Resumen Introduccion y objetivos Se presentan las caracteristicas clinicas y los resultados de los trasplantes cardiacos realizados en Espana con la actualizacion correspondiente a 2019. Metodos Se describen las caracteristicas clinicas y los resultados de los trasplantes cardiacos realizados en 2019, asi como las tendencias de estos en el periodo 2010-2018. Resultados En 2019 se realizaron 300 trasplantes (8.794 desde 1984; 2.745 entre 2010 y 2019). Respecto a anos previos, los cambios mas llamativos son el descenso hasta el 38% de los trasplantes realizados en codigo urgente, y la consolidacion en el cambio de asistencia circulatoria pretrasplante, con la practica desaparicion del balon de contrapulsacion (0,7%), la estabilizacion del uso del oxigenador extracorporeo de membrana (9,6%) y el aumento de los dispositivos de asistencia ventricular (29%). La supervivencia en el trienio 2016-2018 es similar a la del trienio 2013-2015 (p = 0,34), y ambas mejores que la del trienio 2010-2012 (p = 0,002 y p = 0,01 respectivamente). Conclusiones Se mantienen estables tanto la actividad del trasplante cardiaco en Espana como los resultados en supervivencia en los ultimos 2 trienios. Hay una tendencia a realizar menos trasplantes urgentes, la mayoria con dispositivos de asistencia ventricular.

Survival from 2016 to 2018 was similar to that from 2013 to 2015 (P = .34). Survival in both these periods was better than that from 2010 to 2012 (P = .002 and P = .01, respectively). Conclusions: Heart transplant activity has remained stable during the last few years, as have outcomes (in terms of survival). There has been a trend to a lower rate of urgent transplants and to a higher use of ventricular assist devices prior to transplant.

INTRODUCTION AND OBJECTIVES The present report updates the characteristics and results of heart transplantation in Spain, mainly focused in the 2008-2017 period. METHODS We describe the recipient and donor characteristics, surgical procedures, and outcomes of heart transplants performed in 2017. The 2017 data were compared with those obtained from 2008 to 2016. RESULTS A total of 304 cardiac transplants were performed in 2017. Between 1984 and 2017, 8173 procedures were performed, 2689 of them after 2008. Significant temporal trends were observed in recipient characteristics (lower pulmonary vascular resistance, lower use of mechanical ventilation, and a higher percentage of diabetic patients and those with previous cardiac surgery), donor characteristics (older donor age and a higher percentage of female donors and those with a prior cardiac arrest) and procedures (lower ischemia time). Introduccion y objetivos El principal objetivo de este estudio es demostrar que un protocolo para el manejo del sincope en el servicio de urgencias basado en la deteccion precoz de cardiopatia permite diagnosticar a los pacientes de forma rapida y con una baja proporcion de ingresos, sin que ello repercuta de forma negativa en su pronostico. Metodos El estudio se realizo de forma prospectiva en 199 pacientes consecutivos (54% varones; media de edad, 67 ± 17 anos) que acudieron por sincope al servicio de urgencias de nuestro hospital en un periodo de 17 meses. Se desarrollo un algoritmo diagnostico en dos pasos en el que los pacientes eran sometidos a una primera evaluacion clinica y electrocardiografica y posteriormente a un protocolo diagnostico basado en la realizacion secuencial de pruebas diagnosticas en el servicio de urgencias, con lo que se evito su ingreso hospitalario. Resultados Tras la valoracion inicial, se diagnostico a 120 (60%) pacientes. Despues de completado el protocolo, se obtuvo clasificacion diagnostica del 78% de los pacientes, con una estancia media en el servicio de urgencies de 19 ± 15 h; se ingreso al 10% de los pacientes. Con un seguimiento medio de 237 dias, fallecieron 3 pacientes. Conclusiones El manejo protocolizado del sincope en el servicio de urgencias basado en una estratificacion de los pacientes segun tengan o no cardiopatias permite el diagnostico de una elevado porcentaje de pacientes y dar de alta a la mayoria de form a rapida desde el mismo servicio de urgencias, sin que ello repercuta de forma negative en su pronostico a medio plazo.

Introduccion y objetivos Los stents de rapamicina (SR) han demostrado reducir la tasa de reestenosis en multiples estudios, pero se han descrito algunos casos de reestenosis y trombosis tardias que proyectan dudas sobre sus resultados a largo plazo (> 2 anos). Nos planteamos estudiar retrospectivamente la evolucion a largo plazo de pacientes tratados con SR en lesiones de alto riesgo de reestenosis. Pacientes y metodo Desde su introduccion, hemos utilizado SR en los casos con lesiones de mayor riesgo de reestenosis. Estudiamos la evolucion clinica de los pacientes en los que se implantaron SR y que cuentan con un seguimiento clinico superior a 2 anos. Resultados Se estudio a 200 pacientes (edad 60 ± 11 anos, un 22% diabeticos) tratados entre junio de 2002 y abril de 2003 en 309 lesiones: un 16%, oclusiones totales; un 16,8%, reestenosis intra- stent ; un 28%, difusas, y un 30%, en vaso pequeno. La longitud total de stent por paciente fue de 29 ± 16 mm y el diametro del stent, de 2,78 ± 0,27 mm. En el seguimiento clinico de 29 ± 3,2 meses se produjeron los siguientes eventos: 4 muertes, de las que 2 (1%) fueron cardiacas; 4 infartos (2%); 4 trombosis de stent documentadas (2%), todas tardias a los 3, 7, 26 y 31 meses; 4 casos (2%) de revascularizacion por reestenosis del segmento tratado a los 3, 5, 14 y 15 meses, y 6 casos (3%) de revascularizacion de otra lesion. Conclusiones La evolucion a largo plazo de los pacientes con SR en lesiones de alto riesgo reestenotico muestra una tasa de reestenosis clinica muy baja. La incidencia de trombosis tardia parece resultar algo elevada y deberia ser evaluada en series mas amplias.

However, reports of late cases of thrombosis and restenosis have raised questions about long-term outcome in patients treated with these stents. Our aim was to evaluate long-term outcome in patients undergoing SES placement in lesions at a high risk of restenosis. Patients and Methods Since SESs became available, we have used them to treat lesions at risk of restenosis. We studied clinical outcomes in consecutive patients treated with SESs who were followed up for more than 2 years. Results The study included 200 patients (age 60 [11] years, 22% diabetics) who were treated between June 2002 and April 2003 for 309 lesions: 16% were total occlusions, 16.8% in-stent restenoses, 28% diffuse lesions, and 30% small-vessel lesions. The total stent length per patient was 29 (16) mm and the mean diameter was 2.78 (0.27) mm. In a mean clinical follow-up period of 29 (3.2) months (range 24–34 months), there were 4 deaths, 2 (1%) of which were cardiac, 4(2%) non-fatal infarctions, 4 (2%) in-stent thromboses (all occurred late, at 3, 7, 26 and 31 months), 4 (2%) cases requiring target lesion revascularization (at 3, 5, 14, and 15 months), and 6,(3%) requiring revascularization of a new lesion. Conclusions Long-term follow-up of patients undergoing SES placement in lesions at a high risk of restenosis revealed a very low restenosis rate. However, the incidence of late thrombosis appeared to be elevated and warrants further evaluation in larger studies.

Introduction and objectives. The main aim of this study was to demonstrate that a protocol for managing syncope in the emergency department that is based on the early detection of heart disease enables patients to be diagnosed quickly and with few admissions, without there being a negative impact on prognosis. Methods. The study was performed prospectively in 199 consecutive patients (54% male; mean age, 67 [17] years) who presented with syncope at the emergency department of our hospital during a 17-month period. A 2 step diagnostic algorithm was developed in which patients initially underwent clinical and electrocardiographic assessment, and thereafter were submitted to a diagnostic protocol that involved carrying out a sequence of diagnostic tests in the emergency room to avoid hospital admission. Results. A presumptive diagnosis was established in 120 (60%) patients during the initial assessment. After completion of the diagnostic protocol, 78% of patients were given a clear diagnosis within a mean stay of 19 [15] h in the emergency department. Some 10% of patients were admitted. Three patients died during a mean followup period of 237 days. Conclusions. Implementation of a protocol for managing syncope in the emergency department that was based on screening patients according to the presence of heart disease enabled a large percentage to receive a diagnosis. Moreover, the majority of patients could be discharged rapidly from the emergency department without there being any negative impact on medium-term prognosis.

Pulmonary toxicity (PT) is emerging as a frequent and serious complication of sirolimus, a proliferation signal inhibitor (PSI) used in solid-organ transplantation. Everolimus is a more recently developed PSI with molecular structure very similar to that of sirolimus. Surprisingly, although experience with everolimus is increasing and becoming substantial, there remains very little information about everolimus-related PT. Herein we report 2 heart transplant recipients who developed a non-infectious pulmonary syndrome after everolimus treatment was started. Transbronchial pulmonary biopsy specimens showed typical interstitial pneumonitis, and everolimus discontinuation resulted in rapid clinical and radiological improvement. Although PT seems to be more common after sirolimus exposure, everolimus is by no means spared from this potentially lethal complication and should always be suspected in the relevant clinical setting.

Cardiac cephalgia, or headache occurring as manifestation of myocardial ischemia, has only recently been recognized as a distinct entity. In patients with known ischemic cardiopathy, its diagnosis depends on the presence of severe headache that is accompanied by nausea, worsened by physical exercise, and only ceases with nitrate administration. We report on two patients who met diagnostic criteria for this entity. In both, headache was the only symptom of coronary ischemia, and delayed its diagnosis. Headache occurred both at rest and during exertion, and resolved only after the administration of nitrates. Cardiac cephalgia should be suspected in patients with a history of ischemic cardiopathy who present with de novo headache, even when thoracic pain is absent, especially if the headache improves with nitrates. Differential diagnosis with migraine is crucial to avoid the administration of vasoconstrictors.
