A hipomineralização molar-incisivo (HMI) é um defeito qualitativo do esmalte dentário, que afeta um ou mais primeiros molares permanentes e suas repercussões clínicas podem ter impacto na saúde geral e qualidade de vida do indivíduo. Com base na hipótese de que crianças que possuem HMI possivelmente apresentem alterações nas atividades posturais e mastigatórias, na porcentagem da máxima força dos contatos oclusais, na força de mordida molar máxima, na pressão máxima dos músculos bucinadores, da língua e dos lábios, o presente estudo avaliou a funcionalidade do sistema estomatognático nos quesitos acima mencionados. Foram selecionadas 72 crianças, entre 6 a 12 anos de idade, distribuídas em dois grupos: com HMI (GHMI n=36) e sem HMI (GC n=36). Esse foi um estudo observacional transversal comparativo que avaliou crianças com HMI e sem HMI. O Eletromiógrafo Delsys Trigno foi utilizado na atividade eletromiográfica dos músculos masseter e temporal (direitos e esquerdos) nas seguintes condições posturais da mandíbula: repouso, protrusão e lateralidades (direita e esquerda), bem como a eficiência dos ciclos mastigatórios na mastigação habitual; a máxima força dos contatos oclusais foi mensurada com o aparelho T-SCAN®; a força de mordida molar máxima direita e esquerda com o gnatodinamômetro digital; e a pressão máxima dos músculos bucinadores, da língua e dos lábios com o IOPI. A análise estatística foi realizada pelo teste t de Student; p≤0,05. Os resultados permitem observar no GHMI hiperatividade muscular nas condições posturais e mastigatórias em comparação ao GC. Houve significância com maiores valores eletromiográficos para o GHMI quando comparados ao GC nas seguintes condições posturais: repouso para os músculos temporal direito (p=0,00) e temporal esquerdo (p=0,03); protrusão para o músculo temporal direito (p=0,02); lateralidade direita para os músculos masseter direito (p=0,00) e temporal esquerdo (p=0,01); lateralidade esquerda para os músculos masseter direito (p=0,03) e temporal esquerdo (p=0,04). Nas condições mastigatórias com alimento consistente, observou-se significância para o temporal esquerdo (p=0,01) e, com alimento macio, para os músculos temporal direito (p=0,01) e temporal esquerdo (p=0,00). Já nas relações de máxima força dos contatos oclusais, houve significância no dente 46 (p=0,01), e no dente 16 houve uma diminuição dos valores do GHMI em relação ao GC. No lado direito foram observados menores valores na máxima força dos contatos oclusais para o GHMI quando comparado ao GC. Na força de mordida molar máxima, notou-se menor força dos lados direito e esquerdo para o GHMI em relação ao GC. Na pressão dos músculos bucinadores, da língua e dos lábios, apresentaram-se forças diminuídas no GHMI quando comparadas ao GC. Assim, conclui-se que as crianças com HMI apresentaram comprometimento da funcionalidade do sistema estomatognático. Molar-incisor hypomineralization (MIH) is a qualitative dental enamel defect which affects one or more of the first permanent molars and its clinical repercussions may impact on the individual\'s general health and quality of life. Based on the hypothesis that children who have MIH may present with alterations in postural and masticatory activities, in the maximum force percentage of occlusal contacts, in the maximum molar bite force, in the maximum pressure of the buccinator muscle, tongue and lips, the present study evaluated the functionality of the stomatognathic system related to the previously mentioned aspects. We assigned 72 children, aged between 6 and 12 years, and separated them into two groups: one group with MIH (GMIH n=36) and the other one without MIH (GC n=36). It was a comparative cross-sectional observational study which evaluated children with and without MIH. The Delsys Trigno Electromyograph was used to detect electromyographic activity of the masseter and temporalis muscles (right and left) in the following postural conditions of the mandible: rest, protrusion and laterality (left and right), as well as the efficiency of masticatory cycles in habitual mastication; the maximum force of occlusal contacts was measured through the T-SCAN® device; the maximum right and left molar bite force through the digital gnathodynamometer device, and the maximum pressure of the buccinator muscle, tongue and lips using the IOPI device. The statistical analysis was performed using the Student\'s t-test; p≤0.05. The results show muscle hyperactivity in postural and masticatory conditions in GMIH compared to GC. There was significance with higher electromyographic values for GMIH when compared to GC in the following postural conditions: in the rest position for the right (p=0.00) and left temporalis muscles (p=0.03); protrusion for the right temporalis muscle (p=0.02); right laterality for the right masseter muscle (p=0.00) and left temporalis muscle (p=0.01); left laterality for the right masseter muscle (p=0.03) and left temporalis muscle (p=0.04). In masticatory conditions with consistent food, significance was observed for the left temporalis muscle (p=0.01) and with soft food for the right (p=0.01) and left (p=0.00) temporalis muscles. Concerning the maximum force relations of the occlusal contacts, there was significance in tooth 46 (p=0.01) and a decrease in tooth 16 in the GMIH values compared to the ones in GC. The right side presented with decreased values in the maximum force of occlusal contacts for GMIH compared to GC. Regarding the maximum molar bite force, lower force was observed on the right and left sides for GMIH compared to GC. About the pressure of the buccinator muscle, the tongue and the lips, the forces were reduced in GMIH compared to GC. Thus, it is concluded that children with MIH had impaired functionality of the stomatognathic system.
