Abstract

Human infections with Corynebacterium diphtheriae species complex (CdSC) bacteria were rare in French Guiana until 2016, when the number of cases diagnosed increased. We conducted an epidemiologic, multicenter, retrospective study of all human CdSC infections diagnosed in French Guiana during January 1, 2016-December 31, 2021. A total of 64 infectious episodes were observed in 60 patients; 61 infections were caused by C. diphtheriae and 3 by C. ulcerans. Estimated incidence increased from 0.7 cases/100,000 population in 2016 to 7.7 cases/100,000 population in 2021. The mean patient age was 30.4 (+23.7) years, and male-to-female ratio was 1.7:1 (38/22). Of the 61 C. diphtheriae isolates, 5 tested positive for the diphtheria toxin gene, and all results were negative by Elek test; 95% (61/64) of cases were cutaneous, including the C. ulcerans cases. The increase in reported human infections underscores the need to raise awareness among frontline healthcare practitioners to improve prevention. [ABSTRACT FROM AUTHOR]

Abstract

Overview of infectious and non-infectious diseases in French Guiana in 2022 Source of many myths, French Guiana represents an exceptional territory due to the richness of its biodiversity and the variety of its communities. The only European territory in Amazonia, surrounded by the Brazilian giant and the little-known Suriname, Ariane 6 rockets are launched from Kourou while 50% of the population lives below the poverty line. This paradoxical situation is a source of health problems specific to this territory, whether they be infectious diseases with unknown germs, intoxications or chronic pathologies. Some infectious diseases such as Q fever, toxoplasmosis, cryptococcosis or HIV infection are in common with temperate countries, but present specificities leading to sometimes different management and medical reasoning. In addition to these pathologies, many tropical diseases are present in an endemic and / or epidemic mode such as malaria, leishmaniasis, Chagas disease, histoplasmosis or dengue. Besides, Amazonian dermatology is extremely varied, ranging from rare but serious pathologies (Buruli ulcer, leprosy) to others which are frequent and benign such as agouti lice (mites of the family Trombiculidae) or papillonitis. Envenomations by wild fauna are not rare, and deserve an appropriate management of the incriminated taxon. Obstetrical, cardiovascular and metabolic cosmopolitan pathologies sometimes take on a particular dimension in French Guiana that must be taken into account in the management of patients. Finally, different types of intoxication are to be known by practitioners, especially due to heavy metals. European-level resources offer diagnostic and therapeutic possibilities that do not exist in the surrounding countries and regions, thus allowing the management of diseases that are not well known elsewhere. Thanks to these same European-level resources, research in Guyana occupies a key place within the Amazon region, despite a smaller population than in the surrounding countries. Thus, certain pathologies such as histoplasmosis of the immunocompromised patient, Amazonian toxoplasmosis or Q fever are hardly described in neighboring countries, probably due to under-diagnosis linked to more limited resources. French Guiana plays a leading role in the study of these diseases. The objective of this overview is to guide health care providers coming to or practicing in French Guiana in their daily practice, but also practitioners taking care of people returning from French Guiana. Seul territoire européen en Amazonie, entouré du géant brésilien et du méconnu Suriname, on y lance des fusées Ariane 6 depuis Kourou tandis que 50 % de la population vit en dessous du seuil de pauvreté. Cette situation paradoxale est source de problématiques de santé spécifiques à ce territoire, qu’il s’agisse de maladies infectieuses à germes méconnus, d’intoxications, ou de pathologies chroniques. Certaines maladies infectieuses telles que la fièvre Q, la toxoplasmose, la cryptococcose ou l’infection à VIH sont communes aux pays tempérés, mais présentent en Guyane des spécificités entraînant une prise en charge et un raisonnement médical parfois différents. Parallèlement à ces pathologies, de nombreuses maladies tropicales sont par ailleurs présentes sur un mode endémique et / ou épidémique telles que le paludisme, la leishmaniose, la maladie de Chagas, l’histoplasmose ou la dengue. De plus, la dermatologie amazonienne est extrêmement variée, allant de pathologies rares, mais graves (ulcère de Buruli, lèpre), à d’autres fréquentes et bénignes telles que les poux d’agouti (acariens de la famille des Trombiculidae) ou la papillonite. Les envenimations par la faune sauvage ne sont pas rares, et méritent une prise en charge appropriée au taxon incriminé. Les pathologies obstétricale, cardiovasculaire et métabolique cosmopolites prennent parfois en Guyane une dimension particulière à prendre en compte dans la prise en charge des patients. Enfin, différents types d’intoxication sont à connaître par les praticiens, notamment aux métaux lourds. Les ressources de niveau européen offrent des possibilités diagnostiques et thérapeutiques inexistantes dans les pays et régions des environs, permettant ainsi la prise en charge de maladies peu connues ailleurs. Du fait de ces mêmes ressources de niveau européen, la recherche en Guyane occupe une place clé au sein de la région amazonienne, malgré une population moins nombreuse que dans les pays alentour. Ainsi, certaines pathologies telles que l’histoplasmose du patient immunodéprimé, la toxoplasmose amazonienne ou la fièvre Q ne sont pratiquement pas décrites dans les pays voisins, probablement du fait d’un sous-diagnostic lié à des ressources plus limitées. La Guyane joue ainsi un rôle moteur dans l’étude de ces pathologies. 9. Sexually transmitted infections on the border between Brazil and French Guiana

Abstract

PurposeThe border between the State of Amapa, Brazil, and French Guiana is mostly primary forest. In the Oyapock basin, socioeconomic circumstances have fueled sex work, gold mining and the circulation of sexually transmitted infections. Given the lack of comprehensive data on this border area, we describe the different sexually transmitted infections along the Brazil/French Guiana border and the testing and care activity.MethodsWe conducted a review of the available scientific and technical literature on sexually transmitted infections in this complex border area. Temporal trends were graphed and for Human Immunodeficiency Virus (HIV) we estimated incidence using the European Center for prevention and Disease Control modeling tool.ResultsUntil 2019, 26 of the 46 HIV-infected patients followed and treated in Saint Georges de l'Oyapock were residing on the Brazilian side in Oiapoque. Virological suppression was only achieved for 75% of treated patients; but dropped to 62% during the COVID-19 epidemic. In 2019, cooperation efforts allowed HIV care in Oiapoque, resulting in the transfer of Brazilian patients previously followed on the French side and a substantial increase in the number of patients followed in Oiapoque. The average yearly HIV serological testing activity at the health center in Saint Georges was 16 tests per 100 inhabitants per year; in Camopi it was 12.2 per 100 inhabitants. Modeling estimated the number of persons living with HIV around 170 persons, corresponding to a prevalence of 0.54% and about 40 undiagnosed infections. The model also suggested that there were about 12 new infections per year in Saint Georges and Oiapoque, representing an HIV incidence rate of 3.8 cases per 10,000 per year. HPV prevalence in Saint Georges ranges between 25 and 30% and between 35 and 40% in Camopi. Testing activity for other sexually transmitted infections markedly increased in the past 5 years; the introduction of PCR for chlamydiasis and gonorrhea also had a substantial impact on the number of diagnoses.ConclusionsThe ongoing cooperation between multiple partners on both sides of the border has led to remarkable progress in primary prevention, in testing efforts, in treatment and retention on both sides of the border. In a region with intense health professional turnover, nurturing cooperation and providing accurate assessments of the burden of sexually transmitted infections is essential to tackle a problem that is shared on both sides of the border.

Abstract

International audience; Progressive meshes algorithms aim at computing levels of detail from a highly detailed mesh. Many of these algorithms are based on a mesh decimation technique, generating coarse triangulation while optimizing for a particular metric which minimizes the distance to the original shape. However these metrics do not robustly handle high curvature regions, sharp features, boundaries or noise. We propose a novel error metric, based on algebraic spheres as a measure of the curvature of the mesh, to preserve curvature along the simplification process. This metric is compact, does not require extra input from the user, and is as simple to implement as a conventional quadric error metric.

Relationships between producers and consumers reveal renewed forms of food systems that favor a close origin in the context of a local supply. Thèse soutenue publiquement le 1er mars 2010; Cette thèse propose un éclairage sur les préférences des consommateurs pour les produits verts. Elle mobilise les outils de l’économie expérimentale afin de mieux comprendre comment les consommateurs valorisent les caractéristiques environnementales des biens privés. Le chapitre 1 dresse un panorama des principaux enjeux théoriques liés aux propriétés de croyance et de bien public des caractéristiques environnementales. Le chapitre 2 consiste en une discussion des méthodologies de révélation des préférences. Les chapitres 3 et 4 présentent deux études expérimentales de la consommation de produits verts non alimentaires. Le chapitre 3 présente une analyse des consentements à payer pour des produits issus d’un processus de remanufacturing. Les résultats obtenus par un mécanisme d’enchères expérimentales montrent que les préférences environnementales des consommateurs s’expriment dans ce cas, par une forte dévalorisation du produit le plus polluant, voire par une absence de consentement à payer par ce produit. Le chapitre 4 présente une étude des préférences des consommateurs pour des roses offrant deux caractéristiques environnementales indépendantes : un écolabel certifiant des conditions de production plus respectueuses de l’environnement, et un indice carbone plus ou moins important. Un mécanisme de choix discrets expérimentaux révèle une valorisation positive des deux attributs environnementaux, résultat qui confirme l’existence d’un consentement à payer pour des produits dont les caractéristiques environnementales ne sont pas associées à des bénéfices de santé.

National audience; Cette communication a pour objectif de présenter les premiers résultats obtenus via la méthodologie de l'économie expérimentale concernant le comportement des consommateurs vis-à-vis de produits remanufacturés. En récupérant une partie de la valeur intrinsèque des produits en fin de vie, le remanufacturing s'avère prometteur économiquement. Mais s'interroger sur les conditions de sa viabilité économique implique surtout de considérer les comportements des consommateurs vis-à-vis de ces produits. En effet, les consommateurs sont d'une part responsables du retour de leurs produits usagés et influencent donc la quantité, la qualité et le timing des produits rapportés. D'autre part, les consommateurs représentent les acheteurs potentiels de produits remanufacturés. Leurs consentements à payer pour de tels bien influencent directement la rentabilité du processus. Les résultats obtenus cherchent à dévoiler les facteurs permettant d'expliquer les comportements des consommateurs vis-à-vis des produits remanufacturés. Si, en l'absence d'information concernant les conséquences environnementales du remanufacturing, les consommateurs semblent prêts à payer moins cher pour des produits remanufacturés, on observe une réaction à l'information sur les impacts environnementaux des processus industriels (notamment ceux plus limités du remanufacturing). En particulier les consentements à payer pour des produits remanufacturés augmentent lorsque cette information est donnée. Toutefois, les résultats obtenus montrent qu'il apparaît alors un arbitrage dans l'évaluation des sujets entre "qualité" et "environnement".

Abstract

Products' end-of-life management has recently become a critical business issue. One of the possible end-of-life strategies is remanufacturing, which can provide competitive advantages through material and energy savings. Beyond industrial organization challenges, there is a question about the interest of developing a green marketing strategy for remanufactured products. Indeed, remanufactured products can be considered as green products since their industrial process has environmental benefits. Our paper asks whether consumers are willing to pay for remanufactured products, especially when they are informed that these products are 'green'. We use experimental auctions to elicit consumers' WTP for specific characteristics of remanufactured products. Our study indicates that consumers tend to value the remanufactured product less than the conventional one unless they are informed about their respective environmental impacts. We find no evidence that consumers are willing to pay a premium for the green (i.e. remanufactured) product. However, providing environmental information to consumers has an effect on their WTP for the conventional product: they generally decrease significantly their WTP for the conventional (and thus most polluting) product. METHODOLOGY/PRINCIPAL FINDINGS: A cross-sectional survey was performed between 15 July 2020 and 23 July 2020 among individuals who visited 4 medical laboratories or 5 health centers for routine screening or clinical management, with the exception of symptomatic suggestive cases of covid-19. Samples were screened for the presence of anti-SARS-CoV-2 IgG directed against domain S1 of the SARS-CoV-2 spike protein using the anti-SARS-CoV-2 enzyme-linked immunosorbent assay (ELISA) from Euroimmun. CONCLUSIONS/SIGNIFICANCE: The overall seroprevalence was 15.4% [9.3%-24.4%] among 480 participants, ranging from 4.0% to 25.5% across the different municipalities. The seroprevalence did not differ according to gender (p = 0.19) or age (p = 0.51). Among SARS-CoV-2 positive individuals, we found that 24.6% [11.5%-45.2%] reported symptoms consistent with COVID-19. Our findings revealed high levels of infection across the territory but a low number of resulting deaths, which can be explained by French Guiana's young population structure.

Abstract

Background While Latin America has been heavily affected by the pandemic, only a few seroprevalence studies have been conducted there during the first epidemic wave in the first half of 2020. Methodology/Principal findings A cross-sectional survey was performed between 15 July 2020 and 23 July 2020 among individuals who visited 4 medical laboratories or 5 health centers for routine screening or clinical management, with the exception of symptomatic suggestive cases of covid-19. Samples were screened for the presence of anti-SARS-CoV-2 IgG directed against domain S1 of the SARS-CoV-2 spike protein using the anti-SARS-CoV-2 enzyme-linked immunosorbent assay (ELISA) from Euroimmun. Conclusions/Significance The overall seroprevalence was 15.4% [9.3%-24.4%] among 480 participants, ranging from 4.0% to 25.5% across the different municipalities. The seroprevalence did not differ according to gender (p = 0.19) or age (p = 0.51). Among SARS-CoV-2 positive individuals, we found that 24.6% [11.5%-45.2%] reported symptoms consistent with COVID-19. Abstract

French Guiana is a "transcultural world" in its own right, with its indigenous "inland" populations, highly isolated from the healthcare system, more affected by chronic cardiometabolic and infectious diseases than either the coast or mainland France. Transcultural clinics and therapeutic patient education (TPE) therefore play a key role. French Guiana is a pioneer in TPE, health mediation and the practice of "outreach" via local hospitals, delocalized specialty consultations and so on. The proposal is to bring together transcultural TPE workshops for chronic patients in local hospitals in the interior, with a health mediator trained in their pathology, and to combine them with off-site specialist consultations to promote therapeutic adherence., (Copyright © 2024 Elsevier Masson SAS.
