Objetivo: Presentar los resultados de nuestra serie de nefrectomía laparoscópica en pielonefritis xantogranulomatosa (PXG). Método: Se realizó un estudio retrospectivo que incluyó 143 pacientes tratados con nefrectomía laparoscópica por exclusión renal, de los cuales 15 fueron por PXG, en el periodo comprendido de 2011 a 2019. Se recolectaron datos demográficos y clínicos, resultados transoperatorios, complicaciones y días de estancia hospitalaria. Resultados: Se realizó nefrectomía laparoscópica transperitoneal de forma exitosa en 15 pacientes con PXG, sin necesidad de conversión. El tiempo transoperatorio promedio fue de 124.4 minutos (rango: 70-240). El sangrado transoperatorio fue de 148.5 ml (rango: 30-550), sin requerimiento de transfusión sanguínea. No se reportaron complicaciones transoperatorias; se presentó una complicación en el posoperatorio (6.6%) clasificada como Clavien-Dindo I (infección de la herida quirúrgica). La estancia hospitalaria promedio fue de 2.85 días (rango: 2-7). Conclusiones: El manejo definitivo de la PXG es la nefrectomía, y el abordaje laparoscópico debe ser considerado como una modalidad de tratamiento a pesar de ser una patología que presenta un proceso inflamatorio grave y crónico, obteniéndose beneficios como disminución en el tiempo quirúrgico, menor sangrado, menor tasa de complicaciones y menos días de estancia hospitalaria cuando es realizado por un cirujano experimentado.

This article describes the impact of restricting the air intake in industrial 250 m3 WEMCO flotation cells at Los Pelambres concentrator. The influence of air restriction on the hydrodynamic and metallurgical performance of this type of machine was evaluated. The experiments were conducted in single flotation cells and entire rougher banks. In all cases, the gas holdup was measured to estimate the effectiveness of the obstruction system to decrease the air concentration. In single cells, axial profiles for solid percentage and particle size were evaluated. In addition, mass balances were conducted to assess the copper recoveries and concentrate features. In individual cells, air restriction led to a decrease in the gas holdup. However, this slight change was enough to obtain a more stable froth zone and a better solid suspension. The latter was observed as: (i) a higher P80 below the pulp–froth interface, (ii) a less diluted pulp at this level, (iii) a slightly higher Cu recovery, and (iv) a coarser concentrate product. A mineralogical analysis of the concentrate sample also showed the presence of coarser liberated Cu-sulfide particles. The results in single cells suggested an improvement in the recovery of coarse particles via more intense solid suspension. The air intake was also restricted in three rougher banks to assess the impact of air obstruction on the overall performance of the respective circuit. Eleven out of fourteen cells were operated with air restriction, which led to a significant improvement in recovery of 0.9%–1.6% (absolute), at a 95% confidence level. Size-by-size mass balances were also conducted for the rougher circuits, which proved that the recovery improvements were justified by the simultaneous increase in the recovery of coarse and fine particles. Thus, a restriction in the air intake showed that a decrease in the gas holdup (and in the bubble surface area flux) was compensated by better solid suspension and a higher collision efficiency in the draft tube. The former promoted the recovery of coarse particles in the quiescent zone, whereas the latter improved the interaction between bubbles and fine particles. Further developments are being made to implement a regulatory control strategy for the air intake in self-aspirated flotation cells and to use this approach for optimizing industrial flotation banks.

Minerals flotation is a widely used process to produce base metal concentrates through the selective capture of particles on the surface of bubbles. The process performance depends on the size distribution of the bubble population generated by gas dispersion, which is characterized by three variables: superficial gas velocity, gas holdup, and bubble size. A literature review revealed that current instrumentation cannot provide reliable on-line measurement of these variables except for gas velocity. There are some promising alternatives for gas holdup, but bubble size measurement will continue to be unavailable. The use of a model that integrates the three gas dispersion variables makes possible the calculation of one of the variables when knowing the values of the other two. Modelling bubble flow has been pursued using two approaches: determining the bubble terminal velocity reduction by the presence of other bubbles and regarding the bubble swarm as a packed bed through which a fluid is allowed to flow. Models based on these approaches (drift-flux and Molerus, respectively) were found in the literature and used to assess their prediction ability. Gas holdup predictions for known values of gas velocity and bubble size showed similar trends for both models; Molerus results were always higher than those obtained with the drift-flux model, and the difference between both predictions increased with bubble size and gas velocity. A relationship between bubble surface area flux and gas holdup was explored (a single line was expected); Molerus values showed a noticeable effect of bubble size, while drift-flux results showed a minor effect of bubble size only for gas holdups below 10%. Model accuracy was established using a data set collected to characterize frother roles in flotation for six commercial frothers, which included values of the three gas dispersion variables measured simultaneously and reported at the same conditions. The results indicate that bubble size predictions obtained from Molerus model are closer to the measured values than those obtained from the drift-flux model. Drift-flux predictions systematically underestimated the measured bubble size, with relative errors between 10 and 30%, while Molerus predictions showed values around the measured size with relative errors not larger than 15% (and in most cases below 10%). The accuracy of the Molerus predictions is acceptable for applications in the control of individual cells and flotation circuits operation. Further testing to assess the performance of Molerus model with data collected in lab and industrial mechanical cells and columns, where conditions in the test volume may not be stable or homogeneous, is recommended.

Flotation is a complex process that exhibits high dimensionality which makes modeling and optimization very challenging. One technique to alleviate the dimensionality problem is to combine variables together into more informative ones. Bubble surface area flux and air recovery are examples of dimensionality reduction. Gas holdup also captures the effect of a plurality of variables including gas rate, bubble size, surfactant type, and concentration. This work makes use of a dual flotation–conductivity cell to explore the relationship between gas dispersion properties, including frother concentration and flotation performance. Results demonstrate that gas holdup effectively captured the effect of gas rate and frother concentration and better correlates to flotation performance.

Introducción: El uréter retrocavo es una anomalía congénita rara que requiere manejo quirúrgico. Objetivo: Reportar los resultados en el manejo laparoscópico para uréter retrocavo y hacer una revisión bibliográfica. Método: Estudio retrospectivo, descriptivo, transversal, en pacientes con diagnóstico de uréter retrocavo que fueron sometidos a plastia laparoscópica de uréter retrocavo. Resultados: Cuatro pacientes sometidos a plastia laparoscópica por diagnóstico de uréter retrocavo, 1 hombre y 3 mujeres, con una media de edad de 40.7 años. La media del tiempo operatorio fue de 138.2 minutos. La pérdida de sangre cuantificada transoperatoria media fue de 23.7 ml. Ningún paciente presentó complicaciones transoperatorias ni posoperatorias. Los pacientes permanecieron hospitalizados hasta su egreso una media de 2.5 días. Todos los pacientes presentaron mejoría de su sintomatología. Conclusiones: El uréter retrocavo es una condición rara y aún no existe un consenso sobre su diagnóstico, tratamiento y seguimiento. El manejo quirúrgico laparoscópico es una opción para el tratamiento de estos pacientes.

There is a clear need for novel antiviral concepts to control SARS-CoV-2 infection. Based on the promising anti-coronavirus activity observed for a class of 1,4,4-trisubstituted piperidines, we here conducted a detailed analysis of the structure–activity relationship of these structurally unique inhibitors. Despite the presence of five points of diversity, the synthesis of an extensive series of analogues was readily achieved by Ugi four-component reaction from commercially available reagents. After evaluating 63 analogues against human coronavirus 229E, four of the best molecules were selected and shown to have micromolar activity against SARS-CoV-2. Since the action point was situated post virus entry and lying at the stage of viral polyprotein processing and the start of RNA synthesis, enzymatic assays were performed with CoV proteins involved in these processes. While no inhibition was observed for SARS-CoV-2 nsp12-nsp7-nsp8 polymerase, nsp14 N7-methyltransferase and nsp16/nsp10 2’-O-methyltransferase, nor the nsp3 papain-like protease, the compounds clearly inhibited the nsp5 main protease (Mpro). Although the inhibitory activity was quite modest, the plausibility of binding to the catalytic site of Mpro was established by in silico studies. Therefore, the 1,4,4-trisubstituted piperidines appear to represent a novel class of non-covalent CoV Mpro inhibitors that warrants further optimization and development.

Antecedentes: Los quistes suprarrenales son una afección rara. Su diagnóstico y su tratamiento son un desafío debido a la escasa información, lo que genera incertidumbre respecto al mejor manejo. Casos clínicos: Presentamos tres casos de quistes suprarrenales que recibieron tratamiento de mínima invasión con cirugía laparoscópica, sin presentar complicaciones y con una evolución favorable. Discusión: Las nuevas técnicas quirúrgicas de mínima invasión ofrecen una oportunidad terapéutica que permite preservar estructuras y obtener material necesario para su diagnóstico histopatológico, así como la resolución de la sintomatología

Septic arthritis of the facet joint is an extremely rare condition, even more in young immunocompetent patients. There have been approximately only 40 cases of this entity reported worldwide. Here, the authors present a 16-year-old male patient with lower back pain, fever, and cephalea. Blood cultures and cerebrospinal fluid analysis confirmed methicillin-sensitive Staphylococcus aureus infection, and magnetic resonance imaging showed septic arthritis of the lumbar left facet joint L3 and epidural abscess. Intravenous oxacillin was administered, and the patient improved. No other treatment was required.

ABSTRACT Introduction and objective Indwelling double J ureteral stents are used routinely in the resolution of ureteral obstruction caused by different etiologies. Evaluation of urinary symptoms related to double-J stent, indicate that these affect 73-90% of patients. We conducted a prospective, randomized study, to evaluate the efficacy of tamsulosin, oxybutinin and combination therapy in improving the urinary symptoms. Methods Patients who underwent ureteral stent placement after ureterolithotripsy (total 51), were randomized into three groups: Group I: Tamsulosin 0.4 mg. once per day(17 patients), Group II: Oxybutinin 5 mg. once per day (17 patients), Group III: Tamsulosin+ oxybutynin once per day (17 patients). All the groups received the drugs for three weeks and completed a Spanish validated Ureteral Stent Symptom Questionnaire (USSQ) at day 7 and 21. Froth flotation is a process in which hydrophobic particles attach to bubbles forming particle–bubble aggregates, which are transported to the top section of the cell where they overflow to form the concentrate. Particles also reach the concentrate by entrainment, which is a non-selective process that leads to low concentrate grades. The objective of this work was to study and compare the degree of entrainment measured using three methods, in fresh and seawater, and using methyl isobutyl carbinol (MIBC) and DF-250 as frothers. The experimental results indicated that the degree of entrainment decreased in seawater. Besides, water recovery was lower in the experiments with seawater regardless of the frother that was used in the experiments, which correlated with the observed low degrees of entrainment. It is proposed that the reduction of entrainment and water recovery in seawater can be explained by a reduction of air recovery and/or more bubble coalescence in the froth. It is also proposed that surfactant diffusion/convection from the lamellae to the Plateau borders in the froth is reduced in seawater, which reduces the surface tension gradients, and thus the restoring liquid flow from the Plateau borders to the lamellae.

This paper describes a method to predict water recovery rate into and through the foam in a bubble column operating under different gas rates, froth depths, and frother types and concentrations. Three frothers were considered: Metil Isobutil Carbinol (MIBC), a proprietary blend of alcohols, aldehydes, and esters commercialized under the name PINNACLE® 9891, and a PGE-based Dow Froth 1012 (DF1012). The water rate entering into the froth (foam) layer from the bubbly (collection) zone was estimated as the water rate overflowing the column when operating at a thin stable foam layer, i.e., 0.5 cm. It was observed that the rate at which water entered into the froth phase could be modelled as a unique linear function of the gas holdup below the froth, regardless of the frother chemistry. This is a fundamental result not previously found in the literature that also facilitates the calculation of the froth zone water recovery for deeper froths. The water recovery in the froth was found to be an inverse logarithmic function of the average liquid residence time in the froth. Although the same trend was observed for the three frothers tested, they did not converge into a single function, which suggests that frother chemistry plays a role in determining froth structure and then needs to be incorporated when modeling water transport in the froth. Finally, the water overflow rate calculated as the product of the water rate into the froth and froth water recovery predicted the actual measured values fairly well. The water transport model here proposed provides a simple representation of the interactions between collection and froth zone and its relation to easily measure operating variables.

A flotation bank is a serial arrangement of cells. How to optimally operate a bank remains a challenge. This article reviews three reported strategies: air profiling, mass-pull (froth velocity) profiling and Peak Air Recovery (PAR) profiling. These are all ways of manipulating the recovery profile down a bank, which may be the property being exploited. Mathematical analysis has shown that a flat cell-by-cell recovery profile maximizes the separation of two floatable minerals for a given target bank recovery when the relative floatability is constant down the bank. Available bank survey data are analyzed with respect to recovery profiling. Possible variations on recovery profile to minimize entrainment are discussed.

Background: Overweight and obesity, high blood pressure, hyperglycemia, hyperlipidemia, and insulin resistance (IR) are strongly associated with noncommunicable diseases (NCDs), including type 2 diabetes, cardiovascular disease, stroke, and cancer. Different surrogate indices of IR are derived and validated with the euglycemic-hyperinsulinemic clamp (EHC) test. Thus, using a computational approach to predict IR with Matsuda index as reference, this study aimed to determine the optimal cutoff value and diagnosis accuracy for surrogate indices in non-diabetic young adult men. Methods: A cross-sectional descriptive study was carried out with 93 young men (ages 18-31). Serum levels of glucose and insulin were analyzed in the fasting state and during an oral glucose tolerance test (OGTT). Additionally, clinical, biochemical, hormonal, and anthropometric characteristics and body composition (DEXA) were determined. The computational approach to evaluate the IR diagnostic accuracy and cutoff value using difference parameters was examined, as well as other statistical tools to make the output robust. Results: The highest sensitivity and specificity at the optimal cutoff value, respectively, were established for the Homeostasis model assessment of insulin resistance index (HOMA-IR) (0.91; 0.98; 3.40), the Quantitative insulin sensitivity check index (QUICKI) (0.98; 0.96; 0.33), the triglyceride-glucose (TyG)-waist circumference index (TyG-WC) (1.00; 1.00; 427.77), the TyG-body mass index (TyG-BMI) (1.00; 1.00; 132.44), TyG-waist-to-height ratio (TyGWHtR) (0.98; 1.00; 2.48), waist-to-height ratio (WHtR) (1.00; 1.00; 0.53), waist circumference (WC) (1.00; 1.00; 92.63), body mass index (BMI) (1.00; 1.00; 28.69), total body fat percentage (TFM) (%) (1.00; 1.00; 31.07), android fat (AF) (%) (1.00; 0.98; 40.33), lipid accumulation product (LAP) (0.84; 1.00; 45.49), leptin (0.91; 1.00; 16.08), leptin/adiponectin ratio (LAR) (0.84; 1.00; 1.17), and fasting insulin (0.91; 0.98; 16.01). The Liver-Expressed Antimicrobial Peptide 2 (LEAP-2) has emerged as an endogenous GHS-R antagonist and blunts the orexigenic action of ghrelin. This study aimed to determine the Ghrelin/LEAP-2 ratio in humans and rats during pregnancy. In humans, we conducted a nested case-control study within an observational prospective cohort. Healthy and mild preeclamptic pregnant women were studied at each trimester of gestation and three months postpartum. In addition, a group of non-pregnant women was studied into the follicular and luteal phases of the menstrual cycle. Furthermore, Ghrelin/LEAP-2 ratio was investigated in non-pregnant rats and at different periods of rat pregnancy. Human and rat serum ghrelin and LEAP-2 levels were determined using the commercially available ELISA kits. The Ghrelin/LEAP-2 ratio peak around the second trimester of gestation in healthy pregnant women (p < 0.05). Additionally, there were no statistically significant differences in Ghrelin/LEAP-2 ratio between healthy and preeclamptic pregnant women at each trimester of gestation (p > 0.05). The Ghrelin/LEAP-2 ratio in pregnant rat reached the peak around mid-gestation with a similar pattern to the human pregnancy. LEAP-2 was visualized by immunohistochemistry in human term placenta and rat placentas on days 12, 16 and 21 of pregnancy. In conclusion, this study provides the first evidence of a Ghrelin/LEAP-2 ratio peak around the half-way point of pregnancy onwards during human and rat pregnancy, and it might be associated with increased rates of weight gain during pregnancy. Thus, this study suggests that LEAP-2 and Ghrelin/LEAP-2 ratio might play an important role in maternal physiology adaptation of weight gain during pregnancy.
