6. Occupational therapists’ perspectives on an evidence-based, client-centered assistive technology intervention

Introduction: Reviews within the field of assistive technology have shown that a client-centered approach is important for user satisfaction, and that assistive technology service delivery should be evidence-based, systematic, and structured. However, client-centered instruments and systematic, structured models are not used consistently. As part of a larger research project, an evidence-based, client-centered assistive technology service delivery process was developed and piloted. The purpose of this study was to investigate occupational therapists’ perspectives on this evidence-based, client-centered assistive technology intervention. Method: Ten occupational therapists, from two Danish municipalities, participated in focus groups. Data were analyzed based on a hermeneutic approach. Results: Three themes emerged from the analysis: a more refined collaboration with clients; advantages of using theoretical frameworks, structured models and instruments and challenges in using the intervention. Conclusion: By using the evidence-based, client-centered intervention, the occupational therapists found that the collaboration with the clients was refined—more emphasis was placed on shared responsibility. They found that the intervention was time-efficient in the long run; however, there seems to be a particular challenge in designing services that are inclusive of clients with cognitive limitations.

ABSTACT: Background: From the perspective of older adults, the assistive technology application process is complex and their perceived involvement varies. Occupational therapists find it challenging to satisfy their clients’ needs while complying with the eligibility criteria of the service provision system. Research has shown that, from the perspective of both clients and occupational therapists, the application process is perceived to be challenging. However, little is known about which specific actions and experiences during the application process give rise to this perceived complexity. Aim: The aim of this study was to investigate older adults’ and occupational therapists’ actions and experiences of their encounters during the assistive technology application process. Material and method: Data were generated using repeated participant observations and informal interviews. Ricoeur’s theory of interpretation was used to interpret and discuss the data generated. Results: Two themes emerged: Expressing needs and translating needs. Conclusion: Older adults express their needs in different ways. When the older adults’ expression of needs does not correspond with the service provision system’s conditions of eligibility, the occupational therapists act as mediators, by translating experienced needs so that they are expressed in a way that corresponds to the expression of the eligibility criteria.

PURPOSE: To identify, synthesize, and evaluate existing literature concerning the process of becoming a user of assistive technology (AT).METHOD: A systematic review and meta-synthesis were carried out. Five bibliographic databases (MEDLINE via PubMed, CINAHL, Web of Science, PsycINFO and SocINDEX) were systematically searched up to 13th of March 2017, using two sets of search terms: (i) elderly and synonyms and (ii) assistive technology and similar words, and combined with a qualitative research filter. Articles were screened, read and critically assessed. The meta-synthesis was guided by Ricoeur's theory of interpretation.RESULTS: Seventeen out of 4645 articles were included. Five phases emerged relating to the process of becoming a user of AT: phase A: Evaluating need, phase B: Acknowledging need, phase C: Incorporating the AT into daily life, phase D: Using the AT, and phase E: Future use. Three transitions, describing factors essential to moving from one phase to the next, were identified; from phase A-B: Valued activities are threatened, from phase B-C: Obtaining the AT and from phase C-D: Trust in the AT. No transition was identified from phase D-E.CONCLUSION: The meta-synthesis led to a deeper understanding of the process of older adults becoming users of AT, by exploring findings across the included articles. The identified phases and transitions in the systematic review serve as an analytical framework for understanding the process from the older adult's perspective. This review advocates for using a client-centred approach throughout the entire delivery process. Implications for rehabilitation The process of the older adult becoming a user of AT involves an individualized time factor, and this supports the practice of individualized follow-up. The process of becoming a user of AT is closely related to self-image; healthcare professionals should support not only the use of AT but also the older adult's emotional adjustment to a new self-image. The process is highly influenced by the older adult's social context; healthcare professionals should consider involving the client's social network in the AT delivery process.

Purpose: Seaports are to a growing extent controlled by IT systems. Smooth information exchange is crucial for business. Downtimes give rise to substantial financial losses and supply bottlenecks, due to the interconnections in a complex alliance of port stakeholders‘ systems. Hence, Cyber security in an important aspect in Port Communication Systems. Methodology: The project SecProPort will develop a holistic IT security architecture for port communication networks, which will support the security requirements of the stakeholders‘ operating procedures, protect them against sabotage, and prevent third parties from illicitly gathering sensitive data or getting unauthorized access to the communications network. Findings: The desired architecture is to be implemented by first analyzing typical attack scenarios targeted at the data processed in the port communication alliance. The next step entails designing the actual security architecture for the alliance and installing a prototype in collaboration with the application partners. Originality: SecProPort aims at a holistic approach with respect to secure port communications networks, rather than addressing individual stakeholders. The architecture will also provide resilience measures for minimizing the impact on other actors in the alliance in case of an incident, and returning to normal operation in a controlled manner.

Introduction: Previous studies have suggested that, in addition to factors such as sociocultural and physical contexts, the incorporation of assistive technology (AT) into everyday life is influenced by acceptance. Less seems to be known about the actual process by which the AT becomes incorporated. Purpose: To investigate older adults’ experiences of the process of incorporating ATs into occupations. Method and material: A qualitative longitudinal approach using ethnographically inspired fieldwork with repeated interviews and participant observations was taken to explore experiences related to eight older adults’ incorporation of an AT into occupations. A phenomenological-hermeneutical approach was used to interpret the data. Findings: Through the analysis, one main theme, “Becoming acquainted” and six related subthemes, “Acquaintance through use”, “Drawing on previous experiences”, “Engaging in valued occupations”, “Encounters with others”, “Regaining control over everyday life” and “Challenges in using the AT” were identified. Conclusion: Active use of the AT–or “doing”–was experienced as important to the process of, first, learning the AT’s different functions and, later, engaging or re-engaging in valued occupations. Using the AT to engage in occupations had a positive impact on the participants’ minds and inner selves, i.e. it influenced their “being” and enabled a connection to people and places and thereby enhanced the feeling of “belonging”. All these elements contributed to a successful incorporation of the AT into occupations or to the older adults “becoming” users of AT.

Beside physical threats, ports are also vulnerable to cyber attacks due to their dependency on information and communications technology. Port Community Systems (PCSs) are information hubs for ports integrating information from various sources for global supply chains, connecting systems of terminal operators, carriers, freight forwarders and authorities. These systems are critical infrastructures – successful cyber attacks can lead to significant transport disturbances and severe consequences for the whole economy. This paper presents ongoing work within the research project PortSec, aiming at improved resilience of PCSs with respect to cyber attacks.

Maritime Intermodal door-2-door supply chains are complex as many different actors are involved. Beside legal supply chain actors also criminals use logistics transport networks for their operations. For example, illicit narcotics, weapons and counterfeited products are smuggled in these supply chains. There are already mitigation measures for maritime security in place but there is still need for improvement in order to reduce the volume of illicit transports and the social damages related to it. Customs authorities focus on these illicit activities in order to detect and to limit damage to the society. The main aim is to ensure safety and security in international trade. Therefore, customs authorities use risk targeting systems which are based on customs declaration data and intelligence systems to detect high risk trade flows. High quality data has to be used to detect these illicit trade flows. However, today the quality of data is lacking. The CASSANDRA project developed a data pipeline approach to improve the data quality by sharing data between all legal supply chain actors, including businesses and authorities. During the project a socio-economic cost benefit analysis was carried out. A matrix specially designed for the evaluation of improvement of security in maritime supply chains was developed to assess the benefit generated by the data pipeline. The implemented methodology approach for a quantitative assessment considers criteria like risk indicators (available data on which customs decide to inspect a container), kinds of damages (e.g. damages due to smuggling of narcotics or weapons) and inspection rates. For the validation of the methodology interviews with different European customs authorities were carried out. The execution of socio-economic cost benefit analysis was carried out twice: with and without the implementation of the data pipeline approach. Thus, the damage for the socio-economy, which is already prevented by customs (as-is situation), and the possible improvements through the CASSANDRA approach (to-be situation) was compared.

Die Bandbreite dieser moglichen Storungen reicht von Verzogerungen im Transportablauf uber Ladungsdiebstahl bis hin zu terroristischen Angriffen. Der vorliegende Beitrag stellt Ergebnisse verschiedener Forschungsprojekte vor und behandelt das Thema Resilienz und Risikomanagement im Bereich der maritimen Logistik. Dabei wird die Verbindung zwischen der menschlichen Gesundheit und der Widerstandsfahigkeit eines Transportes, z.B. von pharmazeutischen Produkten, aufgezeigt. Neben der Beschreibung verschiedener Risiken werden Masnahmen beschrieben, die eingesetzt werden konnen, um die Eintrittswahrscheinlichkeit oder die Konsequenzen von Risiken zu minimieren. Es wird dargestellt, wie die Erhohung der Transparenz der Lieferkette das Risikomanagement von Transporten vereinfachen kann.

Beside physical threats, ports are also vulnerable to cyber attacks due to their dependency on information and communications technology. Port Community Systems (PCSs) are information hubs for ports integrating information from various sources for global supply chains, connecting systems of terminal operators, carriers, freight forwarders and authorities. In that way, Port Community Systems must be regarded as critical infrastructures – successful cyber attacks can lead to massive problems in port operation, in extreme cases even to a standstill, and thus – depending on the duration – to bottlenecks in the supply of industries and population and to severe consequences for the whole economy. The recent case of the NotPetya attack on Maersk, causing some central systems to be down for several days worldwide, is estimated to have caused a loss of about US$ 200–300 Mio. This paper presents ongoing work within the research project PortSec, aiming at improved resilience of PCSs with respect to cyber attacks.

This paper describes the so-called CORE Simulation Environment (CORE-SE), a scenario based simulation environment which was developed in the scope of the EU project CORE. CORE-SE in particular provides means for the simulation of logistics scenarios. It consists of a software agent platform that facilitates a multiagent-based simulation (MABS) approach. In MABS, the environment and the objects acting therein are modelled by a number of software agents. CORE-SE provides a platform to create and configure different simulation scenarios and finally “run” them by the emulation of the agents’ behaviours in a coordinated and controlled way. CORE-SE, as described in this paper, has been applied in a specific supply chain sce-nario related to product integrity due to cargo temperature requirements where higher temperature (and solar radiation) could result in fluctuating product integrity in terms of quality loss and damage. During the simulated transport, climatic and weather-related factors like ambient temperature, ground temperature, solar radiation, and wind speed together with the resulting effects on the inner container temperature are simulated and respectively calculated according to the actual transport route of the container by usage of a climatic data model. The paper presents first results of the analysis, which are used to optimize the temperature control of the cargo, thereby better taking into account the mentioned external factors.

Background: From the perspective of older adults, the assistive technology application process is complex and their perceived involvement varies. Occupational therapists find it challenging to satisfy their clients' needs while complying with the eligibility criteria of the service provision system. Research has shown that, from the perspective of both clients and occupational therapists, the application process is perceived to be challenging. However, little is known about which specific actions and experiences during the application process give rise to this perceived complexity. Aim: The aim of this study was to investigate older adults' and occupational therapists' actions and experiences of their encounters during the assistive technology application process. Material and method: Data were generated using repeated participant observations and informal interviews. Ricoeur's theory of interpretation was used to interpret and discuss the data generated. Results: Two themes emerged: Expressing needs and translating needs. Conclusion: Older adults express their needs in different ways. When the older adults' expression of needs does not correspond with the service provision system's conditions of eligibility, the occupational therapists act as mediators, by translating experienced needs so that they are expressed in a way that corresponds to the expression of the eligibility criteria. The purpose of the paper is to present and evaluate results of an educational initiative at the occupational therapy programme at University College Lillebaelt in Denmark. The objectives of the initiative were (A) to promote research-based change of practice and knowledge development of municipal practice fields through students’ bachelor projects, (B) to develop students’ professional occupational therapy and practice research competences and (C) to establish an institutional partnership between the educational institutions and various practice fields. The theoretical and methodological approach to the students’ bachelor projects was practice research based on critical psychology. A total of 29 students divided into nine bachelor groups participated in the educational initiatives. Every group conducted a practice research project, they collaborated with occupational therapists in the practice field and citizens, drafted written reports and communicated their results. Concurrently, they received special education and counselling. The initiative was evaluated by focus group interviews and questionnaires. Results of the evaluations showed (A) that the bachelor projects as a new accomplishment lead to research based suggestions for change of the practice field; (B) that the students acquired competences in occupational therapy theory, practice research and collaboration and (C) that institutional partnerships were established promoting the essence of the initiative: change of practice and knowledge development. The practice research undertaken in the students' bachelor's projects had a theoretical and methodological foundation in critical psychology.METHODS: Four bachelor's cohorts with a total of 29 students participated in the educational initiatives from 2008 to 2011. Nine groups conducted practice research projects, wherein they collaborated both with occupational therapists in the practice field and with citizens, and later drafted written reports and communicated their results Concurrent with this, they received special education and counselling from teaching staff.RESULTS: The initiative was evaluated by analysing focus group interviews, questionnaires and the students' bachelor's theses. Results of the evaluations showed that (A) the developed institutional partnerships promoted the goals of the initiative, including research-based proposals for change in the practice field and that (B) the students acquired competences in practice research and collaboration.CONCLUSIONS: On top of what students learned through the initiative, all of this was to the benefit of the university college, the occupational therapy program, and the practice fields and citizens. The results point toward a continuation of the educational initiative.
