30. Allergy development and the intestinal microflora during the first year of life

Abstract

Background: The intestinal microflora is a likely source for the induction of immune deviation in infancy. Objective: The purpose of this study was to prospectively relate the intestinal microflora to allergy development in 2 countries differing with respect to the prevalence of atopic diseases. Methods: Newborn infants were followed prospectively through the first 2 years of life in Estonia (n = 24) and Sweden (n = 20). By that age, 9 Estonian and 9 Swedish infants had developed atopic dermatitis and/or positive skin prick test results. Stool samples were obtained at 5 to 6 days and at 1, 3, 6, and 12 months, and 13 groups of aerobic and anaerobic microorganisms were cultivated through use of standard methods. Results: In comparison with healthy infants, babies who developed allergy were less often colonized with enterococci during the first month of life (72 % vs 96 %, P < .05) and with bifidobacteria during the first year of life (17 % to 39 % vs 42 % to 69 %, P < .05). Furthermore, allergic infants had higher counts of clostridia at 3 months (median value, 10.3 vs 7.2 log(10), P < .05). The prevalence of colonization with Staphylococcus aureus was also higher at 6 months (61 % vs 23 %, P < .05), whereas the counts of Bacteroides were lower at 12 months (9.9 vs 10.6 log(10), P < .05). Conclusion: Differences in the composition of the gut flora between infants who will and infants who will not develop allergy are demonstrable before the development of any clinical manifestations of atopy. Because the observations were made in 2 countries with different standards of living, we believe that our findings could indicate a role for the intestinal microflora in the development of and protection from allergy.

43. Microbial Ecology of Helicobacter pylori in Antral Gastritis and Peptic Ulcer Disease

Abstract

We compared the degree of mucosal colonisation by aerobic and anaerobic bacteria with special attention to Helicobacter pylori in the antrum of the stomach of 19 non-ulceral dyspepsia (NUD), 18 duodenal ulcer (DU) and seven gastric ulcer (GU) patients. In the three cases of normal mucosa bacteria were present in quantities below 103 c.f.u./g and no H.pylori were detected. H.pylori was isolated from the antral mucosa of all patients with DU and GU and of 12 (63 per cent) NUD patients. A high colonisation rate (> 105 c.f.u./g) was found in 25 cases (56 per cent). In NUD patients with superficial gastritis the high degree of colonisation was due to H.pylori, but in the cases of GU and DU with the same state of the mucosa high numbers of H, pylori occurred in association with other microbes (P < 0.02). In the eight cases of slight atrophic gastritis (NUD, GU and DU) the population level of H, pylori was low accompanied by high numbers of other microorganisms. The aetiological role of H.pylori in the development of gastritis is discussed.

44. Effect of Administration of Lactobacillus casei Strain GG on the Gastrointestinal Microbiota of Newborns

Abstract

The aim of the study was to determine whether Lactobacillus casei strain GG could colonise the intestine of newborns and the influence of its administration on establishment of the microbiota. The faecal bacterial population of 25 under 1 mth old newborns was studied: in addition to breastfeeding, 15 babies (GG group) received for 2 wk immediately after birth Lactobacillus GG supplement as freeze-dried powder diluted in water in a dose of 1010-1011 c.f.u./g; 10 newborns (control group) did not receive any supplement to breastfeeding. The faecal bacterial composition of meconium was similar in both groups studied. Ten newborns (67 per cent) excreted Lactobacillus GG, while in eight cases (53-3 per cent) Lactobacillus GG was found even 2 wk after the administration was stopped. The faecal concentrations and the relative proportion of Lactobacillus GG were individually different. In 3-4 d, 5-7 d and 1 mth old newborns of the GG group the faecal concentrations of lactobacilli exceeded those of the control group. The faecal microorganisms predominance pattern did not differ in the case of 1 wk and 1 mth old newborns of the GG group. The study shows that 2 wk administration of Lactobacillus GG, which starts right after birth, increases intestinal lactobacilli concentrations and does not impair the establishment of a normal faecal bacterial microbiota.
