The excavations at “Cova del Randero” (Pedreguer, Alicante, Spain) began in 2007 within the programme of archaeological interventions of the Archaeological Museum of Alicante. The cavity, located in one of the valleys that connect the coast with the inland mountains, presents a wide sequence of occupations that begins in the Upper Palaeolithic and continues throughout the different phases of the Neolithic. The results of a multidisciplinary study, carried out in an archaeological context associated with the first Neolithic presence of the cavity, are presented here. This occupation is defined by a unique combustion structure to which a set of artefacts and biofacts are linked. This archaeological context, probably of a specific nature, is related to the first agro-pastoral communities settled in the area. The fireplace is well defined stratigraphically and sedimentologically because of its reddish soil, which corresponds to hunter-gatherer occupation levels of the cavity, and under the greyish sediments that characterise the use of the cave as a fold during the Middle Neolithic. This occupation event was dated both by the associated materials, among which a fragment of cardial ceramic was found, and by radiocarbon dating of a metacarpus of Ovis aries around 5075–4910 cal BC (epicardial Early Neolithic). This data allows us to link the occupation of the cavity at this time with pastoral activity in a medium mountain environment. However, it also allows us to infer the environmental characteristics in which the first farming communities of the mountains of Alicante were developed.

9. COVID-19 in people living with HIV: A multicenter case-series study

Objectives: Information on how COVID-19 affects people living with HIV (PLHIV) remains scarce. Methods: An observational study was conducted in four public hospitals in Madrid. All HIV patients with confirmed or suspected COVID-19 were included and compared with COVID-19 patients without HIV infection. Results: Sixty-three patients with HIV infection and confirmed or suspected COVID-19 were analyzed. The median age was 46 years (IQR: 37–56 years), and 88.9% were men. The median duration of HIV infection was 10.8 years (IQR: 6.5–16.8 years), and 96.8% were on antiretroviral therapy. 84.1% had previous comorbidities. The most common symptoms were fever (66.1%), cough (66.1%) and dyspnea (46.8%). Pneumonia was found in 47.5%, 28.6% of patients had severe disease, and 32.3% were admitted to hospital. The ICU admission rate and the mortality rate were both 3.17%. A significant association was observed between age, arterial hypertension, overweight, and diabetes mellitus and the severity of COVID-19. No association was observed between HIV-related factors and the severity of COVID-19. The rate of COVID-19 in HIV-patients was 1.68%. Similar hospitalization (31.74% vs 32.57%) and ICU admission (3.17% vs 2%) rates were observed with non-HIV infected patients. A lower mortality rate during hospitalization (10% vs 21.37%) and a lower global mortality rate (3.17% vs 6.96%) were also observed. Conclusions: Established poor prognostic factors for COVID-19 patients, such as age and comorbidities, remain the main determinants for PLHIV. Neither the HIV severity nor the type of ARV treatment seem to influence the outcome of COVID-19. Large prospective cohorts are needed in order to establish the differences between HIV-positive and HIV-negative patients.

Resalta su colección de escultura ibérica de toros sobre piedra, rescatada de una vasta necrópolis de los siglos VI a IV a. C. que existió en los aledaños del río Vinalopó. Los restos de las villas romanas excavadas, una de ellas un vicus identificado con la Mansio Aspis de la Via Augusta, dispone otro ámbito donde se explica la vida en estos grandes complejos rurales de explotación económica del territorio o fundus entre los siglos I a VI de nuestra Era. Con tecnologías de vanguardia y diseño actual, se dota al espectador de las herramientas necesarias para el disfrute y aprendizaje.

The main hypothesis for the aetiology of meningoencephalitis of unknown origin (MUO) in dogs is an autoimmune or genetic cause that is associated with a triggering event (environmental factors/infectious agents). The aim of this ambispective cohort study was to test for Leishmania infantum infection in the blood and cerebrospinal fluid (CSF) of dogs with MUO in an endemic area of canine leishmaniosis. Dogs with MUO were selected amongst all dogs undergoing blood anti-L. infantum antibody testing (control group). The blood plasma or serum samples from all dogs were analysed for anti- L. infantum antibodies by a quantitative indirect fluorescent assay (IFAT). In dogs with MUO, CSF samples were obtained for analysed by PCR detection of L. infantum DNA. Forty-four percent and 22% of the dogs in the MUO group featured magnetic resonance imaging (MRI) findings and CSF cytology respectively, consistent with L. infantum infection. IFAT, PCR, and histological findings were negative for L. infantum. A significant difference in L. infantum infection prevalence was found between the control and MUO group (p = 0.0022). While it seems unlikely that L. infantum plays a role in the aetiology of MUO, in endemic areas, this pathogen should be included in the differential diagnosis of this neurological disorder.

Objective: The health effects of a supplemented Mediterranean diet (SMD) with extra-virgin olive oil (EVOO) and nuts are well documented in non-HIV-infected individuals. We hypothesised that the benefits of an SMD could be mediated by changes in the gut microbiota, even in those with an altered intestinal microbiota such as people living with HIV. Design: Individuals living with HIV (n = 102) were randomised to receive an SMD with 50 g/day of EVOO and 30 g/day of walnuts (SMD group) or continue with their regular diet (control group) for 12 weeks. Methods: Adherence to the Mediterranean diet was assessed using the validated 14-item MD-Adherence-Screener (MEDAS) from the PREDIMED study. A sub-study classifying the participants according to their MEDAS scores was performed. Results: The lipid profile was improved in the SMD group vs. that in the control group (delta-total cholesterol and delta-B-lipoprotein). The immune activation (CD4+HLADR+CD38+ and CD8+HLADR+CD38+ cells) and IFN-γ-producing T-cells significantly decreased at week 12 compared to the baseline in the SMD group but not in the control group. The gut microbiota in those from the high-adherence group presented significantly high diversity and richness at the end of the intervention. Succinivibrio and Bifidobacterium abundances were influenced by the adherence to the MD and significantly correlated with Treg cells. Conclusion: The Mediterranean diet improved metabolic parameters, immune activation, Treg function, and the gut microbiota composition in HIV-1-infected individuals. Further, Mediterranean diet increased the Bifidobacterium abundances after the intervention, and it was associated to a beneficial profile.

La educación secundaria desempeña un importante papel en la alfabetización científica y, en ella, las creencias en pseudociencias tienen una gran relevancia. Por esta razón, los objetivos de este estudio se centraron en la evaluación del nivel de adhesión a las creencias en pseudociencias de los estudiantes de secundaria y en el análisis de la influencia del género, nivel académico y emociones generadas en el aula de ciencias sobre dichas creencias. Participaron 125 estudiantes de tres cursos diferentes de educación secundaria (3º y 4º de ESO, y 1º de Bachillerato, esto es, 9º, 10º y 11º grado). A todos ellos se les administraron dos cuestionarios, uno para evaluar las creencias pseudocientíficas y el otro para determinar las emociones, positivas y negativas, que se experimentan en las clases de ciencias. A partir de los resultados obtenidos mediante un análisis de las correlaciones entre variables y un análisis de regresión, se puede concluir que: a) el nivel de creencias en pseudociencias de los estudiantes es medio-bajo y comparable al de otros países; b) solo las emociones negativas y el nivel académico de los estudiantes influyen de forma significativa en las creencias en pseudociencias, y ambas tienen una contribución similar a la variabilidad de estas creencias., Secondary education plays an important role in scientific literacy and, in it, beliefs in pseudoscience have a great relevance. For this reason, the objectives of this study focused on the evaluation of the level of adherence to pseudoscience beliefs of secondary school students, and on the analysis of the influence of gender, grade level and emotions generated in the science classroom on these beliefs. A total of 125 students from three different grades of Spanish secondary education (9th, 10th and 11th grade, between 14 and 17 years old) participated. Two questionnaires, one to evaluate pseudoscientific beliefs, and the other to determine the positive and negative emotions experienced in the science classroom, were administered to participants. From the results obtained through an analysis of the correlations between variables and a regression analysis, it can be concluded that: a) the level of pseudoscience beliefs of students is medium-low and comparable to that of other countries; b) only negative emotions and grade level of students have a significant influence on pseudoscience beliefs, and both have a similar contribution to the variability of these beliefs

19. Biofilm Eradication by Symmetrical Selenoesters for Food-Borne Pathogens

Infections caused by Salmonella species and Staphylococcus aureus represent major health and food industry problems. Bacteria have developed many strategies to resist the antibacterial activity of antibiotics, leading to multidrug resistance (MDR). The over-expression of drug efflux pumps and the formation of biofilms based on quorum sensing (QS) can contribute the emergence of MDR. For this reason, the development of novel effective compounds to overcome resistance is urgently needed. This study focused on the antibacterial activity of nine symmetrical selenoesters (Se-esters) containing additional functional groups including oxygen esters, ketones, and nitriles against Gram-positive and Gram-negative bacteria. Firstly, the minimum inhibitory concentrations of the compounds were determined. Secondly, the interaction of compounds with reference antibiotics was examined. The efflux pump (EP) inhibitory properties of the compounds were assessed using real-time fluorimetry. Finally, the anti-biofilm and quorum sensing inhibiting effects of selenocompounds were determined. The methylketone and methyloxycarbonyl selenoesters were the more effective antibacterials compared to cyano selenoesters. The methyloxycarbonyl selenoesters (Se-E2 and Se-E3) showed significant biofilm and efflux pump inhibition, and a methyloxycarbonyl selenoester (Se-E1) exerted strong QS inhibiting effect. Based on results selenoesters could be promising compounds to overcome bacterial MDR.

Our aim was to analyze the relationship between plasma inflammatory biomarkers and CD4+ T-cells evolution in human immunodeficiency virus (HIV) elite controllers (HIV-ECs) with a suppressed viremia. We carried out a retrospective study in 30 HIV-ECs classified into two groups: those showing no significant loss of CD4+ T-cells during the observation period (stable CD4+, n = 19) and those showing a significant decrease of CD4+ T-cells (decline CD4+, n = 11). Baseline plasma biomarkers were measured using a multiplex immunoassay: sTNF-R1, TRAIL, sFas (APO), sFasL, TNF-α, TNF-β, IL-8, IL-18, IL-6, IL-10, IP-10, MCP-1, MIP-1α, MIP-1β, RANTES, SDF1α, GRO-α, and CCL11. Baseline levels of sTNF-R1 and CCL11 and sTNF-R1/TNF-α ratio correlated with the slope of CD4+ T-cells (cells/μl/year) during follow-up [r = −0.370 (p = 0.043), r = −0.314 (p = 0.091), and r = −0.381 (p = 0.038); respectively]. HIV-ECs with declining CD4+ T-cells had higher baseline plasma levels of sTNF-R1 [1,500.7 (555.7; 2,060.7) pg/ml vs. 450.8 (227.9; 1,263.9) pg/ml; p = 0.018] and CCL11 [29.8 (23.5; 54.9) vs. 19.2 (17.8; 29.9) pg/ml; p = 0.041], and sTNF-R1/TNF-α ratio [84.7 (33.2; 124.2) vs. 25.9 (16.3; 75.1); p = 0.012] than HIV-1 ECs with stable CD4+ T-cells. The area under the receiver operating characteristic (ROC) curve [area under ROC curve (AUROC)] were 0.758 ± 0.093 (sTNF-R1), 0.727 ± 0.096 (CCL11), and 0.777 ± 0.087 (sTNF-R1/TNF-α). The cut-off of 75th percentile (high values) for these biomarkers had 71.4% positive predictive value and 73.9% negative predictive value for anticipating the evolution of CD4+ T-cells. In conclusion, the loss of CD4+ T-cells in HIV-ECs was associated with higher levels of two plasma inflammatory biomarkers (sTNF-R1 and CCL11), which were also reasonably accurate for the prediction of the CD4+ T-cells loss.

Introducción y objetivo: Nuestra serie es una serie histórica, homogénea en cuanto a tipo de dispositivo y manejo. Esto nos permite conocer la incidencia, características, complejidad y evolución de los pacientes, así como saber si fue un tratamiento adecuado como asistencia ventricular de corta/media duración. Métodos: La población a estudio estuvo formada por 37 pacientes a los que se les implantó la Abiomed BVS 5000® en el período 1992-2001. Se analizan datos demográficos, comorbilidades, causa de la indicación, cuantificación de la disfunción ventricular, características de la asistencia y evolución. Resultados: De los 37 pacientes a los que se les colocó la asistencia, 13 murieron durante la implantación. De los 24 sobrevivientes (64,8%) a 7 de ellos (29%) se les realizó destete y a 17 de ellos (71%) se les practicó un trasplante cardíaco. De los 7 pacientes a los que se les hizo destete 3 murieron durante el ingreso y 4 pudieron ser dados de alta. De los 17 pacientes trasplantados 5 (29%) murieron en el postoperatorio y 12 (71%) recibieron el alta. Del total de la serie, 16 pudieron ser dados de alta (43,2%). Conclusiones: Los pacientes de la serie tenían importantes comorbilidades y presentaron graves complicaciones pre- y postoperatorias. El hecho de obtener una supervivencia inicial del 64% lo avala como opción de tratamiento en la insuficiencia cardíaca crítica. Abiomed BVS 5000® proporcionó una supervivencia final al alta del 43,2% lo cual puede considerarse como un buen resultado en estas circunstancias.

Introduction. The need to integrate local resistances into clinical practice is increasingly urgent, especially in Primary Care where empirical treatment is frequent. Methods. A retrospective observational study of positive microbiological isolates of Neisseria gonorrhoeae from any location (urethral, cervical, pharyngeal, rectal or urine) was carried out in the health area of Alcalá de Henares. Sociodemographic characteristics and resistance to cephalosporins, azithromycin, penicillin and quinolones were analyzed. Each isolate was related to its postal code of origin. Results. We analyzed 256 microbiological samples of N.gonorrhoeae, most of them male (92.9%) with a mean age of 33 years. Half of the samples (49.8%) were resistant to ciprofloxacin. Temporal and spatial evolution of antimicrobial resistance was integrated in heat maps. Conclusion. Knowing local resistances can help to prescribe more adequate empirical treatments, especially in Primary Care, avoiding inadequate antibiotics and decreasing resistance rates.

Up to 22 June 2022, 508 confirmed cases of monkeypox (MPX) have been reported in the Madrid region of Spain, 99% are men (n = 503) with a median age of 35 years (range: 18–67). In this ongoing outbreak, 427 cases (84.1%) reported condomless sex or sex with multiple partners within the 21 days before onset of symptoms, who were predominantly men who have sex with men (MSM) (n = 397; 93%). Both the location of the rash, mainly in the anogenital and perineal area, as well as the presence of inguinal lymphadenopathy suggest that close physical contact during sexual activity played a key role in transmission. Several cases reported being at a sauna in the city of Madrid (n = 34) or a mass event held on the Spanish island of Gran Canaria (n = 27), activities which may represent a conducive environment for MPX virus spread, with many private parties also playing an important role. Because of the rapid implementation of MPX surveillance in Madrid, one of the largest outbreaks reported outside Africa was identified. To minimise transmission, we continue to actively work with LGBTIQ+ groups and associations, with the aim of raising awareness among people at risk and encouraging them to adopt preventive measures.

Nowadays, nanoengineering is being applied to the development of ultra-high strength steels as a key-enabling technology in the steel sector. The present article describes the multiscale structure of nano-grained steels designed using atomic transformation theory and processed by a simple heat treatment. Outstanding mechanical properties for these novel steels are reported, and strain-hardening mechanisms are discussed.

The excavations at “Cova del Randero” (Pedreguer, Alicante, Spain) began in 2007 within the programme of archaeological interventions of the Archaeological Museum of Alicante. The cavity, located in one of the valleys that connect the coast with the inland mountains, presents a wide sequence of occupations that begins in the Upper Palaeolithic and continues throughout the different phases of the Neolithic. The results of a multidisciplinary study, carried out in an archaeological context associated with the first Neolithic presence of the cavity, are presented here. This occupation is defined by a unique combustion structure to which a set of artefacts and biofacts are linked. This archaeological context, probably of a specific nature, is related to the first agro-pastoral communities settled in the area. The fireplace is well defined stratigraphically and sedimentologically because of its reddish soil, which corresponds to hunter-gatherer occupation levels of the cavity, and under the greyish sediments that characterise the use of the cave as a fold during the Middle Neolithic. This occupation event was dated both by the associated materials, among which a fragment of cardial ceramic was found, and by radiocarbon dating of a metacarpus of Ovis aries around 5075–4910 cal BC (epicardial Early Neolithic). This data allows us to link the occupation of the cavity at this time with pastoral activity in a medium mountain environment. However, it also allows us to infer the environmental characteristics in which the first farming communities of the mountains of Alicante were developed. These excavations are part of MARQ’s general plan of ordinary archaeological excavations that are carried out within the framework of the agreement signed between the Provincial Council of Alicante and the Town Council of Pedreguer. Therefore, part of the results of this research are in the framework of the doctoral thesis of SMA (Silvia Martínez Amorós), who has a training contract with the Vice-Rectorate for Research of the University of Alicante (UAFPU2018-045).

Graphical Abstract, Highlights • Lower mortality rate was observed in HIV patients. • Similar hospitalization and ICU admission rate were observed in HIV patients. • Age and comorbidities also represent the main poor prognostic factors for PLHIV. • HIV severity or the type of antiretroviral does not influence the Covid-19 outcome., Objectives The information of how COVID-19 infection affects people living with HIV (PLHIV) is still scarce. Methods Observational study was conducted in four of public hospitals in Madrid. All HIV patients with confirmed or suspected COVID-19 disease were included and compared with COVID-19 infected patients without HIV infection. Results Sixty-three patients with HIV infection were analysed. The median age was 46 years (IQR: 37-56 years), 88.9% were men. The median time of HIV infection was 10.8 years (IQR: 6.5 - 16.8 years), 96.8% were on antiretroviral therapy. 84.1% had previous comorbidities. The most common symptoms were fever(66.1%), cough(66.1%) and dyspnea(46.8%). Pneumonia was found in 47.5%, 28.6% of patients had severe disease and 32.3% was admitted to hospital. The ICU admission rate and the mortality rate were both 3.17%. Significant association was observed between age, arterial hypertension, overweight, diabetes mellitus and the severity of COVID-19 disease. No association was observed between HIV-related factors. The rate of COVID-19 disease in HIV-patients was 1,68%. A similar hospitalization (31.74% vs 32.57%) and ICU admission rate (3.17% vs 2%) was observed with non-HIV infected patients. A lower mortality rate during hospitalization (10% vs 21.37%) and global mortality rate (3.17% vs 6.96%) were observed. Conclusions Established poor prognostic factors, such as age and comorbidities, represent also the main determinants for PLHIV. Neither the HIV severity, nor the type of ARV treatment seem to influence the outcome of COVID-19 infection. Large prospective cohorts are needed in order to establish the differences between HIV positive and negative patients.

Previously, we identified a stabilization of the HIV reservoir size in HIV + monoinfected and HIV+/HCV + coinfected patients who eliminated HCV with DAAs as well as a decrease in HIV reservoir size in HIV+/HCV- spontaneously clearers. However, variations in the distribution of memory CD4 + T cell subpopulations in coinfected patients after HCV clearance remains unclear. To analyze the expression pattern of T cell maturation (CD45RA and CD27) and senescence (CD28 and CD57) markers by flow cytometry in those patients. Longitudinal study (52 weeks follow-up) of 49 HIV+/HCV+, 38 HIV+/HCV- and 59 HIV + subjects, all of them aviremic. A generalized increase in expression of CD28 was observed for CD8 + and in CD4 + CD27 + cells. The proportion of both CD4 + TEM TH1 and TH2 cells decreased significantly in HIV+/HCV- while remained stable in HIV + and HIV+/HCV + individuals. The general decline in cell senescence of cytotoxic lymphocytes may be due to reduced antigenic stimulation of HIV-specific and HCV-specific lymphocytes. The dynamic of HIV viral reservoir in HIV+/HCV- patients matches the dynamic of CD4 + cells with a TH1 phenotype, which could mean a decrease in the number of circulating TFH1 lymphocytes after an improvement in generalized inflammation state.

- Abstract
The objective of this work is to introduce ourselves to the theory of absolutely summing operators, giving an overview of them, based on the first chapter of the book with the same name to this work found in the bibliography. To do this, we will see some important results of absolute and unconditional convergence, establishing the relationship between them. We will also define other types of convergence, such as sign convergence or disordered convergence, and we will establish a list of equivalences that will be very useful. At the end of the second chapter we will and the Grothendieck’s Theorem, which will establish a result that will be the heart of this work. Finally, in the last chapter we will study the well-known Schatten-von Neumann classes, where we will pay special attention to the Hilbert-Schmidt classes and the nuclear operators or the trace class operators, seeing the relationship between them and the p-summing operators., El objetivo de este trabajo es introducirnos en la teoría de los operadores absolutamente sumantes, dando una visión general de los mismos, basándonos en el primer capítulo del libro homónimo a este trabajo que se encuentra en la bibliografía. Para ello, veremos algunos resultados importantes de convergencia absoluta e incondicional, estableciendo la relación entre ellas. También definiremos otros tipos de convergencia, como la convergencia por signos o la convergencia desordenada, y estableceremos una lista de equivalencias que resultarán de gran utilidad. Al final del segundo capítulo encontraremos el teorema de Grothendieck, que establecerá un resultando que será el corazón de este trabajo. Para analizar, en el último capítulo estudiaremos las conocidas clases de Schattenvon Neumann, donde prestaremos especial atención a las clases de Hilbert-Schmidt y a los operadores nucleares o de la clase de la traza, viendo la relación entre ellos y los operdores p-sumantes.

An 11-year-old, female, neutered labrador retriever with a history of chronic and progressive right hindlimb lameness and facial asymmetry was referred for brain and lumbosacral magnetic resonance imaging and cerebrospinal fluid analysis. Multifocal non-enhancing T2-weighted images of hyperintense lesions were observed in the caudate nuclei and medulla oblongata. The right oculomotor nerve was markedly enlarged and showed marked enhancement on T1-weighted images after contrast injection, and there was diffuse enlargement of the sciatic and femoral nerves in the right hindlimb. Cerebrospinal fluid analysis showed a mixed pleocytosis. Histopathology revealed granulomatous inflammation affecting the brain, oculomotor and pelvic limb nerves, consistent with a diagnosis of granulomatous meningoencephalomyelitis.

To investigate the relationship of ambient temperature and atmospheric pressure (AP) at patient discharge after an episode of acute heart failure (AHF) with very early post-discharge adverse outcomes. We analyzed 14,656 patients discharged after an AHF episode from 26 hospitals in 16 Spanish cities. The primary outcome was the 7-day post-discharge combined adverse event (emergency department -ED- revisit or hospitalization due to AHF, or all-cause death), and secondary outcomes were these three adverse events considered individually. Associations (adjusted for patient and demographic conditions, and length of stay -LOS- during the AHF index episode) of temperature and AP with the primary and secondary outcomes were investigated. We used restricted cubic splines to model the continuous non-linear association of temperature and AP with each endpoint. Some sensitivity analyses were performed. Patients were discharged after a median LOS of 5 days (IQR = 1-10). The highest temperature at discharge ranged from - 2 to 41.6 °C, and AP was from 892 to 1037 hPa. The 7-day post-discharge combined event occurred in 1242 patients (8.4%), with percentages of 7-day ED-revisit, hospitalization and death of 7.8%, 5.1% and 0.9%, respectively. We found no association between the maximal temperature and AP on the day of discharge and the primary or secondary outcomes. Similarly, there were no significant associations when the analyses were restricted to hospitalized patients (median LOS = 7 days, IQR = 4-11) during the index event, or when lag-1, lag-2 or the mean of the 3 post-discharge days (instead of point estimation) of ambient temperature and AP were considered. Temperature and AP on the day of patient discharge are not independently associated with the risk of very early adverse events during the vulnerable post-discharge period in patients discharged after an AHF episode.

Background Fast initiation of ART has been associated with higher rates of retention in HIV care and viral suppression at 48 weeks and with lower mortality rates. However, scarce evidence exists in our setting, where diagnosis and treatment are carried out in different contexts. Methods An observational retrospective study evaluating efficacy and safety of ART prescribed at the first specialist appointment, without baseline laboratory data, in a tertiary hospital in downtown Madrid. Individuals with a new diagnosis of HIV infection who initiated treatment at their first appointment with an infectious diseases specialist before receiving baseline laboratory results were included, irrespective of the ART regimen chosen. Results One hundred and eight participants were included. The majority (99.1%) were MSM who had acquired infection during sexual intercourse. The efficacy of ART, without baseline laboratory results at the time of initiation, was 85.2% (92/108) in the ITT analysis and 91.7% (99/108) in the treatment-related discontinuation equals failure analysis. All but nine patients presented an undetectable viral load ( Conclusions Initiating ART at the first available opportunity without baseline laboratory data does not reduce efficacy or safety of ART and achieves rapid virological control with high rates of retention in HIV care.

To determine whether there are differences in the clinical characteristics, management, and outcome of episodes of acute heart failure (AHF) in residents of the Canary Islands, where the climate is subtropical, and episodes in continental Spain.Cases were identified in the registry for Epidemiology of Acute Heart Failure in Emergency Departments and categorized as in the Canary Islands or continental Spain. Data for 38 demographic, baseline, clinical, and therapeutic variables were extracted. We analyzed statistics for in-hospital and 30-day mortality, long hospital stay (more than 7 days), and a composite outcome after discharge (revisits or death within 30 days). The results for island and continental patients were compared and adjusted for between-group differences.A total of 18 390 patients were studied, 697 islanders (3.8%) and 17 673 continental patients (96.2%). Comparisons showed that the islanders were younger; more often women; and more likely to have hypertension, diabetes, and a prior AHF episode. Their New York Heart Association functional class was also likely to be higher. However, their rates of dyslipidemia, valve disease, and functional dependence were lower, and they were also less likely to be on reninangiotensin system blockers. Although the severity of cardiac decompensation was similar in island and continental patients, the islanders received more intravenous treatments in the emergency department and were admitted less often. The adjusted risk of a long hospital stay was higher for the islanders (odds ratio [OR], 2.36; 95% CI, 1.52-3.02) but their risk for mortality and the composite outcome did not differ: in-hospital mortality, OR 0.97 (95% CI, 0.68-1.37); 30-day mortality, OR, 0.9 (95% CI, 0.67-1.27); and the post-discharge composite, OR, 1.19 (95% CI, 0.93-1.53).Baseline patient characteristics and clinical management of AHF episodes differ between the subtropical region and those in southern continental Europe. Hospitalization was required less often, but hospital stays were longer.Investigar si existen diferencias clínicas, asistenciales y evolutivas entre los episodios de insuficiencia cardiaca aguda (ICA) desarrollados en residentes en las Islas Canarias (CAN), de clima subtropical, y en los del territorio español continental (CON).. Los pacientes con ICA incluidos en el registro EAHFE se dividieron en CAN o CON. Se recogieron 38 variables demográficas, de estado clínico basal, clínicas y terapéuticas. Se analizó la mortalidad intrahospitalaria y a los 30 días, la hospitalización prolongada (7 días) y el evento combinado post-alta (reconsulta o muerte a 30 días). Se compararon los pacientes CAN y CON, ajustando los resultados por las diferencias entre grupos.Se incluyeron 18.390 pacientes: CAN = 697 (3,8%), CON = 17.673 (96,2%). Tras el ajuste, los CAN eran más jóvenes, más frecuentemente mujeres, tenían más antecedentes de hipertensión, diabetes e ICA previa, y estaban en clase funcional más avanzada, pero tenían menos dislipemia, valvulopatías y dependencia funcional, y recibían menos frecuentemente inhibidores del eje renina-angiotensina. Aunque la gravedad del episodio de descompensación fue similar, los CAN recibían más tratamientos endovenosos en urgencias y se hospitalizaban menos. El riesgo ajustado de hospitalización prolongada en los CAN resultó incrementado (OR 2,36; IC 95%:1,52-3,02), pero no hubo diferencias en la mortalidad intrahospitalaria (OR 0,97; IC 95%: 0,68-1,37), a 30 días (OR 0,9; IC 95%: 0,67-1,27) o en los eventos combinados post-alta a los 30 días (OR 1,19; IC 95%: 0,93-1,53).Existen diferencias tanto en las características basales como en el manejo clínico de los episodios de insuficiencia cardiaca en los residentes de una región subtropical respecto a los de una zona meridional europea. Además, tienen menor necesidad de hospitalización, aunque estas son más prolongadas.

Fifteen selenocompounds, comprising of eight ketone-containing selenoesters (K1–K8, also known as oxoselenoesters) and seven cyano-containing selenoesters (N1–N7, known also as cyanoselenoesters), have been designed, synthesized, and evaluated as novel anticancer agents. These compounds are derivatives of previously reported active selenoesters and were prepared following a three-step one-pot synthetic route. The following evaluations were performed in their biological assessment: cytotoxicity determination, selectivity towards cancer cells in respect to non-cancer cells, checkerboard combination assay, ABCB1 inhibition and inhibition of ABCB1 ATPase activity, apoptosis induction, and wound healing assay. As key results, all the compounds showed cytotoxicity against cancer cells at low micromolar concentrations, with cyanoselenoesters being strongly selective. All of the oxoselenoesters, except K4, were potent ABCB1 inhibitors, and two of them, namely K5 and K6, enhanced the activity of doxorubicin in a synergistic manner. The majority of these ketone derivatives modulated the ATPase activity, showed wound healing activity, and induced apoptosis, with K3 being the most potent, with a potency close to that of the reference compound. Objetivo: Determinar las características laborales del médico joven egresado de la Facultad de Medicina San Fernando. Diseño: Estudio observacional, transversal. Lugar: Facultad de Medicina San Fernando, Universidad Nacional Mayor de San Marcos. Participantes: Médicos egresados de la Facultad de Medicina. Intervenciones: Se recolectó datos sociodemográficos, del desempeño laboral, grado profesional, nivel de satisfacción y desempeño extracurricular del pregrado, en 232 médicos egresados de la Facultad de Medicina San Fernando entre 1994 y 2003, elegidos al azar. La recolección se llevó a efecto mediante entrevista directa, telefónica o cuestionario vía correo electrónico. También se obtuvo las notas finales del pregrado de los sujetos. Principales medidas de resultados: Índice de empleo, tipo de labor, desempeño, especialización. Resultados: El 72,4% estuvo representado por varones, 73,2% radicaba en Lima, 19,3% en provincias y 7,5% en el extranjero; 97% tenía trabajo remunerado, 51,4% de ellos tenía 2 o más trabajos; 71% trabajaba en el sector público. El tipo de labor predominante fue el asistencial; 78,8% alcanzaba el desempeño laboral esperado; 64,9% era especialista o estaba en proceso de especialización; 15,4% tenía o estudiaba una maestría. No se encontró relación significativa entre factores del pregrado y las características laborales o profesionales. Conclusiones: El índice de empleo fue alto. El centralismo, trabajo médico múltiple, modelo centrohospitalario público asistencial y la tendencia a la especialización fueron predominantes.

Testing software is an expensive and time consuming task. This drawback increases in the case that the system presents timeouts which affect the functional behaviour. In this case, it is necessary to invest more time to apply the test cases. Thus, it is desirable to diminish the amount of test cases to be applied and, consequently, the time devoted to test the system, as long as the fault detection capacity of the test cases is not affected. In this work, we introduce a genetic algorithm that selects a subset of test cases from an initial test suite for systems that present timeouts, with the goal of keeping a good level of effectiveness. We report on several experiments performed to compare the generated solutions with random selection and the original test suite.

Software testing is fundamental to ensure the reliability of software. To properly test software, it is critical to generate test suites with high fault finding ability. We propose a new method to generate such test suites: a coverage-based grammar-guide genetic programming algorithm. This evolutionary computation based method allows us to generate test suites that conform with respect to a specification of the system under test using the coverage of such test suites as a guide. We considered scenarios for both black-box testing and white-box testing, depending on the different criteria we work with at each situation. Our experiments show that our proposed method outperforms other baseline methods, both in performance and execution time.

This paper applies non-parametric techniques to examine the evolution of the entire distribution of regional productivity in the European Union between 1977 and 1999. In addition, we study the importance of the respective roles played by regional and sectoral factors in the convergence of productivity observed in the European context. To achieve this aim, we consider a new methodology involving a modification of conventional shift-share analysis and various results reported in the literature on personal income distribution. The empirical evidence provided suggests that regional inequality in productivity in the European Union is closely linked to intrinsic differences between regions. Likewise, the analyses developed reveal the major role played by the national component in the explanation of regional disparities in product per worker in the European context. Our findings also support the relevance of one-sector growth models for analysing regional disparities in per capita income.

En este trabajo se examina la dinámica de la distribución regional del producto por empleado en la Unión Europea entre 1977 y 1999 a partir de técnicas de estimación no paramétricas. Asimismo, se analiza cuál ha sido la contribución regional y sectorial a la convergencia en productividad observada en el ámbito europeo a lo largo del período contemplado. Para ello se utiliza una metodología novedosa que combina una formulación alternativa del análisis shift-share con diversos resultados procedentes de la literatura dedicada al estudio de la distribución personal de la renta. La evidencia empírica aportada sugiere que la desigualdad regional en productividad en la Unión Europea está estrechamente relacionada con la existencia de diferencias intrínsecas entre regiones. En cualquier caso, los resultados obtenidos también ponen de manifiesto el importante papel desempeñado por el componente nacional a la hora de explicar los desequilibrios territoriales observados. Adicionalmente, las conclusiones alcanzadas permiten respaldar la relevancia teórica de los modelos de crecimiento unisectoriales a la hora de explicar las disparidades regionales en renta por habitante en el ámbito europeo.

Resumen Objetivos Evaluar si existen diferencias en la presentacion clinica y en la atencion de pacientes adultos con crisis epilepticas (CE) en servicios de urgencias hospitalarios (SUH) en funcion de la edad ≥ 75 anos y si esta se asocia de manera independiente a resultados en el SUH y a 30 dias del alta. Material y metodos ACESUR es un registro observacional de cohortes multiproposito, prospectivo y multicentrico, que se llevo a cabo en 2017. Se comparan la distribucion de variables correspondientes a la presentacion clinica y atencion segun la edad ≥ 75 anos. Posteriormente se realizan modelos de regresion logistica con el objetivo de evaluar el efecto de la edad ≥ 75 anos sobre las variables de resultado. Resultados Se analizan 541 (81,5%) pacientes 75 anos. En el grupo de longevos se detecto de forma significativa: mayor probabilidad de dependencia, comorbilidad, polifarmacia, visita previa al SUH, llegada en ambulancia, primera CE y clasificacion etiopatogenica sintomatica. En el analisis multivariado tras ajustar por las variables anteriores, se objetiva que la edad ≥ 75 anos se asocia de manera independiente a una mayor incidencia de prueba complementaria especifica (OR: 2,31; IC95%: 1,21-4,44), pero no a intervencion farmacologica (OR: 1,63; IC95%: 0,96-2,80) ni hospitalizacion o estancia prolongada en SUH (OR: 1,56; IC95%: 0,94-2,59). Al ajustar por todas las variables anteriores, la edad ≥ 75 anos se asocia a menor incidencia de algun resultado adverso (recurrencia de CE, revisita al SUH, hospitalizacion y/o fallecimiento) a 30 dias (OR: 0,43; IC95: 0,25-0,77). Conclusiones En el registro ACESUR se identificaron diferencias en la presentacion clinica y en la atencion de pacientes con CE en SUH al comparar longevos frente a jovenes. La edad ≥ 75 anos no se asocia de manera independiente a mayor incidencia de intervencion en el SUH ni a algun resultado adverso a 30 dias del alta.

The main hypothesis for the aetiology of meningoencephalitis of unknown origin (MUO) in dogs is an autoimmune or genetic cause that is associated with a triggering event (environmental factors/infectious agents). The aim of this ambispective cohort study was to test for Leishmania infantum infection in the blood and cerebrospinal fluid (CSF) of dogs with MUO in an endemic area of canine leishmaniosis. Dogs with MUO were selected amongst all dogs undergoing blood anti-L. infantum antibody testing (control group). The blood plasma or serum samples from all dogs were analysed for anti- L. infantum antibodies by a quantitative indirect fluorescent assay (IFAT). In dogs with MUO, CSF samples were obtained for analysed by PCR detection of L. infantum DNA. Forty-four percent and 22% of the dogs in the MUO group featured magnetic resonance imaging (MRI) findings and CSF cytology respectively, consistent with L. infantum infection. IFAT, PCR, and histological findings were negative for L. infantum. A significant difference in L. infantum infection prevalence was found between the control and MUO group (p = 0.0022). While it seems unlikely that L. infantum plays a role in the aetiology of MUO, in endemic areas, this pathogen should be included in the differential diagnosis of this neurological disorder.

- Abstract
Aims: Benzodiazepines have been used as safe anxiolytic drugs for decades and some authors have suggested they could be an alternative for morphine for treating acute cardiogenic pulmonary oedema (ACPE). We compared the efficacy and safety of midazolam and morphine in patients with ACPE.Methods and Results: A randomized, multicentre, open-label, blinded endpoint clinical trial was performed in seven Spanish emergency departments (EDs). Patients >18 years old clinically diagnosed with ACPE and with dyspnoea and anxiety were randomized (1:1) at ED arrival to receive either intravenous midazolam or morphine. Efficacy was assessed by in-hospital all-cause mortality (primary endpoint). Safety was assessed through serious adverse event (SAE) reporting, and the composite endpoint included 30-day mortality and SAE. Analyses were made on an intention-to-treat basis. The trial was stopped early after a planned interim analysis by the safety monitoring committee. At that time, 111 patients had been randomized: 55 to midazolam and 56 to morphine. There were no significant differences in the primary endpoint (in-hospital mortality for midazolam vs. morphine 12.7% vs. 17.9%; risk ratio[RR] 0.71, 95% confidence interval [CI] 0.29-1.74; p = 0.60). SAE were less common with midazolam versus morphine (18.2% vs. 42.9%; RR 0.42, 95% CI 0.22-0.80; p = 0.007), as were the composite endpoint (23.6% vs. 44.6%; RR 0.53, 95% CI 0.30-0.92; p = 0.03).Conclusion: Although the number of patients was too small to draw final conclusions and there were no significant differences in mortality between midazolam and morphine, a significantly higher rate of SAEs was found in the morphine group. People living-with HIV (PLHIV) presented less richness and diversity in the intestinal microbiota than non-infected individuals. Immunopathogenesis of HIV-1 infection has been extensively studied, however the influence of microbiota in those parameters that are key in the pathogenesis are less well characterized. The aim of the current study was to characterize the faecal microbiota composition in a cohort of PLHIV and its relationship with the clinical, metabolic, nutritional, inflammation, bacterial translocation and immunological markers. Baseline faecal samples from seventy-six HIV infected individuals with undetectable viral load (uVL) from a prospective, randomized feeding trial (NCT03846206) were collected and processed. Objective: The health effects of a supplemented Mediterranean diet (SMD) with extra-virgin olive oil (EVOO) and nuts are well documented in non-HIV-infected individuals. We hypothesised that the benefits of an SMD could be mediated by changes in the gut microbiota, even in those with an altered intestinal microbiota such as people living with HIV. Design: Individuals living with HIV (n = 102) were randomised to receive an SMD with 50 g/day of EVOO and 30 g/day of walnuts (SMD group) or continue with their regular diet (control group) for 12 weeks. Methods: Adherence to the Mediterranean diet was assessed using the validated 14-item MD-Adherence-Screener (MEDAS) from the PREDIMED study. A sub-study classifying the participants according to their MEDAS scores was performed. Results: The lipid profile was improved in the SMD group vs. that in the control group (delta-total cholesterol and delta-B-lipoprotein). The immune activation (CD4+HLADR+CD38+ and CD8+HLADR+CD38+ cells) and IFN-γ-producing T-cells significantly decreased at week 12 compared to the baseline in the SMD group but not in the control group. The gut microbiota in those from the high-adherence group presented significantly high diversity and richness at the end of the intervention. Succinivibrio and Bifidobacterium abundances were influenced by the adherence to the MD and significantly correlated with Treg cells. Conclusion: The Mediterranean diet improved metabolic parameters, immune activation, Treg function, and the gut microbiota composition in HIV-1-infected individuals.
