Eight experiments (N = 2,003) assessed the relation between working memory capacity (WMC) and performance on the antisaccade task. Experiments 1-5 and 7 examined individual differences in aspects of goal management processes occurring during the preparatory delay of the antisaccade task. WMC tended to interact with delay interval suggesting that high WMC individuals better prepared for the upcoming trial by activating the task goal to a higher level than low WMC individuals (although these effects were generally small). Experiments 3a, 4, and 7 further demonstrated that individual differences in the consistency of attention (i.e., lapses of attention) were partially important for the relation between WMC and antisaccade performance. Experiment 5 demonstrated that knowledge of the likelihood of target location increased overall performance, but did not interact with WMC. Experiment 6 manipulated stimulus onset asynchrony and suggested that speed factors are also likely important for the relation between WMC and antisaccade performance. Finally, structural equation models in Experiment 7 suggested that lapses of attention and speed factors partially accounted for the relation between WMC and antisaccade, but WMC still accounted for unique variance in antisaccade. Collectively, the results suggest that multiple factors (goal activation, consistency of attention, and speed factors) contribute to the relation between variation in WMC and performance on the antisaccade task.

The growth of the self-concept through increasing perspectives, identities, resources, and efficacy is known as self-expansion and typically involves novelty, challenge, interest, and/or excitement. Self-expansion is positively associated with health factors including self-reported physical activity (PA). This study is the first to investigate self-expansion and daily PA, and with a PA monitor. Fifty community participants completed baseline questionnaires, wore a Fitbit One and completed daily self-report questionnaires for 28 days, and completed follow-up questionnaires. Daily surveys included questions about both general and PA-specific self-expansion. Across the 4 weeks, steps taken was positively correlated with both general (all maximum likelihood r = 0.17) and PA-specific self-expansion (maximum likelihood rs of 0.15 and 0.16), and PA-specific self-expansion was positively correlated (maximum likelihood rs of 0.38 and 0.50) with aerobic activity. Future research should investigate this relationship in a larger more diverse sample and test whether PA-specific self-expansion can be utilized as an acceptable, feasible, and effective intervention to increase daily steps and other forms of PA.

PremisePhloem tissue allows for sugar transport along the entirety of a plant and, thus, is one of the most important anatomical structures related to growth. It is thought that the sugar-conducting sieve tube may overwinter and that its cells persist multiple seasons in deciduous trees. One possible overwintering strategy is to build up callose on phloem sieve plates to temporarily cease their function. We tested the hypothesis that five deciduous tree species produce callose on their sieve plates on a seasonal basis.MethodsYoung shoots of five deciduous tree species were sampled periodically between April 2019 and February 2020 in Edmonton, Alberta, Canada. After enzymatic digestion of cytoplasmic constituents, cross sections were imaged using scanning electron microscopy to observe and quantify the level of callose deposition at monthly intervals, and sieve plate pore size was measured. Using a conductivity apparatus, we measured xylem native embolism during these sampling periods.ResultsContrary to past work on some of the same species, we found little evidence that sieve tubes overwinter by becoming occluded with callose. Instead, we found that most sieve plates remain open. Xylem embolism was minimal during the peak growing season, but increased over winter.ConclusionsMany species had been assumed to deposit callose on sieve plates over winter, though anatomical and phenological phloem data were sparse. Our data do not support this notion.

In 2 experiments, eye-tracking was used to examine individual differences in attention during encoding and their relation to associative learning. Pupillary responses were used as an indicator of the amount of attention devoted to items, whereas eye fixations provided a means of assessing attentional focus among items within each to-be-remembered word pair. In both experiments, participants performed a paired associates (PA) cued recall task while pupil diameter and eye fixations were simultaneously recorded. Results from Experiment 1 revealed that larger pupillary responses at encoding, as well as more alternating fixations between the Cue and Target items for each word pair (i.e., switches), was associated with increased learning ability. Critically, while Experiment 2 revealed that effective strategy use partially accounted for the relationship between pupillary responses and PA recall accuracy, pupillary responses still explained unique variance in PA recall accuracy--a result that held even when controlling for the influence of working memory capacity and long-term memory ability. Collectively, the results suggest attentional abilities at encoding are vital for successful learning.

Little is known about the extent of prior authorization requirements in otolaryngology. We performed a secondary analysis of data comparing prior authorization (PA) policies across 5 major Medicare Advantage insurers to estimate the counterfactual proportion of 2021 Medicare Part B fee‐for‐service spending and utilization for commonly performed otolaryngologic procedures that would have required PA. The counterfactual proportion of spending (range: 20.4%–27.6%) and utilization (range: 1.8%–4.5%) requiring PA was relatively consistent across insurers and largely attributable to rhinologic procedures. However, PA requirements for specific services varied widely among insurers. Among the 70 (of 196; 35.7%) services subject to PA by any insurer, nearly half were subject to PA by a single insurer (n = 34; 48.6%). Only 10 (14.3%) services were subject to PA by 4 (n = 6; 8.6%) or 5 (n = 4; 5.7%) insurers. These discrepancies illustrate the challenges of navigating discordant insurer policies for otolaryngologists and raise concerns about the validity of certain PA requirements. [ABSTRACT FROM AUTHOR]

Three experiments examined if deep processing would aid in reducing off-task thoughts during learning. In each experiment, participants learned words or pairs of words under deep or shallow conditions. During learning, participants were periodically presented with thought-probes to examine if they were experiencing off-task thoughts (mind wandering, external distraction, mind blanking). Levels of processing was manipulated both within (Experiment 1) and between subjects (Experiments 2 and 3) using structural, rhyme, and semantic judgements and testing memory with cued-recall (Experiments 1 and 2) or recognition (Experiment 3). All three experiments demonstrated a levels of processing effect on memory with better performance in deep compared to shallow conditions. Importantly, in all three experiments rates of off-task thinking (and mind wandering more specifically) were the same across conditions and Bayes factors suggested moderate evidence in favour of the null hypothesis. Health care professionals can enhance conversations about serious illness and medical decision-making by adopting a transparent, standardized approach. This article critiques established communication strategies, which often emphasize patient values and goals without providing the necessary medical information to align these goals with a shared understanding of prognosis. We propose an alternate strategy that (1) provides detailed explanations of medical conditions at the beginning of the conversation, (2) includes support persons in discussions, (3) considers capacity, and (4) offers tailored advice by clinicians. The proposed framework aims to provide patients (or their delegates) with the information they need to integrate their values in pursuit of well-informed medical decisions. This strategy builds trust by providing honest information about medical conditions and their trajectories. It empowers decision makers to consider realistic outcomes, allowing them to accept or reject treatments in accordance with their preferences. This article presents a thorough step-by-step guide on how to conduct a serious illness conversation and facilitate medical decision-making, including a supplement that provides example phrases for use in clinical practice. The current study investigated how social support may mitigate the risk of procrastination, particularly among those high in intolerance of uncertainty. This study examined associations between personality traits, procrastination, and perceived social support among 394 undergraduate students. Participants completed self-reported measures of intolerance of uncertainty, procrastination, and social support from family, friends, and significant others. Regression analyses revealed a significant interaction between family social support and intolerance of uncertainty in predicting procrastination. AbstractObjectiveMethodsResultsConclusionsWe aim to explore the concept of distance and journey to goal, and consideration of these 2 elements a priori when choosing LLT.Modelling of expected % LDL-C reductions was carried out on a range of hypothetical patients’ baseline LDL-C values prior to any LLT being commenced. Therapies were then added in a stepwise manner based on the pathway demonstrated in current national guidance and compared with goal achievement on a novel LLT optimization pathway implemented in Morecambe Bay NHS Trust.Modelling of a stepwise lipid management pathway shows that high-intensity statin monotherapy is not sufficient in most modelled baseline LDL-C scenarios to achieve guideline-recommended goals. Furthermore, ezetimibe second line may preclude 3rd line injectable prescribing and lead to “ezetimibe limbo” where the patient is now below the reimbursement threshold for injectable prescribing but still not achieving their LDL-C target. Overall goal achievement is poor across the spectrum of modelled LDL-C levels. In contrast, by following the Morecambe Bay pathway all patients on statin for the range of hypothetical baseline LDL-C levels can reach an LDL-C target of < 1.8 mmol/L.This study identifies a therapeutic gap when following a stepwise approach highlighted by recent national guidance. Our proposal of a novel pathway highlights that the order in which drugs are added is important in the context of national reimbursement thresholds and allows LDL-C goal to be reached in a timely manner, regardless of the starting baseline LDL-C level. Relations between conative factors (task-specific motivation, attention self-efficacy, and self-set goals) and individual differences in attention control (AC) performance were investigated in two latent variable studies. Participants performed AC tasks along with measures of working memory and processing speed. During the AC tasks, participants self-reported their motivation, self-efficacy, and self-set goals for the tasks. Task-unrelated thoughts were also assessed. Confirmatory factor analyses demonstrated that latent factors for the constructs could be formed and the conative factors were each related to the AC factor. Structural equation modeling further suggested that the conative factors tended to account for unique variance in attention, even after accounting for shared variance with working memory and processing speed. These results provide evidence that conative factors are important for individual differences in AC and further suggest that multiple factors likely contribute to variation in performance on AC tasks. Working memory capacity (WMC) has received a great deal of attention in cognitive psychology partly because WMC correlates broadly with other abilities (e.g., reading comprehension, second-language proficiency, fluid intelligence) and thus seems to be a critical aspect of cognitive ability. However, it is still rigorously debated why such correlations occur. Some theories posit a single ability (e.g., attention control, short-term memory capacity, controlled memory search) as the primary reason behind WMC's predictiveness, whereas others argue that WMC is predictive because it taps into multiple abilities. Here, we tested these single- and multifaceted accounts of WMC with a large-scale (N = 974) individual-differences investigation of WMC and three hypothesized mediators: attention control, primary memory, and secondary memory. We found evidence for a multifaceted account, such that no single ability could fully mediate the relation between WMC and higher order cognition (i.e., reading comprehension and fluid intelligence). Further, such an effect held regardless of whether WMC was measured via complex span or n-back. Public Significance Statement: Individual differences in working memory capacity predict a host of important outcomes such as intelligence, academic performance, emotion regulation, and second-language acquisition. This study indicates that these relationships are likely multifaceted. Complex cognitive processes require the instantiation of task goals, the maintenance of goal-relevant information in short-term memory, and the controlled retrieval of information from long-term memory. Children's early experiences have potential to shape their development through early childhood, middle childhood, and into adolescence. Family structure at birth and material hardship may offer insight into how children's health and well‐being are shaped within their family of origin. The current paper examined (a) the association between family structure at birth and material hardship 1‐year post‐birth and (b) the differential impact of race on the associations between family structure and material hardship. We further explored the association between family structure and material hardship on self‐reported health in (a) middle childhood and (b) adolescence. Using the Future of Families and Child Wellbeing dataset, an ordinary least squares (OLS) regressions and structural equation path models (SEM) were used to examine associations between family structure, material hardship, and health outcomes (body mass index [BMI], anxiety, and suicidal ideation). Results indicated that family structure predicted material hardship at 1‐year until maternal characteristics were taken into account. Race/ethnicity did not moderate the association. Also, family structure at birth was indirectly associated with youth anxiety through material hardship but did not account for BMI or sucidical ideation. Social supports and state policies should prioritize maternal health access and ensure families are able to meet their basic needs to mitigate children's health risks. [ABSTRACT FROM AUTHOR]

BACKGROUND:Patients undergoing salvage laryngectomy are predisposed to radiation-induced hypothyroidism and impaired wound healing secondary to the tissue effects of prior treatment. The impact of hypothyroidism on postoperative wound healing is not established. METHODS:A single-institution retrospective case series was performed. The inclusion criteria specified preoperatively euthyroid adults who underwent salvage laryngectomy with concurrent neck dissection between 1997 and 2015 for persistent or recurrent laryngeal squamous cell carcinoma after radiation or chemoradiation therapy (n = 182). The principal explanatory variable was postoperative hypothyroidism, defined as thyroid-stimulating hormone (TSH) higher than 5.5 mIU/L. The primary end points of the study were pharyngocutaneous fistulas and wounds requiring reoperation. Multivariate analysis was performed. RESULTS:The fistula rate was 47% among hypothyroid patients versus 23% among euthyroid patients. In the multivariate analysis, the patients who experienced hypothyroidism in the postoperative period had a 3.6-fold greater risk of fistula [95% confidence interval (CI) 1.8-7.1; p = 0.0002]. The hypothyroid patients had an 11.4-fold greater risk for a required reoperation (24.4 vs 5.4%) than the euthyroid patients (95% CI 2.6-49.9; p = 0.001). The risk for fistula (p = 0.003) and reoperation (p = 0.001) increased with increasing TSH. This corresponds to an approximate 12.5% incremental increase in the absolute risk for fistula and a 10% increase in the absolute risk for reoperation with each doubling of the TSH. CONCLUSION:Postoperative hypothyroidism independently predicts postoperative wound-healing complications. The association of hypothyroidism with fistula formation may yield opportunities to modulate wound healing with thyroid supplementation or to provide a biomarker of wound progression.

Structured afterschool programs are often perceived as a service for young children only. Communities often overlook teenagers, expecting more substantial benefits from investments in programs for younger children (Hall & Gruber, 2007). Of about 8.4 million children participating in afterschool programs nationwide, only 1 million are high school students (Afterschool Alliance, 2009b). In addition, only 15 percent of the programs funded by the 21st Century Community Learning Centers (CCLC) program include high school students (Afterschool Alliance, n.d.). Recent budget cuts in many schools have reduced or eliminated high school extracurricular activities such as music and athletics, leaving some teenagers without safe, enriching activities after school (Hall & Gruber, 2007). A small body of research identifies characteristics of afterschool programs that enhance the academic and social development of high school youth. Given the relatively small number of afterschool programs that serve high school students, ensuring that the programs that do exist follow these promising practices is critical. If existing programs maximize the academic and social benefits of participation by following these practices, more investment in out-of-school time programming for high school youth may be possible. To determine the extent to which high school afterschool programs followed promising practice research, the authors studied 19 21st CCLC high school afterschool programs in one Midwestern state. They looked for research-based promising practices in three key areas identified in the literature: program activities, recruitment and retention, and student choice and voice. They found that evidence-based academic practices such as tutoring services and homework help or credit recovery opportunities were implemented more often than were practices related to student choice and voice. These findings have implications for practice in other afterschool programs serving high school youth.
