Abstract

Background and Objective: When geriatric patients are admitted for an orthopedic problem, their risk for complications is increased due to underlying chronic illness, chronic drug therapy and additive treatment changes requiring pharmacist counseling. The clinical pharmacist can counsel the patient and focus on transmural communication concerning polypharmacy to improve continuity of care after discharge by primary care providers. We assessed whether a pharmacist-led intervention aimed at optimizing transmural communication of drug-related problems (DRPs) to general practitioners (GPs), could lead to an additional DRP reduction in orthogeriatric patients. Method: An interventional study (pre-post design) was performed (October to December 2021 and January to March 2022). Orthopedic patients (≥65 years) were included. The pre-group received usual care, the post-group received the pharmacist-led transmural care program. Main outcome measures: The reduction of DRPs one month after discharge was calculated in both periods and, in addition, the GP acceptane rate was determined for the proposed interventions. Readmissions one month after discharge and the clinical impact of the pharmaceutical interventions (PIs) were evaluated, using the Clinical, Economic and Oganisational (CLEO) tool. Finally, it was determined whether the intervention, and other patient- or drug-related characteristics, influenced the DRP reduction rate. Abstract

Alterations in musculoskeletal health with advanced age contribute to sarcopenia and decline in bone mineral density (BMD) and bone strength. This decline may be modifiable via dietary supplementation. To test the hypothesis that a specific oral nutritional supplement can result in improvements in measures of bone health. Participants (n 380) were participants of the PROVIDE study, a 13-week, multicenter, randomized, controlled, double-blind, 2 parallel-group study among non-malnourished older participants (≥ 65 years) with sarcopenia [determined by Short Physical Performance Battery (SPPB; 0-12) scores between 4 and 9, and a low skeletal muscle mass index (SMI; skeletal muscle mass/BW × 100) ≤ 37% in men and ≤ 28% in women using bioelectric impedance analysis] Supplementation of a vitamin D, calcium and leucine-enriched whey protein drink that comprises a full range of micronutrients (active; 2/day) was compared with an iso-caloric control. Abstract

Background. There is growing evidence for the significant involvement of inflammatory processes in the development of muscle wasting in old age. Therefore, any disease accompanied by inflammation can be threatening to the muscle function in geriatric patients. Methods. Sixty-three hospitalized geriatric patients (42 female, 21 male; mean age 84.2 [+ or -] 5.7 years) were monitored weekly for muscle function (grip strength, fatigue resistance, shoulder extension strength, and hip extension strength) and for concentration of circulating C-reactive protein (CRP), fibrinogen, interleukin 6 (IL-6), and tumor necrosis factor-[alpha] alpha (TNF-[alpha]). Results. On the basis of circulating CRP and fibrinogen concentrations, 42 patients were categorized on admission as inflammatory and 21 as noninflammatory. Inflammatory patients presented significantly weaker grip strength, shoulder extension strength, and a worse fatigue resistance than did noninflammatory patients. These muscle functions were negatively correlated with the concentrations of circulating CRP and IL-6, but not with fibrinogen or TNF-[alpha]. In noninflammatory patients, the fatigue resistance improved significantly during the first week of hospitalization. In patients admitted with inflammation, no improvement of muscle function was observed. Patients who remained inflammatory for 2 weeks or more presented a significant worsening of fatigue resistance. Conclusions. Geriatric hospitalized patients presenting with inflammation show significantly worse muscle functions, which do not improve during hospitalization despite adequate treatment of the primary disease. Reduced strength and fatigue resistance are significantly related to the concentration of circulating CRP and IL-6. Standard treatment of the underlying illness and classic physical therapy are not sufficient to normalize the skeletal muscle strength and fatigue resistance in these hospitalized patients.

Abstract

Background. The value of the 6-minute walk in a population of elderly patients with chronic heart failure (CHF) has not yet been established, as it has been in a younger population. Methods. In a prospective trial, 2 exercise tests were compared: the 6-minute walk (6 MW) and a treadmill test with progressive increments in workload. 37 patients (mean age 81.3, SD 5.6 years): 11 untrained controls, 16 patients with New York Heart Association (NYHA) class II and 10 patients with NYHA class III participated. The main outcome measures were the distance walked in 6 minutes, the symptomatic V[O.sub.2]max in the progressive treadmill test and the degree of CHF as scored by the NYHA classification and the Boston Study Group (BSG) score. Results. For both the 6 MW and the treadmill test there was a significant difference in distance between NYHA class II and III patients (p < .001), between the controls and NYHA class III patients (p < .001), but not between the controls and NYHA class II patients. The distance walked in the 6 MW was well correlated with the distance walked in the treadmill test (p < .001). Eight participants (22%), however, were unable to perform this treadmill test and 6 participants (17%) covered very low distances compared to the 6 MW. The V[O.sub.2]max was lower in NYHA class III than in class II patients (p = .032). NYHA classification and BSG score correlated reasonably well. Conclusion. A treadmill exercise test with V[O.sub.2]max measurement in elderly patients with CHF is difficult to accomplish. On the contrary, the 6 MW gives a good impression of the remaining exercise capacity. It is well correlated with the treadmill test. The 6 MW is well tolerated by elderly patients and differentiates between NYHA classes II and III. Untrained controls could not be differentiated from NYHA class II patients.
