBV serostatus of the recipient is routinely obtained in 69/71 centres (97%) and 64 (90%) have access to EBV DNAemia assays. EBV monitoring is routinely used in 85.9% of the programmes and 77.4% reported performing pre-emptive treatment for patients with significant EBV DNAemia levels. Pre-emptive treatment for EBV DNAemia included reduction of immunosuppression in 50.9%, switch to mammalian target of rapamycin inhibitors in 30.9%, and use of rituximab in 14.5% of programmes. Imaging by whole-body 18-fluoro-deoxyglucose positron emission tomography (FDG-PET) is used in 60.9% of centres to rule out PTLD and complemented computer tomography is used in 50%. In 10.9% of centres, FDG-PET is included in the first-line diagnostic workup in patients with high-risk EBV DNAemia. Despite the lack of definitive evidence, EBV load measurements are frequently used in Europe to guide diagnostic workup and pre-emptive reduction of immunosuppression. We need prospective and controlled studies to define the impact of EBV monitoring in reducing the risk of PTLD in SOT recipients.
