Abstract

Background: The SARS-CoV-2 pandemic is a worldwide challenge. The CRIT-CoV-U pilot study generated a urinary proteomic biomarker consisting of 50 peptides (COV50), which predicted death and disease progression from SARS-CoV-2. After the interim analysis presented for the German Government, here, we aimed to analyse the full dataset to consolidate the findings and propose potential clinical applications of this biomarker. Methods: CRIT-CoV-U was a prospective multicentre cohort study. In eight European countries (Austria, France, Germany, Greece, North Macedonia, Poland, Spain, and Sweden), 1012 adults with PCR-confirmed COVID-19 were followed up for death and progression along the 8-point WHO scale. Capillary electrophoresis coupled with mass spectrometry was used for urinary proteomic profiling. Statistical methods included logistic regression and receiver operating characteristic curve analysis with a comparison of the area under curve (AUC) between nested models. Hospitalisation costs were derived from the care facility corresponding with the Markov chain probability of reaching WHO scores ranging from 3 to 8 and flat-rate hospitalisation costs adjusted for the gross per capita domestic product of each country. Findings: From June 30 to Nov 19, 2020, 228 participants were recruited, and from April 30, 2020, to April 14, 2021, 784 participants were recruited, resulting in a total of 1012 participants. The entry WHO scores were 1–3 in 445 (44%) participants, 4–5 in 529 (52%) participants, and 6 in 38 (4%) participants; and of all participants, 119 died and 271 had disease progression. The odds ratio (OR) associated with COV50 in all 1012 participants for death was 2·44 (95% CI 2·05–2·92) unadjusted and 1·67 (1·34–2·07) when adjusted for sex, age, BMI, comorbidities, and baseline WHO score; and for disease progression, the OR was 1·79 (1·60–2·01) when unadjusted and 1·63 (1·41–1·91) when adjusted (p<0·0001 for all). Abstract

Proteome analysis has emerged as a powerful tool to decipher (patho)physiological processes, resulting in the establishment of the field of clinical proteomics. One of the main goals is to discover biomarkers for diseases from tissues and body fluids. Due to the enormous complexity of the proteome, a separation step is required for mass spectrometry (MS)-based proteome analysis. In this review, the advantages and limitations of protein separation by two-dimensional gel electrophoresis, liquid chromatography, surface-enhanced laser desorption/ionization and capillary electrophoresis (CE) for proteomic analysis are described, focusing on CE-MS. CE-MS enables separation and detection of the small molecular weight proteome in biological fluids with high reproducibility and accuracy in one single processing step and in a short time. As sensitive and specific single biomarkers generally may not exist, a strategy to overcome this diagnostic void is shifting from single analyte detection to simultaneous analysis of multiple analytes that together form a disease-specific pattern. Such approaches, however, are accompanied with additional challenges, which we will outline in this review. Besides the choice of adequate technological platforms, a high level of standardization of proteomic measurements and data processing is also necessary to establish proteomic profiling. In this regard, demands concerning study design, choice of specimens, sample preparation, proteomic data mining, and clinical evaluation should be considered before performing a proteomic study.

Abstract

Die vorliegende Stellungnahme der Expertenkommission zum Monitoring-Prozess „Energie der Zukunft“ kommentiert den achten Monitoring-Bericht zur Energiewende der Bundesregierung für die Berichtsjahre 2018 und 2019. Die Stellungnahme bezieht sich auf den Entwurf des achten Monitoring-Berichts, welcher der Expertenkommission im Dezember 2020 vom Bundesministerium für Wirtschaft und Energie (BMWi) in einer vorläufigen Fassung zur Verfügung gestellt wurde. Soweit zeitlich möglich, wurden auch Änderungen des achten Monitoring-Berichts im Kontext der Ressortabstimmung berücksichtigt. Die vorliegende Stellungnahme basiert auf dem am 18. Dezember 2020 für die Expertenkommission verfügbaren Informations- und Datenstand. Zur Dokumentation des Fortschritts bei der Umsetzung des Energiekonzepts veröffentlicht die Bundesregierung jährlich einen faktenbasierten Monitoring-Bericht und zudem alle drei Jahre einen Fortschrittsbericht mit einer vertieften Analyse der Entwicklungen und Maßnahmen sowie einem Ausblick. In diesem Jahr veröffentlicht die Bundesregierung ihren mittlerweile achten Monitoring-Bericht. Seit 2011 steht der Bundesregierung in diesem Prozess eine unabhängige Kommission aus vier Expertinnen und Experten beratend zur Seite. Die Expertenkommission zum Monitoring-Prozess „Energie der Zukunft“ legt ihrerseits jährliche Stellungnahmen zum Fortschritt der Energiewende vor, die den Berichten der Bundesregierung beigefügt und dem Kabinett sowie dem Bundestag zugeleitet werden. Die vorliegende Stellungnahme der Expertenkommission bezieht sich auf den Entwurf des achten Monitoring-Berichts, der der Expertenkommission am 09. Dezember 2020 vom Bundesministerium für Wirtschaft und Energie (BMWi) zur Verfügung gestellt wurde.

Abstract

Background Kidney transplantation is the best treatment option for end-stage kidney disease but is still associated with long-term graft failure. In this study, we evaluated the application of urinary proteomics to identify grafts with high failure risk before initial decline of estimated glomerular filtration rate (eGFR) with irreversible graft changes. Methods Fifty-two living donor kidney transplant recipients (KTR) with 8-year follow-up were enrolled. All patients underwent clinical examination and had a routine laboratory screening at 3, 6, 12, 24, 36, 48 and 96 months post-transplantation, including creatinine, urea, albumin and 24-h proteinuria. Graft function was estimated according to Nankivell. Urine samples at Month 24 were analysed by capillary electrophoresis coupled mass spectrometry followed by classification with the chronic kidney disease classifier CKD273. Results CKD273 showed significant correlation with serum creatinine at every time point and moderate inverse correlation for the slope in glomerular filtration rates by Nankivell (r = −0.29, P = 0.05). Receiver operating characteristics analysis for graft loss and death within the next 6 years after proteomic analysis resulted in an area under curve value of 0.89 for CKD273 being superior to 0.67 for Nankivell eGFR. Stratification into CKD273-positive and -negative patient groups revealed a hazard ratio of 16.5 for prevalence of graft loss in case of CKD273 positivity. Conclusions Using a representative KTR cohort with 8-year follow-up, we could demonstrate significant value of CKD273 for risk stratification of graft loss. This study provides the conceptual basis for further evaluation of CKD273 as a prognostic tool for long-term graft function risk stratification by large prospective clinical trials. Abstract

The possible role of megalin in the cellular delivery of sex steroids and the consequences of receptor deficiency for androgen and estrogen action in vivo are investigated. The findings uncover the existence of endocytic pathways for protein bound androgens and estrogens and their critical role in development of the reproductive organs.
