Abstract

The development of FVIII/FIX inhibitor alloantibodies represents a severe complication requiring specific laboratory evaluation for establishing a life-saving therapy regimen. Our preliminary study aimed at elaborating a laboratory strategy for monitoring the effectiveness of Activated Prothrombin Complex Concentrate (APCC) and recombinant activated factor VII (rFVIIa) in haemophiliacs with inhibitors, by checking the reliability and clinical value of three complementary assays: clotting-time based coagulometry, thrombelastography (TEG) and thrombin generation assay (TGA). The investigations were performed on 7 patients with severe haemophilia A with high titer inhibitors treated for 12 episodes of severe bleedings, 5 of them for surgical interventions. Abstract

Aim. In COVID-19 Pandemic, a new hyperinflammatory syndrome was reported with clinical features of Kawasaki disease, named PIMS-TS. We want to present a single center experience where the patients were diagnosed with Kawasaki-like in PIMS-TS with cardiac affliction. Material and method: The study was observational and retrospective, enrolled 14 patients fulfilling the criteria of PIMS-TS with the median age of 9 (IQR, 1.6-11), 9 male (64.2 %) and 5 female (35.8 %). Results: ECG revealed tachycardia and ST-T changes in 60% of patients. In evolution, ECG modified in 20% and consisted of long QT in 7% of cases, bradycardia in 7%, 3% transitory sick sinus syndrome and 3% grade I/II degree atrio-ventricular block. Cardiac disfunction was evidenced in 4 patients (28%), with reduced ejection fraction under 50%, mitral insufficiency in 6 (42.8%), pericardial fluid in 8 (57.1%) and perivascular brightness in 8 (57.1%). The cardiac biomarkers: NT-proBNP (increased in 9), cTroponin T (increased in 7) and cTroponin I (increased in 5) confirmed heart dysfunction. During the hospitalization and under medical treatment, all the modifications recover. Evolution was good for 12 children. Conclusions: Cardiac dysfunction and myocardial injury were confirmed by elevated cardiac biomarkers. Rapid recognition allows prompt treatment for a good outcome. NT-proBNP, cTroponin T and I are of capital significance in monitoring the myocardial injury, the treatment and evolution of these patients. [ABSTRACT FROM AUTHOR]

11. P375 Accidental poisoning in paediatric patients – latest folow-up

Abstract

Background and aimsAcute accidental poisoning represent o major cause of morbidity and also sometimes mortality among young children and teenagers. In preschool age, poisoning seems to be mainly accidental. The child needs to explore, but also has a specific curiosity which leads to accidental ingestion on unattended pills or different household products. Parasuicidal behaviour or suicidal attempts remain prerogative for teenage period, especially in disorganised families or in case of psycho-affective inappropriate environment for child development.This study aims to alarm for the constantly increasing number of cases of acute accidental poisoning and to identify risk factors.MethodsThe present paper represents a retrospective study of 225 paediatric patients admitted in Emergency Children Hospital ‘Louis Turcanu’ Timisoara between 2013 and 2015 for acute accidental poisoning. We used Microsoft Excel for analysing data.ResultsOf a total of 406 patients admitted on Toxicology Department, more than half (225) were accidental events and mainly non-drug poisonings (67,5%). Accidental/Voluntary poisoning ratio was 1,24:1 with a slightly predominance in boys for accidental poisoning with male : female ratio is 1,27:1. The majority of accidental poisoning cases (58%, 130 cases) occurred in small childhood (under 3 years of age) due to the lack of child surveillance or/and inappropriate use or storage of household products, fuel or unsupervised medication. Carbon monoxide poisoning shows homogenous distribution by age group, while nitrite poisoning is typical for toddler. Analysing environmental origin we showed a mild frequency in rural versus urban environment.ConclusionsAccidental poisonings represent a frequent reason for hospital presentation and/or admission. Risk factors include inappropriate environment, young parents, disorganised families, insufficient child surveillance for small age, curiosity, ADHD behaviour. We support ‘Keep away from child’ sign on each harmful product or medication and awakening parents of accidental harmful poisoning in a carelessness moment.

Abstract

Background and aimsCerebral sinovenous thrombosis (CSVT) are increasingly recognised in children but still underdiagnosed. CSVT in children may be fatal and frequently associated with adverse outcomes.MethodsSeven children diagnosed with CSVT between March 2012 and November 2016 were treated in the Haematology Oncology Department of the Emergency Clinical Hospital for Children ‘Louis Ţurcanu’ Timişoara (current physician Dr.Carmen Petrescu). The cases were carefully investigated: clinical manifestations, neuroimaging aspects, congenital and acquired risk factors, treatment, complications, and outcome.ResultsThe initial manifestations of CSVT in infants and small children (5/7), all with severe iron deficiency anaemia, preceding respiratory or digestive infections and severe dehydration, were nonspecific (vomiting, lack of appetite, lethargy), followed by seizures and coma (3/7); the older children (2/7) with chronic adeno-tonsillitis, had ocular disturbances, and respectively vomiting, headache and vertigo as onset symptoms. CSVT was diagnosed after cerebral CT scan/MRI, one to 14 days after the onset; the extension of thrombosis and parenchymal complications (venous/haemorrhagic infarctions) were correlated with the clinical manifestations and complications and presented for each case. Genetic predisposition for thrombosis was found in all cases: positive family history (4/7), and positive genetic profile (7/7). The anticoagulant treatment with unfractionated heparin was unsatisfactory (administration, monitoring, maintaining APTT in the therapeutic range, haemorrhagic complications), while treatment with low molecular weight heparins (enoxaparin, dalteparin) was efficient and safe. Two small children had long-term complications (e.g., hydrocephalus, cerebral atrophy; symptomatic epilepsy, hemiparesis).ConclusionsAlthough CSVT are rare in children, their potentially fatal/severe outcome, especially in small children, justifies increased paediatricians attention; an early diagnosis based on a high index of suspicion (especially tacking into account the nonspecific initial manifestations) and adequate neuroimaging techniques, correction of age-specific acquired risk factors (e.g., severe iron deficiency anaemia, dehydration), and efficient and safe anticoagulant treatment could reduce immediate complications and long-term sequelae.
