Background: Health professional education has been criticized for not integrating patient expertise into professional curricula to develop professional skills in patient empowerment. Objective: To develop and translate a new expert patient-centered model for teaching empowerment into professional education about routine chronic care management. Methods: Eight Finnish patients (known as expert patients), 31 students, and 11 lecturers from 4 European countries participated in a new pilot intensive educational module. Thirteen focus groups, artefacts, and an online student evaluation were analyzed using a thematic analysis and triangulated using a meta-matrix. Results: A patient-centered pedagogical model is presented, which describes 3 phases of empowerment: (1) preliminary work, (2) the elements of empowerment, and (3) the expected outcomes. These 3 phases were bound by 2 cross-cutting themes “time” and “enabling resources.” Conclusion: Patient expertise was embedded into the new module curriculum. Using an example of care planning, and Pentland and Feldman’s theory of routine organization, the results are translated into a patient-centered educational model for teaching empowerment to health profession students.

This contributed volume is based on the "European Core Competences Framework for health and social care professionals working with older people" (ECCF), developed and verified in a unique international cooperation between 26 universities and universities of applied sciences in 25 European countries, part of the European Later Life Active Network (ELLAN). In addition to the framework, the book outlines the necessary qualifications and describes the roles of professionals working with older people in health and social services. It explores healthy ageing for older people from different perspectives and describes the seven roles of health and social care professionals (Expert, Communicator, Collaborator, Organizer, Health and Welfare Advocate, Scholar, and Professional), before going on to define 18 related competences and elaborating them in performance indicators. Beyond the ECCF, the book explains the widely used CanMED role model and puts forward theories to support a client centered and integrated approach on health and social care in order to change attitudes toward older clients and offer better care and support. It also provides health and social care professionals, for example nurses, allied health professionals and social workers with new contextual information and cultural awareness. It gives a voice to students by addressing selected perspectives for professional development. The book includes questions for reflective learning helping to make the book a vital practical instrument for use in the educational context throughout Europe. Europe’s ageing populations represent a major challenge for both public health and social care systems. 18% of the population is 65 years old and over, and this proportion will increase in the coming years. As a result, more and more health and social care professionals will work with older people in different settings – at home, in the community, in hospitals or in long-term care settings. Older people, and especially the frail, face a host of interrelated issues, e.g. cognitive restrictions, functional restrictions, psychosocial problems, multimorbidity, polypharmacy and social isolation. These problems call for an integrated approach to health and social care, which this book supplies. It is intended for health and social care professionals, students and educators, for a better understanding of Europe’s ageing society and of the impact on care and services. Furthermore, the ECCF offers educational institutes a unique resource for curriculum development, education, training and assessment.

As a Scholar, the health and social care professionals pursue excellence by continually evaluating the processes and outcomes of their daily work, comparing their work with that of others, and by actively seeking feedback to improve the quality of care and support they provide for the older person and their family. The main competence part of the role of the Scholar is described in this chapter. The first competence is about growing one’s expertise by different ways of learning and disseminating this expertise. The second competence of this role concerns evidence-based practice and the innovation of care and services.

Over the last two decades, much work has been done for developing person-centred approaches in social and health services in order to deliver better treatment and care for older people. Communication is pivotal in person-centred care and services. Communication skills are necessary to understand the needs and wishes of the older people and their families and be able to decide for the best person-centred care. Competences of the communicator role are maintaining relationships and effective communication, empowerment and coaching.

Pro gradu -tutkielmani aiheena on Tampereen evankelisluterilaisen seurakuntayhtymän yhteiskunnallisen työn sinkkutoiminnan merkitys yksinelävien selviytymisensä. Vuosi 2010 oli Euroopan köyhyyden ja sosiaalisen syrjäytymisen torjunnan teemavuosi, jolloin muun muassa Terveyden ja hyvinvoinnin laitoksella kiinnitettiin huomiota yksineläjien ja -asujien tilanteeseen yhteis­kunnassamme. Yksin asuvien suureen ja alati kasvavaan määrään kiinnitetään kuitenkin julkisessa puheessa tuskin ollenkaan huomiota. Samoin pienituloisten yksin asuvien toimeentulon ongelmia tulisi tutkia enemmän ja pyrkiä löytämään niihin ratkaisuja. Yksinelävät nähdään usein vain asunnottomuuden ja päihdeongelman kautta, vaikka yksineläjiä on paljon ja vaikka useimmat heistä ovat työikäisiä. Tässä tutkimuksessani kohteenani on sekä työelämässä että eläkkeellä olevat yksinelävät. Tutkimukseni tarkoituksena on saada tietoa sinkkutoimintaan osallistuneiden naisten ja miesten elämäntilanteesta, heidän kokemuksistaan, sinkkutoimintaan osallistumisen merkityksestä ja kokonaisvaltaisesta selviytymisestä. Kokonaisvaltaiseen selviytymiseen kuuluu sosiaalinen ja taloudellinen selviytyminen, koska huono taloudellinen tilanne on vaikutuksiltaan myös sosiaalista. Henkilön fyysinen ja psyykkinen selviytyminen voi heijastua myös sosiaaliseen osallistumiseen, koska vakava sairaus tai masennus yksinäisyyden vuoksi voi muodostua esteeksi lähteä kotoa tutustumaan ihmisiin ja jonkin yhteisön toimintaan. Yksineläjät ovat myös ryhmä, jonka toimeentulo on erityisen herkkä työelämästä ulkopuolelle jäämisen vaikutuksille. Yksinelävällä ei välttämättä ole mitään turvaverkkoa työttömyyden tai sairauden kohdatessa ja taloudellisesti huonossa tilanteessa yksinelävä voi joutua siirtämään esimerkiksi sairauden hoitamiseen tarvittavien lääkkeiden ostamista. Tutkimukseni teoreettinen viitekehys muodostuu aikaisemmista yksinäisyyttä, köyhyyttä, yksinelämistä, vertaisryhmätoimintaa ja sinkkutoimintaa koskevista tutkimuksista ja kirjallisuudesta. Yksineläjistä on maassamme kuitenkin tehty melko vähän tutkimuksia, joten tutkimukseni on siten poikkeuksellinen. Kyseessä on laadullinen tapaustutkimus, jonka aineisto muodostuu haastateltavilleni tekemistä tiettyihin teemaan liittyvistä kysymyksistä. Empiirinen osa koostuu neljästä Tampereen evanke­lis­luterilaisen seurakuntayhtymän sinkkutoimintaan osallistuneen naisen ja kahden miehen haastatteluista, jotka kokosin kevätlukukaudella 2009. Olen analysoinut aineiston temaattisen analyysin avulla ja keskiössä on ollut aineistolähtöisyys. Olen halunnut liittää tutkimustuloksiin runsaasti haastateltavien suoria sitaatteja, jotta yksinelävien ääni tulisi tutkimukseni kautta hyvin kuuluville.
