Abstract

This Consensus Statement covers recommendations for the diagnosis and management of patients with pseudohypoparathyroidism (PHP) and related disorders, which comprise metabolic disorders characterized by physical findings that variably include short bones, short stature, a stocky build, early-onset obesity and ectopic ossifications, as well as endocrine defects that often include resistance to parathyroid hormone (PTH) and TSH. The presentation and severity of PHP and its related disorders vary between affected individuals with considerable clinical and molecular overlap between the different types. A specific diagnosis is often delayed owing to lack of recognition of the syndrome and associated features. The participants in this Consensus Statement agreed that the diagnosis of PHP should be based on major criteria, including resistance to PTH, ectopic ossifications, brachydactyly and early-onset obesity. The clinical and laboratory diagnosis should be confirmed by a molecular genetic analysis. Patients should be screened at diagnosis and during follow-up for specific features, such as PTH resistance, TSH resistance, growth hormone deficiency, hypogonadism, skeletal deformities, oral health, weight gain, glucose intolerance or type 2 diabetes mellitus, and hypertension, as well as subcutaneous and/or deeper ectopic ossifications and neurocognitive impairment. Overall, a coordinated and multidisciplinary approach from infancy through adulthood, including a transition programme, should help us to improve the care of patients affected by these disorders.

Abstract

In this paper, we report on the synthesis and applications of semiconducting nanostructures. Nanostructures of interest were zinc oxide (ZnO) nanowires and tungsten disulfide WS2 nanotubes where transistors/phototransistors and photovoltaic (PV) energy conversion cells have been fabricated. ZnO nanowires were grown with both high- and low-temperature approaches, depending on the application. Individual ZnO nanowire side-gated transistors revealed excellent performance with a field-effect mobility of 928 cm2 V·s. ZnO networks were proposed for large-area macroelectronic devices as a less lithographically intense alternative to individual nanowire transistors where mobility values in excess of 20 cm2 /V·s have been achieved. Flexible PV devices utilizing ZnO nanowires as electron acceptors and for photoinduced charge separation and transport have been presented. Phototransistors were fabricated using individual WS2 nanotubes, where clear sensitivity to visible light has been observed. The results presented here simply reveal the potential use of inorganic nanowires/tubes for various optoelectronic devices. © 2008 IEEE.

Abstract

OBJECTIVE: The aim was to formulate practice guidelines for the diagnosis and management of congenital hypothyroidism (CH). EVIDENCE: A systematic literature search was conducted to identify key articles relating to the screening, diagnosis, and management of CH. The evidence-based guidelines were developed with the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) system, describing both the strength of recommendations and the quality of evidence. In the absence of sufficient evidence, conclusions were based on expert opinion. CONSENSUS PROCESS: Thirty-two participants drawn from the European Society for Paediatric Endocrinology and five other major scientific societies in the field of pediatric endocrinology were allocated to working groups with assigned topics and specific questions. Each group searched the literature, evaluated the evidence, and developed a draft document. These papers were debated and finalized by each group before presentation to the full assembly for further discussion and agreement. RECOMMENDATIONS: The recommendations include: worldwide neonatal screening, approaches to assess the cause (including genotyping) and the severity of the disorder, the immediate initiation of appropriate L-T4 supplementation and frequent monitoring to ensure dose adjustments to keep thyroid hormone levels in the target ranges, a trial of treatment in patients suspected of transient CH, regular assessments of developmental and neurosensory functions, consulting health professionals as appropriate, and education about CH. The harmonization of diagnosis, management, and routine health surveillance would not only optimize patient outcomes, but should also facilitate epidemiological studies of the disorder. Individuals with CH require monitoring throughout their lives, particularly during early childhood and pregnancy Objective: The aim was to formulate practice guidelines for the diagnosis and management of congenital hypothyroidism (CH). Evidence: A systematic literature search was conducted to identify key articles relating to the screening, diagnosis, andmanagementof CH. The evidence-based guidelines were developed with the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) system, describing both the strength of recommendations and the quality of evidence. In the absence of sufficient evidence, conclusions were based on expert opinion. Consensus Process: Thirty-two participants drawn from the European Society for Paediatric Endocrinology and five other major scientific societies in the field of pediatric endocrinology were allocated to working groups with assigned topics and specific questions. Each group searched the literature, evaluated the evidence, and developed a draft document. These papers were debated and finalized by each group before presentation to the full assembly for further discussion and agreement. Abstract

The solid state characterization of stereoregular poly vinyl chloride (PVC) prepared by urea clathrate polymerization was carried out in comparison with free radically prepared sample by using various instrumental analyses. The principle of urea clathrate polymerization by inclusion phenomenon was described from a purely experimental point of view. The stereo-chemical difference of the resulting PVC appeared in discoloration, solubility into solvents, IR, DSC, and TG curves. Solid state 13C NMR spectra also clearly indicate the structural difference of this polymer. In FT-IR spectra, particularly, Krimm’s novel results were confirmed in the 700-600 cm-1 region. A new characteristic band (at 2933 cm-1) appeared which was explained in terms of Fermi resonance, the enhanced intermolecular hydrogen bonding in highly ordered syndiotactic sequences. In the dynamic ESR measurements in the solid state, signal intensity of the bulk PVC increased rapidly, which was directly connected with TG curve under the linear heating conditions (~10°C/min).

Abstract

When developing an expert system, the difficulty in acquiring knowledge poses a bottleneck. It is essential that the system possess a means to modify its knowledge bases. This study details the construction of a versatile inference system equipped with a rule-base refinement function that is expressed by a network with relations between the hypotheses as compositional elements. As an example for practical use, this paper examines the effectiveness of the system being proposed, using the rule base of an existing expert system to diagnose cracks in damaged bridge slabs. As a result, it was found that, by presenting adequate examples as training samples, the rule base is refined along with a remarkable increase in damage-cause inference accuracy.

Abstract

Microscaled non-woven fabrics with fiber diameters in the range 1.8–3.1 μm were prepared by electrospinning from a chloroform solution of a degradable multiblock copolymer (PDLCL) consisting of crystallizable poly(ω-pentadecalactone) hard segments (PPDL) and poly(ε-caprolactone) switching segments (PCL). The surface morphology and microstructure of the non-woven fabrics were characterized by scanning electron microscopy, temperature-controlled scanning probe microscopy, wide angle X-ray diffraction (WAXD), and small angle X-ray scattering (SAXS). The microstructural analysis demonstrated that the rod-like domains of hard segment (PPDL) formed in the electrospun PDLCL fiber functioned as physical crosslinks. Cyclic, thermomechanical tensile tests showed that the electrospun PDLCL fabrics exhibited good shape-memory properties with strain recovery rates of Rr = 89–95% and strain fixity rates of Rf = 82–83% after the 2nd cycle, when small deformations were applied at clinically relevant temperatures.

42. The Last 41.000 Years Fluctuation in Atmosperic CO2 Concentration Inferred From the Changes in Oxygen and Carbon Stable Isotopes Ratios of the Marine Sediment

Abstract

The past atmospheric CO2 concentrations were reconstructed based on the results of measurements of stable oxygen and carbon isotopic ratios of fossil foraminifer and total organic carbon contained in marine sediment taken from the Okinawa Trough, East China Sea. In this study, we utilized two models of Popp et al and Rau et al. for the reconstruction. The results show that the whole trends of the changes in CO2 concentrations are very similar, even when it is compared to the atmospheric CO2 concentration of air trapped in ice core from southern pole. Changes in atmospheric CO2 concentrations are interpreted as a consequence of fluctuation in ocean surface water utilization of CO2 by marine organism and those are closely related to glacial-interglacial (cold-warm) fluctuations between maximum and minimum values through most Quaternary. Rekonstruksi terhadap Perubahan konsentrasi CO2 yang terkandung dalam udara telah dilakukan berdasarkan hasil pengukuran rasio isotop stabil oksigen dan karbon dalam fosil foraminifera dan total karbon organik yang terkandung dalam sedimen dasar laut dari Okinawa Trough, Laut Cina Timur. Dalam studi ini, dipakai model dari Popp et al. dan Rau et al. untuk rekonstruksi. Hasil studi menunjukkan bahwa kedua tren dari Perubahan kandungan CO2 udara sangat mirip, bahkan bila dibandingkan dengan kandungan CO2 udara yang terperangkap dalam inti es di Kutup Selatan sekalipun. Perubahan kandungan CO2 udara diinterpretrasikan sebagai akibat fluktuasi konsumsi CO2 di permukaan air laut oleh mikro-organisme yang juga sangat erat hubungannya dengan fluktuasi glasial-interglasial (dingin-panas) antara suhu udara bumi maksimum dan minimum sepanjang masa Kuarter.
