Uterine leiomyomas (fibroids) are the most common tumors in women of reproductive age and a cause of significant morbidity in this patient population. Depending on the fibroid location, they can be the cause of a variety of symptoms, such as abnormal uterine bleeding, constipation, urinary frequency, and pain. Historically, hysterectomy has been the primary treatment option, and uterine fibroids remain the leading cause for hysterectomy in the United States. However, women who do not wish to undergo hysterectomy now have a variety of less invasive options available, including uterine artery embolization. This article discusses uterine artery embolization as well as some of the other treatment strategies for symptomatic uterine fibroids. In many situations, there may be no single best treatment option but several viable alternatives. Each option is discussed with consideration of outcomes, complications, and, when possible, cost-effectiveness. Acute upper gastrointestinal (UGI) tract bleeding is best initially investigated and treated with endoscopy. For patients who fail therapeutic endoscopy, both surgery and transcatheter arteriography and intervention (TAI) are equally effective. Transcatheter arteriography and intervention should be considered as a treatment option in patients with UGI bleeding, particularly those at high risk for surgery. Transcatheter arteriography and intervention for UGI bleeding has a low rate of major complications, and prolonged clinical success is seen in at least 65% of patients. Transcatheter arteriography and intervention is the best method of treatment for bleeding occurring into the biliary tree or pancreatic duct. In patients with acute lower gastrointestinal (LGI) tract bleeding who are hemodynamically stable, either colonoscopy or nuclear medicine scans can be used for diagnosis. Colonoscopy will identify the site of bleeding more frequently than other methods and can provide effective treatment. The use of emergent TAI is most appropriate for patients with massive LGI bleeding, because contrast extravasation is more likely to be seen on diagnostic arteriography, and this can then guide therapeutic embolization. Transcatheter arteriography and intervention may successfully stop bleeding in 40% to 85% of patients. Major complications from TAI are uncommon, but the risk for rebleeding is quite high, particularly when LGI bleeding originates from the jejunum, ileum, or cecum. Transcatheter arteriography and intervention is most effective for the treatment of bleeding from colonic diverticulitis and for bleeding occurring distal to the cecum. The choice of colonoscopy, TAI, or surgery for hemodynamically unstable patients with acute LGI bleeding will depend on institutional expertise and whether the site of bleeding has been localized. Upper gastrointestinal bleeding is a significant cause of morbidity and mortality, affecting 36 to 48 per 100,000 persons annually. Aggressive resuscitation and upper endoscopy remain the cornerstones of therapy; however, in cases refractory to endoscopic diagnosis and management, radiology plays an increasingly vital and often lifesaving role, thanks to improvements in both imaging and interventional techniques. The various etiologies of upper gastrointestinal bleeding are discussed along with specific management recommendations based on an extensive literature review of current radiographic methods., (Copyright © 2010 American College of Radiology. Abnormal fluid collections occur throughout the body and represent a wide range of pathologies, including abscesses, pseudocysts, cysts, lymphoceles, seromas, bilomas, hematomas, urinomas, and infected neoplasms. Appropriate management often depends on clinical presentation, location, type of collection, early response to treatment, and the presence of complicating factors such as fistulas, septations, and increased viscosity. Physicians should carefully review clinical and imaging findings and make evidence-based recommendations for the best treatment, which may include antibiotics, needle aspiration, percutaneous drainage, endoscopic drainage, or surgical drainage. This paper addresses percutaneous catheter drainage and alternative treatment options for the management of fluid collections and is the result of evidence-based consensus by the ACR Appropriateness Criteria Expert Panel on Interventional Radiology.

Focal tumor ablation--whether applied percutanously, laparoscopically, or by means of open surgery--is an effective therapy for selected liver tumors. The choice of liver ablation as well as the choice between percutaneous and surgical approaches is dependent on tumor factors, patient factors, and other viable treatment options. Currently, the largest cumulative reported experience is with radiofrequency (RF) ablation of hepatocellular carcinoma and colorectal metastases. This document is a position statement of the Interventional Oncology Task Force and the Standards Division of the Society of Interventional Radiology regarding the use of percutaneous RF ablation for the treatment of liver tumors.

The field of interventional oncology includes tumor ablation as well as the use of transcatheter therapies such as embolization, chemoembolization, and radioembolization. Terminology and reporting standards for tumor ablation have been developed. The development of standardization of terminology and reporting criteria for transcatheter therapies should provide a similar framework to facilitate the clearest communication among investigators and provide the greatest flexibility in comparing established and emerging technologies. An appropriate vehicle for reporting the various aspects of catheter directed therapy is outlined, including classification of therapies and procedure terms, appropriate descriptors of imaging guidance, and terminology to define imaging and pathologic findings. Methods for standardizing the reporting of outcomes toxicities, complications, and other important aspects that require attention when reporting clinical results are addressed. It is the intention of the group that adherence to the recommendations will facilitate achievement of the group's main objective: improved precision and communication for reporting the various aspects of transcatheter management of hepatic malignancy that will translate to more accurate comparison of technologies and results and, ultimately, to improved patient outcomes.

The role of endovascular therapy in the management of inflammatory aneurysms of the infrarenal abdominal aorta has been controversial. Review of our endovascular database identified six patients who have undergone treatment for preoperatively diagnosed inflammatory abdominal aortic aneurysms. Outcomes measured were primary success of the procedure, variation in computed tomographic (CT) scan-defined perianeurysmal fibrosis, change in aneurysm size, development of endoleak, requirement of reintervention, aneurysm rupture, and progression or resolution of symptoms. At a median follow-up of 20 months (range 4-56 months), endovascular repair has been successful in all six patients. All patients demonstrated CT reduction of perianeurysmal fibrosis, with a median of 47% absolute reduction (range 33-69%, p = 0.014). All patients had aneurysm sac shrinkage, with a mean of 41% (range 6-86%, p = 0.04). There were no aneurysm ruptures or persistent endoleaks. Of the three patients who presented with abdominal or back pain, all are now symptom-free. One patient required reintervention for limb thrombosis of a bifurcated graft after 2 years. In conclusion, endovascular treatment of an inflammatory abdominal aortic aneurysm is safe and effective and the treatment of choice in anatomically suitable patients.

Regular vascular access blood flow (Qa) surveillance is recommended to detect graft stenosis; however, there is little evidence that monitoring and correcting with angioplasty improves graft survival. This blinded, randomized, controlled trial of 112 patients studied time to graft thrombosis and graft loss, comparing monthly Qa plus standard surveillance (dynamic venous pressure and physical examination) (treatment group) to standard surveillance alone (control group). Only the treatment group was referred for angiogram if Qa <650 ml/min or a 20% decrease in Qa from baseline. Percutaneous angioplasty was performed for stenosis >50%. The rate of graft thrombosis per patient-year at risk was 0.41 and 0.51 in the control and treatment groups, respectively. Fifty-one interventions (0.93/patient-years at risk) were performed in the treatment group versus 31 interventions (0.61/patient-years at risk) in the control group. There was no difference in time to graft loss (P = 0.890). In a multivariate analysis, aspirin (ASA) therapy at baseline was associated with an 84% reduction in risk of graft thrombosis (odds ratio [OR], 0.14; P = 0.002). Higher baseline Qa (OR, 0.84; P = 0.05) and longer interval since graft insertion (OR, 0.97; P = 0.07) were associated with a decrease in graft thrombosis. Results reveal that graft surveillance that uses Qa increases the detection of stenosis, compared with standard surveillance; however, intervention with angioplasty does not improve the time to graft thrombosis or time to graft loss.

Purpose: To report data collected by the Canadian Registry of the Günther Tulip Retrievable Filter (GTF)., Materials and Methods: Between February 1998 and December 2000, 90 patients at eight hospitals underwent implantation of 91 GTFs. There were 45 male patients and 45 female patients, age 17-88 years, with a mean age of 49 years. Indications for filter placement were pulmonary embolism (PE) or deep vein thrombosis (DVT) with a contraindication to anticoagulation in 83 patients, prophylaxis after massive PE in one, prophylaxis for proximal free-floating thrombus in one, and prophylaxis with no DVT or PE in six patients (major trauma, n = 4; high preoperative risk, n = 2). GTF retrieval was attempted in selected patients from a right internal jugular vein approach., Results: One GTF was inadvertently placed in the right iliac vein and could not be retrieved. There were no other major placement complications. GTF retrieval was attempted in 52 patients (53 GTFs); 52 GTFs were successfully retrieved from 51 patients. Implantation times were 2-25 days (mean, 9 d). Of these 51 patients, 37 underwent follow-up for 5-420 days (mean, 103 d) after filter retrieval. Four patients (8% of retrieved GTFs) required reinsertion of a permanent filter 17-167 days (mean, 78 d) after GTF retrieval as a result of bleeding from anticoagulation (n = 2) or because the patient required further surgery (n = 2). One other patient had recurrent DVT 230 days after retrieval; no PE or other complication was documented in the retrieval group. GTFs were not retrieved from 39 patients for various reasons. Of these 39 patients, 25 underwent follow-up 7-420 days (mean, 85 d) after filter placement. Two patients developed filter occlusion (5%); no other complications were documented., Conclusion: The GTF has a broad range of utility: it can be used as a permanent filter or retrieved after implantation periods of 15 days and possibly longer. However, indications for retrieval require further study, as does the maximum implantation time.
