Abstract

The demand of social skills interventions for people with ASD has grown in recent years. The main goal of this research was to study social skills: "responding to interaction" and "initiating interaction", and to capture whether there were differences between an initial and a final session in a program for children with ASD. Additionally, we aimed to compare social skills patterns according to the VIQ level. The sample (N = 20) was divided into 2 subgroups depending on whether the VIQ was > 90 or < 90. We employed a mixed methods approach based on a systematic observation of social behaviors. The observational design was nomothetic, follow-up, and multidimensional. Once we confirmed inter-observer reliability for the ad hoc observational instrument we performed descriptive statistics and polar coordinate analysis using LINCE software. The results show high intragroup and intergroup variability. In general, participants with VIQ < 90 showed a better improvement in responding to interaction, whereas participants with VIQ > 90 showed more complex patterns to initiate interactions. The polar coordinate technique was useful for detecting significant relationships between autism's social micro-behaviors. Results and information obtained through observational methodology could allow professionals to understand communication and interaction of participants.

Abstract

Over the last 20 years, researchers have been mixing qualitative and quantitative approaches, but mixed methods research represents a new movement that arose in response to the currents of qualitative and quantitative research, considered separately. Little has been published on the use of polar coordinate analysis in psychotherapy. This type of analysis can provide detailed information and integrate the qualitative-quantitative analysis. Even less has been published on the analysis of ASD children's behavior. The main aim of this study was to implement this mixed methods methodology to analyze patterns of social behaviors in a group of adolescents with ASD during a group social competence intervention program. Moreover, we wanted to see whether an observational scale could be combined fruitfully with polar coordinate analysis and to investigate whether typical ASD behaviors show similar interrelations (prospective and retrospective sequentialities) as behaviors observed in psychotherapy. We used an adaptation from the Social Skills Training Program (UC Davis, California). We observed that each participant took a unique course, increasing or decreasing the number and quality of their social behaviors. In accordance with previous literature, results suggest some increment in the amount of appropriate social conduct. We did not detect a generalized progress pattern but agreed that there were changes between the beginning and end of the intervention. Therefore, we consider that observational methodology is useful in the field of psychotherapy and ASD, offering detailed information about changes and development that cannot be obtained with other traditional measures, such as questionnaires.

Abstract

To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause.Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms.Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year.Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women agedor = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73).This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

Abstract

OBJECTIVE To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

Abstract

ABSTRACT River basin managers responsible for water allocation decisions are increasingly required to evaluate tradeoffs between environmental flow protections and human water security. However, the basin-scale effects of environmental flow regulations on water users are not well understood, in part because analyses are complicated by the spatial and temporal variation in water availability, human demands, and ecosystem needs. Here, we examine alternative regional environmental flow policies and their effects on a distributed network of water users in a small (182 km2) river basin in coastal California. We use a hydrologic model to simulate water diversion operations under three policy scenarios and quantify potential impacts to bypass flows for adult migrating salmon and agricultural water storage. The results indicate that there are inherent tradeoffs between environmental flows and agricultural water security, with the most restrictive environmental policy associated with the greatest impacts to water users. Surprisingly, the moderate environmental flow policy had larger impacts to bypass flows than the unregulated management scenario, suggesting that ecological benefits of the moderate policy are small relative to the adverse effects on agricultural water users. Conflicts between environmental and human water needs were greatest in upper catchments (<2.5 km2), where flow protections caused the greatest reduction in water storage. Although natural supplies were adequate for meeting water needs in most years regardless of policy restrictions, potential for conflict between environmental flow protections and water security was evident in dry years. Therefore, strategies are particularly needed for drought-year water management to ensure adequate environmental flows while reducing human water allocations in an equitable manner. Abstract

The ampicillin/sulbactam combination is one of several such drug combinations of a beta-lactam and suicide inhibitor having a wide spectrum of activity. These characteristics induced us to evaluate the in vitro activity of this combination towards 54 strains of Haemophilus sp. (38 beta-lactamase producers) and 20 strains of Branhamella catarrhalis (16 beta-lactamase producers). All strains were isolated from sputum, sinusal aspiration and tympanocentesis. In the case of Haemophilus sp beta-lactamase producers, minimal inhibitory concentrations of ampicillin were reduced 8 times by the use of the inhibitor; good results were also obtained for B. catarrhalis. Haemophilus influenzae, B. catarrhalis together with Streptococcus pneumoniae are recognized as the major pathogens involved in upper respiratory tract infections. The increasing frequency of beta-lactamase producing strains has impaired the use of aminopenicillins. The combination of an inhibitor and beta-lactam restore the activity of the latter, suggesting that this combination can serve as first choice in therapy.

Abstract

The activity of linezolid, a new oxazolidinone, was tested against 862 Gram-positive cocci isolated in Italy and compared with the activities of 12 antibiotics. Overall, MIC[sub 90]s for linezolid (2-4 mg/L) indicated an in vitro activity comparable to that of vancomycin in methicillin-resistant Staphylococcus aureus (4 mg/L), S. epidermidis (2 mg/L) and methicillin-susceptible strains. Enterococcus faecalis strains were susceptible to linezolid (MIC[sub 90] 2-4 mg/L), glycopeptides and β-lactams. In E. faecium, only glycopeptides (MIC[sub 90] 2 mg/ L) and linezolid (MIC[sub 90] 2 mg/L) were active. Linezolid was the only drug active against two strains of Enterococcus showing a VanA phenotype. Owing to its antibacterial profile, linezolid represents a promising drug for the treatment of Gram-positive infections. [ABSTRACT FROM AUTHOR]

Abstract

Bloodstream infection (BSI) due to extended-spectrum β-lactamase (ESBL) producing Enterobacteriaceae are a major cause of in-hospital mortality. The effect on survival of empirical and targeted antibiotic therapy in these patients remains controversial.A prospective cohort study was conducted analyzing data from 94 patients (age 59 ± 21 years) with BSI due to ESBL producing strains (Sixty-one E. coli, 26 K. pneumoniae, 4 Proteus spp and 3 Enterobacter spp).Risk factors associated with 21-day mortality at univariate analysis were: recent administration of antibiotic therapy (p=0.049), higher SOFA score (p=0.05), ICU stay (p0.01), hypotension at presentation (p =0.001) or septic shock (p0.001) and bacteremia from source other than urinary tract (p=0.03). Regardless of antibiotic class used, no differences in survival were noted between patients receiving or not adequate initial antimicrobial treatment (37.1% vs 23.7% p=0.23); on the other hand, compared with the administration of other in vitro active antibiotics, the use of carbapenem as definitive therapy was associated with a significantly lower 21-day mortality (54.3% vs 28.5% p=0.02).These findings suggest that the administration of an adequate initial therapy is not associated with mortality in hospitalized patients with BSI due to Enterobacteriaceae. The severity of clinical conditions at presentation and the administration of carbapenems as definitive therapy seems to be really important in affecting the outcome of patients with BSI due to ESBL producing strains.

Abstract

In a 1-year multicenter study the microbial colonization of 1154 central venous catheters (CVCs) was investigated. Catheters explanted either from immunocompromised or immunocompetent patients were collected and analyzed by five clinical microbiology laboratories located in Ancona, Aviano, Catania, Pavia and Rome, Italy.A further aim was to investigate, by scanning electron microscopy, the features of currently used catheters, both new and explanted from patients, analyzing their surface quality, the influence of the host protein biofilm on their microbial colonization, the modifications caused by their permanence in the body and the relationship between these factors and the occurrence of infections.

Il contributo si sofferma sulla diffusione del Sars-Cov-2 quale epifenomeno della gestione capitalistica neoliberista del pianeta e sul suo ruolo rivelatore, per un verso, di tutte le contraddizioni e i disastri del neoliberismo, e detonatore, per un altro verso, delle nuove strategie capitalistiche per una nuova fase di accumulazione del capitale. Dentro a questo quadro cercheremo di far luce sulla ulteriore spinta alla crescita delle diseguaglianze, delle povertà e delle marginalità sociali ed economiche di cui è responsabile il modo di produzione capitalistico ma che la pandemia sta accentuando, consentendo agli attori economici, individuali e istituzionali di usare quest’ultima come alibi per nuove politiche e interventi economici che incidono notevolmente sulla polarizzazione tra ricchi e poveri. The contribution dwells on the diffusion of Sars-Cov-2 as an epiphenomenon of the neoliberal capitalist management of the planet of the planet and its role in revealing, on the one hand, all the contradictions and disasters of neoliberalism, and detonator, on the other hand, the new capitalist strategies for a new phase of capital accumulation. Within this framework we will try to shed light on the further push for the growth of inequalities, poverty and social and economic marginality for which the capitalist mode of production is responsible, but which the pandemic is accentuating, allowing economic, individual and institutional actors to use it as an alibi for new policies and economic interventions that greatly affect the polarization between rich and poor.

Il capitolo presenta un’analisi delle organizzazioni di volontariato della provincia di Palermo, sulla base dei dati di una survey regionale, condotta su iniziativa dei tre Centri di servizio per il Volontariato e del Comitato di Gestione dei fondi speciali per la Sicilia. L’obiettivo e quello di delineare un’immagine articolata delle organizzazioni, in ragione di alcune dimensioni essenziali: 1) identificazione anagrafica (denominazione, comune della sede sociale, forma giuridica, iscrizione/non iscrizione al registro regionale del volontariato/della protezione civile e al Centro di Servizio per il Volontariato); 2) informazioni circa il profilo socio-professionale, la rete di appartenenze organizzative e lo stile di leadership del legale rappresentante; 3) numerosità e caratteristiche salienti dei soci iscritti, dei soci volontari e dei lavoratori alle dipendenze delle organizzazioni; 4) settori di attività e tipologie di servizi erogati; 5) condizione economica e patrimoniale e forme di informazione/accountability pubblica; 6) rapporti delle organizzazioni con i Centri di servizio e grado di soddisfazione per i servizi ricevuti; 7) destinatari delle attività, radicamento territoriale ed esigenze di rafforzamento; 8) forme e contenuti delle reti di relazioni tra organizzazioni e con le istituzioni pubbliche e del terzo settore presenti nel territorio. L’analisi dei dati empirici e inquadrata nell’ambito della ricerca nazionale sull’evoluzione del fenomeno all’interno del panorama della crisi e trasformazione delle politiche sociali, evidenziando come, pur in una fase di crescita quantitativa delle organizzazioni e dei/delle volontari/ e, il ruolo “politico” e di innovazione del volontariato organizzato – nel territorio in esame, ma più in generale nelle regioni del Mezzogiorno – appaia caratterizzato da debolezze e incertezze – di natura tanto endogena quanto esogena – che si proiettano anche nel prossimo futuro. he chapter presents an analysis of volunteer organizations in the province of Palermo, based on data from a regional survey, conducted at the initiative of the three Volunteer Service Centers and the Special Funds Management Committee for Sicily. The objective is to outline an articulated image of the organizations, due to some essential dimensions: 1) biographical identification (name, municipality of registered office, legal form, registration/non-registration with the regional register of volunteers/civil protection and the Volunteer Service Center); 2) information about the socio-professional profile, the network of organizational affiliations and the leadership style of the legal representative; 3) numerosity and salient features of registered members, volunteer members and workers employed by the organizations; 4) sectors of activity and types of services provided; 5) economic and patrimonial condition and forms of public information/accountability; 6) relationships of organizations with Service Centers and degree of satisfaction with the services received; 7) recipients of activities, territorial rootedness and strengthening needs; 8) forms and contents of networks of relations between organizations and with public and third sector institutions in the territory. The analysis of the empirical data and framed within the framework of the national research on the evolution of the phenomenon within the landscape of the crisis and transformation of social policies, highlighting how, despite a phase of quantitative growth of organizations and volunteers, the "political" and innovative role of organized volunteering - in the territory under review, but more generally in the regions of Southern Italy - appears to be characterized by weaknesses and uncertainties - of both endogenous and exogenous nature - that are projected also in the near future.

Il volume raccoglie alcuni contributi scaturiti da attività di ricerca e didattica di due laboratori dei Corsi di studio in Servizio sociale del Dipartimento Culture e società dell’Università degli Studi di Palermo: il Laboratorio Interdisciplinare di ricerca su Corpi, Diritti, Conflitti e il Laboratorio di Servizio sociale. Marginale è chi vive una condizione di estraneità e non-appartenenza al contesto sociale, chi viene a trovarsi ai margini, ai confini del sistema sociale, a distanza dai processi centrali di distribuzione delle risorse sociali, non solo materiali, a indicare che le disuguaglianze sociali sono strettamente riconducibili a gradi diversi di integrazione sociale, e che la questione non coincide con quelle, pur connesse, concernenti i processi di inclusione/esclusione sociale, i diritti di cittadinanza e le politiche sociali. Marginal is someone who lives a condition of alienation and non-belonging to the social context, someone who comes to be on the margins, on the borders of the social system, at a distance from the central processes of distribution of social resources, not only material, indicating that social inequalities are closely related to different degrees of social integration, and that the issue does not coincide with those, though connected, concerning processes of social inclusion/exclusion, citizenship rights and social policies. Abstract

This paper analyzes the phenomenon of desistance from crime by deviant young people in some Sicilian cities. In these paths, individual and relational reflexivity is central. This is an original study, which focuses on the importance of the role of social workers as actors capable of strengthening the socio-educational rehabilitation pathways of deviant young people. The author demonstrates how the presence of relational reflexivity in the intervention of social workers and social services contributes to (re)activating in young people an individual reflexivity (proactive, responsible, prosocial) capable of directing them towards objectives of personal growth and adult responsibility.

Abstract

During active surveillance at the Mediterranean Institute for Transplantation and Advanced Specialized Therapies (ISMETT, Palermo, Italy) with the CARBA screening medium, five pairs of Klebsiella pneumoniae carbapenemase (KPC)-producing K. pneumoniae and Escherichia coli strains were isolated in each of five colonized patients. In each patient, lateral gene transfer was demonstrated by comparing K. pneumoniae and E. coli strains, both possessing KPC-3, Tn 4401a and pKpQIL-IT elements. The isolates were found to be multiclonal by multilocus sequence typing (sequence type (ST) 512 related to ST258, and ST307 belonging to a clonal complex different from the habitual sequence clone ST258 isolated in Italy) and pulsed-field gel electrophoresis. The results of our study highlight the easy transfer of KPC among Enterobacteriaceae colonizing the human intestine, and the active and careful surveillance required to identify and prevent the spread of these multidrug-resistant microorganisms.
