Abstract

One of the determinants of the capture threshold of an endocardial pacing lead is the configuration of the electrode tip. To evaluate whether micro- and macroporous electrodes have better initial and chronic thresholds than nonporous electrodes, acute and chronic capture thresholds, stimulation impedance and sensing thresholds were determined in 22 patients in whom a ventricular ring-tip electrode and a unipolar, dual chamber pacemaker with bidirectional telemetry had been implanted. These values were compared to those obtained from 25 patients receiving an electrode constructed with a platinized groove surface at the time of implant of an identical pulse generator. The ventricular capture threshold at implant was 0.7 V ±0.3 at 0.6 msec pulse width/or both groups. The capture threshold was significantly greater in the ring tip electrode group at follow-up periods of 1 month (1.1V ±0.5vs 1.6 V ±0.6, P < 0.008), 4 months (1.0 V ±0.2 vs 1.7 V ±0.8. P < 0.002), and 10 months (1.2 V ±0.4 vs 1.7 V ±0.5, P < 0.04) following implantation. The stimulation impedance at the time of implantation was lower in the ring-tip electrode group (530 ohms vs 603 ohms, P < 0.03), but thereafter no significant difference was seen between the two groups. The acute and chronic sensing thresholds were similar in both groups. While the microporous electrode had significantly lower chronic capture thresholds, the magnitude of this difference is small, and probably clinically inconsequential. Since three patients with ring-tip electrodes and two patients with microporous electrodes had at least one chronic threshold exceeding 2.5 V, periodic threshold determination is prudent in all patients. Abstract

A patient having high grade AV block with intact sinus node function is presented in whom DDDR pacing provided the benefit of preventing 2:1 pacemaker block in response to exercise-induced sinus tachycardia. In paired treadmill tests with the patient blinded as to pacing mode, she was able to exercise longer (7.5 vs 6.6 METS] when programmed in DDDR than in DDDO. This is attributable to circumvention of 2:1 pacemaker block which had resulted in abrupt onset of fatigue and SOB (shortness of breath) when the sinus rate exceeded the maximum tracking rate of 130/min. Outpatient ambulatory electrocardiographic monitoring confirmed this phenomenon during relatively strenuous activity. The theoretic advantages of dual chamber rate modulated pacing compared to the DDDO and WIH modes are discussed. [ABSTRACT FROM AUTHOR]

Abstract

ObjectivesThis trial examined the relative clinical efficacy, angiographic outcomes, and safety of zotarolimus-eluting coronary stents (ZES) with a phosphorylcholine polymer versus sirolimus-eluting stents (SES).BackgroundWhether a cobalt-based alloy stent coated with the novel antiproliferative agent, zotarolimus, and a phosphorylcholine polymer may provide similar angiographic and clinical benefit compared with SES is undetermined.MethodsA prospective, multicenter, 3:1 randomized trial was conducted to evaluate the safety and efficacy of ZES (n = 323) relative to SES (n = 113) in 436 patients undergoing elective percutaneous revascularization of de novo native coronary lesions with reference vessel diameters between 2.5 mm and 3.5 mm and lesion length ≥14 mm and ≤27 mm. The primary end point was 8-month angiographic in-segment late lumen loss.ResultsAngiographic in-segment late lumen loss was significantly higher among patients treated with ZES compared with SES (0.34 ± 0.44 mm vs. 0.13 ± 0.32 mm, respectively; p < 0.001). In-hospital major adverse cardiac events were significantly lower among patients treated with ZES (0.6% vs. 3.5%, p = 0.04). In-segment binary angiographic restenosis was also higher in the ZES cohort (11.7% vs. 4.3%, p = 0.04). Total (clinically and non-clinically driven) target lesion revascularization rates at 9 months were 9.8% and 3.5% for the ZES and SES groups, respectively (p = 0.04). However, neither clinically driven target lesion revascularization (6.3% zotarolimus vs. 3.5% sirolimus, p = 0.34) nor target vessel failure (12.0% zotarolimus vs. 11.5% sirolimus, p = 1.0) differed significantly.ConclusionsCompared with SES, treatment with a phosphorylcholine polymer-based ZES is associated with significantly higher late lumen loss and binary restenosis at 8-month angiographic follow-up.(The Endeavor III CR; http://clinicaltrials.gov/ct/show/NCT00265668?order=1?)

26. Double vs Single Balloon Technique for Aortic Balloon Valvuloplasty

Abstract

Percutaneous aortic valvuloplasty using a single dilating balloon has been associated with significant but modest reduction in transvalvular pressure gradient and increase in valve area. The balloon diameter is usually 20 mm or smaller to avoid disruption of aortic root structure and to permit forward blood flow during inflation. To evaluate the safety and efficacy of valvuloplasty using a combination of balloons with larger maximum inflated diameters, we compared results of aortic valvuloplasty in 21 patients using either the single or double balloon technique. Mean maximum inflated balloon diameter was 19.4 mm ± 1.4 for the single balloon technique, while the mean sum of diameters for the simultaneous double balloon technique was 36.3 mm ± 3.9. The mean age, aortic annulus diameter, and predilatation aortic valve area were not different among groups. Mean aortic transvalvular gradient reduction and mean aortic valve area increase were greater for the double balloon technique. The procedure was well tolerated with no major complications. No change in the degree of aortic regurgitation was noted. The double balloon technique for aortic valvuloplasty is safe and more effective at improving aortic valve area and transvalvular gradient than the conventional single balloon technique.

Abstract

Because it is uncertain whether high-pressure stent deployment may contribute to more exuberant neointimal proliferation, a new stent, the LP , was constructed to allow full expansion at 10 atmospheres (atm). We compared in a randomized trial the 6-month target vessel revascularization (TVR) and other clinical outcomes of the LP stent with the most commonly used Food and Drug Administration-approved stent (Guidant Duett and Tristar stents) in 1,003 patients without a recent myocardial infarction. The first 150 patients assigned the LP stent also underwent repeat angiography at 6 months. Baseline characteristics for the two groups were balanced, except for an excess of unstable angina in the LP group. There was slight excess in failure to deliver the LP stent (3.4% versus 1.4%; p = 0.04), and similar 7-day rates of procedural success without ischemic complications (92% versus 93%; p = 0.53). More patients in the LP group had pre-dilation (72% versus 58%; p < 0.001) and post-dilatation (61% versus 50%; p = 0.001). The stent deployment pressure was significantly lower, as expected, in the LP group (10 2 ATM versus 13 4 atm; p < 0.001). At 30 days, the incidence of major adverse cardiac events was similar. At 6 months, the incidence of TVR was 10% and 7.8%, respectively (p = 0.28), hazard ratio was 0.79 (0.52 1.21), and absolute difference was 2.2% (-2.3% to 6.7%), well within the range of equivalency set at 7.5%. Patients without post-dilatation had a significantly higher rate of revascularization with the LP stent than with the Guidant stent (p = 0.02). Thus, the new LP stent achieves rates of revascularization that are not inferior to the Guidant stent, but this effect cannot be linked to lower-pressure deployment.
