The study focussed on two professional groups: the public notary and the bailiffs’ professions. Indeed, these groups fulfil a particular place within society, which is among other things manifested by the fact that they are subject to a code of confidentiality and the privilege of non-disclosure. Although the main thrust of this article is on their contribution to the rule of law, this article will also highlight the various manifestations of administrative mechanisms borrowed from the civil service system and how these two professionals execute the law. The article presents shortly each one of them tasks, professional standards and the legal framework. After the introduction of each professions, there is a discussion of a number of factors that may influence the execution of the law. Thirdly, the article gives an overview of their contribution. Finally, attention is paid to their Romanian and European organisation within Chambers, Unions and European Union Councils.

The aim of the paper is to identify corruption prevention tools and new ways of approaching them. The research question is how society coped with corruption in an era heavily marked by the media and the internet. The hypothesis that led us to the study was that new strategies of communication are necessary and based on them the civil society capacity to monitor and evaluate can be improved. The new approaches highlighted new entities, such as education, NGOs, media and internet, or new concepts, such as prevention, local strategies, audit, image and identity. Among these new entities involved in prevention, education should act as a real inner actor to change.

A new democratic tool, the European Citizens’ Initiative (ECI), starting with April 2012, will allow one million European Union (EU) citizens to ask the European Commission to propose EU legislation. The ECI could thus create a new space inside the EU policy-making machine for ordinary EU citizens. The purpose of the paper is to analyse the early implementation of the Treaty of Lisbon provisions concerning citizens’ initiative. At the level of the European Union, member states in their fundamental laws set up the democratic initiative of the people. The following Member States have citizens’ initiatives at national level: Austria, Hungary, Italy, Latvia, Lithuania, Poland, Portugal, Romania, Slovakia, Slovenia, Spain and The Netherlands. These initiatives differ considerably in scope and generally operate according to different procedures. Because at EU level, there is no experience to build upon this, we will analyze the national citizens’ initiative and the problems which occurred in practice. So, the citizens’ initiative must accomplish few conditions which we intend to discover and unveil in a comparative study with the national initiative.

The concept of “unified” or “homogeneous” state authority (in which the local authorities act as representatives of the central government, equivocally subordinated to its directive and control)was rejected and replaced with a dual system, in which the state and the local management act each in its own sphere of influence. However, we should not be surprised by the fact that the reality of local management partly lags behind the normative ideal. Europe is a space of decentralized local communities, the emphasis being placed on decentralization to enable the development of contacts which the hyper-centralized state would not have promoted and could not have tolerated. The decentralization is one of the ways which leads to a sort of European “normality” and that it participates in achieving this goal. Thus, the actual context is quite favourable to diminishing the role of the state, which should focus on its major functions: diplomacy, defence, monetary policy, preserving the economic macro-balance etc. those which stem directly from the national sovereignty, which only the state holds, no matter if it is a unitary or federal one.

Within the European Union order, the national public administrations play a very important role.They are responsible for the enforcement and the control of the execution of the policies of the EuropeanCommunities, in all the states of the European Union, in the interest of the respective governments. Althoughthe European Union has a central administration, she doesn’t have external agencies. Applying the Europeaninstructions and regulations depends on the national governments. Because of rules such as mutualrecognition, each government depends on the quality of executing the Community policy, for the purpose ofachieving its own responsibilities. The role of the public administration in the European unification is adecisive one; and this is not only because the public administration is connected to the institutional andgovernmental mechanisms, but also because the real, practical, technical and “aesthetic” convergence ofdifferent administrative cultures is the key element of the European integration. Thus we are faced not onlywith a new supra-national institutional order, but also with a new juridical order given by the integrationthrough law.

Asocierea hipertrofiei VS este un factor de risc independent pentru mortalitate și morbiditate cardio-vasculară, fiind un biomarcher al leziunilor multiple de organe-țintă. Remodelarea miocardului VS este manifestarea cea mai frecventă a afectării de organ-țintă, iar denervarea arterelor renale reprezintă o metodă inovativă în tratamentul hipertensiunii arteriale rezistente care prezintă interes nu numai ca o metodă de reducere suplimentară a tensiunii arteriale la pacienții rezistenți la terapia standard cu antihipertensive, dar atestă impact şi asupra indicilor remodelării miocardului VS. Articolul este o sinteza a studiului desfăşurat in Departamentul Hipertensiuni arteriale ale IMSP Institutului de Cardiologie, scopul căreia a fost elucidarea modificărilor structural-geometrice a cordului în contextul manifestărilor clinice. Acest studiu a demonstrat că sub influența ambelor scheme de tratament farmacologic s-a atestat o dinamică statistic semnificativă începând cu 6 luni, neatingând însă valorile normale de referință în comparație cu tratamentul minim invaziv prin DASAR, care a indus o superioritate absolută în revers-remodelarea miocardului VS.

Hipertensiunea arterială (HT) reprezintă o problemă majoră de sănătate publică. În pofida eforturilor comune aplicate de comunitatea medicală globală, HT rămâne a fi rău controlată. Între 5-30% dintre pacienţi cu HT în tratament nu vor atinge obiectivele în materie de valori-țintă a TA, condiție definită ca „rezistență la tratament” (HTRT). În dezvoltarea HTRT sunt incriminați un grup heterogen de factori de risc, iar factorii genetici şi de mediu ar juca roluri echiimportante. Aprecierea determinantelor genetice individuale ar facilita realizarea unui „profil individual“ al acestuia și ar face posibilă alegerea unei terapii individualizate și, deci, optime pentru fiecare bolnav. Această abordare individualizată deschide calea medicinii personalizate. În hipertensiologie, existența acestui profil genetic personalizat reprezintă o veritabilă sursă de vulnerabilitate genetică individuală pentru fiecare bolnav și constituie în egală măsură un obstacol, dar și o țintă majora pentru modelarea unor scheme farmacoterapice personalizate în scopul atingerii cât mai eficiente a valorilor țintă a TA

În pofida progreselor majore în managementul farmacologic al HTA, există o categorie de pacienți cu HTA rezistentă cu o prevalență de 10%, care constituie o entitate provocatoare în abordarea noilor metode de tratament intervențional care ar oferi noi oportunități în tratamentul HTA. De acea trebuie luate în considerare noile terapii intervenționale disponibile – stimularea electrică a baroreceptorilor carotidieni care este o procedură neinvazivă, realizată prin: stimulator implantabil în carotidă bilateral, plasat permanent în spațiul perivascular în jurul sinusurilor arterelor carotide. Scopul cărora este o îmbunătățire a controlului pe termen lung al valorilor TA pentru reducerea morbidității și mortalității cardiovasculare.

Imagistica cardiacă prin rezonanță magnetică (RMNc) este o metodă informativă de evaluare diagnostică a patologilor sistemului cardiovascular, inclusiv bolilor aortei, cardiomiopatiilor, bolilor pericardului, viciilor cardiace congenitale și valvulopatiilor dobândite. Metoda este utilă în diferențierea cardiomiopatiilor ischemice de non-ischemice cu implicații majore în managementul pacientului cardiovascular. Este o metodă sigură și inofensivă în evaluarea pacientului caridovascular, deoarece nu se bazează pe radiații ionizante și astfel nu interferează cu structurile nucleare (în special cu ADN-ul). RMNc are avantajul de a nu fi limitată de fereastră acustică redusă, fenomen frecvent întâlnit în cadrul examenului ecocardiografic transtoracic. Este necesară integrarea RMNc în planul de evaluare imagistică de rutină a pacientului cardiovascular, considerând potențialul diagnostic superior și absența complicațiilor post-procedurale pe termen lung.

Denervarea arterelor renale reprezintă o metodă inovațională și de perspectivă în tratamentul hipertensiunii arteriale rezistente la tratament, care denotă aplicări tot mai vaste. În ultimii ani, această procedură induce interes nu numai ca o metodă de reducere suplimentară a tensiunii arteriale la pacienții care nu răspund la terapia standard cu antihipertensive, dar, de asemenea, și datorită efectelor sale pleiotrope asupra altor sindroame și patologii patogeneza cărora este asociată cu hiperactivitatea sistemului nervos simpatic. Articolul oferă o sinteză per ansamblu a studiilor clinice de denervare renală cu diabet zaharat, hipertrofi e ventriculară stângă, insuficiență cardiacă congestivă, fibrilație atrială și boala renală cronică. În pofida rezultatelor contradictorii ale studiilor de denervare renale la pacienții cu hipertensiune arterială, se atestă impactul pozitiv al procedurii asupra metabolismului glucidic, dinamica hipertrofiei ventriculare stângi și a fracției de ejecție. Există tot mai multe date despre siguranța denervării renale în boala cronică renală. În prezent, sunt în derulare diverse studii în cohorta de pacienți care implică denervarea renală.

Boala mitrală mixomatoasă (BMM) este cea mai frecventă afecțiune valvulară. Prezentarea clinică este variabilă și depinde de grosimea cupelor mitrale, gradul de prolaps și starea cordajelor. Studiile experimentate au modifi cat conceptul despre mecanismele celulare responsabile de dezvoltarea BMM și au sugerat că hipercolesterolemia ar putea contribui la inițierea/perpetuarea alterării valvulare prin activarea Lrp5 receptorilor. În absența unui tratament medicamentos efi cient, corecția chirurgicală este unica opțiune în cazul instalării regurgității mitrale severe,dar alegerea momentului optim pentru intervenție rămâne a fi un subiect controversat. Astfel, sunt necesare studii extensive care ar stabili rolul factorilor de risc tradiționali în dezvoltarea BMM și ar stabili exact timing-ul tratamentului chirurgical.

În articol este abordată utilizarea dozelor fixe de medicamente în prevenția secundară a patologiilor cardiovasculare. Sunt revizuite date epidemiologice recente privind morbiditatea, mortalitatea și cost-eficiența acestora la nivel european. Prin prisma acestor date este analizat rolul potențial al polypill-urilor, pozițiile actuale ale cercetărilor, punctele forte și limitările, dar și direcțiile de viitor pentru folosirea acestora.

the treatment with antibiotics represents one of the main components of the medical treatment of neglected peritonitises. Material and method: in principle, the antibiotics therapy must take place based on the bacteriological examination and with the results of the antibiogram in mind. However, in practice, because of the emergency situation and the polymorphism of bacteria we could also apply certain theoretical criteria in order to properly select the antibiotics treatment, but: the antibiotic must be active for the peritonitis germs; it must have a good penetration and it must concentrate in the peritoneal cavity; it must not any have toxic effects and must not be conducive to kidney failure. In our trial we have used a systematic antibiotics treatment in emergency situations, and we have often chosen combination of antibiotics following the analysis of the peritoneal pus and the antibiogram outcome. Results: The antibiotics used the most were Penicillin, Gentamicin, Metronidazole and Ampicillin. The combination of three antibiotics was used in 67.35% of all the cases when antibiotics were used. The antibiotics mostly used in an association of two were: Penicillin, Gentamicin, Ceftriaxone, Cefazolin. This association was used in 26.53% of the cases when antibiotics were used in the treatment. The most frequent antibiotics used in a combination of two for the treatment of acute appendicitis were: Ampicillin and Gentamicin, respectively Ampicillin and Metronidazole. The percentage of this association of the whole antibiotherapy for acute appendicitis represents 23.81%. In our trial we have compared the therapeutic proficiency of one antibiotic vs the combination of antibiotics in cases of neglected peritonitises. From our results we have found that there was an increased sensibility and a high rate of response (92%) in the patients treated with Imipenem and Netilmicin, in comparison with the rate of positive response of 83%, which we achieved when Imipenem was administrated in monotherapy. Conclusions: We have applied an algorithm of antibiotics therapy in a growing scale: Tienam (Imipenem) usually, in the 3 days until we have obtained the result of the antibiogram and we have become aware of the sensibility of the bacteria and had the possibility to choose a combination of antibiotics.

The Romanian society, the state, the administration and the judiciary are facing several essential challenges of the contemporary world, and these challenges have to be addressed as soon as possible. The position and the role of the Public Prosecutor’s Office within the rule of law should be clarified since it is a well known fact that the judiciary and public administration systems are not yet capable to manage the changes necessary for the integration in the European juridical space. With the advancement of the Romanian public administration reform, the Public Prosecutor’s Office and the public administration have become the two main actors involved in the struggle against corruption. Corruption is considered to be an important factor underlying the inability of the public administration and the judiciary systems to act and to meet the citizen’s needs. The Romanian citizen has been facing the burden of corruption and bureaucracy, and the Public Prosecutor’s Office is one of the institutions called upon not to eradicate this phenomenon, but to prevent it and to keep it under control. Within the current framework, when the political and legal debate raises issues regarding a new review of the Romanian Constitution, one of the questions raised is whether the Public Prosecutor’s Office is a public authority belonging to the executive or to the judiciary. The following paper studies the place of the Public Prosecutor’s Office within the legal systems and consequently will indentify and determine its place and role at the intersection between the executive and the judiciary.
