Abstract

The Questionnaire of Cognitive and Affective Empathy (QCAE) is a tool for self-assessing the cognitive and emotional components of empathy. A study showed that a two-factor model fits the data of patients with schizophrenia, whereas other reports on healthy subjects have suggested a five-factor decomposition. We aimed to replicate the model of Horan et al. in a French population with schizophrenia spectrum disorders (i.e., schizophrenia and schizoaffective disorders) participating in the EVACO Study (NCT02901015). In total, 133 patients were assessed with the QCAE, the Positive and Negative Symptom Scale (PANSS), the Personal and Social Performance Scale (PSP), and the Self rating Quality of Life Scale (S-QoL). The two-factor model demonstrated an adequate fit with the data, comparable to that reported by Horan et al. Males scored higher on the Affective subscore than females. After correction for multiple tests, psychopathology (PANSS) and functioning (PSP) did not correlate significantly with the QCAE subscores. However, quality of life (S-QoL) correlated positively with the Emotional Contagion subscore. Thus, the variability of empathetic disposition in schizophrenia may be considered through the cognitive versus affective dichotomy and properly investigated with the QCAE. The results support further investigation of the relationship between QCAE scores and subjective outcome measurements, such as quality of life, and emphasize the importance of cross-cultural comparisons.

Abstract

BackgroundThe determinants of quality of life (QoL) in schizophrenia are largely debated, mainly due to methodological discrepancies and divergence about the concepts concerned. As most studies have investigated bi- or tri-variate models, a multivariate model accounting for simultaneous potential mediations is necessary to have a comprehensive view of the determinants of QOL. We sought to estimate the associations between cognitive reserve, cognition, functioning, insight, depression, schizophrenic symptoms, and QoL in schizophrenia and their potential mediation relationships.MethodsWe used structural equation modeling with mediation analyses to test a model based on existing literature in a sample of 776 patients with schizophrenia from the FondaMental Foundation FACE-SZ cohort.ResultsOur model showed a good fit to the data. We found better functioning to be positively associated with a better QoL, whereas better cognition, better insight, higher levels of depression, and schizophrenic symptoms were associated with a lower QoL in our sample. Cognitive reserve is not directly linked to QoL, but indirectly in a negative manner via cognition. We confirm the negative relationship between cognition and subjective QoL which was previously evidenced by other studies; moreover, this relationship seems to be robust as it survived in our multivariate model. It was not explained by insight as some suggested, thus the mechanism at stake remains to be explained.ConclusionThe pathways to subjective QoL in schizophrenia are complex and the determinants largely influence each other. Longitudinal studies are warranted to confirm these cross-sectional findings.

Abstract

Aim: The main purpose of this feasibility study was to estimate compliance and acceptability rate of a 9-Steps mobile app preparation program. Subsequently, to assess its impact regarding attitudes towards smoking cessation. Methods: The program was only available on versions above iOS 13.3. The ecological study collected user’s data via the app from 04/07/2021 to 28/07/2021. Results: A total of 2331 ecological users completed onboarding; 1926 users (73.9%) completed the step 0 and only 265 users continued in step 1. On average, participants actively engaged with intra-steps activities (85,1%). Most users were highly satisfied at the end of each step. On average 86% found the content of each step comprehensive and relevant. Abstract

The increased prevalence and adverse health consequences of obesity have made it one of the leading public health issues in recent years. Importantly, several epidemiological studies have revealed significant associations between BMI and organic food consumption. However, although these studies have suggested that this factor holds promise to prevent obesity, they all suffer from methodological limitations, including self-reporting methods to assess BMI, not controlling for potential confounding factors or using a non-representative sample. Moreover, all were restricted to an adult sample. We present the results of a cross-sectional epidemiological study assessing the association of organic food consumption with BMI and obesity in a representative lifespan French sample (INCA3 study). Objective methods were used to measure BMI, and several potentially confounding variables were controlled for. In total, 1775 children and adolescents and 2121 adults underwent anthropometric measurements and completed questionnaires concerning their dietary habits and lifestyle. Unadjusted models systematically revealed negative associations between organic food consumption and both BMI and obesity across all age groups. These associations tended to remain statistically significant even after controlling for several confounding variables concerning socio-economic status, quality of the diet and physical activity. The effect sizes were, however, small. These data confirm the association between organic food consumption and obesity during both childhood and adulthood. Evidence from randomised controlled trials is required to investigate causality between organic food consumption and lower BMI or obesity rate.

Abstract

Aims Establishing the minimum clinically important difference (MCID) in functioning and cognition is essential to the interpretation of the research and clinical work conducted in bipolar disorders (BD). The present study aimed to estimate the MCID for the Functioning Assessment Short Test (FAST) and a battery of neuropsychological tests in BD. Methods Anchor-based and distributive methods were used to estimate the MCID for the FAST and cognition using data from a large, multicentre, observational cohort of individuals with BD. The FAST and cognition were linked with the Clinical Global Impressions Scale-Severity (CGI-S) and Global Assessment of Functioning (GAF) using an equipercentile method. The magnitude of the standard error measurement (s.e.m.) provided another estimate of the MCID. Results In total, 570 participants were followed for 2 years. Cross-sectional CGI-S and GAF scores were linked to a threshold ⩽7 on the FAST for functional remission. The MCID for the FAST equalled 8- or 9-points change from baseline using the CGI-S and GAF. One s.e.m. on the FAST corresponded to 7.6-points change from baseline. Cognitive variables insufficiently correlated with anchor variables (all ρ s.e.m. for cognitive variables corresponded to a range of 0.45 to 0.93-s.d. change from baseline. Conclusions These findings support the value of the estimated MCID for the FAST and cognition and may be a useful tool to evaluate cognitive and functional remediation effects and improve patient functional outcomes in BD. The CGI-S and GAF were inappropriate anchors for cognition. Further studies may use performance-based measures of functioning instead.

Abstract

Objectives: The aim of this study is to design a questionnaire, the Versailles Metacognitive Strategies Evaluation Questionnaire, for assessing the use of metacognitive and help-seeking strategies in three key-domains of impaired daily functioning in schizophrenia. To evaluate its psychometric properties (internal consistency, factor structure, convergent and divergent validity, and stability). Design: Development of a questionnaire and psychometric validation procedure in patients with schizophrenia compared with healthy controls. Stability over one year was assessed in the patient group. Setting: Schizophrenia Centers of Expertise (French FondaMental Network). Subjects: A total of 141 patients with schizophrenia, among whom 77 participated in the second evaluation; 97 healthy subjects. Main measures: The Versailles Metacognitive Strategies Evaluation Questionnaire, Positive and Negative Symptoms Scale, Personal and Social Performance Scale, Evaluation of Cognitive Processes involved in Disability in Schizophrenia Scale, Schizophrenia Quality of Life Questionnaire, and Stages of Recovery Instrument. Results: From the 36-items version, stepwise exploratory factor analysis (oblimin) produced a 25-items scale which had a 3-factors structure (hygiene concern, social relationships, and hygiene help-seeking). Cronbach’s were respectively equal to 0.91, 0.82, and 0.78. One-year stability was good (intra-class correlation coefficient = 0.7). The three factors showed good convergent validity with measures of quality of life (rho = 0.34, P ⩽ 0.001). The first two factors correlated with recovery ( N = 34, rho = 0.53, P ⩽ 0.001). On the contrary, the factors exhibited divergent validity, with no significant correlation, with symptoms and cognitive and psychosocial functioning ( P > 0.05). Factor structure in healthy controls did not match with that of patients, all items but one were found significantly different among groups. Conclusion: The Versailles Metacognitive Strategies Evaluation Questionnaire provides a simple and valid means to assess metacognitive strategies in individuals with schizophrenia.

Abstract

International audience; Background: Up to half of the patients with schizophrenia attempt suicide during their lifetime. Better insight is associated with better functioning but also with increased suicidality. The direction of the relationship between insight and suicidality is not clear, hence we aimed to provide new elements using structural equation modeling. Methods: Insight, quality of life (QoL), depression, and suicidality were measured at baseline and at 12 months in individuals with schizophrenia spectrum disorders. The relationships between these variables were investigated by latent difference score models, controlling for chlorpromazine doses, positive and negative symptoms, and general psychopathology. Results: 738 patients were included, and 370 completed the study. Baseline levels of insight predicted changes in suicidality, whereas baseline levels of suicidality did not predict changes in insight, suggesting that better insight underlies suicidality and predicts its worsening. Our results suggest this temporal sequence: better insight → worse QoL → increased depression → increased suicidality, while insight also affects the three variables in parallel. Conclusion: Better insight predicts a worsening of QoL, depression and suicidality. These findings contribute to our global understanding of the longitudinal influence of insight on suicidality. We advocate that insight-targeted interventions should not be proposed without the monitoring of depression and suicide prevention.

Background Rapid eye movement sleep behavior disorder and sleepiness precede or accompany idiopathic Parkinson's disease (PD), but their presence in subjects with leucine-rich repeat kinase 2 mutations is unknown. Methods Ten patients with leucine-rich repeat kinase 2-associated PD, four healthy leucine-rich repeat kinase 2 mutation carriers, 20 patients with idiopathic PD, and 12 healthy controls underwent clinical assessments and a nighttime video-polysomnography. Results No sleep changes, no rapid eye movement sleep behavior disorder, or rapid eye movement sleep without atonia was found in the 14 subjects with leucine-rich repeat kinase 2mutations compared with controls, whereas 41% of patients with idiopathic PD had rapid eye movement sleep behavior disorder. Eventually, 20% of patients with leucine-rich repeat kinase 2–associated PD had abnormal periodic leg movements, a frequency similar to the idiopathic PD group frequency. Conclusions The sleep phenotype in leucine-rich repeat kinase 2 mutations parallels that of idiopathic PD, except for absent rapid eye movement sleep behavior disorder here in the presymptomatic and symptomatic stages. © 2015 International Parkinson and Movement Disorder Society

Abstract

International audience; Idiopathic rapid eye movement sleep behaviour disorder is characterized by nocturnal violence, increased muscle tone during rapid eye movement sleep and the lack of any other neurological disease. However, idiopathic rapid eye movement sleep behaviour disorder can precede parkinsonism and dementia by several years. Using 3 T magnetic resonance imaging and neuromelanin-sensitive sequences, we previously found that the signal intensity was reduced in the locus coeruleus/subcoeruleus area of patients with Parkinson’s disease and rapid eye movement sleep behaviour disorder. Here, we studied the integrity of the locus coeruleus/subcoeruleus complex with neuromelanin-sensitive imaging in 21 patients with idiopathic rapid eye movement sleep behaviour disorder and compared the results with those from 21 age- and gender-matched healthy volunteers. All subjects underwent a clinical examination, motor, cognitive, autonomous, psychological, olfactory and colour vision tests, and rapid eye movement sleep characterization using video-polysomnography and 3 T magnetic resonance imaging. The patients more frequently had preclinical markers of alpha-synucleinopathies, including constipation, olfactory deficits, orthostatic hypotension, and subtle motor impairment. Using neuromelanin-sensitive imaging, reduced signal intensity was identified in the locus coeruleus/subcoeruleus complex of the patients with idiopathic rapid eye movement sleep behaviour. The mean sensitivity of the visual analyses of the signal performed by neuroradiologists who were blind to the clinical diagnoses was 82.5%, and the specificity was 81% for the identification of idiopathic rapid eye movement sleep behaviour. The results confirm that this complex is affected in idiopathic rapid eye movement sleep behaviour (to the same degree as it is affected in Parkinson’s disease). Neuromelanin-sensitive imaging provides an early marker of non-dopaminergic alpha-synucleinopathy that can be detected on an individual basis.

11. Le trouble comportemental en sommeil paradoxal est-il un marqueur pré-symptomatique chez les sujets mutés LRRK2 (syndrome parkinsonien juvénile) ?

Abstract

Objectif Le trouble comportemental en sommeil paradoxal (TCSP) est fortement associe a la maladie de Parkinson idiopathique (MPi) : il la precede dans 30 % des cas et l’accompagne dans 60 % des cas. Les mutations du gene LRRK2 sont rares et induisent un syndrome parkinsonien juvenile dont l’association avec la synucleine n’est pas encore claire. Ces mutations induisent-elles un TCSP chez les sujets mutes pre-symptomatiques ou symptomatiques ? Methodes Nous avons inclus 15 sujets mutes sur LRRK2 (11 parkinsoniens, 4 porteurs sains), 22 parkinsoniens idiopathiques, et 17 temoins sains apparies en âge et en sexe. Ils ont beneficie d’une videopolysomnographie, avec mesure du pourcentage de sommeil paradoxal sans atonie, et d’echelles motrices (UPDRS motrice OFF). Resultats Il n’y avait pas de TCSP ou de pourcentage de sommeil paradoxal sans atonie superieur a 15 % ni chez les porteurs sains et ni chez les porteurs symptomatiques d’une mutation du gene LRRK2, alors que les patients MPi avaient un TCSP dans 41 % des cas, et presentaient plus d’activite tonique en sommeil paradoxal que les parkinsoniens LRRK2 (p Conclusion Sur cet echantillon, le controle de l’atonie musculaire en sommeil paradoxal ne semble pas associe aux lesions induites par les mutations du gene LRRK2.

Objectif Etablir un lien entre la perte neuronale dans le complexe du locus coeruleus-subcoeruleus (LC/LSC, responsable de l’atonie en sommeil paradoxal chez l’animal) et la presence d’un TCSPi, une parasomnie souvent inauguratrice d’une synucleinopathie. Methodes Les patients avec TCSPi et les volontaires sains apparies en âge et en sexe ont beneficie d’une IRM encephalique 3 T avec sequence sensibles a la neuromelanine (paramagnetique, hyperintense en T1), en comparant le signal des 10 voxels d’intensite maximale dans la region du LC/LSC au signal de 10 voxels de reference dans le pont. La neuromelanine est contenue dans les neurones catecholaminergiques du LC/LSC, ainsi un signal faible reflete une perte de ces neurones. Les participants ont aussi beneficie d’une evaluation cognitive (MoCA), psychologique (HAD-RS), sensorielle (vision des couleurs et olfaction), et motrice (UPDRS, tests de dexterite) ainsi que d’une video-polysomnographie pour calculer le pourcentage de sommeil paradoxal sans atonie musculaire. Resultats Le signal du LC/LSC etait moins intense chez les 16 patients avec TCSPi que chez les 16 temoins (p = 0,001) sur les images ponderees en T1. L’intensite du signal IRM correlait avec la perte d’atonie en sommeil paradoxal (r = –0,52). Les patients avaient aussi plus de troubles olfactifs (p = 0,005), autonomiques (hypotension orthostatique : p = 0,035, selles par semaine : p = 0,016) et un score UPDRS-3 plus eleve (p = 0,002) que les temoins, sans difference aux niveaux cognitif, psychologique, visuel ou aux tâches de dexterite. L’architecture du sommeil n’est pas non plus differente chez les patients, hormis un pourcentage de sommeil paradoxal sans atonie plus eleve chez les patients avec TCSPi (p Conclusion Le signal du LC/LSC en IRM sensible a la neuromelanine est reduit chez les patients avec TCSPi, de facon proportionnelle a la perte d’atonie en sommeil paradoxal, suggerant un lien direct entre ces deux phenomenes. Cette methode d’imagerie pourrait devenir un moyen diagnostique precoce des synucleinopathies, un moyen de suivi de la perte neuronale et de suivi d’une eventuelle intervention neuro-protectrice.
