1. Vertical structure of buoyancy transport by ocean baroclinic turbulence

Abstract

Ocean mesoscale eddies enhance meridional buoyancy transport, notably in the Antarctic Circumpolar Current where they contribute to setting the deep stratification of the neighboring ocean basins. The much-needed parameterization of this buoyancy transport in global climate models requires a theory for the overall flux, but also for its vertical structure inside the fluid column. Based on the quasi-geostrophic dynamics of an idealized patch of ocean hosting an arbitrary vertically sheared zonal flow, we provide a quantitative prediction for the vertical structure of the buoyancy flux without adjustable parameters. The prediction agrees quantitatively with meridional flux profiles obtained through numerical simulations of an idealized patch of ocean with realistic parameter values. This work empowers modelers with an explicit and physically based expression for the vertical profile of buoyancy transport by ocean baroclinic turbulence, as opposed to the common practice of using arbitrary prescriptions for the depth-dependence of the transport coefficients., Comment: Geophysical Research Letters, in press

2. A direct derivation of the Gent-McWilliams/Redi diffusion tensor from quasi-geostrophic dynamics

Abstract

The transport induced by ocean mesoscale eddies remains unresolved in most state-of-the-art climate models and needs to be parameterized instead. The natural scale separation between the forcing and the emergent turbulent flow calls for a diffusive parameterization, where the eddy-induced fluxes are related to the large-scale gradients by a diffusion tensor. The standard parameterization scheme in climate modeling consists in adopting the Gent-McWilliams/Redi (GM/R) form for the diffusion tensor, initially put forward based on physical intuition and educated guesses before being put on firm analytical footing using thickness-weighted average (TWA). In the present contribution we provide a direct derivation of this diffusion tensor from the quasi-geostrophic (QG) dynamics of a horizontally homogeneous three-dimensional patch of ocean hosting a large-scale vertically-sheared zonal flow on the beta plane. While less general than the TWA approach, the present QG framework leads to rigorous constraints on the diffusion tensor. First, there is no diapycnal diffusivity arising in the QG GM/R tensor for low viscosity and small-scale diffusivities. The diffusion tensor then involves only two vertically dependent coefficients, namely the GM transport coefficient $K_{GM}(z)$ and the Redi diffusivity $K_R(z)$. Secondly, as already identified by previous authors the vertical structures of the two coefficients are related by the so-called Taylor-Bretherton relation. Finally, while the two coefficients generically differ in the interior of the water column, we show that they are equal to one another near the surface and near the bottom of the domain for low-enough dissipative coefficients. We illustrate these findings by numerically simulating the QG dynamics of a horizontally homogeneous patch of ocean hosting a vertically sheared zonal current resembling the Antarctic Circumpolar Current.

Abstract

In 2013, Stockwell and Hubbard published an article on emerging principles in Mobile Assisted Language Learning (MALL). In that article, they mentioned three issues that could perhaps impede learning in the domains of the physical, pedagogical, and pyscho-social. The physical issue they imply, refers to the general size of screens, which at the time were deemed small, or not big enough to have an impact on learning. The pedagogical issue involved with MALL, according to Stockwell and Hubbard (2013), is to ensure that tasks introduced with mobile tools are suited to the affordances of the devices. They questioned the need for mobile devices for language learning and whether tasks that were being introduced were suitable and worthwhile and not simply replicating what could normally be done without them, with pen and paper for example. The psycho-social issue they refer to implies that the primary function of mobile devices is generally regarded as one for “personal and social purposes, as opposed to work or study purposes” (Stockwell & Hubbard, 2013, p. 4). In this short paper, we would like to argue this point by introducing seven tools currently available for free on mobile applications that can be adapted to foreign language learning in several ways.

12. Dépistage des anomalies visuelles des enfants de 0 à 2 ans par les médecins généralistes de Gironde

Abstract

Introduction. La prévalence des déficits visuels en France à l’âge de 5 ans a été évaluée en 2002 à 14,5% malgré des recommandations nationales mises à jour en 2005. Cette étude va évaluer l’application des recommandations HAS par les médecins généralistes de Gironde pour le dépistage des anomalies visuelles des enfants de 0 à 2 ans. Matériel et Méthode. Cette étude descriptive quantitative a été réalisée auprès de 47 médecins généralistes de la Gironde, à l’aide d’un questionnaire d’enquête de pratique élaboré en miroir des dernières recommandations. Résultats et discussion. 20 médecins ont terminé l’étude. 44 questionnaires ont été analysés soit 44 enfants. 80% des médecins recherchent systématiquement des facteurs de risque. 90% des médecins étudient la lueur pupillaire, 85% la poursuite du regard et 75% les réflexes photomoteurs. Tous s’aident d’une source lumineuse. Aucun médecin n’a utilisé un test de Lang 1 ou des lunettes à secteur de dépistage. 100% des enfants présentant des anomalies ont été adressés à un confrère, 70% à un ophtalmologiste libéral. Les femmes respectent mieux les recommandations (70,2% de conformité) que les hommes (60,6%). Un seul médecin a été en mesure de réaliser l’examen clinique complet selon les recommandations de l’HAS et l’âge de l’enfant. Conclusion. Les recommandations sont peu respectées. L’examen visuel de base est réalisé par une majorité. Manque de formation initiale, recommandations non adaptées à la pratique ? Les médecins généralistes sont-ils les mieux placés pour réaliser ce dépistage ?

Background and Purpose: Communication with patients is a core clinical skill in medicine that can be acquired through communication skills training. Meanwhile, the importance of transfer of communication skills to the workplace has not been sufficiently studied. This study aims to assess the efficacy of a 40-hour training program designed to improve patients' satisfaction and residents' communication skills during their daily clinical rounds. Methods: Residents were randomly assigned to the training program or to a waiting list. Patients' satisfaction was assessed with a visual analog scale after each visit. Transfer of residents' communication skills was assessed in audiotaped actual inpatient visits during a half-day clinical round. Transcripted audiotapes were analyzed using content analysis software (LaComm). Training effects were tested with Mann-Whitney tests and generalized linear Poisson regression models. Results: Eighty-eight residents were included. First, patients interacting with trained residents reported a higher satisfaction with residents' communication (Median = 92) compared to patients interacting with untrained residents (Median = 88) (p = .046). Second, trained residents used more assessment utterances (Relative Risk (RR) = 1.17; 95% Confidence intervals (95%CI) = 1.02-1.34; p = .023). Third, transfer was also observed when residents' training attendance was considered: residents' use of assessment utterances (RR = 1.01; 95%CI = 1.01-1.02; p = .018) and supportive utterances (RR = 0.99; 95% CI = 0.98-1.00; p = .042) (respectively 1.15 (RR), 1.08-1.23 (95% CI), p<.001 for empathy and 0.95 (RR), 0.92-0.99 (95% CI), p = .012 for reassurance) was proportional to the number of hours of training attendance. Abstract

There are few studies which have investigated variables associated with the development of burnout among residents working with cancer patients. The aim of this study is to identify variables leading to residents' burnout in order to develop effective interventions. Burnout was assessed with Maslach Burnout Inventory. Person- (i.e., emotional-focused coping) and work-related (i.e., changes in lack of organizational support index) variables explain 28% of the variance in changes in emotional exhaustion. Training programs may be improved by adding specific modules for residents, about problem-focused coping in interviewing patients, and for supervisors, about effective team management.

Abstract

BACKGROUND: This study aims to assess the efficacy of a 40-h training programme designed to teach residents the communication skills needed to break the bad news. METHODS: Residents were randomly assigned to the training programme or to a waiting list. A simulated patient breaking bad news (BBN) consultation was audiotaped at baseline and after training in the training group and 8 months after baseline in the waiting-list group. Transcripts were analysed by tagging the used communication skills with a content analysis software (LaComm) and by tagging the phases of bad news delivery: pre-delivery, delivery and post-delivery. Training effects were tested with generalised estimating equation (GEE) and multivariate analysis of variance (MANOVA). RESULTS: The trained residents (n=50) used effective communication skills more often than the untrained residents (n=48): more open questions (relative rate (RR)=5.79; P<0.001), open directive questions (RR=1.71; P=0.003) and empathy (RR=4.50; P=0.017) and less information transmission (RR=0.72; P=0.001). The pre-delivery phase was longer for the trained (1 min 53 s at baseline and 3 min 55 s after training) compared with the untrained residents (2 min 7 s at baseline and 1 min 46 s at second assessment time; P<0.001). CONCLUSION: This study shows the efficacy of training programme designed to improve residents' BBN skills. The way residents break bad news may thus be improved.

Several studies indicate that family functioning and parental expressiveness can influence children's glycaemic control. However, previous studies have had contradictory findings. Furthermore, no previous work has simultaneously explored the mother's and father's perception of family cohesiveness together with maternal and paternal alexithymia in relation to a child's diabetic control. Abstract

Dans le cadre d’une étude belge multicentrique, cet article rapporte l’évaluation qualitative et quantitative des besoins psychosociaux des proches aidants principaux des patients atteints d’un cancer, ainsi que les difficultés vécues par ces proches à faire face aux besoins psychosociaux des malades. De janvier à novembre 2004, l’ensemble des proches aidants principaux des patients atteints d’un cancer et hospitalisés au sein de sept unités de soins en oncologie, ainsi que le proche aidant principal d’un patient sur deux consultant au sein de ces mêmes unités ont étéinvités à prendre part à l’étude. Les proches aidants principaux ont rempli une adaptation du questionnaire Cancer Rehabilitation Evaluation System (CARES) permettant l’investigation de 24 types de difficultés ayant pu être rencontrées au cours du dernier mois, ainsi qu’une seconde adaptation du CARES permettant d’évaluer leur perception de 38 types de difficultés potentielles chez leur proche malade au cours du dernier mois. Les sujets ont également étéinvités à indiquer s’ils avaient eu des difficultés à faire face à chaque difficulté perçue chez leur proche malade. Les informations médicales relatives aux patients ont étérécoltées auprès de leurs oncologues. Parmi les 284 proches aidants principaux inclus, une grande majoritéa rencontré au cours du dernier mois une à plusieurs difficultés physiques, psychosociales, sexuelles de même que des difficultés de communication avec leur partenaire. La majorité des proches aidants principaux perçoit de nombreuses difficultés chez leur proche malade et 20 à 60 % d’entre eux rapportent éprouver au moins un peu de difficulté à faire face à ces difficultés. À côtéde leurs nombreuses difficultés personnelles, les proches aidants principaux des patients atteints d’un cancer perçoivent donc de nombreuses difficultés psychosociales chez leur proche malade. Abstract

Ce chapitre est organisé autour de la question d’une double responsabilité : d’une part, la responsabilité du personnel de santé d’être informé et compétent sur le sujet de violences masculines envers les femmes (VeF); de l’autre, la responsabilité de ceux/celles qui interviennent dans le social d’évaluer leur interventions, puisque une intervention, même bien construite, pourrait avoir des effets négatifs ou n’avoir pas d’effets du tout. La violence a des sérieuses conséquences sur la santé. Les médecins devraient être en mesure d’aider les victimes, mais ils manquent souvent des connaissances, ou ne se sentent pas concernés : sensibiliser les étudiant-e-s est donc une étape nécessaire. En 2009, un cours sur la VeF a été proposé aux étudiant-e-s en médicine à Trieste (Italie). Pour l’évaluation du cours nous avons utilisé une approche quasi-expérimentale, avec des questionnaires pré et posttest, et avec un groupe expérimental (N=75) et un groupe de contrôle (N=63). Les résultats montrent que, au départ, il n’y avait pas de différences entre les deux groupes concernant connaissances et attitudes relatives à la VeF. Après le cours, les étudiant-e-s du groupe expérimental avaient plus de connaissances, se sentaient plus concernées et présentaient moins de préjugés, alors que aucun changement n’a été observé pour le groupe de contrôle. Les commentaires des étudiant-e-s confirment que la participation au cours a modifié leur perception de la VeF. Tous/toutes ont qualifié ces changements de très positifs; toutefois une minorité dit avoir été mal à l’aise face à ce qu’ils considéraient une approche féministe. Cette étude indique que qu’il est possible de mettre en œuvre une évaluation rigoureuse et que ce cours sur la VeF a eu un impact positif.
