7. Monitoring Wetlands in a Salinizing Landscape: Case Studies from the Wheatbelt Region of Western Australia

Abstract

Three elements of wetland biodiversity (aquatic invertebrates, waterbirds and overstorey vegetation of the wetland edge) have been monitored since 1998 at Lake Eganu and Paperbark Swamp in the Western Australian Wheatbelt to provide information about the changes occurring in wetland biodiversity in a landscape that is severely affected by dryland salinization. Changes in extent of wetland vegetation since the 1960s were examined using historical aerial photographs and waterbird use of Lake Eganu during the early 1980s was compared with recent waterbird survey results. Lake Eganu, which is within a major drainage line, started to become salinized in the mid-1960s, about 70 years after land clearing began in the catchment, and its salinity has increased an order of magnitude. The extent of wetland overstorey vegetation and the richness of freshwater aquatic invertebrates have both declined about 80%. Waterbird richness has also declined over the past 20 years, with changes in species composition. Salinization has not occurred at Paperbark Swamp, which is in a small catchment off the main drainage line, and there has been no consistent change in the biodiversity elements monitored

30. Exogenous substrate preference of the post-ischaemic myocardium.

Abstract

Myocardial exogenous substrate preference was studied under conditions of increased plasma lactate concentration before and after a severe (halving of tissue ATP concentration, sixfold increase in tissue lactate concentration) but reversible (<1% necrosis on reperfusion) global ischaemic stress produced by continuous hypothermic electromechanical arrest of the heart of four hours' duration by aortic cross clamping and multidose potassium cardioplegia. Fatty acid oxidation was studied using l-l4C-palmitate under steady state conditions and under similar isovolumic fixed pressure conditions with the heart at a constant rate using a left ventricular intracavitary balloon. Exogenous free fatty acid oxidation during the pre-ischaemic period with an increased lactate concentration (3.9-5.8 mmol·litre−1) was 0.62(0.21) μmol·min−1·100 g−1 (mean (SEM)). This represented a mean of 32% of the total carbon dioxide produced in contrast to a post-ischaemia free fatty oxidation rate of 2.67(0.87) μmol·min−1·100 g−1, in the presence of even further increased plasma lactate concentrations (8.47-11.17 mmol·litre−1), representing a mean of 82% of the total carbon dioxide output. These data suggest that the substrate preference of the myocardium, under conditions of increased plasma lactate concentration, shifts to greater oxidation of exogenous free fatty acids after ischaemic stress. 34. Morbidity outcome in early versus conventional tracheal extubation after coronary artery bypass grafting: A prospective randomized controlled trial

Abstract

Introduction: We undertook a prospective, randomized, controlled clinical trial to evaluate morbidity outcomes and safety of a modified anesthetic technique to provide shorter sedation and early extubation (1 to 6 hours) than those of the conventional anesthetic protocol used for prolonged sedation and extubation (12 to 22 hours) in patients after coronary artery bypass grafting. Methods: One hundred twenty patients undergoing elective coronary artery bypass grafting were prospectively assigned randomly to either an early extubation group (n = 60; 15 @mg.kg^-^1 fentanyl and 2 to 6 mg . kg^-^1. hour^-^1 propofol and isoflurane) or to a conventional extubation group (n = 60; 50 @mg.kg^-^1 fentanyl and 0.1 mg.kg^-^1 midazolam and isoflurane). Cardiac morbidity (postoperative myocardial ischemia, postoperative myocardial infarction, and perioperative sympathoadrenal stress response), respiratory morbidity (postextubation apnea, alveolar-arterial oxygen gradient, pulmonary shunting, oxygen consumption, atelectasis, and reintubation), hemodynamic values and vasoactive medication requirements, intraoperative awareness, postoperative cognitive function, 30 day mortality, and intensive care unit and hospital lengths of stay were compared between the two groups. Results: Fifty-one of the 60 patients in each group (85%) were extubated within the defined time period. Postoperative extubation time and intensive care unit and hospital lengths of stay were significantly shorter in the early group. At 48 hours after operation, there were no significant differences between the two groups in myocardial ischemia incidences, ischemia burdens, or creatine kinase isoenzyme MB levels. Four patients in the conventional group, but not in the early group, had postoperative myocardial infarction. The extubation anesthetics used were effective in suppressing the perioperative plasma catecholamine stress response in both groups. Postextubation apnea characteristics were similar between the groups. Intrapulmonary shunt fraction improved significantly in the early group at 4 hours after extubation. The incidences and degree of atelectasis did not differ significantly between the two groups. The incidences of treated postoperative complications were comparable between the two groups, but three patients in the conventional group died as a result of stroke or postoperative myocardial infarction. Conclusion: Early extubation after coronary artery bypass grafting is safe and does not increase perioperative morbidity. 38. Myocardial free-radical injury after cardioplegia

Abstract

Although cold blood cardioplegia provides excellent myocardial protection for elective coronary bypass surgery, myocardial metabolic recovery is delayed postoperatively, perhaps because of free-radical injury during reperfusion. To assess free-radical reperfusion injury, we measured the products of lipid peroxidation and the cardiac concentrations of alpha tocopherol in 10 patients undergoing elective surgical revascularization. Arterial and coronary sinus blood measurements revealed a delayed recovery of myocardial oxygen consumption and lactate utilization and the myocardial release of conjugated dienes (chemical signatures of free-radical injury) at 3 and 60 minutes after reperfusion. In addition, myocardial concentrations of alpha tocopherol decreased after reperfusion, suggesting consumption of the major membrane antioxidant. These results support the hypothesis that oxygen-derived free radicals contribute to myocardial injury after cardioplegic arrest and that antioxidant therapy should improve myocardial protection.
