Der fruhe Therapiebeginn der rheumatoiden Arthritis (RA) ist ein entscheidender Faktor fur die weitere Prognose der Erkrankung. Um diese zu ermoglichen existieren seit 2010 die ACR-EULAR-Klassifikationskriterien der RA, welche klinische Kriterien und Laborbefunde verbinden und in einem Punktesystem die fruhe Klassifikation ermoglichen. Vorausetzung ist der mindestens einmalige Nachweis einer Synovitis. Aber auch eine Stufe fruher, also wenn der Patient z. B. nur Arthralgien hat, ist es moglich, Kriterien anzuwenden, welche die fur eine entzundliche Erkrankung suspekten Patienten identifizieren konnen. Sowohl der Nachweis von Antikorpern gegen anti-citrullinierte Peptide (Anti-CCP) im Serum wie auch Methoden der Bildgebung wie Sonografie, Magnetresonanztomografie und auch noch die 3-Phasenskelettszintigrafie konnen wesentlich fur die fruhe Diagnose sein. Schlieslich spielen die versorgungsmedizinischen Aspekte eine wichtige Rolle. Eine fruhe Diagnose ist nur moglich, wenn dem Patienten der fruhe Zugang zu Rheumatolog*innen ermoglicht wird. Die groste Herausforderung dafur ist es, geeignete Strukturen zu schaffen, welche die Auswahl der richtigen Patient*innen ermoglichen und damit die rheumatologischen Ressourcen entlasten. Verschiedene Screeningmodelle unter Nutzung von rheumatologischen Fachassistent*innen, Fragebogensystemen oder aber arztlichen Kurzsprechstunden werden in Deutschland dafur erprobt. Jedes hat seine spezifischen Vor- und Nachteile, alle konnen aber die fruhe Diagnose und Therapie ermoglichen. The ACR-EULAR classification criteria for RA, introduced in 2010 and combining clinical criteria with laboratory test results facilitating an early classification system based on scores, allow us to do so. Proof of synovitis is required at least once. Already in earlier stages, e. g. in cases of patients suffering from just arthralgia, it is however possible to apply criteria that are able to identify patients that can be suspected to develop inflammatory diseases. Anti-citrullinated peptides (anti-CCP) in serum as well as imaging methods such as sonographies, MRI and three phase bone scans can be vital in early diagnostics; and, in addition, healthcare itself comes into play as an important aspect. An early diagnosis is only possible if patients have early access to rheumatologist care. The major challenge therefore remains to set up structures capable of choosing the right patients and relieve the strain on resources in rheumatology. In Germany, a variety of screening models using rheumatologist assistants, questionnaires or short specialist consultation hours are currently tested. Each of these models has its specific advantages as well as disadvantages; all of them, however, are able to facilitate early diagnostics and treatment.
