Abstract

BACKGROUND There were gaps between guidelines and practice when surgery was the only treatment for aortic stenosis (AS). OBJECTIVES This study analyzed the decision to intervene in patients with severe AS in the EORP VHD (EURObservational Research Programme Valvular Heart Disease) II survey. METHODS Among 2,152 patients with severe AS, 1,271 patients with high-gradient AS who were symptomatic fulfilled a Class I recommendation for intervention according to the 2012 European Society of Cardiology guidelines; the primary end point was the decision for intervention. RESULTS A decision not to intervene was taken in 262 patients (20.6%). In multivariate analysis, the decision not to intervene was associated with older age (odds ratio [OR]: 1.34 per 10-year increase; 95% CI: 1.11 to 1.61; P = 0.002), New York Heart Association functional classes I and II versus III (OR: 1.63; 95% CI: 1.16 to 2.30; P = 0.005), higher age adjusted Charlson comorbidity index (OR: 1.09 per 1-point increase; 95% CI: 1.01 to 1.17; P = 0.03), and a lower transaortic mean gradient (OR: 0.81 per 10-mm Hg decrease; 95% CI: 0.71 to 0.92; P < 0.001). During the study period, 346 patients (40.2%, median age 84 years, median EuroSCORE II [European System for Cardiac Operative Risk Evaluation II] 3.1%) underwent transcatheter intervention and 515 (59.8%, median age 69 years, median EuroSCORE II 1.5%) underwent surgery. A decision not to intervene versus intervention was associated with lower 6-month survival (87.4%; 95% CI: 82.0 to 91.3 vs 94.6%; 95% CI: 92.8 to 95.9; P < 0.001). CONCLUSIONS A decision not to intervene was taken in 1 in 5 patients with severe symptomatic AS despite a Class I recommendation for intervention and the decision was particularly associated with older age and combined comorbidities. Abstract

Transthoracic echocardiographic examination is known to be a safe, non-invasive and reproducible method, used in every day clinical practice to obtain important information about cardiac structure and function. Unfortunately, a significant proportion of studies have highlighted the considerable technically difficultly in producing diagnostic images due to a poor acoustic window and more than 33% of patients undergoing stress echocardiography have suboptimal echocardiographic images. All these limitations have led to the use of contrast agents to improve the quality of standard ultrasound examination to provide a better delineation of left ventricle endocardial borders or to obtain information that cannot be achieved by using standard echocardiography, such as assessing myocardial microcirculation and therefore perfusion. This paper sought to review the clinical efficacy and safety of ultrasound contrast agents focusing on stress echocardiography.

Abstract

Basal cell carcinoma represents the most common skin and epithelial cancer. Most of the patients are cured by surgery. However, some cases display fullblown aggressiveness which has a dual connotation. The tumour may reach an impressive size being locally destructive while in rare cases, basal cell carcinomas may metastasize. If this agressivetumour is located on the face or neck, the surgeon is confronted with a clinical dilemma since total removal of the lesion with clear margins is impossible. Therefore, nonsurgical approaches need to be adopted in these cases. A better understand of the molecular pathways could theoretically lead to new improved therapeutic treatments. The current paper presents an update on the molecular factors with clinical importance for the treatment of basal cell carcinoma in parallel with presentation of an aggressive case as well as a review of the current therapeutic methods.

Abstract

To present a hypothesis regarding the pathways of angiogenesis in primary versus secondary hip osteoarthritis (OA).In synovial tissue samples provided by 57 consecutive patients who underwent hip arthroplasty, immunohistochemical examinations were performed using the following angiogenesis-related antibodies: VEGF-A, COX-2, maspin and the endothelial cells markers CD31 and CD105. The cases were divided into three categories: classic primary hip OA (group A; n = 16), rapidly destructive hip OA (group B; n = 24) and hip OA secondary to avascular osteonecrosis of the femoral head (group C; n = 17). The endothelial area (EA) was digitally quantified for both CD31 and CD105.The large mature vessels with CD105-positive activated endothelium predominated in group C, which also showed the highest CD105 median EA value (7.31 ± 4.01, compared to 4.76 ± 3.73 for group A and 6.69 ± 3.53 for group B). In groups A and B, synovial cell hyperplasia and the predominance of small immature vessels were characteristic. CD105, VEGF-A and COX-2 were focally seen in the synovial membrane, without maspin positivity.The severity of hip OA can be related to angiogenesis pathways that are not maspin-mediated. In primary hip OA, angiogenesis may be induced by a combined mechanism: hypoxia-related VEGF-dependent vasculogenesis and endothelial differentiation of the activated pluripotent cells, which are released from the hyperplastic synovial cells layer. An endothelial mesenchymal transition is assumed to be involved in the fibrotic process.

6. Constrictive pericarditis - a difficult diagnosis

Abstract

Constrictive pericarditis is an uncommon condition, often of unknown etiology. The diagnosis of constrictive pericarditis can be difficult and is often delayed, because the signs and symptoms of this disease can be falsely attributed to other causes. We report the case of a 62-year-old woman presented with a one year history of progressively worsening dyspnea, peripheral oedema, prominent jugular distension, hepatomegaly, ascites. The patient is known with a history of effusive pericarditis. Blood test showed a normal white cell count, anaemia, raised CRP. Chest X-Ray shows a normal sized heart and without calcification of the pericardium. The echocardiographic exam showed septal bounce-abrupt transient rightward movement, left, right ventricular size decreased-heart tubularin shape, mild atrial enlargement, IVC plethoric and unresponsive to respiration, hepatic veins dilated. Doppler echocardiographic findings were consistent with constrictive pericarditis. Cardiac catheterization showed elevation and equalization of diastolic filling pressures, and dips and plateau configuration of ventricular pressure during diastole (square root sign). Based on these results pericardiectomy was necessary. Constrictive pericarditis was also confirmed at the time of surgery. The pericardium was found with thickening of up to 30 mm in some areas. Histopathological exam showed fibrosis and calcification within the pericardium with no evidence of malignancy or tuberculosis. The patient was discharged 10 days later. At 3 months she had no significant symptoms. Diagnosis of constrictive pericarditis remains challenging. Constrictive pericarditis should be suspected in patients with clinical features of right-sides heart failure. Echocardiography and cardiac catheterisation are important investigation in diagnosis of constrictive pericarditis and avoiding unnecessary treatments.

A fatal complication of an incarcerated diaphragmatic hernia: pyo-pneumopericardium due to a gastro-pericardial fistula, case report and literature reviewThe pyo-pneumopericardium is a very rare complication of an incarcerated diaphragmatic hernia. Usually, it depends on the existence of a gastropericardial fistula and, in most cases, it is a deadly complication.We present the case of a 65 year old man who was admitted to our emergency service with clinical signs of septic shock. With the help of the CT scan a pyo-pneumopericardium and incarcerated diaphragmatic hernia was diagnosed. He was subjected to emergency surgical intervention with double approach (thoracic and abdominal), discerning during surgery the existence of a pericardial abscess due to a gastro-pericardial fistula due to an incarcerated diaphragmatic hernia. The gastric fundus was resected with abolition of the gastro-pericardial fistula, evacuation of the pericardial abscess, performing the splenectomy (in order to have a better surgical access) and adequate treatment of the diaphragmatic hernia. Despite the medical efforts, the patient died on the 10th postoperative day due to septic complications. To our knowledge this is the first case reported in the literature of gastro-pericardial fistula complicated with pyo-pneumo-pericardium due to an incarcerated diaphragmatic hernia. In such cases we consider that only by performing a double surgical approach (thoracic and abdominal) we can provide a proper resolution of the case, also based on the objective situation found during surgery, where the splenectomy can provide an easier approach to the diaphragmatic hernia.Il pio – pneumopericardio è una complicanza particolarmente rara di un’ernia diaframmatica incarcerata. Di solito la causa del pio – pneumopericardio è rappresentata dall’esistenza di una fistola gastro – pericardica e nella maggior parte dei casi, è una complicanza mortale.Presentiamo il caso di un uomo di età 65 anni il quale è stato ricoverato nel nostro servizio di urgenza con segni clinici di shock settico. In seguito all’esame di tomografia computerizzata è stata messa la diagnosi di pio – pneumopericardio ed ernia diaframmatica incarcerata. Si è intervenuto chirurgicamente d’urgenza con doppio approccio (toracico e addominale), in via intraoperatoria, evidenziandosi l’esistenza di un ascesso pericardico dovuto a una fistola gastro - pericardica formata a seguito di un’ernia diaframmatica incarcerata. Si è praticata la resezione del fondo gastrico con la rimozione della fistola gastro – pericardica, l’evacuazione dell’ascesso pericardico, splenectomia (per avere un accesso chirurgico più facile) e il trattamento adeguato dell’ernia diaframmatica. Nonostante gli sforzi medici, il paziente è deceduto nel giorno 10° post – operazione a causa delle complicanze settiche. Dalle nostre conoscenze, questo è il primo caso presentato nella letteratura di fistola gastroperocardica complicatacon pio – pneumopericardio a causa di un’ernia diaframmatica incarcerata. In questi casi consideriamo che solo praticando un approccio chirurgico doppio (toracico e addominale) è possibile garantire una risoluzione corretta del caso; inoltre, a seconda della realtà intraoperatoria, l’effettuazione della splenectomia può garantire un approccio più adatto al livello dell’ernia diaframmatica.

Abstract

Introducere. Cu o incidenţă estimată la nivel mondial de 6 în 1.000 de nou-născuţi, malformaţiile cardiace congenitale (MCC) sunt una dintre principalele cauze ale mortalităţii în primul an de viaţă. Principalele probleme în managementul malformaţiilor cardiace congenitale (MCC), în România, sunt lipsa de date care să descrie incidenţa reală la nivel populaţional, lipsa de standardizare în diagnosticul şi raportarea MCC la nivel naţional. În România, există doar încercări locale de a dezvolta un registru de malformaţii cardiace. Obiective. Măsurarea şi analiza incidenţei MCC la copii în Centrul Pediatric Cardiovascular al Institutului de Urgenţă pentru Boli Cardiovasculare şi Transplant Tg. Mureş (IUBCvT) în vederea determinării ratei de înrolare, stabilirea formularului de înregistrare şi urmărire al Registrului Clinic de Monitorizare al MCC. Material şi metodă. A fost realizat un studiu descriptiv retrospectiv al copiilor externaţi cu un diagnostic principal sau comorbiditate din categoria Q20-Q28 (Malformaţii cardiace congenitale – ICD 10 AM), în perioada 2008- 2013, la nivelul Centrului Pediatric Cardiovascular al Institutului de Urgenţă pentru Boli Cardiovasculare şi Transplant Târgu-Mureş. Lotul de studiu a cuprins un număr de 839 de nou-născuţi externaţi din secţia Neonatologie a Spitalului Clinic Judeţean de Urgenţă Târgu-Mureş (SCJU), respectiv 4.423 de copii externaţi din IUBCvT. Rezultate. Incidenţa MCC la nivelul nou-născuţilor externaţi din SCUJ a variat între 5,23% şi 11,47%. Din cei 839 nou-născuţi, 139 au fost operaţi în cadrul IUBCvT în primele 30 zile de viaţă. Cele mai frecvente malformaţii sunt Comunicaţie interauriculară (46,52%), Permeabilitatea canalului arterial (22,10%), Comunicaţia inter ventriculară (9,04%). Morbiditatea spitalizată pe grupe de boli la nivelul IUBCvT demonstrează ponderea ridicată a Comunicaţiei interventriculare (20,05%), a Comunicaţiei interauriculare (15,98%) şi a Permeabilităţii canalului arterial (13,05%). Din cei 1.255 de copii la care s-a intervenit chirurgical, cele mai frecvente intervenţii chirurgicale (17,93%) au fost pen tru Comunicaţie interauriculară. Concluzii. Întocmirea unui Registru al MCC este importantă pentru depistarea, diagnosticul, monitorizarea şi instituirea unei atitudini terapeutice adecvate.

Abstract

Introduction. With an estimated incidence of 6 cases to 1000 births worldwide, congenital cardiac malformations (CHM) are one of the main causes of death during the fi rst year of life. The main issues regarding CHM management in Romania are the lack of data that describe the true incidence on population level, lack of standardized diagnosis and CHM reporting at national level. In Romania we see only local attempts at establishing such a CHM registry. Objectives. Measuring and analysing the incidence of CHM in children within the Pediatric Cardiovascular center of the Emergency Cardiovascular and Transplant Institute in Targu Mures (IUBCvT) in order to determine de rate of enrollment, conturing the registration and follow-up form for CHM. Material and methods. We have conducted a retrospective, descriptive study concerning children discharged between 2008-2013, with a main diagnosis or comorbidity belonging to the category Q20-Q28 (Congenital cardiac malformations – ICD 10 AM), belonging to the Pediatric Cardiovascular center of IUBCvT. The study group was made up of 839 new-borns discharged from the Neonatology clinic of the Mures County Emergency Clinical Hospital (SCJU) and 2181 children discharged from IUBCvT. Results. The incidence of CHM in new-borns discharged from SCJU varied between 5.23% and 11.47%. Of the total 839 new-borns, 139 underwent surgery in the IUBCvT within the fi rst 30 days of life. The most frequent malformations found were interatrial communication (46.52%), persistent ductus arteriosus (22.10%), interventricular communication (9.04%). Disease group based morbidity within the IUBCvT demonstrates the large share of interventricular communication (20.05%), interatrial communication (15.98%) respectively of persistent ductus arteriosus (13.05%). Of the 1255 children that underwent surgery, the most frequent procedures (17.93%) were conducted for interatrial communication. Conclusions. Establishing a CHM clinical registry is important for the screening, diagnosing, monitoring of congenital cardiac malformations as well as aiding in choosing the right treatment path.
