5. Frequency of Helicobacter pylori infection and antibiotic resistance in patients with acute liver decompensation

Introduction: Helicobacter pylori (H. pylori) infection is a global healthcare challenge. In its course of infection, Helicobacter pylori infection leads to a systemic increase of post-inflammatory cytokines and can have extra gastric manifestations, affecting the worsening of metabolic and cardiovascular diseases and endangering normal liver function, especially when liver cirrhosis is present. Aim: The aim of this study was to examine the frequency of H. pylori infection in patients with cirrhosis of the liver in stage AD or ACLF. The frequency of antibiotic resistance to clarithromycin and fluoroquinolones was also investigated. Material and methods: A prospective study with 45 patients was conducted which included both sexes. Samples of gastric mucosa were taken from all patients included in the study during the upper endoscopy and were sent to the Institute of Microbiology of the Faculty of Medicine University of Belgrade. A molecular method, Polymerase chain reaction (PCR) was used to detect H. pylori and mutations responsible for resistance to clarithromycin and fluoroquinolones. Results: The presence of H. pylori infection was registered in 15 decompensated patients (33.33%), while 30 (66.67%) were negative. In H. pylori positive patients, PCR detection of genotypes responsible for antibiotic resistance revealed clarithromycin resistance in 11 patients (73.33%), while fluoroquinolone resistance was detected in 7 patients (46.66%). In 6 patients (40.0%), an associated resistance to clarithromycin and fluoroquinolones was found. Conclusion: The H. pylori infection in cirrhotic patients with acute decompensation or ACLF has no significant effect on the severity of the clinical condition, elevated laboratory parameters and survival. Resistance rates to fluoroquinolones and clarithromycin or both antibiotics are high in decompensated patients with cirrhosis. There is a need to increase awareness of the rational use of antibiotics based on further investigations.

Timely and adequate treatment of Helicobacter pylori (H. pylori) infection is essential, bearing in mind its high prevalence rate (> 50%), the fact that infection with this pathogen causes chronic gastritis, as well as significant complications in the form of ulcer disease, MALT lymphoma, metaplasia and adenocarcinoma of the stomach. Numerous national recommendations and international guidelines dictate the effectiveness of therapy, but their inconsistent implementation disrupts the cure rate and increases the possibility of developing antibiotic resistance. According to preliminary data, the resistance rate in Serbia is high. The goal of therapeutic guidelines should be efficient therapy, which is safe and which, if possible, prevents the growing resistance to antibiotics. New diagnostic techniques such as bacterial culture and PCR (Polymerase chain reaction) can facilitate diagnosis and determine antibiotic resistance, which further leads to the possible application of a tailored made therapy that can enable a better cure rate by preventing further antibiotic resistance.

- Abstract
Helicobacter pylori eradication is characterized by decreasing successful eradication rates. Although treatment failure is primarily associated with resistance to antibiotics, other unknown factors may influence the eradication outcome. This study aimed to assess the presence of the antibiotics resistance genes in H. pylori and the presence of Candida spp., which are proposed to be endosymbiotic hosts of H. pylori, in gastric biopsies of H. pylori-positive patients while simultaneously assessing their relationship. The detection and identification of Candida yeasts and the detection of mutations specific for clarithromycin and fluoroquinolones were performed by using the real-time PCR (RT-PCR) method on DNA extracted from 110 gastric biopsy samples of H. pylori-positive participants. Resistance rate to clarithromycin and fluoroquinolone was 52% and 47%, respectively. Antibiotic resistance was associated with more eradication attempts (p < 0.05). Candida species were detected in nine (8.18%) patients. Candida presence was associated with older age (p < 0.05). A high rate of antibiotic resistance was observed, while Candida presence was scarce, suggesting that endosymbiosis between H. pylori and Candida may not be a major contributing factor to the eradication failure. However, the older age favored Candida gastric mucosa colonization, which could contribute to gastric pathologies and microbiome dysbiosis.

The measurement of inclination on gravity concrete dams using the instrument tiltmeter is described and discussed with special reference on obtained results on the dam 'Đerdap 2' acquired in the three years period. Tiltmetar way of operation is presented both through physical principle of vibrating wire sensors and through described design of the instrument. The influence of the temperature on the measurement of the slope is specially emphasized and presented through temperature correction. Processing the results of real inclination measurements on the dam 'Đerdap 2' with and without temperature correction showed the significant difference. Statistical analysis of measurement data consisted of performed regression analysis and forming of corresponding series with the expected measurement values depending on environmental conditions. At the end we give a summary conclusion on the instrument, the influence of temperature on the measurement and statistical model.

- Abstract
This paper provides insight into the Institute's long-standing engagement in the design and implementation of hydroinformatics systems in Serbia and the region. The introduction provides a definition of the hydroinformatics system and different areas of application, as well as an overview of the best practices in the world. An overview of the general platform that was created based on experience in the development of different systems has been presented here. We have described the functionalities integrated into a single software platform based on mathematical models and computational services. Various practical examples of application by the Institute are presented with the specifics of implementation in line with the purpose and characteristics of the studied systems. The conclusion highlights the role of applied hydroinformatics systems and the effects of application by users. Possible further development and implementation directions in water management and hydropower systems in Serbia and the region have been also presented.

The goal of this paper is to assess the effectiveness of using artificial neural networks in the prediction of concrete dam deformation. The aging of dams as a concrete structures poses a significant risk for the environment, and many of them are at the stage where it is necessary to pay attention to their behaviour. Short-term term prediction of deformation is very important for rapid response in the case of any adverse events. Two examples are presented in this paper to investigate how short-term prediction of deformation can be realized by using artificial neural networks. As a comparison, a set of statistical linear regression models are established using the same data. The conclusions on the basis of the analysis and the established models are presented. The paper provides an assessment of the network structure and a comparison of neural network models for the usual concept of statistical models for the monitoring of dams.

The measurement of inclination on gravity concrete dams using the instrument tiltmeter is described and discussed with special reference on obtained results on the dam 'Đerdap 2' acquired in the three years period. Tiltmetar way of operation is presented both through physical principle of vibrating wire sensors and through described design of the instrument. The influence of the temperature on the measurement of the slope is specially emphasized and presented through temperature correction. Processing the results of real inclination measurements on the dam 'Đerdap 2' with and without temperature correction showed the significant difference. Statistical analysis of measurement data consisted of performed regression analysis and forming of corresponding series with the expected measurement values depending on environmental conditions. At the end we give a summary conclusion on the instrument, the influence of temperature on the measurement and statistical model. U ovom radu prikazuje se način merenja nagiba kod gravitacionih betonskih brana primenom instrumenta tiltmetar, teorijski i preko opisa praktičnih merenja na brani 'Đerdap 2'. U prvom delu se objašnjava princip na kojem radi instrument tiltmetar i način na koji se vrši merenje nagiba primenom ovog instrumenta. Uticaj temperature na merenje nagiba je posebno razmatran kroz uvođenje temperaturske korekcije. U drugom delu se prikazuje obrada rezultata realnih merenja nagiba na jednom od instrumenata na brani 'Đerdap 2' sa i bez temperaturske korekcije. U trećem delu se prikazuje statistička obrada podataka merenja, u okviru koje se vrši regresiona analiza rezultata merenja i formiraju odgovarajuće serije sa očekivanim vrednostima merenja u zavisnosti od spoljašnjih uslova. 25. Erythropoietin in Predicting Prognosis in Patients with Acute-on-Chronic Liver Failure

- Abstract
Background & Aims: Acute-on-chronic liver failure (ACLF) is characterized by a rapid progression to multiple organ failure and is associated with a very high mortality rate of 50-90%. Novel therapies are being investigated such as Erythropoietin (EPO). The aim of this prospective cohort study was to analyse the value of EPO in predicting prognosis and determine which patients may benefit most from EPO therapy. Methods: According to the EASL-CLIF criteria, 104 consecutive patients were diagnosed with ACLF, and separated into two groups based on the type of insult: bleeding (Group A=31) or non-bleeding (Group B=73). In addition to a complete biochemical work-up and calculation of relevant prognostic scores, levels of EPO were measured on admission and correlated to the type of insult and final outcome. Results: Fifteen patients from Group A (mean age 60.32 +/- 9.29 years) had a lethal outcome and higher values of EPO on admission (319.26 +/- 326.58 mIU/ml) (p lt 0.005), compared to the 37 patients from Group B (mean age 59.9 +/- 10.19 years) with EPO levels at admission of 29.88 +/- 34.6 mIU/mL. In Group B, a cut-off EPO value of 30.65 mIU/mL had a sensitivity of 87.5% and a specificity 57.4% in predicting lethal outcome with an AUROC of 0.823. In Group A, a cut-off value of 229.95 mlU/mL had a sensitivity and specificity of 53.3% and 92.7%, respectively. The AUROC for this cut-off was 0.847. Conclusions: Erythropoietin is superior to the standard prognostic scores in predicting 28-day mortality. Lower levels of EPO were detected in patients without bleeding as an insult indicating a possible therapeutic benefit in these patients.

- Abstract
This paper discusses harnessing Internet of Things in monitoring and managing dams in Republic of Serbia. Large dams are of major importance, primarily because of their use for electricity, but risks which are associated with it should be greatly taken into account. There is a need to consolidate information related to dam facilities in order to use them for dam management in the Republic of Serbia. An information system has been developed based on the existing systems, allowing utilization of intelligent network sensors. The aim of the paper is to describe possibilities of the Internet of Things application within a specific system for dam safety management. In order to facilitate the inclusion of a large number of intelligent sensors, a new data acquisition module for communication with sensors in the monitoring network is defined. The system should provide on time alerting in case security parameters deviate from the expected values.

This paper shows the structure of the SWAT-based model used in modeling of the 'rainfallrunoff' process. The SWAT model is hydro-dynamic and physically-based model for application in complex and large basins. Model inputs are as follows: rainfall, air temperature, soil characteristics, topography, vegetation, hydrogeology and other relevant physical parameters. The model is based on five linear reservoirs as follows: reservoir of the vegetation cover, snow accumulation and melting, surface reservoir, underground reservoir and surface runoff reservoir. The model uses GIS tools for preprocessing and post-processing. The basic modeling unit is the hydrologic response unit (HRU), defined as the network of elementary hydrologic areas with the selected discretization, measure of which is dependent upon the desired accuracy, as well as upon data accuracy. The total runoff on the exit profile of the catchment is computed by convolution of the sum of runoffs (surface and base runoffs). The model can be applied at the daily and hourly level of discretization and used for multiannual simulations. Illustration of operation of the SWAT based model will be presented on a selected part of the River Drina basin (with the total area of around 20.000 km2). .

In this paper the authors discuss the Digital Elevation Model (DEM) based GIS algorithms, applicable for automatic creation of input data needed by hydrological models. The expected source of DEM data are standard digital data sets that a modeler can purchase from the official data sources. The DEM has to be cleared from errors and prepared for standard hydrology usage as the depression less version, or a version of DEM where only small depressions are filled-in, while larger depressions are extracted as separate objects. The slopes and aspects are calculated from such DEM. The flow accumulation image is created using slopes and aspects and using information about large ponds. From flow accumulation image the surface drainage network is extracted with user-specified density. Based on slopes and aspects, the catchment is delineated into subcatchments, the areas drained by each river segment. Finally, the model-specific parameters like length of flow segment along the terrain and along the river to the subcatchment outlet, slope of those segments, subcatchment shape factor, or subcatchments mean or weighted slope are calculated. Paper explains the used algorithms and emphasizes the problems one can encounter during the usage of DEM data using examples from River Drina catchment. Finally, the authors comment on overall usability of presented GIS algorithms, especially if purchased data are of low quality. .
