5. Beyond the body – Rethinking the architectural module to promote social inclusion

The architectural module has long been associated with the concept of measurement, where standardisation of the human body is used to define absolute modularity. With the awareness of human diversity, this narrow view of the module’s applicability is problematic, particularly in processes of environmental accessibility and inclusion. This paper critically explores the limits of the traditional measurement module, rethinking the concept of modularity to account for physical and perceptive diversity. By doing so, we aim to promote social inclusion and universal design in architectural projects for people. The paper concludes that the evolution of the concepts of the module and the human being requires a revision of their very meanings, calling for a more inclusive approach to design and planning in our contemporary world. 9. How Covid-19 changed the epidemiology of febrile urinary tract infections in children in the emergency department during the first outbreak

Abstract Background The first Covid-19 pandemic affected the epidemiology of several diseases. A general reduction in the emergency department (ED) accesses was observed during this period, both in adult and pediatric contexts. Methods This retrospective study was conducted on the behalf of the Italian Society of Pediatric Nephrology (SINePe) in 17 Italian pediatric EDs in March and April 2020, comparing them with data from the same periods in 2018 and 2019. The total number of pediatric (age 0–18 years) ED visits, the number of febrile urinary tract infection (UTI) diagnoses, and clinical and laboratory parameters were retrospectively collected. Results The total number of febrile UTI diagnoses was 339 (73 in 2020, 140 in 2019, and 126 in 2018). During the first Covid-19 pandemic, the total number of ED visits decreased by 75.1%, the total number of febrile UTI diagnoses by 45.1%, with an increase in the UTI diagnosis rate (+ 121.7%). The data collected revealed an increased rate of patients with two or more days of fever before admission (p = 0.02), a significant increase in hospitalization rate (+ 17.5%, p = 0.008) and also in values of C reactive protein (CRP) (p = 0.006). In 2020, intravenous antibiotics use was significantly higher than in 2018 and 2019 (+ 15%, p = 0.025). Urine cultures showed higher Pseudomonas aeruginosa and Enterococcus faecalis percentages and lower rates of Escherichia coli (p = 0.02). Conclusions The first wave of the Covid-19 pandemic had an essential impact on managing febrile UTIs in the ED, causing an absolute reduction of cases referring to the ED but with higher clinical severity. Children with febrile UTI were more severely ill than the previous two years, probably due to delayed access caused by the fear of potential hospital-acquired Sars-Cov-2 infection. The possible increase in consequent kidney scarring in this population should be considered.

Within the 9th European Framework programme, since 2021 EUROfusion is operating five tokamaks under the auspices of a single Task Force called ‘Tokamak Exploitation’. The goal is to benefit from the complementary capabilities of each machine in a coordinated way and help in developing a scientific output scalable to future largre machines. The programme of this Task Force ensures that ASDEX Upgrade, MAST-U, TCV, WEST and JET (since 2022) work together to achieve the objectives of Missions 1 and 2 of the EUROfusion Roadmap: i) demonstrate plasma scenarios that increase the success margin of ITER and satisfy the requirements of DEMO and, ii) demonstrate an integrated approach that can handle the large power leaving ITER and DEMO plasmas. The Tokamak Exploitation task force has therefore organized experiments on these two missions with the goal to strengthen the physics and operational basis for the ITER baseline scenario and for exploiting the recent plasma exhaust enhancements in all four devices (PEX: Plasma EXhaust) for exploring the solution for handling heat and particle exhaust in ITER and develop the conceptual solutions for DEMO. The ITER Baseline scenario has been developed in a similar way in ASDEX Upgrade, TCV and JET. Key risks for ITER such as disruptions and run-aways have been also investigated in TCV, ASDEX Upgrade and JET. Experiments have explored successfully different divertor configurations (standard, super-X, snowflakes) in MAST-U and TCV and studied tungsten melting in WEST and ASDEX Upgrade. The input from the smaller devices to JET has also been proven successful to set-up novel control schemes on disruption avoidance and detachment.

In 2021 JET exploited its unique capabilities to operate with T and D–T fuel with an ITER-like Be/W wall (JET-ILW). This second major JET D–T campaign (DTE2), after DTE1 in 1997, represented the culmination of a series of JET enhancements—new fusion diagnostics, new T injection capabilities, refurbishment of the T plant, increased auxiliary heating, in-vessel calibration of 14 MeV neutron yield monitors—as well as significant advances in plasma theory and modelling in the fusion community. DTE2 was complemented by a sequence of isotope physics campaigns encompassing operation in pure tritium at high T-NBI power. Carefully conducted for safe operation with tritium, the new T and D–T experiments used 1 kg of T (vs 100 g in DTE1), yielding the most fusion reactor relevant D–T plasmas to date and expanding our understanding of isotopes and D–T mixture physics. Furthermore, since the JET T and DTE2 campaigns occurred almost 25 years after the last major D–T tokamak experiment, it was also a strategic goal of the European fusion programme to refresh operational experience of a nuclear tokamak to prepare staff for ITER operation. The key physics results of the JET T and DTE2 experiments, carried out within the EUROfusion JET1 work package, are reported in this paper. Progress in the technological exploitation of JET D–T operations, development and validation of nuclear codes, neutronic tools and techniques for ITER operations carried out by EUROfusion (started within the Horizon 2020 Framework Programme and continuing under the Horizon Europe FP) are reported in (Litaudon et al Nucl. Fusion accepted), while JET experience on T and D–T operations is presented in (King et al Nucl. Fusion submitted).

JET, the world’s largest operating tokamak with unique Be/W wall and tritium handling capability, completed a Deuterium-Tritium (D-T) campaign in 2021 (Maggi et al 29th Fusion Energy Conf. ) following a decade of preparatory experiments, dedicated enhancements, technical rehearsals and training (Horton et al 2016 Fusion Eng. Des. 109–111 925). Operation with tritium raises significant technical, safety and scientific challenges not encountered in standard protium or deuterium operation. This contribution describes the tritium operational requirements, pulses and technical preparations, new operating procedures, lessons learned and details on the achieved operational availability and performance. The preparation and execution of the recent JET tritium experiments benefitted from the previous experience in 1991 (Preliminary Tritium Experiment), 1997 (DTE1 campaign) and 2003 (Trace Tritium Campaigns) and consisted of the following five phases: technical rehearsals and scenario preparation, tritium commissioning, 100% tritium campaign, D-T campaign (DTE2), tritium clean-up. Following the clean-up JET resumed normal operation and is currently undertaking a further D-T campaign (DTE3).

The contribution presents state-of-the-art critically through the results of some applied research financed with public funds in areas related to the concept of innovability understood as an ‘innovative’ approach to the environmental accessibility of urban places. This condition is functional to planning, implementing, and managing urban contexts from an environmental and economically ‘sustainable’ point of view. Through these experiences, it is possible to give a sufficiently an in-depth picture of the ICT tools adopted, and the potential of the tools and experiences created. Focusing on users’ needs and evaluating urban/environmental performance leads to evaluating the current situation in medium-sized urban centres. The reported case studies show the high potential of computerisation of the urban management processes, which does not correspond to a convincing social, economic, political and cultural context due to its partial and inefficient application of ICT. These results present some possible actions on the interface and governance of the human-built environment to design a future ‘smart’, ‘innovative’ and ‘sustainable’ city.

In the process towards energy transition from fossil fuel based energy to renewables systems, the way to involve final users in the energy production is still complex. Although many steps forward have been made by community energy around the world and especially in the EU, there are still many barriers and entry obstacles that do not allow a revolution in the energy market. In this paper, various aspects that distinguish a community energy have been explored. After presenting a state of the art on the development of community energy in Europe, the technologies supporting these cooperative realities were studied. This work reports a complete analysis on renewable energy production systems, community management software and the most innovative user engagement tools. This paper also presents the results of a survey, with the aim of studying how people can be more involved in the energy market. The outcomes show how people needs should be considered, in order to increase awareness and control over the energy that they consume for a better development of future community energy.

Despite the alarming and growing burden of cardiovascular diseases in sub-Saharan Africa (SSA), there is still a huge lack of specialised institutions in the region with a mean of one cardio-surgical unit for 33 million inhabitants. Despite the numerous efforts from humanitarian organisations made in recent years, the setting up of cardio-surgical units in the region remains challenging with regards to long-term sustainability. Indeed, besides the lack of financial resources, the insufficient local expertise in addition to the inadequate health infrastructure, unpredictable threats from external factors such as recurrent conflicts and humanitarian crises are still major concerns in an environment characterised by endemic socio-political instability. In Cameroon, located in the North West Anglophone region at 500 km from the capital, the cardiac centre of Shisong (CCS) is currently the lone cardio-surgical institution of the country. Fruit of a joint initiative of two Italian Non-governmental organisations namely, Bambini Cardiopatici nel Mondo (ABCnM) and Cuore Fratello (CF), and a local religious partner, the Tertiary Sisters of Saint Francis (TSSF), the CCS was faced with in the middle of a socio-political crisis that led to the urgent need of revision of the cardio-surgical project. The current paper reviews the impact of the ongoing socio-political crisis on the CCS over the past 3 years, in terms of clinical activities, staff perspectives, and long-term sustainability.

The research refers to the eco-social theory and environmental accessibility principles towards inclusive strategies and it targets public spaces used as a requirement for an autonomous and independent life. The contribution, referring to the 2030 Agenda, presents the experiment applied to the Municipality of Udine, a medium-size Italian city. While drafting the PEBA, the research focuses on urban itineraries, adopting and updating the regulatory tools in force. The administration, supported by the university, commuted the drafting process into a strategic opportunity of knowledge of the context defining the relationships between citizens and urban habitat. The research provides the possibility to qualify some strategic objectives and identify potential actions for an increasingly people-friendly city.

Data Center (DC) management aims at promptly serving user requests while minimizing the energy consumed. This is achieved by turning off unnecessary servers to save energy and adapting the number of servers that are $on$ to the time-varying and heterogeneous user requests. A great change in the number of servers $on$ leads to a considerable management effort, also called control effort in the literature, which should be reduced as much as possible. Since feedback control can improve the performance of computing systems and networks, we propose to use it to achieve this dynamic capacity provisioning of the DC. In order to design this feedback control, first, we developed a dynamic model of the DC. The purpose of this paper is to design a feedback control strategy based on the DC model, able to optimize i) the Quality of Service, ii) the energy consumed and iii) the management effort. A simple Reactive open-loop Control which provides an amount of energy equal to the amount requested in the previous time interval is considered as a benchmark for comparison. Second, two feedback controls based on the balance equations of the DC are studied, namely i) Reactive Feedback Control providing an amount of energy equal to that provided by the reactive open-loop control but adding the accumulated demand that has not yet been served, and ii) Model Predictive Control optimizing a constrained cost that weights the management effort and the prediction error. Reactive Control, Reactive Feedback Control and Model Predictive Control are compared in terms of energy consumed, energy error and management effort. Quantitative results of the comparative performance evaluation are given, based on a data set collected from a real DC.

The JET hybrid scenario has been developed from low plasma current carbon wall discharges to the record-breaking Deuterium-Tritium plasmas obtained in 2021 with the ITER-like Be/W wall. The development started in pure Deuterium with refinement of the plasma current, and toroidal magnetic field choices and succeeded in solving the heat load challenges arising from 37 MW of injected power in the ITER like wall environment, keeping the radiation in the edge and core controlled, avoiding MHD instabilities and reaching high neutron rates. The Deuterium hybrid plasmas have been re-run in Tritium and methods have been found to keep the radiation controlled but not at high fusion performance probably due to time constraints. For the first time this scenario has been run in Deuterium-Tritium (50:50). These plasmas were re-optimised to have a radiation-stable H-mode entry phase, good impurity control through edge T _i gradient screening and optimised performance with fusion power exceeding 10 MW for longer than three alpha particle slow down times, 8.3 MW averaged over 5 s and fusion energy of 45.8 MJ.
